WELCOME TO THE WORLD OF ORTHOKERATOLOGY! It’s an exciting time to get involved in one of the fastest growing modalities in the global contact lens market.

The processes and programs defined in this manual have been implemented, tested, and can be effective in practices of varying sizes and in various stages of growth around the world. Reviewing this information will not only assist you in the timely integration of Paragon CRT® Contact Lenses, but can increase patient satisfaction and enhance the overall growth of your practice.

Orthokeratology has entered the mainstream of contact lens fitting, and thousands of practices around the world have begun capturing new market share, while enjoying the incremental revenue that Paragon CRT® Contact Lenses can provide. With mainstream practices focusing on increased profit-per-patient, Paragon CRT® Contact Lenses can provide a seamless way to achieve higher profit margins while minimizing chair time. There are two key areas where Paragon CRT® Contact Lenses are making a strong impact: The first market where Paragon CRT® Contact Lenses has performed strongly is the tweens and teens, referred to as the “8 to 18” market. High levels of myopia double the risk of serious ocular health problems such as retinal detachment, cataract and glaucoma, which can lead to vision loss and blindness.1 Orthokeratology with Paragon CRT® Contact Lenses can provide adolescent patients with a treatment option for addressing the unprecedented rise of myopia. The second market is the current soft contact lens wearers that are experiencing discomfort, manifested as feelings of dryness or scratchiness. Myopia can also occur in adults (ages 20 to 40) with no prior history of problems in childhood, termed early adult-onset myopia.

THE SURVEY SAYS! Paragon Vision Sciences conducted a survey regarding chair time and the use of staff members when fitting Paragon CRT® Contact Lenses. Of the surveyed Certified Practitioners:

93%

Paragon CRT® Contact Lens practitioners take between 15-30 minutes for each follow up appointment.

78%

Paragon CRT® Contact Lens practitioners report using a technician to assist with fitting.

70%

Paragon CRT® Contact Lens practitioners require 30-60 minutes for the initial fitting visit.

TIME ALLOTMENT FOR A NEW FITTING Review the Paragon CRT® Contact Lens Certification & Resource Guide. This guide provides you with basic fitting instructions and fluorescein pattern guidelines. You may purchase the Technical Kit or Practice Management Kit, described in this section, for an additional fee. You may also choose to upgrade to the Paragon CRT® Contact Lens Diagnostic Dispensing System. We recommend allowing extra time to fit your first few Paragon CRT® Contact Lens patients. Many practitioners will designate a half-day to fit new Paragon CRT® Contact Lens patients. Because patients will require a check-up the morning following their dispensing visit, Friday fittings are appropriate if there are Saturday morning office hours; or instruct the patient not to wear the lenses until Sunday evening for a Monday morning appointment. Allow 15-30 minutes for each follow-up exam. As always, the time allotted for fitting a new patient is dependent on the level of delegation of routine tasks to your staff, i.e. visual acuity testing, corneal topography, over-refraction, history, etc. We recommend 45 minutes per initial fitting. Paragon CRT Dual Axis® Contact Lenses can be used to successfully fit a patient when lens centration is an issue with Paragon CRT® Contact Lenses. Paragon CRT Dual Axis® Contact Lenses allow for modulation of the lens periphery in two meridians when corneal elevation or curvature differences limit the success of a fit. It features auto-orientation, with no prism or slab off, while maintaining flexure resistance and an overall thin lens design. The enhanced design of Paragon CRT Dual Axis® Contact Lenses maintains an optimum treatment zone and provides a lens perimeter that remains round and planar. The product’s ability to permit independent manipulation of a second RZD and LZA, that doesn’t alter the dimension found optimum in a first meridian, expands Paragon CRT® Contact Lens technological superiority. For more information on Paragon CRT Dual Axis® Contact Lenses, contact a Paragon Vision Sciences consultant for assistance at 800-528-8279 x 2. 5

While we are not telling you what to charge, as that is your decision to make, we would to like share what your Paragon CRT® Contact Lens colleagues reported in a 2016 survey. ELEMENTS TO CONSIDER WHEN ESTABLISHING YOUR GLOBAL SERVICE FEES • If you are using a corneal topographer, the use of this technology should be slightly reflected in your fees. • Remember to describe Paragon CRT® Contact Lenses as a “therapy” when presenting the option to your patient. You are providing a therapy, not just a contact lens. • Paragon CRT® Contact Lenses provide visual freedom, without the hassles of traditional daytime contacts or glasses. Visual freedom is invaluable and priceless. • Do you want more volume at a lower fee, or less volume at a higher fee? • What will be your global service fee after the first year? To avoid surprises, it is recommended that you advise all patients on what to expect in annual fees. • We recommend establishing a cancellation policy in the event a patient decides that Paragon CRT® Contact Lenses are not for them. Your cancellation policy may include a partial credit towards the purchase of contact lenses, solutions or optical shop purchases. • Offer CareCredit® as an option for patients who cannot afford to pay for Paragon CRT® Contact Lenses up front. CareCredit® offers interest-free financing plans. (800) 340-3047 ext 4133.

