Admission

ELECTRICAL APPLIANCES

When you were admitted, you were asked to provide certain information, such as address, Social Security number, phone number, date of birth, next of kin, employment and insurance coverage. A copy of your insurance card(s) was scanned to your electronic medical record. Although your personal and insurance information may be on file already, the hospital representative will verify on each admission that the information is still accurate. Upon completion of your admission, you signed consent for treatment and financial responsibility. Consents for minors must be signed by parents or guardians.

Your Room: Getting Acquainted NURSE CALL SYSTEM The Nurse Call System contacts the nurses’ station, allowing you to get in touch with your nurse. A remote device with a cord will be placed within your reach on your bed. This remote allows you to call the nurses’ station, and also operates the television. If you need nursing assistance, please press the button marked “NURSE” on the remote. Pressing this button activates a signal to the nurses’ station where a member of your care team will answer your call and forward your request to your nurse or the appropriate caregiver. If you have any questions about the Nurse Call System, please ask for the nurse coordinator.

PERSONAL BELONGINGS

Electrical appliances, such as those used for personal grooming, may be used in your room. Coffee pots, hot plates, heating pads, privately owned televisions and DVD players are considered fire hazards and are not permitted in the hospital.

MAIL AND FLOWERS Patient mail is delivered daily to patients’ rooms Monday through Friday. Flowers are delivered to patients’ rooms by the individual florist or by Sacred Heart’s own Gift Shop volunteers, if purchased there. Flower delivery may be arranged by calling (877) 887-3200. Flowers and cards also can be ordered by calling the Gift Shop at (850) 416-7885.

LINEN SERVICE Prior to your admission, your bed was prepared with fresh linens. During your stay, your nurse will straighten your linens daily and change bed sheets and pillowcases on an as-needed basis. If your linens become soiled, a member of your patient-care team will change them as often as needed. Please contact your nurse to request fresh linens. Our goal is to help conserve resources and our environment while still providing comfort and excellent care for our patients.

WIRELESS INTERNET (WI-FI) As a convenience for our patients, family members and visitors, wireless Internet access is available in all patient rooms and throughout the hospital. However, we are not able to provide technical support for guest Wi-Fi access at this time.

We recommend that patients bring only sleeping garments, slippers, a robe and personal toiletries. We strongly urge you to leave valuables at home, or send them home with a family member. If that is not possible, you may deposit such items in the hospital’s Security Office. The hospital will not be responsible for money or other valuables lost or stolen.

Your Patient Care Team PHYSICIAN

Excellent, Safe Patient Care

Our active medical staff consists of approximately 100 primary care and specialty physicians. Your physician is responsible for coordinating all aspects of your care while you are in the hospital. In addition to the physician you will meet with in person, other specialists also may be involved in your care, including pathologists and radiologists.

At Sacred Heart Hospital, the safety of our patients is our first priority. The most important thing you can do to prevent errors in your care is to be an active member of your healthcare team. That means taking part in every decision about your care. Patients who are more involved with their care tend to get better results.

HOSPITALISTS

PREVENTING INFECTION: FIVE THINGS YOU CAN DO

Hospitalists are dedicated physicians who specialize in caring for hospitalized patients and will work with your primary care provider to coordinate all aspects of your care while you are in the hospital.

NURSING TEAM Sacred Heart Hospital has an outstanding group of dedicated nurses who coordinate patient care 24 hours a day, seven days a week. Our nurses are our patients’ primary advocates, keeping other staff members involved and informed while providing an excellent continuity of care. Our nurses are certified in basic life support, advanced cardiac life support and pediatric advanced life support.

NUTRITION SUPPORT TEAM Patients are closely monitored to ensure that their nutritional needs are met during their stay. Our registered dietitian will work with your nurse and physician to evaluate special nutritional needs or meal plans.

OTHER SUPPORT STAFF In addition to physicians and nurses, many other Sacred Heart support staff members may be involved in your care, including respiratory therapists, physical therapists, lab personnel, housekeeping, transport staff, and volunteers, depending on your needs. Our entire healthcare team is committed to providing you with excellent care.

