“Success has many parents.” That’s a saying with which many of us are familiar.
This March I had the chance to meet with many OREF donors at the AAOS Annual Meeting. It was inspiring to hear from members of the orthopaedic community, who, like you, embrace giving back to the profession. OREF staff photographed as many donors as possible at the meeting. As I look back on my past year as OREF’s board president and reflect upon our challenges and triumphs, it’s essential to recognize the efforts of so many “parents” like you who make OREF’s successes possible. Last year, OREF provided $6.9 million in support of research and education. Since its founding by orthopaedic surgeons nearly 60 years ago, OREF has cumulatively provided $139 million for more than 4,600 grants and awards.
That great success is a result of you! The theme for this year’s annual report is “You complete the picture,” which has a particular resonance for me as a watercolorist. When I think of how OREF serves the orthopaedic community and what it accomplishes—how it supports research that transforms orthopaedics and improves patient care—I know that the foundation for all that important work comes from you, our donors. This year’s annual report is our first foray into digital publications. By using a hybrid of printed and digital materials, we can distribute the report to four times as many people without increasing costs. 2012 I FINANCIALS OREF’s 2012 financial results reflect the continuing effects of post-recession, post-governmental oversights, as well as economic pressure on the orthopaedic device industry. Contributions from individual donors and industry were affected by competitive fundraising efforts in the orthopaedic industry as well as the economic forecasts anticipated by industry.
Investment earnings were improved over prior years and strong relative to the general investment markets. Fundraising and general and administrative expenses were lower than prior years as a result of focused cost controls. The financial statement below reflects accrual accounting with grants and awards made in 2012 funded by 2011 contributions.

On what drew him to orthopaedics: I had a compartment syndrome in my leg from a football injury that took me about nine months to recover from through rehab. During recovery I had a long acquaintance with an orthopaedic surgeon and physical therapy. On why orthopaedic research is important: Research is the only way we’re going to figure out what works and learn how to improve patient care. On being a past peer review committee member: It was a lot of work, but I met a lot of people, learned something about research, and helped out some young people. I also learned a lot about reviewing grants, which gave me an idea of what to do when I began reviewing many larger grants for the NIH. On being a faculty member for New Investigator Workshops: The workshops were great because I met people who had common interests in research, and I was able to exchange ideas with them. I also met some nice young people I wound up mentoring over time. On being a mentor: Mentoring is critical. Very few people are successful on their own. Most everyone has mentors who have helped them succeed along the way. On being a grant recipient: I’m tremendously and forever indebted to OREF for the ability to get a start. I wouldn’t have nine years of NIH funding without the ability to get the career development and the grant awards through OREF first. I think that’s why OREF needs to continue to focus on funding research. Funding clinician scientists, funding independent research—funding those aspects— is going to create the individuals who make things happen and continue to make our field grow.

On what drew him to orthopaedics: My commitment to orthopaedics began serendipitously in a pub as I undertook my doctoral studies at Oxford University under the suasion of a large pint and the enthusiasm of Mr. John Goodfellow and Dr. Peter Bullough. They were certain I could solve a problem relating to the weightbearing surfaces of the human hip and their relation to observed patterns of degenerative changes noted on femoral head articular cartilage. It began a career journey, which I still ply with vigor for the enhancement of medical devices and the ongoing contemporary education of orthopaedic surgeons. At the end of the day, I received Oxford’s first combined D.Phil. in Orthopaedic & Engineering Sciences, which was subsequently recognized through the Kappa Delta. On how he first learned about OREF: Returning to the United States, I sought government funding to continue my studies and involve other synovial joint articulations. Total hip arthroplasty was still in its infancy and much was yet to be learned about the quality of trabecular bone and the acrylic bone cement used to hold hip implants in place over time. OREF provided seed money to investigate these fixation interactions and evolving biological alternatives. On why supporting OREF is important: Giving back to a discipline that has helped millions of people find pain relief and return to function by virtue of contemporary joint arthroplasty is an obligation all in orthopaedics should have. OREF has played a pivotal role in supporting young investigators, filling the breach at a time when receiving one’s next NIH R01 grant is so precarious.