USE YOUR STAFF TO IDENTIFY PARAGON CRT® CONTACT LENS CANDIDATES A well-trained staff is the key to having a successful practice. Your staff comes into contact with your patients before you see them in the chair and, in your patient’s eyes, your staff is an extension of the services they expect from you. Use your staff to help you identify potential Paragon CRT® Contact Lens candidates during the work-up process and reward them when you develop a new patient in the practice. Rewards can be as simple as Starbucks® gift cards or a nominal bonus in their paycheck. Without the support of your staff, integrating Paragon CRT® Contact Lenses into your practice may be a challenge. Instruct your staff on what a good Paragon CRT® Contact Lens candidate looks like: • FDA approved up to -6.00D, with or without 1.75D astigmatism, and unlike LASIK, there is no age restriction • The patient is myopic, falls within the FDA guidelines and is concerned about their nearsightedness getting worse • Is between the ages of 8-to-18 • Has an active or athletic lifestyle and currently wears spectacles or daytime soft contact lenses that interfere with their daily activities • Adults who suffer from uncomfortable daytime soft contact lens wear that may cause dryness symptoms • Adults who are apprehensive about having eye surgery or afraid of a bad surgical result, the reversibility of Paragon CRT® Contact Lenses is attractive • Is not a candidate for laser surgery, due to age (too young) or contraindicated • Has discretionary income and prefers quality and convenience CareCredit® Gives Your Patients a Payment Option for Paragon CRT® Contact Lenses Often it’s not the total cost of recommended refractive or other ophthalmic procedures that causes patients to delay or decline care – It is fitting payments into their monthly budget. That’s why more than 20 million patients have used the CareCredit® healthcare credit card to get the care they need and want. For over 25 years CareCredit® has been serving the healthcare community, earning the exclusive recommendation of refractive and ophthalmology practices. CareCredit® can be used in your practice for your patient’s out-of-pocket healthcare expense. CareCredit® picks up where insurance leaves off. Your patients can use it for co-payments and deductibles. CareCredit® makes it easy for patients to say “yes”! THE ROLE OF CORNEAL TOPOGRAPHY Paragon Vision Sciences strongly recommends the use of corneal topography in the management of Corneal Refractive Therapy. Through Paragon Vision Sciences’ partnership programs with Medmont and OCULUS®, investing in Paragon CRT® Contact Lenses and a corneal topographer has never been easier – or more financially attractive. Visit the Practice Management Section of our website for full details. A corneal topographer is an important tool in pre-fit evaluation, baseline documentation and ongoing management of Paragon CRT® Contact Lens patients. Corneal topography also provides accurate treatment assessment during followup and improves chair time efficiency and practice profitability. All contact lens wear, even soft spherical contact lens wear, has an effect on the epithelial layer and its topography. If the pre-treatment topography shows a generally symmetrical presentation with a centrally located corneal apex, practitioners should proceed with the fitting and dispensing of Paragon CRT® Contact Lenses. If topography is unavailable, or if topography indicates contact lens wear has substantially redistributed the epithelium from pre-contact lens baseline, then it may be prudent to discontinue contact lens wear 1-to-2 weeks before Paragon CRT® Contact Lens fitting and allow the cornea to return to baseline curvatures. Paragon Vision Sciences has partnered with two leading corneal topography companies to provide options if you’re considering investing in new equipment. Our partnership programs with Medmont and OCULUS® include the option to bundle Paragon CRT® Contact Lens Diagnostic Dispensing System (100 Lens) to your order. See below for more details.

MARKETING PARAGON CRT® CONTACT LENSES IN YOUR PRACTICE Implementing a Paragon CRT® Contact Lens marketing campaign doesn’t have to cost a fortune, involve an outside marketing firm or consume your valuable time. Paragon strongly recommends taking action to alert your existing patients about the new technology offered in your practice. Here are some quick, easy and inexpensive ways to begin your Paragon CRT® Contact Lens marketing campaign.

Start with Consumer Brochures

LIKE US ON FACEBOOK

After a decade of post FDA clearance, the “teen and tween” markets have evolved as one of the leading growth segments for Paragon CRT® Contact Lenses. Children and adolescents between the ages of “8-to-18” are great candidates because: • There is no age restriction • They are too young for LASIK • They have active, sport-related lifestyles • Glasses or daytime contacts may interfere with some activities

Follow us on Facebook and share status updates on your practice Facebook page.