Avoiding contagious diseases like the common cold, strep throat and the flu is important to everyone. Here are five things you can do to fight the spread of infection: 1. Clean your hands. 2. Make sure your healthcare providers clean their hands and/or wear gloves. 3. Cover your mouth and nose when you cough or sneeze. 4. If you are sick, avoid close contact with others. 5. Get vaccines to avoid disease and fight the spread of infection. For more information, visit www.jointcommission. org and search for “infection control brochure.”

PREVENTING PRESSURE ULCERS A pressure ulcer or bedsore occurs when there is pressure on the skin for too long. This can happen to people who lie in bed, sit in a chair or wheelchair for long periods of time, and are not able to shift their weight or change their position themselves. If you have a concern about risk for pressure ulcers for you or a patient at Sacred Heart, please tell your nurse and care team. PAIN MANAGEMENT At Sacred Heart, we want to make your stay with us as comfortable as possible. During your stay, if you are having any pain, please let your nurse know. Sometimes, patients hesitate to share their pain because they want to “tough it out,” but this can prolong your recovery time. Please let us know if you are experiencing any pain.

Pain management can help you get well faster. With less pain from illness or surgery, you can increase your daily activities, sleep better, eat better and feel more positive. You may gain your strength back more quickly and may even leave the hospital sooner. It is best to ask for pain medicine when the pain starts rather than waiting until it is very bad. Pain medicines often work better if you take them at regular times. We will be assessing your pain throughout your visit and every 30 minutes to one hour after you receive pain medication to ensure that it is working properly. Together with your healthcare team, you can make a pain management plan that meets your needs and goals. Your team will need to ask you questions. They may want to learn about: • What your pain feels like and when you have it.

• Any other types of pain you have had. • Medications or other treatments you have tried and how they worked. • How the pain is affecting your life (work, sleep, social activities, etc.). Your team may include several people, such as your primary healthcare provider, along with a: • Doctor who is a pain specialist, such as a neurologist or anesthesiologist.

• Nurse. • Rehabilitation specialist, such as a physical or occupational therapist. • Psychologist or other mental-health professional. • Social worker. • Pharmacist. • Chaplain.

• Dietitian. Everyone feels and reacts to pain differently. Pain is not just a physical experience. A person’s pain also can involve his or her: • Emotions and attitude. – After the brain receives a pain signal, it sends it to another part of the brain, which is linked to emotions. When a person feels pain, he or she also may have feelings such as anger, anxiety, sadness or a sense of loss.

– Positive feelings and thoughts may keep a person from focusing on pain, or help him or her take more steps to control it. The person may not notice or have as much pain. • Culture, upbringing or personality. – Some people may be very expressive when in pain, while others bear it silently. – Some people may expect relief from pain. Others may view pain as part of life and not seek relief. • Memories. – How much pain a person feels in a certain situation can be affected by his or her own experiences with pain and/or experiences with a loved one’s pain. • Fears or expectations. – For example, if a person fears the dentist, he or she may feel more pain from dental procedures. – If a person worries that pain is from a serious disease, the pain may feel worse until he or she learns it’s not.

COMMUNICATION Please speak up if you have questions or concerns, and don’t be afraid to ask if you don’t understand something. Good communication not only will help us provide excellent care, but also can be a source of comfort to you. By working together, you and your patient care team will have the information to make the best decisions regarding your health. Please inform your nurses, doctors and other professional staff about your important health information so that we can provide you with excellent care. In addition, whenever you receive new medications, be sure to tell your caregivers about your allergies or problems you have had in the past when taking medications.

