Types of Facilities, Care Options A number of broad categories of care options and facilities are included in this guide. Terminology may vary from facility to facility. Residential Living is for individuals who are responsible for their own basic needs. Nursing services are not available on site nor are residential communities affiliated with nursing care providers. No meal service or activity programs are offered. Active Adult Communities are residential-type communities for mature adults only. Individuals are responsible for their own basic needs. Nursing services are generally not available on site. Meal service is not provided. Outdoor maintenance service may be provided. Continuing Care Retirement Communities/Life Plan Communities To portray the “vibrant, forward-looking nature of the contemporary senior living community,” some continuing care retirement communities have chosen to describe themselves as life plan communities. A life plan community follows the definition of a CCRC set by The National Investment Center: Age-restricted properties that include a combination of independent living, assisted living, and skilled nursing services (or independent living and skilled nursing) available to residents all on one campus. Resident payment plans vary and

include entrance fee, condo/co-op, and rental programs. Retirement Communities are planned for those who are able to care for their own basic needs (or almost so) and choose to live in a community with other seniors. Organized social programs, meal service, transportation, recreational activities, and access to health and shopping are typical amenities. Nursing care is not usually provided. Some do offer assisted living/personal care designed for individuals who function mainly on their own but may need help with medications, bathing, or dressing. Services offered are similar to those provided by assisted living/personal care facilities. Retirement communities can be freestanding or affiliated with a complex offering nursing care services. Personal Care Homes offer food, shelter, and personal assistance or supervision. They are ideal for people who do not want to forfeit independence and do not require the services of a licensed long-term care facility but do require assistance or supervision in activities of daily living (ADL). Some ADL provided are transferring in and out of a bed or chair, toileting, bladder and bowel management, personal hygiene, securing and managing healthcare, selfadministering medication, and proper turning and positioning in a bed or chair. 50plus Living 2023
Assisted Living Residences provide an environment that combines housing and supportive services to allow residents to “age in place” without having to move to a licensed long-term care facility when their care needs increase. ALRs have living units with kitchen capacity, private bathrooms, and only one person per unit. An ALR may provide services that would be provided in a longterm care facility (personal care homes may not). As of January 2011, changed regulations regarding licensure for assisted living communities became effective. Nursing/Rehab Facilities provide nursing care and related medical or other personal health services 24 hours a day, seven days a week, to individuals who require full-time care or supervision but do not need more intensive, hospital-based care. Nursing/rehab facilities may be independent or part of another community. Individuals may be temporarily placed for rehabilitation following an illness, injury, or surgery or reside long term due to illness or aging. Nursing facilities offer skilled medical care under the supervision of licensed nurses, and at least one registered nurse must be on duty during the day. Custodial care is also offered and includes services that assist in the activities of daily living, which include bathing, dressing, using the bathroom, or transferring from a bed to a chair. Nursing centers are licensed by the State Department of Health. Acute Care Facilities are licensed as hospitals. They provide the highest level of care under a physician. Some services provided at an acute care facility include emergency services, critical care, medical/ surgical services, and maternal/childbirth services. Dementia/Alzheimer’s services offer assistance and support to those living with memory issues. These units/communities www.onlinepub.com
enable the person living with memory issues to be as independent as possible while enhancing freedom of movement in a safe, secured environment. Adult Day Centers offer programs in facilities or function as independent organizations. The services may include but are not limited to personal care; assistance with eating, using the toilet, and taking medication; and social assistance. Centers offer a protective, supervised setting and generally operate during normal business hours. Respite Care provides caregiving opportunities on a short-term basis. Care may range from personal to nursing care. Home Care Services may be provided in a residential setting or as ancillary services, such as in personal care, assisted living, or nursing care environments. Services include skilled nursing services, therapies, and changing and reinforcing simple wound dressing, as well as nonmedical services, such as light housekeeping, transportation to doctor visits, shopping, respite, and more. Hospice Care is for families living and coping with life-limiting illnesses and provides professional treatment of pain and symptom management with support and counseling for medical, psychological, and spiritual needs. Palliative Care also focuses on providing relief from the symptoms and stress of serious illness but, unlike hospice, can be provided along with curative treatment. Living Independence for the Elderly (LIFE) is a managed care program that provides a comprehensive, all-inclusive package of medical and supportive services. The program is known nationally as the Program of All-Inclusive Care for the Elderly (PACE). All PACE providers in Pennsylvania have “LIFE” in their name.