Paragon Vision Sciences can provide your office with educational consumer brochures to explain what orthokeratology with Paragon CRT® Contact Lenses is, and how it can improve a patient’s quality of life.

It may sound “too simple,” but by placing the Paragon CRT® Contact Lens consumer brochures at your check-in counter and having your receptionist offer the patient a brochure, which explains “the technology offered in the practice,” you’re marketing Paragon CRT® Contact Lenses to someone who you’ve already won over. The patient is in your office and ready to see the doctor they trust!

PATIENT TESTIMONIALS

PATIENT ADVOCATE PROGRAM

Patient testimonials are a powerful marketing tool that can help other patients become comfortable with the treatment option by learning about the experience and success of others. We recommend you receive permission from patients to use their testimonials and we have included a sample form (page 16) to help you gather this information. The following are some suggestions on how to implement patient testimonials:

Many Paragon CRT® Contact Lens patients enjoy telling their personal success stories to others. Capitalize on these patients by asking them to participate in a Patient Advocate Program. These patients can be used as a resource for patient testimonials at educational and product information seminars and answer questions via email or telephone or write articles for your patient newsletter.

Testimonial Book

Adolescent patient advocates and their parents can be especially helpful to practices by addressing the concerns of other children and their parents.

Fill the pages of a binder with Paragon CRT® Contact Lens patient stories, thank you cards and other practice memorabilia and place in your reception area to generate patient interest.

PATIENT REFERRAL PROGRAM

Practice Website

Most Paragon CRT® Contact Lens patients will refer an average of four new patients. To support this opportunity, we suggest developing a “Patient Referral Program.”

Place Paragon CRT® Contact Lens testimonials on your website so consumers have the opportunity to read what satisfied patients have to say about Paragon CRT® Contact Lenses and your practice.

Gather Information Ask each new patient how they found out about your practice by making it a part of your new patient intake form.

Testimonial Fliers Provide your patients with a copy of your favorite Paragon CRT® Contact Lens testimonial(s) to generate discussion.

Referral Cards

Wall of Fame

A preprinted special business card designed for satisfied Paragon CRT® Contact Lens patients to give to friends and family. On the back you could offer a Paragon CRT® Contact Lens “free screening” to all referrals.

Display Paragon CRT® Contact Lens patient testimonials and/or photos on the walls of your office.

Most practices find they convert approximately 75% of free screening Paragon CRT® Contact Lens consultations. Acknowledge each Paragon CRT® Contact Lens referral with a personal “Thank You Letter” (page 16).

11

YOUR ON-HOLD MESSAGE Put a Paragon CRT® Contact Lens announcement on your on-hold message system. This will alert your patients to the innovative technology offered in your practice. Below are two suggestions for an on-hold message. With minimal modification, you can use the following scripts on your website, in a newsletter, in a blog, as a social media post, in a yellow pages ad, as part of a newspaper ad or a radio script.

Option # 1 Introducing orthokeratology with Paragon CRT® Contact Lenses. Vision correction therapy that works while you sleep. Paragon CRT® Contact Lenses are a therapeutic contact lens that gently and temporarily reshapes your cornea while you sleep. You simply remove your lenses when you wake and experience clearer vision all day. No more torn lenses, broken frames or discomfort sometimes associated with daytime contact lens wear. Whether you are playing sports or simply applying makeup, with Paragon CRT® Contact Lenses, nothing stands between you and clearer vision. Paragon CRT® Contact Lenses are FDA approved for all ages to correct myopia, also known as nearsightedness. Ask us to set up an appointment to see if orthokeratology with Paragon CRT® Contact Lenses is right for you.

Option # 2 Do you want freedom from the hassles of daytime contact lenses and glasses? (Practice name) can offer you a nonsurgical, effective option called orthokeratology with Paragon CRT® Contact Lenses. Paragon CRT® Contact Lenses are a therapeutic contact lens that gently temporarily reshapes your cornea while you sleep. EMPLOYEE RECOGNITION PROGRAM

APPOINTMENT REMINDER PHONE CALLS

An employee recognition program can be an excellent tool to expand your Paragon CRT® Contact Lens patient base. An employee recognition program can consist of:

If someone in your staff places a reminder call to the patient prior to their appointment, have the staff member end each call by saying something similar to:

Recognition Certificate

“Our practice is offering an innovative technology called orthokeratology with Paragon CRT® Contact Lenses. It’s a process that allows you to wear a special contact lens while you sleep so that you can see during the day without wearing contact lenses or glasses. Ask us about orthokeratology with Paragon CRT® Contact Lenses during your appointment.”