INFECTION CONTROL It is important for patients, visitors and healthcare workers to work together to reduce the risk of infection. Patients are susceptible to infections due to their illnesses and injuries. Infections may be spread by things such as contact with unwashed hands or objects, as well as through the air, such as when people sneeze or cough. Visitors with recent colds, flu or other illnesses should avoid patient contact. All visitors should wash their hands prior to patient contact and before going home. If you are sick, avoid close contact with others. Make sure that your vaccinations are current, even for adults. Check with your doctor about shots you may need. If isolation precautions are used for you during your stay, please follow all recommendations and review the given education. Sacred Heart invites you to become our partner in ensuring patient safety by asking your caregivers if they have washed their hands before and after their encounters with you.

PREVENTING FALLS You can help prevent falls by following the guidelines below: If your nurse asks you to call for assistance when getting out of bed or up from a chair, please call before you begin to get up. Certain medications may make you at high risk for falls, such as diuretics, sedatives, pain medications, tranquilizers, blood pressure medications, antidepressants, cardiac medicines or any medication that is likely to affect your balance. Ask our staff for assistance.

MEDICATIONS During your hospital stay, your doctor may order medications for you from our pharmacy. You will need to provide an up-to-date list of all your medications upon admission. This includes prescription and overthe-counter medications, as well as dietary supplements such as vitamins and herbs. Please remember: Do not take any medications on your own while in the hospital. If you bring any medications from home, you will be asked to send them back home with a friend or family member. When you are discharged from the hospital, your nurse will reconcile your current medication list with any additional medications or changes your physician has ordered.

In addition, certain diagnoses also may mean you are at high risk for falls, including previous falls, fear of falling, stroke, problems with vision or hearing, dehydration, low blood sugar, heart-rhythm problems, Parkinson’s disease, depression, confusion and gait disturbances. If you have any of these conditions, please follow these tips to help prevent falls: • Rise or sit slowly to prevent becoming light headed or dizzy. • Use a night light. • Follow the safety precautions as explained to you by your nursing or rehabilitation staff. • Minimize clutter near the bedside and throughout the room. • Report any spills or bathroom mishaps so they can be cleaned up promptly.

SIDE RAILS The side rails on your bed may be raised for your protection, especially at night and after surgery. Please do not try to adjust the side rails without assistance.

SMOKING To promote a healthy, healing environment for our patients, associates and guests, the use of any tobacco products is not permitted at Sacred Heart Hospital, nor on any property owned by Sacred Heart, inside and outside. Upon admission, your nurse will ask you about your smoking habits and provide you with information on smoking cessation services at Sacred Heart. The Smoking Cessation Department can be reached at 416-7764.

EMERGENCY DRILLS For your safety, we occasionally hold fire/disaster drills in order for our staff to remain well-prepared for an emergency situation. In the unlikely event of a fire or other disaster, hospital employees are prepared and will assist you as necessary.

SPEAK UP! HOW TO REPORT SAFETY CONCERNS We encourage you to let us know anytime you have concerns about your safety. Please tell your physician, nurse or other caregiver, or you may call the operator at 416-7000 (or 0 from inside the hospital) anytime you have questions about your safety.

Visiting Hours and Policies Your family and friends are welcome to visit you during your stay. Please review the following guidelines that have been designed to ensure your privacy, as well as that of other patients.

GENERAL VISITING POLICIES 1. To allow adequate rest, recommended visiting hours are from 6:00 a.m. to 8:30 p.m. 2. For the protection of our patients, we ask visitors showing any signs of infection or illness to not visit hospitalized patients. This includes but is not limited to: cough, fever, runny nose, rash, vomiting, diarrhea or sore throat. 3. One responsible adult (age 18 or older) will be permitted to stay overnight to provide support to the patient. All overnight guests must register with security. 4. There is a waiting room on the third floor for families of patients undergoing surgery. 5. Visitors of patients in isolation must follow the instructions provided on the isolation signage. Depending on the situation, visitation may be limited for the safety of the patients and others. 6. Please limit your visitors to a few at a time. Children ages 15 and younger must be accompanied by an adult, other than the patient, at all times. 7. If the nursing staff needs to perform a procedure, we may ask your visitors to go to the waiting room. We will gladly notify them when we are finished so they can continue their visit. 8. The hospital dedicates the hours from 2:00 p.m. to 4:00 p.m. as Quiet Time. Please keep the noise and visitation to a minimum during this time to allow our patients adequate rest. 9. Keeping the television volume low and turning it off at bedtime will help you and your fellow patients get much-needed rest. Please keep noise levels to a minimum in consideration of other patients. 10. We welcome members of the clergy to visit at any time.