Susquehanna Valley Profile Weather Average high temperatures are the 80s in the summer months and the 30s in the winter. The warmest month is July, with an average maximum temperature of 85 degrees. The coldest month is January, with an average minimum temperature of 20-23 degrees. Average annual precipitation is 41-47 inches.
Taxes Sales tax is 6%. Sales tax exemptions include groceries, health-related items, medical services, prescription drugs, and groceries. State income tax is 3.07%; retirement income is tax-exempt. Property tax is determined by the local government according to school districts and municipalities.
State Lottery Benefits The Pennsylvania Lottery contributes to programs such as Property Tax/Rent Rebate; the Mass Transit Program; PACE, PACENET, and PACE Plus Medicare; long-term care services administered by the Department of Public Welfare; and the 52 Area Agencies on Aging.
PACE To be eligible for PACE, you must be 65 years of age or older and a Pennsylvania resident at least 90 days before the date of application. You cannot be enrolled in the Department of Public Welfare’s Medicaid prescription benefit. PACE and PACENET eligibility is determined by your previous calendar year’s income. Effective in 2014, Social Security Medicare Part B premiums are

excluded from income. For a single person, your total income must be $14,500 or less. For a married couple, your combined total income must be $17,700 or less. Once you are enrolled in the PACE program, a benefit card will be sent to you, and you will pay no more than $6 for each generic prescription medication and no more than $9 for each brand-name prescription at your pharmacy. These copayments are based on a 30-day supply. Effective Jan. 1, 2022, the PACE Program is encouraging cardholders to enroll in a Part D plan because it may provide additional benefits for members. Effective August 2021, PACE reinstated prescription limits when PACE is the primary payer. PACE legislation limits prescriptions to a 30-day supply or 100 units, whichever is less. Enrollees must use 75% of their supply before refills will be reimbursed.
PACENET To be eligible for PACENET, you must be 65 years of age or older, a Pennsylvania resident at least 90 days before the date of application, and you must not be enrolled in the Medicaid prescription benefit program. As of February 2022, PACENET income limits increased by $6,000. Residents who were not eligible in the past may now be eligible with the new income limits in place. A single person’s total income must be between $14,500 and $33,500. A couple’s combined total income must be between $17,700 and $41,500. PACENET cardholders who do not enroll in a Part D plan will pay a nominal deductible each month at the pharmacy, which will be calculated through the cost 50plus Living 2023

In addition, the individual will pay no more than $8 for each generic prescription medication and no more than $15 for each brand name. PACENET cardholders enrolled in one of the program’s partner Part D plans will pay the Part D premium at the pharmacy each month, which will be calculated through the cost of the medications. PACENET cardholders enrolled in a Part D plan that is not one of the program’s partner plans will pay the Part D premium directly to the Part D plan. In addition, they will pay no more than the PACENET co-payments of $8 for each generic prescription medication and $15 for each brand name. Effective Jan. 1, 2022, PACENET cardholders not enrolled in a Part D plan will pay a $40.74 premium at the pharmacy each month they use their PACENET benefit. PACE Plus Medicare Under PACE Plus Medicare, PACE/ PACENET coverage is supplemented by federal Medicare Part D prescription coverage and offers older Pennsylvanians the best benefits of both programs. Older adults continue to receive the same prescription benefits while, in many cases, saving more money.
Transportation Pennsylvania Free Transit Program – Provides free transit service on bus routes, trolley, and rapid-transit lines. Individuals 65 and older may ride free on fixed-route www.onlinepub.com
services at all regular operating times on weekdays, weekends, and holidays. You will need a senior citizen transit identification card to ride free. The identification card is available free of charge from participating local transit operators. Shared-Ride Program – Provides reduced-fare transportation services to persons 65 and older utilizing shared-ride transportation services. Reduced-fare transportation is provided to medical appointments, senior community centers, and adult daycare centers. Mature Driver Safety Program – There is a mandated auto insurance discount for older drivers who complete a mature driver improvement course. Courses are offered through AARP, AAA, and the National Safety Council. It is intended for drivers 55 and older. Low-Vision License – Persons who do not meet the vision standards for a driver’s license may qualify for a restricted (lowvision) license. A restricted license allows the driver to drive during daylight hours on roadways other than freeways. A person with visual acuity of less than 20/70 combined vision, but at least 20/100 combined vision with best correction and 120 degrees’ field vision in the horizontal meridian, may qualify with a recommendation from an optometrist or physician. Retired Status Registration – Customers who qualify for retired status registration pay a $10 per year processing fee instead of the full $38 registration renewal fee. Total income must not exceed $19,200 and vehicles must weigh less than 9,000 lbs.

Falls Can Have Severe Health Ramifications for Older Adults
By Kimberly Blaker Trips, stumbles, and minor tumbles may not seem like a big deal when you’re younger. But for seniors, any fall has the potential to result in severe injury or a health crisis. According to the Centers for Disease Control, a senior over the age of 65 falls every second of every day; 1 in 5 falls ends in serious injury. So understanding the risk factors that lead to both fatal and nonfatal falls and taking precautions are vital to avoid the potentially debilitating consequences seniors can face. 10

What are the Causes of Falling? A variety of factors increase an individual’s likelihood of falling. Usually, it’s a combination of multiple factors that lead to a fall. As you age, there’s a natural degeneration in various bodily functions. This may include: • A decrease in strength • Difficulty with balance • Slowed reaction times and reflexes • Worsened vision • Impaired hearing • Cognitive impairments Any of these individual issues can cause difficulty walking, going up and down stairs, sitting, standing, or lying down. These problems can also lead to increased fatigue and less awareness of external factors that could potentially contribute to a fall. Medical factors. Aging may lead to higher susceptibility to certain medical conditions that increase your risk of falling, including dementia, osteoporosis, vertigo, depression, COPD, diabetes, arthritis, and disabilities, particularly in the feet or lower limbs. Surgical procedures for medical conditions or injuries can also affect an individual’s mobility, strength, and function. Medications taken for medical conditions can increase the risk of falling because of the side effects that compromise balance. Taking four or more drugs is also associated with falling. Environmental factors. Falls are more common in environments with slippery floors, poor lighting, uneven surfaces, stairs, and clutter. Many falls occur in the bathroom. Getting in or out of the shower or bath can be particularly hazardous to older adults. Interestingly, however, household and environmental factors aren’t as significant an indicator as health factors. Other factors. The potential for serious or fatal injury from a fall is especially concerning if a person lives alone. Also, senior women are more likely to fall and have serious hip injuries. www.onlinepub.com

Being less active means more muscle atrophy and an increased likelihood of falling. Likewise, being overly active can lead to more opportunities to fall and potentially pushing your physical limits. Finally, a history of previous falls means you have a higher risk of falling again. What Happens to Older Adults Who Suffer a Fall? Just one fall can have a significant negative impact on a senior’s quality of life. Consider these facts. • Most fatal injuries occur from falling. • Of all hip fractures, 95% happen during a fall. • Falling is the leading cause of traumatic brain injury. • A fear of falling again is common. • Seniors who’ve fallen may start avoiding activities that could lead to another fall. • Injuries from a fall may contribute to an inability to function independently and require care or moving to a nursing home. • The consequences of falling may increase the risk of depression. • Medical costs after a fall can be expensive. What are Some Prevention Strategies to Avoid Falling? Make sure your prevention strategies address your individual risk factors. List out your personal risk factors, look at which ones can be changed, and create a plan to decrease those risks. Stay active at an appropriate level for your health and ability. Try lowerintensity workouts like yoga, tai chi, walking, swimming, water aerobics, and weight training, especially for leg muscles. Work with a physical therapist to prevent falls or to heal from injury, especially if you have health risk factors. A physical therapist can help you improve balance and build strength while taking into account your individual needs. Get regular physical exams to check for any risk factors or problems that could lead to falls. Be honest about any concerns you have and discuss any side effects of your medication. 12

Wear well-fitting, comfortable, wide, and flat shoes for a stable base. If needed, be sure you actually use devices like canes or walkers for assistance. Assess your environment and take steps to make it safer. Consider features like grab bars in the bathroom, secure handrails on stairs, a more accessible tub or shower, and plenty of light. Also, avoid small rugs that can bunch up or catch on your shoes, and remove clutter to keep floors and spaces clear. Have a plan in place in case you fall and know what to do and how to get help. If you’re at risk, get a wearable emergency fall device so you can call for medical assistance. New technology is available for automatic fall-risk detection. There are also manual devices to push in case of an emergency. For mature adults, falling can be a significant concern in trying to stay safe and healthy. But falling isn’t an inevitable part of growing older. By taking these steps to lessen your risk of falling and being prepared in the event of a fall will go a long way toward living a healthy, fulfilling life throughout your retirement.