Acknowledge staff members for their contribution to practice growth with a certificate or thank you note for the efforts put forth.

Recognition Program Consider offering employees a program that provides rewards, such as bonuses or gifts for referrals they generate that result in a new fit.

12

PATIENT SEMINARS

ROUTINE PATIENT COMMUNICATION METHODS

Patient Seminars can be a helpful tool to grow Paragon CRT® Contact Lenses within your practice. The goal of a patient seminar is to fill a room with pre-qualified patients and educate them on how orthokeratology with Paragon CRT® Contact Lenses works and its benefits. Some key elements of a successful seminar are as follows:

Communication is key to any relationship. Routinely sending communication to your patient base helps retain patient loyalty, reminds patients it’s time for their exam and promotes special Paragon CRT® Contact Lens offerings or announcement of new products throughout the year.

• Consider setting up your waiting room theater-style. • Screen your patient charts or database and invite those who are good candidates. • Have printed fliers available to hand out, or mail fliers to candidates beginning a month prior to your seminar. • If you choose to do a mailing, send at least 1,000 invitations, which should yield 20 to 30 participants. • Confirm their attendance on the day prior to the seminar.

If your practice has an electronic or printed newsletter, it is an excellent way to inform your patient base about Paragon CRT® Contact Lenses. Repeating the message about new products is the most effective.

• Keep a record of all seminar registrants. • Have a prepared sign-in sheet with the key patient information already included. Attendees need only confirm information and sign in.

The majority of patients will read an email message from their doctor. Using email to develop and send newsletters, seminar invitations, special offers and recalls is an efficient and cost effective method of communicating with your patient base.

• Have a table to sign up and schedule patients for their Paragon CRT® Contact Lens evaluation or fitting appointment. • Consider offering a discounted price for Paragon CRT® Contact Lenses and a free evaluation.

There are several companies that offer email service for handling your marketing needs. They can develop and send a custom personalized email to your patient database saving valuable marketing dollars over traditional direct mail. Paragon Vision Sciences recommends Constant Contact – visit them at constantcontact.com.

• Seminar content should include product information, explanation of fees, fee policies and payment options. • Send a thank you letter as a follow-up to those candidates who did not sign up during the seminar and invite them to call if they have any questions.

MARKETING TIP Market Paragon CRT® Contact Lenses to local schools, boys and girls clubs, and church groups. Consider attending “Employee Benefit Fairs,” provided by local companies. Take the opportunity to meet with different organizations and present your practice and the benefits of Paragon CRT® Contact Lenses. 13

APPOINT A PARAGON CRT® CONTACT LENS STAFF CHAMPION Designate one staff member to be your Paragon CRT® Contact Lens staff champion or coordinator, placing them in charge of in-office education, marketing and Paragon CRT® Contact Lens inventory management. We suggest having a staff meeting to review the following:

FORMS, LETTERS, CONSENT DOCUMENT AND QUIZ Paragon CRT® Contact Lens Doctor Referral Letter Date Eye Care Practitioner Name Address City, State Zip Dear _________, My name is (Doctor Name) and I practice at (Practice Name) located in (City, Town). I am reaching out to inform you of my clinical expertise in an FDA approved modality, orthokeratology with Paragon CRT® Contact Lenses, which you may find useful if you need to refer patients to a local expert. FDA approved orthokeratology (ortho-k) with Paragon CRT® Contact Lenses may not be offered by your practice for a variety of reasons, however that shouldn’t mean your patients don’t have access to the modality. Since 1971, the incidence of myopia has increased 66% in the USA.1 As consumer awareness increases about the growing incidence of myopia, the FDA approved vision treatment option of ortho-k could meet your patient’s visual requirements and improve their quality of life. Ortho-k with Paragon CRT® Contact Lenses is ideal for: • Kids and teens with active lifestyles who may be concerned about their myopia, or who want freedom from the hassle or discomfort of daytime contacts. There is no age restriction for wearing Paragon CRT® Contact Lenses. • Parents concerned about their child losing or breaking their contacts or glasses while at school or play. FORMS, LETTERS, CONSENT DOCUMENT AND QUIZ Example: Paragon CRT® Contact Lens Informed Consent Document You are being fitted with gas permeable (GP) contact lenses, also known as orthokeratology (ortho-k). Ortho-k refers to the use of specially designed GP contact lenses to temporarily reshape the cornea (the clear layer on the front of the eye), allowing you to see clearly without the use of glasses or contact lenses during waking hours. Ortho-k with Paragon CRT® Contact Lenses must be worn on a regular basis during sleep in order to reduce the need for glasses or contact lenses during the day. Complications and Side Effects Ortho-k with Paragon CRT® Contact Lenses carries the same risks as other types of contact lenses, such as swelling of the cornea, scratching of the eye, irritation, infection, unusual eye discharge, excessive tearing, dry eyes, sensitivity to light, pain, redness, and distorted vision. These risks are usually temporary if the contact lenses are removed promptly and if appropriate professional care is received. In some instances permanent corneal scarring, infection, or blood vessel growth on the cornea may occur, which can lead to reduced sight in rare cases. Although uncommon, infection of the cornea can develop rapidly and lead to loss of vision. The risk of infection of the cornea has been shown to be greater among patients who wear their lenses overnight than among those who do not sleep in their lenses. Paragon CRT® Contact Lenses also has risks that are not typically associated with other types of contact lenses, such as blurry or variable vision, especially late in the day. The blurry vision and how long it lasts each day should decrease with time. You may also experience distortions or ghost images, particularly outside at night which may affect night driving. For the first few weeks, the risk may be increased in patients with a high degree of correction or large pupils. All risks are minimized if you follow the correct contact lens wearing schedules and care procedures, remove your contact lenses if problems occur, and report to your primary eye care practitioner as soon as possible. With any procedure, there may be unforeseeable risks. If you experience any of the symptoms listed above, remove your lenses immediately. If the condition continues after lens removal, you should immediately call for an appointment or consultation with your eye care practitioner who will provide the necessary treatment. I understand that if my condition requires a procedure that is not indicated by the FDA and deemed “off label,” I release all liability from (Practice Name) and the practitioner(s) and accept the treatment under my own volition. Signature: ________________________________________________________________ Lens wear Schedule: Your doctor will recommend a wearing schedule for you to follow. The wearing time necessary for Paragon CRT® Contact Lenses is typically 7 to 8 hours per night. Your doctor will also recommend a follow-up schedule to check your vision and contact lenses. It is important that you attend every visit (with your lenses) that your eye care practitioner recommends in order to maintain the health of your eyes. Alternatives to ortho-k with Paragon CRT® Contact Lenses include, among others, eyeglasses, traditional contact lenses, and refractive surgical procedures. Pregnancy: Pregnancy could adversely affect treatment results with ortho-k with Paragon CRT® Contact Lenses. If problems exist during pregnancy, you may need to temporarily discontinue ortho-k with Paragon CRT® Contact Lenses wear. STAFF TRAINING AND EDUCATION What is orthokeratology with Paragon CRT® Contact Lenses?

How will our practice fit and dispense Paragon CRT® Contact Lenses?

Paragon CRT® Contact Lenses are a specially designed oxygen permeable therapeutic contact lens used in orthokeratology (ortho-k). Ortho-k is a sophisticated nonsurgical process that temporarily reshapes the cornea while you sleep. You remove the lenses when you wake and should be able to go throughout the day without any other vision correction.

The eye care professional who is certified to fit Paragon CRT® Contact Lenses chose one of the options below. It is important to know which method your practice will utilize. • Paragon CRT® Contact Lens Diagnostic Dispensing System (100 Lens): Paragon CRT® Contact Lenses can be prescribed through a Diagnostic Dispensing System (DDS) utilizing routine clinical data: patient refraction, corneal measurement and fluorescein pattern evaluation. The Paragon CRT® Contact Lens DDS includes 100 or 136 lenses, with a range of Base Curves, Return Zone Depths and Landing Zone Angles. All parameters in the 100 and 136-Lens DDS were carefully selected, based on outcomes analysis of successful patient data. Practitioners can expect to successfully dispense and begin immediate treatment on four out of five patients directly from their diagnostic dispensing inventory.

How new are Paragon CRT® Contact Lenses? The idea of contact lens orthokeratology is not a new concept. In fact, some historians believe the original concept dates back to Ancient China. Paragon Vision Sciences has taken advancements in computerized corneal mapping, computerized manufacturing, and oxygen permeable contact lens materials and produced a therapeutic contact lens design that is like no other. This has allowed Paragon CRT® Contact Lenses to become the global benchmark in science and safety and the first to earn FDA approval (June 2002) for nighttime orthokeratology.