SPECIAL VISITING HOURS AND GUIDELINES At Sacred Heart, we believe visits from family and friends aid in the emotional well-being of patients. In addition to our general visiting guidelines, some areas of the hospital have special visiting hours and policies.

AFTER-HOURS VISITING AND ENTRY TO THE HOSPITAL After 8:30 p.m., guests who want to visit a patient must obtain a temporary ID badge either at The Children’s Hospital or Bayou Tower entrance information desk. The visitor must present a driver’s license, military ID or state-issued ID to the security officer. An ID badge displaying the visitor’s photo, name, date, time of entry and destination will be issued. The visitor’s access is limited to the patient-care area designated on the temporary ID, as well as public areas, such as the cafeteria. Visitors must display the temporary ID in a visible location above the waist at all times while on SHHP premises. Authorized visitors (designated family member or designated support person) for med-surge/ICU patients who are in the facility at the conclusion of visiting hours will be asked to go to either The Children’s Hospital or Bayou Tower entrance to receive a visitor badge. Visitors without a valid visitor badge after normal visiting hours will be asked to leave the facility.

ADULT CRITICAL AND PROGRESSIVE CARE UNITS

NEONATAL INTENSIVE CARE UNIT (NICU)

Sacred Heart has open visitation hours in its Intensive Care Units and Progressive Care Unit. Two visitors or family members may visit ICU and Progressive Care patients at one time. One family member may stay overnight with the patient.

The NICU wants you as a parent to spend as much time as you can visiting and caring for your baby. Here are some things to be aware of when visiting your baby:

For safety reasons, we ask that children under the age of 12 refrain from visiting these areas. We know that exceptions are necessary on certain occasions, and we will be happy to discuss this with you and your family. We also ask that live plants and flowers be sent directly to the patient’s home and not to their hospital room in these units. Please contact the patient care manager or house supervisor (416-7000) if you have any questions.

CHILDREN’S HOSPITAL Parents of patients in the Children’s Hospital have unlimited visiting hours and are encouraged to visit. Children under the age of 12 should not visit the Children’s Hospital without permission from the patient’s nurse or physician. For the safety and privacy of our patients, no more than two people may visit at a time, and visitors age 18 and younger may not stay overnight in the hospital lobbies or rooms. Children’s Hospital Visiting Hours: Visiting hours are from 11 a.m. to 8 p.m. for the general public. In addition, there is a Quiet Time from 2 p.m. to 4 p.m. Please keep the noise level and visitors to a minimum during this time.

• The NICU has an open visitation policy with the exception of change of shift, which is limited to parents only. Change of shift is 6:45 a.m. to 8:00 a.m. and 6:45 p.m. to 8:00 p.m. • Due to space constraints, visitors are limited to two people at your baby’s bedside at a time. • All visitors must be ages 15 years or older. • Visitation may be limited due to emergencies or special procedures going on in the NICU. • Parents must have their ID bands on them at all times. If the ID bands have been removed, you will be asked to show a photo ID. • Parents may select two visitors, ages 15 or older, to visit when they are not present. These two visitors may bring back additional visitors as well. Parents should complete a visitor consent form and select what activities they allow their visitors to provide. Once the visitor consent form has been completed, we ask that visitor names not be changed in an effort to limit the amount of different people in and out of the NICU. • No food is allowed in the NICU, but mothers are encouraged to bring in a bottle of water to assist in breast milk production. • Designated members of the clergy may visit when parent(s) have given prior approval either by phone or in person.