On what drew her to orthopaedics: My interest in orthopaedics mainly stems from a curiosity in joint biomechanics, amazement for the mechanical properties of bone tissue and how human physiology plays a role in altering their function. I find orthopaedic medicine appealing because of its likeness to carpentry and basic mechanics but with the complexity of an organic living tissue that has the ability to remodel itself based on external loads and physiology. On how she first learned about OREF: We have been a long-time sponsor for OREF and heard about OREF through its work at conferences in support of research and education. We also knew of the collaborative studies between surgeons, medical device manufacturers and researchers that OREF makes possible through its fundraising efforts. On why Sawbones Worldwide/Pacific Research Labs supports OREF: We support OREF because the work it does to supports medical advancements and our mission is to service and support medical education and research through the hands-on anatomical models that we manufacture. On the importance of orthopaedic research: Orthopaedic research is important to human movement and quality of life.

On what drew him to orthopaedics: I was always fascinated by the human aspects of science. The idea that technology could help a bone heal or that a damaged part of the body might be replaced really caught my imagination. Biomedical engineering was just starting as a defined discipline when I was in school, ultimately leading me to medicine and orthopaedic surgery. On how he first learned about OREF: I was aware of OREF as a resident. I remember Jim Urbaniak and Leonard Goldner making a point of its importance for our specialty. On why he supports OREF: I rely on OREF to support young investigators and significant contributors to the science that drives our work. OREF is in a unique position to encourage investigation and career development for orthopaedic surgeons. OREF supports the work that sustains and advances our specialty. On the importance of research: Research is the single most important long-term strategy for growth and development of a field, orthopaedic surgery included. It is through research and innovation that we remain important and relevant to society. On why supporting OREF is important: Support and commitment to the future of our specialty starts at home. We want to be part of the vision and the success of orthopaedic surgery.

On what drew him to orthopaedics: My introduction to orthopaedics was a joint replacement, a failed outcome. I was a medical student. It was an infected joint but I was interested in the structural anatomy of the knee and how they could put this device in. It was fascinating, and it looked like fun. I became more involved with that patient and actually saw the patient recover function and walk following surgery. It’s really fulfilling to see that happen. On why he supports OREF: To go forward in any arena you have to give and be a part of the process. OREF has certainly been an integral part of orthopaedics, in the development of our craft and our ability to care for patients. It’s incumbent upon all of us to give, to make sure that this type of support can be maintained and our field can go on, doing good for our patients and allowing us to practice as we do. On how he encourages colleagues to support research: What I have said to my colleagues who are actively involved in research is that they have reaped the benefits of organizations such as OREF, and it is incumbent upon us to give back so that people in generations going forward have the resources necessary to answer the questions of the future. Many treatments come from the efforts of someone who thought of something new and took it forward through scientific testing. That process is what OREF is about. So I remind colleagues in non-academic institutions that it’s incumbent upon them to give as well. It doesn’t matter how much; many small efforts of giving collectively make a big sum.

On what drew him to orthopaedics: It was serendipity more than analysis or cogitation that led me to a career in orthopaedics. The 1970s, when I graduated from college, were not a particularly good time for graduates, but I persevered and gained an interview with a small company in Santa Monica— Richards Medical—which was later acquired by Smith & Nephew. Total joint replacement was an emerging industry and I was entranced by the idea of manufacturing hips and knees. After my interview, I was so enamored of the idea that I asked to tour the factory. I later learned that the request set me apart from the other candidates and thus launched my career in orthopaedics. On why Kinamed supports OREF: Our inclination is for new and emerging technologies. We want to find products that are technically distinct and advanced. While we develop those products ourselves, clinical validation is critically important to us and is just one way OREF has impacted our industry. OREF research improves the field and it moves the art forward. Both these things are beneficial to industry, my company and ultimately patients. On the importance of orthopaedic research: On a basic level, working in orthopaedics you see your handiwork impact people in a profound and gratifying way. Without research, our industry would be impoverished and would not be able to serve our customers well.