• Paragon CRT® SureFit® Contact Lens Fitting & Dispensing System: Paragon CRT® Contact Lenses can be prescribed using Paragon CRT® SureFit® Contact Lenses, which provides 3 lenses per eye to fit and dispense on day one. A Paragon CRT® SureFit® Contact Lens candidate should fall within the following parameter range:

How will Paragon CRT® Contact Lenses benefit our patients? Paragon CRT® Contact Lenses offer freedom from glasses and the hassle of wearing contact lenses during the day. This can enhance the lifestyle of those requiring vision correction. Active individuals can freely participate in sports without the interference of glasses or bother of contacts. Eye irritation or dryness, sometimes associated with contact lens wear due to outside dust and pollutants, is eliminated.

- Flat “K” Readings between 42.50D and 44.50D - Spectacle Rx (vertexed power) from -1.00D to -4.00D - Corneal Cylinder up to -1.00D Paragon CRT® SureFit® Contact Lenses saves chair time and increases profitability. It means fewer patient visits, delivers improved patient convenience and provides immediate fit assessment with in-office lens changes, if needed.

How will Paragon CRT® Contact Lenses benefit our practice? Paragon CRT® Contact Lenses provide an opportunity for your practice to take patient care to the next level. You can share in the fun and excitement of patients who experience visual freedom using Paragon CRT® Contact Lenses. The result will be higher patient satisfaction along with increased patient retention and practice growth.

Paragon CRT® Contact Lens Traditional Warranty Program: Paragon CRT® Contact Lenses can be ordered using “K” readings and Spectacle Rx, or by ordering the specific parameters needed. 20

STAFF TRAINING AND EDUCATION (CONT.) Why is the Paragon CRT® Contact Lens certification number important?

Contact lens and spectacle-wear patients with complaints about their current correction option: • Interference with outdoor sporting activities and hobbies • Lost and torn lenses • Irritation from allergies, dust and wind • Daily care and handling problems • Scratched lenses and broken glasses

Once certified, practitioners are issued a Paragon CRT® Contact Lens Certification number. This number is important when ordering lenses to replace dispensed lenses and patient education materials. Your certification number is also used to login to the paragonvision.com website.

Paragon CRT® Contact Lens patient selection guidelines:

Refractive surgery candidates. Paragon CRT® Contact Lenses offer a safe, non-surgical option for many patients interested in the benefits of refractive surgery to reduce their dependence on glasses and contact lenses, but do not proceed for the following reasons: • Fear of Surgery • Fear of Poor Outcome • Fear of Irreversibility • Too Young for Refractive Surgery • Cost

The Paragon CRT® Contact Lens is the first overnight wear lens to be approved for its safety and efficacy by the FDA for the treatment of myopia and with or without moderate astigmatism. The approval range is up to -6.00D of myopia, with or without up to 1.75D of astigmatism. Optimal vision is usually reached in 10 to 14 days. There are no age restrictions and the treatment is reversible. For most patients their vision returns to its original state in as little as 72 hours.

Paragon CRT® Contact Lens Fees and payment policy

Who is a good candidate for Paragon CRT® Contact Lenses?

Typically the fees involved for Paragon CRT® Contact Lenses are the initial fitting, professional care, follow-ups and the therapeutic Paragon CRT® Contact Lenses. After the first year, the annual cost of Paragon CRT® Contact Lenses is about the same as wearing contact lenses or glasses. Many eye care practitioners offer optional payment plans, similar to the plans used for LASIK.

After more than a decade of post FDA clearance, the “teen and tween” markets have evolved as one of the leading growth segments for Paragon CRT® Contact Lenses. Children and adolescents between the ages of 8 and 18 are great candidates for Paragon CRT® Contact Lenses because: • There are no age restrictions with Paragon CRT® Contact Lenses • They are too young for LASIK • They have active, sport related lifestyles • Glasses or daytime contacts may interfere with some activities

happy to help you. Are you calling today to schedule an examination for contact lenses, glasses, LASIK or orthokeratology?”

Patients may also want to consult their employer about Flexible Spending Accounts (FSA). A Flexible Spending Account (also called flex plan, reimbursement account, Flex 125, Tax Saving Plan, Medical Spending Account, a Section 125, or a Cafeteria Plan) is an employer-sponsored benefit that allows you to pay for eligible medical expenses on a pre-tax basis. Expenses such as corneal refractive therapy, laser vision correction, eyeglasses, and contact lenses may be covered under this program.

If the patient expresses an interest in LASIK and/ or orthokeratology, your staff could say: “We offer consultation for the LASIK procedure. If surgery is not for you, our practice also offers non-surgical vision correction, called orthokeratology with Paragon CRT® Contact Lenses. You simply wear specially designed lenses at night, remove them when you wake up and the result is corrected daytime vision without wearing glasses or contacts. Our doctor will be happy to discuss the latest vision correction options, including orthokeratology, with you during your exam. Is morning or afternoon better for you?”