Any tips to help us find someone? – Searching Sarah Dear Sarah, Getting your mother some help at home to handle some of her day-to-day chores is a smart idea that can make a big difference in keeping her independent longer. Here are some tips to help you find someone reliable. 28

In-Home Help For seniors who could use some help at home — but don’t need a caregiving aide for personal care — there are homemakers/home helpers you can hire who can make life a little easier. Most in-home helpers can assist with any number of things, like shopping, running errands, transportation, light housekeeping, laundry, meal preparation, arranging services (home maintenance, lawn care, etc.), and other household chores, along with providing companionship and support. And, if your mom gets to the point she needs personal/physical care like bathing or dressing, many home helpers can assist with this too. Most home helpers are part-time workers who work a few hours a day or a few days per week. You also need to know that while Medicare does cover home healthcare services if a doctor orders it, they do not cover in-home homemaker/helper services. There are two ways in which you can go about hiring someone for your mom: either through a home care agency or you can hire someone directly on your own. Home Care Agency Hiring a home helper through a nonmedical home care or nonmedical companion care agency is the easiest and perhaps safer option, but it is the more expensive option of the two. Costs typically run anywhere from $15 to $25 an hour, depending on where you live. How it works is you pay the company, and they handle everything, including assigning appropriately trained and prescreened staff to care for your mother and finding a fill-in on days her helper cannot come. Some of the drawbacks, however, are that you may not have much input into the selection of the aide, and the helpers may change or alternate, which can cause a disruption. To find a home care agency in your area, use search engines like Google or Yahoo and type in “nonmedical home care” followed by the city and state your mom lives in. Or you can use Medicare’s home health services search tool at medicare.gov/ care-compare — click on “home health services.” Most home health agencies offer some form of nonmedical home care services too. You can also check your local Yellow Pages under “home health services.”

Hiring Directly Hiring a personal assistant/home helper on your own is the other option, and it’s less expensive. Costs typically range between $12 and $20 per hour. Hiring directly gives you more control over who you hire so you can choose someone you feel is right for your mom. But be aware that if you do hire someone on your own, you become the employer, so there’s no agency support to fall back on if a problem occurs or if the assistant doesn’t show up. You’re also responsible for paying payroll taxes and any worker-related injuries that may happen. If you choose this option, make sure you check the person’s references thoroughly and do a criminal background check. To find someone, ask for referrals through friends, or you can search online. I need to find some help moving my mother from her fourbedroom home — where she’s lived for nearly 50 years — to an apartment near me. – Overwhelmed Daughter Dear Overwhelmed, The process of downsizing and moving to a new home is a big job for anyone, but it can be especially overwhelming for seniors who are moving from a longtime residence filled with decades’ worth of stuff and a lifetime of memories. Fortunately, there’s a specialized service available today that can help make your mom’s move a lot easier for her and for you. Senior Move Manager To help your mom get packed up and moved into her new home, you should consider hiring a “senior move manager.” These are trained organizers (they are not moving companies) who assist older people with the challenges of relocating and can minimize the stress of this major transition by doing most of the work for you. A senior move manager can help your mom pare down her belongings, decide what to take and what to dispose of, recommend charities for donations, and help sell her unwanted items. They can even create a customized floor plan of her new home so your mom can visualize where her belongings will fit. Senior move managers can also get estimates from moving companies, oversee the movers, arrange the move date, supervise the packing and unpacking and help set up her new home, have the house cleaned, and just about anything you need related to her move. If you want to do some of the work yourself, you can pick and choose only the services you want. For example, you may only want a move manager’s help with downsizing and selling excess furniture and unwanted belongings but plan 32

on doing the actual packing and moving yourself. The cost of working with a senior move manager will vary depending on where you live, the services you want, and size of the move, but you can expect to pay somewhere between $60 and $125 per hour or more, not including the cost of movers. How to Find One To locate a senior move manager in your area, visit the National Association of Senior Move Managers website at nasmm.org or call (877) 606-2766. The NASMM is a trade association with an accreditation program that requires its members to abide by a strict code of ethics that ensures integrity. They currently have around 1,000 members across the U.S. But, before you hire one, be sure you ask for references from previous clients and check them, and check with the Better Business Bureau too. Also find out how many moves they have actually managed and get a written list of services and fees. And make sure they’re insured and bonded. If you can’t find a senior move manager in your area, another option is to hire a certified professional organizer who specializes in downsizing and relocating.
