TIME ALLOTMENT FOR A NEW FITTING Review the Paragon CRT® Contact Lens Certification & Resource Guide. This guide provides you with basic fitting instructions and fluorescein pattern guidelines. You may purchase the Technical Kit or Practice Management Kit, described in this section, for an additional fee. You may also choose to upgrade to the Paragon CRT® Contact Lens Diagnostic Dispensing System. We recommend allowing extra time to fit your first few Paragon CRT® Contact Lens patients. Many practitioners will designate a half-day to fit new Paragon CRT® Contact Lens patients. Because patients will require a check-up the morning following their dispensing visit, Friday fittings are appropriate if there are Saturday morning office hours; or instruct the patient not to wear the lenses until Sunday evening for a Monday morning appointment. Allow 15-30 minutes for each follow-up exam. As always, the time allotted for fitting a new patient is dependent on the level of delegation of routine tasks to your staff, i.e. visual acuity testing, corneal topography, over-refraction, history, etc. We recommend 45 minutes per initial fitting. Some key elements of a successful seminar are as follows:

Communication is key to any relationship. Routinely sending communication to your patient base helps retain patient loyalty, reminds patients it’s time for their exam and promotes special Paragon CRT® Contact Lens offerings or announcement of new products throughout the year.

Ortho-k with Paragon CRT® Contact Lenses must be worn on a regular basis during sleep in order to reduce the need for glasses or contact lenses during the day. Complications and Side Effects Ortho-k with Paragon CRT® Contact Lenses carries the same risks as other types of contact lenses, such as swelling of the cornea, scratching of the eye, irritation, infection, unusual eye discharge, excessive tearing, dry eyes, sensitivity to light, pain, redness, and distorted vision. These risks are usually temporary if the contact lenses are removed promptly and if appropriate professional care is received. In some instances permanent corneal scarring, infection, or blood vessel growth on the cornea may occur, which can lead to reduced sight in rare cases. The blurry vision and how long it lasts each day should decrease with time. You may also experience distortions or ghost images, particularly outside at night which may affect night driving. How will our practice fit and dispense Paragon CRT® Contact Lenses?

Paragon CRT® Contact Lenses are a specially designed oxygen permeable therapeutic contact lens used in orthokeratology (ortho-k). Ortho-k is a sophisticated nonsurgical process that temporarily reshapes the cornea while you sleep. You remove the lenses when you wake and should be able to go throughout the day without any other vision correction.

The eye care professional who is certified to fit Paragon CRT® Contact Lenses chose one of the options below. It is important to know which method your practice will utilize. • Paragon CRT® Contact Lens Diagnostic Dispensing System (100 Lens): Paragon CRT® Contact Lenses can be prescribed through a Diagnostic Dispensing System (DDS) utilizing routine clinical data: patient refraction, corneal measurement and fluorescein pattern evaluation. The Paragon CRT® Contact Lens DDS includes 100 or 136 lenses, with a range of Base Curves, Return Zone Depths and Landing Zone Angles. How long does it take to reach good vision? Most patients show accelerated improvement in the first few days of treatment and achieve their optimum vision in 10 to 14 days.

Each Paragon CRT® Contact Lens is laser-marked with a series of very small numbers and/or letters. If the Paragon CRT® Contact Lens does not have a laser mark of small numbers and/or letters, you do not have an authentic Paragon CRT® Contact Lens and you should consult with your eye care practitioner or practice administrator immediately.

What should a patient expect during the 10 to 14-day treatment process? It’s important to understand that for a short time after a patient has begun treatment, but before sufficient treatment is realized, old glasses or contacts will no longer be the appropriate prescription. Paragon CRT® Contact Lenses were the first therapeutic lens design approved by the FDA for overnight contact lens orthokeratology for the temporary reduction of myopia. Paragon Vision Sciences, the manufacturer of Paragon CRT® Contact Lenses, received approval on June 13, 2002. The issuance of this approval is the culmination of an extensive clinical study to establish the safety and efficacy of orthokeratology in overnight use.

FREQUENTLY ASKED QUESTIONS BY CONSUMERS Are there any risks involved with wearing Paragon CRT® Contact Lenses?

Are Paragon CRT® Contact Lenses difficult to apply or remove?