Not all employers participate in FSA programs. Recommend the patient check with their Benefits Manager to see if their employer participates in an FSA.

Using the Paragon CRT® Contact Lens consumer brochures throughout your office

• Patient Check-In at the Reception Desk At the time of patient check-in, staff members can introduce Paragon CRT® Contact Lenses by handing the patient a brochure. The staff member could say: “Mrs. Jones, while you are waiting to see the doctor, you may wish to hear about an interesting technology we are offering.”

Paragon Vision Sciences can provide your office with educational consumer brochures to explain what orthokeratology with Paragon CRT® Contact Lenses is, and how Paragon CRT® Contact Lenses can improve a patient’s quality of life. It may sound “too simple,” but by placing the Paragon CRT® Contact Lens consumer brochures at your check-in counter and having your receptionist offer the patient a brochure which explains “the innovative technology offered in the practice,” you’re marketing Paragon CRT® Contact Lenses to someone who you’ve already won over. The patient is in your office and ready to see the doctor they trust!

• Paragon CRT® Contact Lens Patient Scheduling Protocol Planning for the efficient handling of pre-appointed Paragon CRT® Contact Lens fittings will help eliminate delays in your daily schedule. The time needed to educate and fit an orthokeratology patient is similar to that of LASIK patients. Establish all future appointments immediately after the initial consultation. • Initial Exam: 1 Hour • Follow-up Exams: 30 Minutes

Generating patients through internal patient education Plan for a proactive introduction to Paragon CRT® Contact Lenses that is incorporated into the daily flow of key areas within the practice. • Patient Scheduling for Routine Exam . The introduction to Paragon CRT® Contact Lenses begins when a new or established patient calls your office for an appointment. Train your staff to greet the caller by saying: “I’d be

FREQUENTLY ASKED QUESTIONS BY CONSUMERS How can I tell if I have an authentic Paragon CRT® Contact Lens?

How long does it take to reach good vision? Most patients show accelerated improvement in the first few days of treatment and achieve their optimum vision in 10 to 14 days.

Each Paragon CRT® Contact Lens is laser-marked with a series of very small numbers and/or letters. If the Paragon CRT® Contact Lens does not have a laser mark of small numbers and/or letters, you do not have an authentic Paragon CRT® Contact Lens and you should consult with your eye care practitioner or practice administrator immediately.

What should a patient expect during the 10 to 14-day treatment process? It’s important to understand that for a short time after a patient has begun treatment, but before sufficient treatment is realized, old glasses or contacts will no longer be the appropriate prescription. It may involve re-insertion of Paragon CRT® Contact Lenses for part of the day or the use of temporary soft lenses in different prescriptions. The doctor will discuss the options with each patient for visual correction during that temporary period of time.

Paragon CRT® Contact Lens w/Lasermark

Is orthokeratology (ortho-k) FDA-approved? Yes. Paragon CRT® Contact Lenses were the first therapeutic lens design approved by the FDA for overnight contact lens orthokeratology for the temporary reduction of myopia. Paragon Vision Sciences, the manufacturer of Paragon CRT® Contact Lenses, received approval on June 13, 2002. The issuance of this approval is the culmination of an extensive clinical study to establish the safety and efficacy of orthokeratology in overnight use.

How much do Paragon CRT® Contact Lenses cost? Our initial cost for orthokeratology with Paragon CRT® Contact Lenses is $______________. This includes: (list products and services included in the fee). We also offer optional payment plans, which we will be happy to review with you. Payments can be as low as $___________ per month. Also, for subsequent years, the cost of Paragon CRT® Contact Lenses is about the same as regular contact lenses or glasses.

Is ortho-k permanent? No. Ortho-k is temporary. If you stop wearing the lenses regularly while you sleep, your vision will return to its original state in as little as 72 hours.

Can I see with my Paragon CRT® Contact Lenses in?

Can everyone wear Paragon CRT® Contact Lenses?

Yes. One of the great features of the Paragon CRT® Contact Lens is that if you get up in the middle of the night, you will be able to see.

Paragon CRT® Contact Lenses treat nearsightedness (myopia up to –6.00D) with or without minor astigmatism (up to 1.75D). Because there are no age restrictions with Paragon CRT® Contact Lenses, it may be a great option for patients that experience discomfort while wearing daytime contact lenses. At this time, Paragon CRT® Contact Lenses is not approved for treatment of farsightedness.

How do I care for my Paragon CRT® Contact Lenses? Your lenses should be chemically disinfected after every use (not heat). Your doctor will instruct you about which care system is best for you.

FREQUENTLY ASKED QUESTIONS BY CONSUMERS Are there any risks involved with wearing Paragon CRT® Contact Lenses?

Are Paragon CRT® Contact Lenses difficult to apply or remove?

There is a small risk involved when any contact lens is worn. It is not expected that the Paragon CRT® Contact Lenses for orthokeratology will provide a risk that is greater than other contact lenses that are worn during sleep. There were no serious adverse events reported in the Paragon CRT® Contact Lens FDA clinical study. (See the package insert online at paragonvision.com)

No. The lenses are generally no more difficult to insert or remove than any other similar contact lens modality. You may want to instill comfort or rewetting drops prior to insertion of the lenses and immediately upon waking. The lenses must move freely prior to removal. If you have difficulty removing the Paragon CRT® Contact Lens, there are lens removal aids available to assist this process.

What is the difference between orthokeratology (ortho-k) and Paragon CRT® Contact Lenses?

How often will I have to replace my Paragon CRT® Contact Lenses?

Ortho-k is the process of using a therapeutic contact lens to reduce myopia by reshaping the corneal surface while you sleep. Paragon CRT® Contact Lenses are the product used for this therapy.

Generally, Paragon CRT® Contact Lenses will have to be replaced annually. However, depending on factors such as protein buildup, how well the lenses are taken care of, etc., the lenses may have to be replaced more or less frequently.

Are there age restrictions for ortho-k with Paragon CRT® Contact Lenses?

If I discontinue Paragon CRT® Contact Lenses, will my vision return to normal?

No. The FDA placed no age restrictions on candidates for ortho-k with Paragon CRT® Contact Lenses. Ortho-k is being successfully performed on children of all ages. It can be a great option for children and teens that are active in sports activities or any other extra curricular activities.

Yes. There are no recorded permanent corneal changes post-treatment. Generally, the more myopia being reduced, the longer it will take for the corneal curvature to return to normal. For instance, if your pretreated correction was -4.50D, it would take a few days longer to return to normal than if the pretreated correction was -1.50D.

Those who are older and presbyopic (requiring reading glasses or bifocals) may still be candidates for Paragon CRT® Contact Lenses. They may correct their distance vision with Paragon CRT® Contact Lenses and wear near vision glasses for close work. At the doctor’s discretion you may be fitted in a monovision mode (one eye corrected for near vision and one corrected for distance vision).

What happens if I lose or damage a lens? Ortho-k is different than wearing regular contact lenses. If you discontinue wear for one night, your vision may be impaired the next day. Previously worn glasses or contact lenses may not help. Immediate lens replacement is necessary! Having a spare pair of lenses is strongly recommended.

Visual requirements due to age or activities can only be made after a thorough eye exam and recommendations of a Paragon CRT® Contact Lens certified eye care professional.

FREQUENTLY ASKED QUESTIONS ABOUT PARAGON CRT® CONTACT LENSES BY CONSUMERS Is orthokeratology (ortho-k) painful?

I used to wear contact lenses, but stopped due to discomfort. Am I a candidate?

No. The therapy is not painful. Initially, you may have a slight awareness of the therapeutic lens. You should not feel the lenses when you sleep and there is no sense of physical corneal change…just visual improvement when the lenses are removed.

Paragon CRT® Contact Lenses are worn at night while you are sleeping. This removes any discomfort normally experienced while wearing contact lenses during the day.

I have astigmatism. Am I a candidate for Paragon CRT® Contact Lenses?

Will my insurance plan cover ortho-k with Paragon CRT® Contact Lenses?

Paragon CRT® Contact Lenses are designed to correct myopia with up to 1.75D of astigmatism. Each individual should be evaluated by a certified eye care professional to determine if Paragon CRT® Contact Lenses are right for your visual requirements.

This is an elective procedure and, to the best of our knowledge, is not usually covered by benefit programs. If regular contact lenses are covered under your vision care plan, ortho-k may be partially covered. You should check with your benefits plan administrator.

My driver’s license says I need corrective lenses to operate a motor vehicle. What should I do?

I suffer from “dry eyes.” Am I a candidate for Paragon CRT® Contact Lenses?

You will be supplied with a document that states you are participating in a visual therapy program that eliminates the need for corrective lenses while operating a motor vehicle. The document should have an expiration date for this treatment and you should have your driver’s license updated with a new vision screening through your local DMV office to reflect this change before the expiration date.

Only your eye care professional can determine if you are a Paragon CRT® Contact Lens candidate. Many patients with slightly dry eyes do well with Paragon CRT® Contact Lenses. Since you are wearing the lenses only while sleeping, the closed eye minimizes evaporative loss of tears and may give dry-eye patients suitable vision without compromising the ocular surface.