PEDIATRIC INTENSIVE CARE UNIT (PICU) In addition to the visiting policies in Children’s Hospital, a few special guidelines apply to the Pediatric Progressive and Intensive Care Units. To provide the best care and safety for our patients, these units are located in a secured area. For patient safety, visitors and family members are limited to

two at a time and must call on the phone outside the door for entry into this unit. Four ID bands are issued on admission for designated visitors. Banded visitors must be 18 years or older. The ID bands are not interchangeable and must be worn by the designated person during the patient’s PICU stay. If for any reason these designated visitors must remove their baby band from their wrist, they will be required to present a photo ID for verification when visiting. All visitors must be accompanied by a banded person.

WOMEN’S AND MATERNITY CENTER

One banded visitor may stay overnight with the PICU patient, as the patient’s condition allows. Due to the seriousness of their condition, patients who require the use of a ventilator do not have couches, cots or recliners available in their room. The waiting room is available for a second banded visitor. In addition, visitors 18 and younger may not stay overnight in the hospital lobbies or rooms.

Patients in the Labor and Delivery suites may have up to five visitors at the discretion of the nurse in charge and patient’s physician. Visitors will be limited as needed to promote a safe environment for the patient. No children under age 12 are allowed in the labor room unless he or she is the patient’s child, and must be accompanied by an adult at all times, preferably not the significant other.

Sibling Visitation:

On admission, the patient will select a designee/ significant other to wear an armband; this is the person she wants to attend the delivery. Four additional visitor passes will be provided and given out by the patient to those who she desires to visit her while she is in labor and at delivery.

While children under the age of 18 are not allowed to visit the PICU, siblings of the patient may visit from 4 p.m. to 6:30 p.m. daily. For the safety of all of our patients, two banded visitors with a maximum of two siblings may be in the room together during this time.

Spouses and/or significant others may visit the Women’s and Maternity Center at any time. Visiting hours for the general public are from 10 a.m. to 8:30 p.m.; however, we ask that no more than four visitors be in the room at a time. In addition, brothers and sisters under the age of 12 may make short visits when accompanied by another parent/adult.

LABOR AND DELIVERY

PICU Visitors’ Hours: Banded visitors are welcome at all times. We ask that only two banded visitors be at the bedside from 6:30 a.m. to 8 a.m. and 6:30 p.m. to 8 p.m. to allow for a nursing report. We ask that no one enter or leave the PICU during this time. We encourage one banded member to stay at the bedside and participate in a report with the nursing staff. From 2 p.m. to 4 p.m., only banded individuals are allowed to visit the PICU. All other visitors may visit with a banded visitor.

Hospital Conveniences PARKING Parking is available at no charge to our patients and visitors. Parking garages are located off Ninth Avenue and Brent Lane/Bayou Boulevard. Additional parking lots can be found near Brent Lane/Bayou Boulevard, the Women’s and Children’s Hospital and the Bayou Tower.

CARE CHANNEL 23 The hospital now offers the CARE television channel, which provides relaxing music and images to help our patients enjoy a soothing environment for healing and rest. The 24-hour format includes unique daytime and nighttime images of beautiful natural landscapes that reduce anxiety, support the circadian rhythm, enhance sleep, and reduce the negative impact of hospital noise on patient floors.

GIFT SHOP Sacred Heart Hospital’s two gift shops are operated by the volunteers of the Mother Seton Guild. They are located in the main entrance lobby and The Children’s Hospital lobby. Personal necessities are available, as well as flowers, balloons, small gifts, cards, reading material and other items. Call 850-416-7885. All major credit cards are accepted. LOST AND FOUND The Lost and Found Department is managed by the Sacred Heart Hospital Security Department. Please report any lost or found items to your caregiver immediately. Items found on the Sacred Heart Hospital campus will be held in the Security Office for 30 days, after which they will be discarded or donated to a local charity organization. VOLUNTEERS Volunteers give generously of their time to help Sacred Heart run smoothly and efficiently. We are proud to have more than 500 volunteers who assist in the operation of Sacred Heart Hospital in Pensacola. Many of these volunteers are members of the Mother Seton Guild. You will recognize them by their peach (women) and red (men) uniforms. Other volunteers include Friends of the Hospital (blue tops) and teen volunteers (yellow tops).

Special, your host or hostess will assist you in making choices that are allowed on your diet from the AllTime Favorites List. If you have any questions about your diet and the meals you receive, or if you wish to speak with a registered dietitian, please call 416-7958. Patients’ family members are welcome to purchase a meal tray from our hospital cafeteria to be delivered to the room. Please call Food Services for Visitors at 416-7462.

CAFETERIA Visitors are welcome to enjoy our cafeteria, which is located on the first floor between the Women’s and Children’s Hospital and the main hospital. Hours are: Monday – Friday: Breakfast: 6:30 a.m. – 10 a.m. Lunch: 10:45 a.m. – 2 p.m. Dinner: 4:30 p.m. – 6:30 p.m. Night Meal: 2 a.m. – 4 a.m. Please remember that patients are not permitted to eat in the cafeteria. Your diet will be determined by your physician and managed through the Dietary Department.

Volunteers perform a number of essential duties, including assisting at information desks and the Admitting Office, helping out in the Surgery waiting areas, and providing comfort to patients and family members. MEALS AND DIETARY SERVICE Our Dietary Department is fully equipped to meet your nutritional requirements while you are in the hospital. Your physician will determine whether your diet is routine or if you should receive special meals. Medication Safety Pledge for Patients Patient safety is our highest priority at Sacred Heart. Please review our Medication Safety Pledge below and comply with the directives provided. Medications may be needed to improve my health. By working as a partner with my healthcare team, I can learn to use these drugs safely. I promise to: • Tell all caregivers (doctors, nurses, pharmacists, etc.) about my allergies (food and drug). • Learn about all of my drugs, including the name, strength or dose, why I am taking the drug and how to take it. • Tell all physicians when new drugs are ordered for me by another physician. • Talk with my caregivers about my use of herbal products, vitamins, food supplements, or other over-the-counter drugs I may be taking.

• Identify myself before taking any type of drugs given to me by a nurse, physician, pharmacist or caregiver. • Question my caregiver about any drug with which I am not familiar. • Tell my caregivers if I have any problem taking the drug for any reason, including the cost. • Check with my caregiver before changing the way I take my drugs. • Faithfully renew my prescriptions on time. • Never share medications with others or use drugs belonging to someone else. • Store drugs properly and safely, away from children and pets. • While in the hospital, I will not take home any medication unless my physician and nurse approve.

Bioethics Committee Patients, relatives and medical personnel sometimes face difficult treatment and life-and-death decisions. The Bioethics Committee at Sacred Heart is composed of a variety of professionals, including physicians, nurses, social workers, pastoral ministers and administrators. This Committee, upon request, will seek to provide confidential consultation and emotional support for bioethical issues. Advance Directives Under Florida law, all hospital patients have a right to sign documents that make choices about treatment known to their doctor or family in advance. You also have a right to sign papers that name someone you trust to make medical decisions for you in certain situations. These patient rights are known as advance directives. The directives state in advance what kind of treatment you want—or do not want—if serious conditions develop that would prevent you from telling your doctor whether you want the recommended treatment. For example, if you lapsed into a coma, would you want Sacred Heart’s staff to know your specific wishes about your treatment? You may choose one of two types of advance directives: a living will or a healthcare surrogate designation. With a living will, you state the kind of medical care you want or do not want if you become unable to make your own decisions. With a directive naming a healthcare surrogate, you sign a paper naming another person as your agent to make medical decisions for you, should you become unable to make them for yourself. If you choose to complete an advance directive, you may change or cancel it at any time. If you have prepared a living will or appointed a surrogate, make sure someone, such as your doctor, lawyer or family member, knows about the document and where it can be found. The hospital will ask you for a copy upon admission. Advance directives are important. You do not have to have an attorney prepare an advance directive, but you may wish to talk about it with your family, your attorney or a member of the clergy.

Palliative Care Palliative care (pronounced pal-lee-uh-tiv) is specialized medical care for people who have a serious illness. It focuses on providing patients with relief from the symptoms, pain and stresses of a serious illness—whatever the diagnosis. The goal is to improve quality of life for the patient and the family. Palliative Care at Sacred Heart Hospital is provided by a team of specialty trained nurse practitioners, registered nurses, a chaplain, social worker and other specialists who work together with the patient’s other doctors to provide an extra layer of support. It also enables patients to have more control over their care by improving their understanding of treatment options. It is appropriate at any age and at any stage in a serious illness, and can be provided along with curative treatment. The overall purpose of palliative care is to relieve suffering and provide the best possible outcome and quality of life for the patient and the family. Social Services At Sacred Heart, our team of social workers and case managers provides support and counseling for patients with traumatic diagnoses, patients in need of specialized rehabilitative care, patients admitted from a nursing home, or patients having problems adjusting to illness or hospitalization. The Social Services and Case Management Office can be reached at 416-6881 and is open Monday through Friday from 8 a.m. to 4:30 p.m. There is no charge for this patient service.

CASE MANAGERS Case managers are registered nurses who work closely with your physician and social worker. The case manager reviews your medical chart on a daily basis and assists the physician in coordinating your care during your hospital stay. Case managers can assist patients in understanding their health benefits and available services provided by their health insurance. The case manager routinely provides information to your insurance company outlining your treatment plan, as well as plans for your care after you leave the hospital. The case manager and social worker work together to develop a sound plan for care following a hospitalization. For more information about Case Management, please call 416-6881.

SOCIAL WORKERS Social workers are knowledgeable about state and federal programs, including Social Security Disability, Vocational Rehabilitation and SSI/Medicaid benefits, and can assist with the application to these programs. They also can assist in completing advance directive and designation of healthcare surrogate forms. Social workers also help families prepare for a patient’s return home or find alternatives when returning home is not possible. In addition, the department can link families to home healthcare services or community resources that may assist with family needs.

CHAPEL For the spiritual needs of you and your family, the chapel is located on the second floor of the main hospital. It is open 24 hours a day, seven days a week. All faiths and denominations are welcome. Catholic Mass is offered: Monday - Friday at 11:30 a.m. Saturday at 8 p.m. Sunday at 8 a.m. These Masses are televised on hospital Channel 2.

Social workers assist with families of children admitted to a specialty-care unit, children who need specialized equipment or supplies for continuity of care, and children who are diagnosed with terminal and chronic illnesses. For more information, call 416-6881.

Spiritual Care Patients often have spiritual and emotional needs that go beyond the care that doctors and nurses can provide. The staff of our Pastoral Care Department strives to serve you and your family as unique human beings with individual needs. Pastoral Care chaplains are daily visitors to the patient areas and see persons of all religious denominations. To request that a chaplain visit you, contact the hospital operator by dialing 0. The priest chaplain visits all newly admitted Catholic patients and makes available the Sacraments as desired and appropriate. They see other patients upon request. Leaders from many faith traditions are available to minister to the spiritual needs of members who are hospitalized. If your insurance policy does not cover all charges or if you anticipate having difficulty paying your hospital bill, please call our Business Office at 416-7190. Going Home PATIENT DISCHARGE At Sacred Heart, we recognize that most patients are eager to go home from the hospital. To arrange for your ride, we encourage you to allow for two to three hours for your nurse to finish your discharge process. Nurses will be busy arranging for your prescriptions, discharge instructions and follow-up appointments, and your doctor may request that you complete tests before you go home. We will do our best to keep you informed of your discharge process and notify you of any delays.

HOME HEALTHCARE AND SKILLED NURSING FACILITIES After your discharge from Sacred Heart hospital, you or your loved one may need the services of a home-healthcare agency, skilled nursing facility or rehabilitation center.