On what drew her to orthopaedics: My undergraduate degree was in physical education and athletic training. In this program, I had an opportunity to work with athletes to prevent injuries as well as work with these students as they rehabilitated after a variety of treatments, including surgery. I used to say I helped people not get hurt, and if they did get hurt, I’d pack them up and ship them out and be there for them when they returned. I realized I spent a lot of time wondering what happened after I shipped them out. I became interested in the orthopaedic care that was helping these athletes. One other important influence was Saul Trevino, MD, a mentor to me in orthopaedics who shaped my interests in surgery and specifically foot and ankle care. On why she supports OREF: I’m a research nut. After undergrad I went to graduate school for a couple years at Middlebury College to prepare myself for medical school. It was at Middlebury where I was bit by the research bug. I realized there were so many unanswered questions and we needed to support efforts to answer those questions. We need to prioritize giving to OREF. It’s putting your money where your mouth is. On how she encourages colleagues to support research: Research is a critical element in our care, and we have such creative people in orthopaedics thinking of fantastic questions and methodology to answer those questions. We need to support those people. On what she finds most challenging about orthopaedics: There are still many unanswered questions. Questions that affect patients’ lives—mobility, longevity and quality of life. Patients want it all and I want to give it to them. I want to maximize their outcomes. There is a sense of urgency to get these answers.

Below is just a sampling of the work undertaken by grant recipients, both current and past. Danielle benoit, PhD, is investigating allograft failure and hopes to devise simple, biologically active coatings that can be applied to allografts to enhance healing and integration. She just received an R01 grant from the National Institutes of Health (NIH) for her research. Quanjun cui, mD, explored how to create better synthetic grafts to treat nonhealing fractures and other bone defects. Dr. Cui also has two NIH R21 awards. chitra Dahia, PhD, is investigating signaling pathways that could lead to disk regeneration, which will help develop treatments that will better preserve flexibility and alleviate pain for patients suffering from spinal disorders. matthew Dobbs, mD, localized a gene responsible for idiopathic clubfoot, which will result in the development of better treatment strategies. Young-Jo Kim, mD, PhD, researched how to pinpoint when a pelvic osteotomy patient has the greatest potential for cartilage repair and joint recovery. alfred Kuo, mD, PhD, investigated tissue regeneration that will improve current surgical procedures to treat cartilage injury. Jonathan Schoenecker, mD, PhD, is studying what role the coagulation system plays in bone development, degeneration and repair to learn if drug therapies used for clotting problems can improve bone health and healing. Jennifer Wolf, mD, investigated treatments that may prevent carpometacarpal arthritis, especially in women.

On why he became a clinician scientist: I had an interest in research as a medical student. I remember asking probing questions about whether the things we did were based on science. As soon as I finished medical school and during residency I continued to engage in research. I found it gratifying trying to find an answer for questions that we had in clinical practice. On how he ďŹ rst learned about OREF: I learned of OREF before I graduated from residency. It was the first year that OREF had a meeting for residents. I attended and found it stimulating. On the importance his first OREF grant: The OREF Clinician Scientist grant was elemental in allowing me to move into the clinician scientist track; in setting up a research infrastructure from the beginning. That has allowed me to exploit numerous avenues in clinical practice. Without that grant, all my dreams and intentions of becoming a clinician scientist probably would have evaporated. On the importance of his current OREF grant: The OREF Prospective Clinical grant is generous and it will help us answer a clinical question [on periprosthetic infection] much faster than going through other tracks. The grant will allow us to generate the pilot data that will then hopefully serve as the entry into an R01 or other types of NIH grants. On the importance of OREF funding: The major difference between OREF and other funding bodies is that OREF funds research that has real translational elements. You will see the implication of research findings for a particular grant and how those findings will impact clinical practice. I can think of numerous grants that OREF has funded that have all led to a change in our clinical practice. Without this type of activity and support from OREF, our profession is not going to be able to advance.