There is a small risk involved when any contact lens is worn. It is not expected that the Paragon CRT® Contact Lenses for orthokeratology will provide a risk that is greater than other contact lenses that are worn during sleep. There were no serious adverse events reported in the Paragon CRT® Contact Lens FDA clinical study. (See the package insert online at paragonvision.com)

No. The lenses are generally no more difficult to insert or remove than any other similar contact lens modality. You may want to instill comfort or rewetting drops prior to insertion of the lenses and immediately upon waking. The lenses must move freely prior to removal. If you have difficulty removing the Paragon CRT® Contact Lens, there are lens removal aids available to assist this process.

What is the difference between orthokeratology (ortho-k) and Paragon CRT® Contact Lenses?

How often will I have to replace my Paragon CRT® Contact Lenses?

Ortho-k is the process of using a therapeutic contact lens to reduce myopia by reshaping the corneal surface while you sleep. Paragon CRT® Contact Lenses are the product used for this therapy.

Generally, Paragon CRT® Contact Lenses will have to be replaced annually. However, depending on factors such as protein buildup, how well the lenses are taken care of, etc., the lenses may have to be replaced more or less frequently.

Are there age restrictions for ortho-k with Paragon CRT® Contact Lenses?

If I discontinue Paragon CRT® Contact Lenses, will my vision return to normal?

No. The FDA placed no age restrictions on candidates for ortho-k with Paragon CRT® Contact Lenses. Ortho-k is being successfully performed on children of all ages. It can be a great option for children and teens that are active in sports activities or any other extra curricular activities.

Yes. There are no recorded permanent corneal changes post-treatment. Generally, the more myopia being reduced, the longer it will take for the corneal curvature to return to normal. For instance, if your pretreated correction was -4.50D, it would take a few days longer to return to normal than if the pretreated correction was -1.50D.

Those who are older and presbyopic (requiring reading glasses or bifocals) may still be candidates for Paragon CRT® Contact Lenses. They may correct their distance vision with Paragon CRT® Contact Lenses and wear near vision glasses for close work. At the doctor’s discretion you may be fitted in a monovision mode (one eye corrected for near vision and one corrected for distance vision).

What happens if I lose or damage a lens? Ortho-k is different than wearing regular contact lenses. If you discontinue wear for one night, your vision may be impaired the next day. Previously worn glasses or contact lenses may not help. Immediate lens replacement is necessary! Having a spare pair of lenses is strongly recommended.

Visual requirements due to age or activities can only be made after a thorough eye exam and recommendations of a Paragon CRT® Contact Lens certified eye care professional.

FREQUENTLY ASKED QUESTIONS ABOUT PARAGON CRT® CONTACT LENSES BY CONSUMERS Is orthokeratology (ortho-k) painful?

I used to wear contact lenses, but stopped due to discomfort. Am I a candidate?

No. The therapy is not painful. Initially, you may have a slight awareness of the therapeutic lens. You should not feel the lenses when you sleep and there is no sense of physical corneal change…just visual improvement when the lenses are removed.

Paragon CRT® Contact Lenses are worn at night while you are sleeping. This removes any discomfort normally experienced while wearing contact lenses during the day.

I have astigmatism. Am I a candidate for Paragon CRT® Contact Lenses?

Will my insurance plan cover ortho-k with Paragon CRT® Contact Lenses?

Paragon CRT® Contact Lenses are designed to correct myopia with up to 1.75D of astigmatism. Each individual should be evaluated by a certified eye care professional to determine if Paragon CRT® Contact Lenses are right for your visual requirements.

This is an elective procedure and, to the best of our knowledge, is not usually covered by benefit programs. If regular contact lenses are covered under your vision care plan, ortho-k may be partially covered. You should check with your benefits plan administrator.

My driver’s license says I need corrective lenses to operate a motor vehicle. What should I do?

I suffer from “dry eyes.” Am I a candidate for Paragon CRT® Contact Lenses?

You will be supplied with a document that states you are participating in a visual therapy program that eliminates the need for corrective lenses while operating a motor vehicle. The document should have an expiration date for this treatment and you should have your driver’s license updated with a new vision screening through your local DMV office to reflect this change before the expiration date.

Only your eye care professional can determine if you are a Paragon CRT® Contact Lens candidate. Many patients with slightly dry eyes do well with Paragon CRT® Contact Lenses. Since you are wearing the lenses only while sleeping, the closed eye minimizes evaporative loss of tears and may give dry-eye patients suitable vision without compromising the ocular surface.
