Additional Medical Plan Features Included in all medical insurance plan options are pharmacy, vision, mental health and substance abuse benefits. • The prescription plan offers 3-tiered pricing at the pharmacy for generic, name-brand and non-preferred drugs. Mail order prescriptions for maintenance drugs are also available through Prime Therapeutics.

• The vision benefits include annual eye exams at participating doctors’ offices and $50 reimbursement available for contacts/glasses every two years.

• Mental health and substance abuse benefits are available in the plan options and are coordinated through Magellan Behavioral Health.

Flexible Spending Account and Dependent Care What is a flexible spending account? Flexible spending accounts (FSAs) are a convenient, pre-tax way to pay for eligible out of pocket health care expenses as well as dependent care expenses. Pre-tax payroll deductions are deposited into your account and you are then reimbursed for eligible expenses using these pre-tax dollars from your account.

How does the flexible spending account work? Oticon offers an option for healthcare FSA as well as dependent care. You have the option to elect an annual amount for either healthcare, dependent care or both according to the chart on the following page. The amount you elect is deducted from your paychecks in equal amounts throughout the year. You save your receipts for services rendered and submit these receipts to Horizon for reimbursement. Funds elected remain in effect for the plan year and cannot be revoked or changed. Funds not used during the plan year are forfeited, but you may continue to file claims for expenses incurred during the plan year until three months following the end of the plan year.

• $100 - $5000 per year • $2500 maximum for married person filing taxes separately • The total amount cannot exceed the lesser of $5000 ($2500 for a married person filing separately) or spouse’s earned income. • If spouse does not work and is not disabled or a full-time student, employee cannot participate in Dependent Care.

Life Insurance Plan Options Group Term Life Insurance Life and accidental death and dismemberment (AD&D) insurance is provided through Lincoln Financial at no cost to the employee for a coverage amount of one times the salary for non-exempt (hourly) employees and two times the salary for exempt (salaried) employees.

Supplemental Life Insurance Through Boston Mutual, supplemental coverage is available for you, your spouse, children, step-children and grandchildren if they qualify. This supplemental life insurance has affordable premiums, convenience of payroll deductions and portability.

Business Travel Accident Insurance Oticon automatically provides you with company-paid business travel accident insurance that pays benefits to you (if you sustain certain injuries) or your beneficiary (if you die) as a result of an accident while on a business trip. The benefit maximum per covered accident is $2,500,000 Aggregate Policy Limit.

Workers Compensation All employees are covered by Workers Compensation Insurance from the first day of employment. Coverage under Workers Compensation is governed by statute.

Disability Programs Short Term Disability • Private Plan with National Benefit Life or Lincoln Financial • Coverage begins on 8th consecutive day out of work and provides 66.67% of pay up to a weekly maximum and for a maximum benefit period of 26 weeks

Long Term Disability • Provided at no cost to employees through Lincoln Financial • Begins after STD benefits are exhausted, replaces 66.67% of base salary up to maximum of $10,000/month minus all other disability payments to which employee may be entitled (ie: Social Security)

401(k) Retirement Savings Plan Employees of Oticon are eligible to participate in the 401(k) retirement savings plan with Wells Fargo after meeting eligibility requirements of 21 years of age and 250 hours and 3 months of service. The plan features automatic enrollment, optional automatic escalation of contributions, pre-tax and Roth contributions and matching contributions from the company. You can contact Wells Fargo directly to manage your retirement savings and access a full range of resources including:
