“OUR noble intention has been unfairly misinterpreted.” That’s the stance of the now infamous Obedient Wives Club (OWC) — launched only four days ago — which felt that its proclamation that wives should serve their husbands “better than a first-class prostitute” was taken in the wrong context. Ever since the club’s vicepresident, Dr Rohaya Mohamed, made the controversial statement last Saturday, it has been savaged and subjected to ridicule by the Press, women’s groups and religious bodies locally and internationally. Dr Rohaya’s call was viewed by most as denigrating, as it

painted a picture that women should always play a subservient role in a marriage. The Malay Mail visited Bandar Country Homes in Rawang, where Global Ikhwan Sdn Bhd — the multi-business corporation that runs OWC — is based and spoke with the club’s national director Fauziah Ariffin. “I have been receiving nonstop calls since the launch,” said the jovial 40-something Fauziah, just after she hung up on a phone conversation with Time magazine, which was seeking to interview her. “I believe we have been misunderstood and misinterpreted. When we said that husbands should treat their wives

like first-class prostitutes, we were not putting wives on the same level with prostitutes. We are talking about first-class elite types, not street hooker types. Our wives provide men with top-level service. However, ordinary prostitutes can only provide good sex, but not love and affection which only a wife can provide. “Hence, as wives, we must treat our husbands better. It’s not just in bed, but everything that a wife can offer. Optimise your role. HER boyfriend wanted her to be slimmer and have bigger breasts and constantly pestered her about it during their five-year relationship. To please him, 30-year-old clerk Tham Mei Leng, from Pahang, underwent liposuction of her tummy and transferred the fat to her breasts. But the procedure by an unlicensed cosmetic surgery company, which cost RM13,100, turned out to be a disaster — her breasts were infected with pus and Wong suffered ugly surgical scars and constant itchiness. On top of that, PAINFUL EXPERIENCE: Tham says she thought what her boyfriend has she was doing would please her boyfriend dumped her.

Relating her ordeal at the MCA Public Services and Complaints Bureau here yesterday, Tham said a friend had recommended the company to her. "My breasts became infected because tummy fat is not suitable for breast enlargement and I found this out from a specialist after undergoing the surgery," she said. "The specialist told me the fat should have been from the thighs and not the tummy. This specialist is now helping me prevent the infection from spreading into my bloodstream." Tham also said she underwent the cosmetic surgery without any anaesthetics or medicine to relieve the pain. "I endured the pain during the two-hour surgery by constantly re-

minding myself that what I was doing would please my boyfriend. "It was very painful and I could not help crying during the surgery. The infection also causes constant pain and itchiness." Bureau chief Datuk Michael Chong said the unlicensed cosmetic surgery company was operating at a shoplot in Kuala Lumpur and the surgeon came from southern China. "Tham's friend had previously undergone liposuction at the same company without complications. Tham thought it would also work for her although she also wanted breasts enlargement. "We will provide her legal assistance and seek the help of the Health Ministry on possible action against the unlicensed company."

Ensure surgeons are qualified, says Chong

THIRTEEN cases of women who had fallen victim to botched cosmetic surgeries have been reported to the MCA Public Services and Complaints Bureau since 2009. "These women feel they had been cheated by unqualified cosmetic surgeons after they suffered side-effects," bureau chief Datuk Michael

Chong told The Malay Mail. "The surgeries include nose sharpening, having double eye-lids, lips enhancement and skin beautification. In all the cases, the women wanted to please their husbands or boyfriends so they would not stray." Chong said there was also a case of a woman who wanted to make her breasts smaller.

"She preferred to be a tomboy and felt her big breasts were a hindrance. "Ironically, when the surgeries went awry, many of these women ended up being dumped by their lovers." Chong advised women to choose their cosmetic surgeons carefully. "Do not be duped by adver-

tisements and by cheap pricing and always ensure the surgeons are qualified and reputable. "The government should also vet advertisements on cosmetic surgeries to prevent fraud and weed out the quacks." The most prominent case of a cosmetic surgery gone wrong in Malaysia was

that of Datin Fatimah Wan Chik, wife of former Labuan MP Datuk Suhaili Abdul Rahman, who suffered complications after having reportedly undergone a tummy tuck, liposuction and surgery for her eye bags at a clinic in Klang on Jan 9, 2008. She died on Oct 28 that year after being in a coma for more than 10 months.

DAYS after Nicole Scherzinger was tapped to officially replace Cheryl Cole on The X Factor, series creator Simon Cowell says he did all he could to keep Cole around. Though a source confirmed to Us Weekly her lack of chemistry with the other judges was a key factor in her departure, Cowell said his primary goal in sending Cole packing was to ease her homesickness. Michael Silverleaf, a lawyer for News Group Newspapers, the publisher of The News of The World, offered "sincere apologies" to Miller in London's High Court yesterday. A SINGLE ticketing system for public transport in the Klang Valley should have been implemented a long time ago, say commuters. They were commenting on the front page report in The Malay Mail yesterday, headlined 'Ticket to ride', on hopes for smooth road-rail travel by 2013 when the electronic pass systems of two major transport operators merge. Syarikat Prasarana Negara Berhad (Prasarana) — the

operator of the LRT, monorial and RapidKL bus services — and Keretapi Tanah Melayu Berhad (KTMB), which runs the Komuter service, have confirmed they were working towards a single ticketing system. Zarina Abdul Halim, an assistant manager with a government-linked company, said a common ticketing system to encourage seamless journeys for all rail networks and bus services was long overdue.

some men. “Ever since the launch, OWC offices in Rawang have been visited by many people, including the Press, seeking interviews with club members,” said a male religious teacher at the headquarters of the club, Global Ikhwan Sdn Bhd. “What is wrong with asking wives to obey their husbands? This is a practice that dates back to ancient times.”

Fauziah also argued that obedient wives will not cause husbands to take their partner for granted, but in fact, it will make them better husbands. “When a husband comes home and receives good treatment from the wife, he generally becomes more loving towards her. “I also heard footballers often engage in sex with their partners or prostitutes before a match as this will boost their confidence. Likewise, when a husband gets a healthy dose of sex they become motivated at work.” Fauziah stressed that in Islam, there are four things that wives must do to enter heaven: to pray, to fast during Ramadan, to protect their chastity, and to be obedient wives – and it is often the fourth aspect that modern wives neglect.

She clarified that the club will not be giving out practical sex lessons. “Of course not! There won’t be practical sex classes. That’s haram.” She was asked to explain another controversial statement by Dr Rohaya earlier, that the club would provide bedroom lessons to the OWC's 1,000 members. Fauziah said their lessons will consist of motivational talks and discussions among the members. The talks will be conducted by her, Dr Rohaya, and eight other committee members. The club was first introduced in Amman, Jordan on May 1. On June 18, the club will launch its chapter in Jakarta, Indonesia. "There are plans to spread the club to Europe," said Fauziah, adding Dr Rohaya is currently in Paris to organise talks there.

HELPING the homeless in the streets is gratifying for Justin Cheah, one of the project directors of Kechara Soup Kitchen (KSK) which offers food, drinks and basic medical first aid to the urban poor in KL. “When people ask me why I do this, I tell them that helping the homeless motivates me," said Cheah, 35, who is involved in the day-to-day operations of KSK on a full-time basis. "There are a lot of underprivileged people who need support, and we give out not just food but also care and comfort." Cheah, a former marketing executive, said it was fulfilling whenever a homeless person no longer required the assistance of KSK or from other similar charitable outfits. "I can never forget the occasion when a former homeless

man patted me on my back to happily inform he was able to support himself again. That makes what we do worthwhile," he said. "While giving out free food to the homeless, the intention is also to encourage them to return to normal society." KSK, founded in 2006, was inspired by Tsem Tulku Rinpoche, the

FOR the homeless in Jalan Masjid India, selfless volunteers like Richard Thong are godsend as every Sunday at 5pm, he and a group of others would distribute free food to the underprivileged at the compound of the Sri Bunus building there. During these sessions, the recipients would be given numbers to queue for their portions. Thong, 56, a co-coordinator of the Klang-based Grace Community Services Centre, said: "Our volunteers are mostly church-goers from Puchong and from the local Lion's Club. The food is occasionally sponsored by companies such as Yeo Hiap Seng who supply canned food, and shops at nearby malls and bakeries. We also sometimes receive financial aid from CIMB Bank." Thong, who has been volunteering since 2002, is a full-time counselor for the National Population and Family Development Board. When asked about the rising number of homeless people in the streets of KL, he said: "We help find shelter for those who approach us. We would be able to do more if there were more volunteers and supplies from sponsors. We have five volunteer doc-

Pillai who has been doing full-time charity work for the past 30 years, said: "We assist destitute people with family problems, marriage problems, mental illness, unemployment issues or are homeless. We also have a programme called Birthright in which we provide shelter for female runaways coping with abandonment, abuse, unwanted pregnancies and

other social issues." Among those who queued for portions last Sunday was Rosman Alwi, a 43-yearold unemployed man from Perak. "I have been homeless for three years now and came to know of Grace Community from other homeless people," said Rosman, adding that he sleeps in the Jalan Masjid India area. Another homeless person was 52-year-old Hong Kam Fatt from Tanjung Malim, Perak, who is HIV positive. "Because of the stigma of my condition, I have problems finding work and accomodation," said Hong. "So I have to depend on the kindness of others to survive."

ONE of the charitable organisations providing free meals to the underprivileged in KL is Food Not Bombs Kuala Lumpur (FNBKL) whose volunteer members distribute vegetarian meals at the walkway at St John's Cathedral in Jalan Bukit Nanas here at 6pm every Sunday. FNBKL is among the 400-plus chapters of FNB from around the world. The organisation, which originated in the United States in the early 1990s, has about 50 per cent of its chapters operating outside the US in about a thousand cities. The Malay Mail visited FNBKL last Sunday and one of its volunteers, 27-year-old freelance photographer Yuu Han said: "We have been giving out free food here for almost a decade. nancial support from individuals and organisations." Each FNB chapter is independent, do-it-yourself and nonhierarchical. It does not condone wastage of food, sexism, patriarchal behaviour or attitude, queerphobia, racism, classism, speciesism, and anything else that is oppressive. FNB members also actively participate in issues pertaining to anti-war, anti-arms and antinuke and also protest against the destruction of the environment. It has been estimated that over a billion people go hungry each day — that would be one in seven people as the world population is now seven billion. Aussie UFO files disappear THE defence department has been unable to find a host of records relating to UFO sightings collected over several decades. The disappearance of the documents came to light when the military received a freedom of information request from the Sydney Morning Herald Newspaper asking for any documents mentioning sightings of UFOs or “extraterrestrial organisms” in Australia. However, after two months of searching the relevant archives, the department said the documents could not be found. UFO experts in Australia believe the military knowingly shredded the documents.

Uncivil obedience

I CANNOT believe how the issue of the ‘Kelab Isteri Taat’ (directly translated as ‘Obedient Wives Club’ or OWC) has grown into such a huge firestorm the last week or so. As those following the story may have read, Dr Rohaya Mohamad, the vice-president of the now-world-infamous club, reportedly said something along the lines of “wives must learn to satisfy their husbands’ sexual needs to prevent him from seeking other women”. She was also quoted as saying that part of the club’s goals is to educate women how to please their husbands “like a first class prostitute”. Naturally, such politically incorrect statements did not sit too kindly with a lot of people, especially women. The club’s stand is widely seen as taking a few steps backwards in the longfought battle for women’s rights and gender equality. Going by the fiery comments and condemnations the OWC had drawn, it seemed pretty obvious among many the club’s very existence is unacceptable in a modern, progressive world.

Assuming, of course, that everything reported before this was NOT taken out of context or misquoted, the anger is fully justified. A wife should not be held accountable if her husband strays. He’s the bad guy, he betrayed her trust and his sacred vow to commit only to her. Blaming the wife as ‘not obedient or sexually fulfilling enough’ for his infidelity is not only grossly unfair, it is also quite frankly, downright stupid. Such quick escape is a scrotumless way of solving problems many selfish people take. Instead of making the effort to solve his marital problems, he looks outside for distractions, ending up inviting more problems. Instead of facing the painful issues and making difficult decisions, he blames everyone else and absolves himself from guilt. Such actions also degrade both women and men, and the whole idea of marriage itself. It is absolutely insulting to suggest women are nothing but sexual objects whose sole purpose in

ON WEDNESDAY life is to make their men ‘happy’. It is equally insulting to say that men are mindless animals who are driven only by their lust. Women are not, and should not, behave like submissive whores, and men are not, and should not, behave like brainless, horny swine. So it’s not fair to even insinuate those negative stereotypes. What’s worse is that such a notion would drive a wedge between good-thinking women and men. It feeds the polar opposite sides of the spectrum — lusty and irresponsible men go ‘Yay!’ at the suggestion, taking it as a licence to go further astray, while men-hating women take it as fuel to further bash all men, good and bad. While it makes for good jokes and heated debates, it doesn’t serve any good purpose.

Let’s just all calm down and try to sift through all the angst and name-calling. First of all, let’s accept that despite our differences, men and women are equal in many, many ways, especially in this age of enlightenment. Most times, we look for the same things — happiness, love, security, comfort, companionship, compassion, passion, sense of belonging, purpose and other good things. MORE than 200 hawkers and petty traders at the Jalan Merlimau wet market off Jalan Kenanga here staged a protest yesterday against the market's proposed relocation by Kuala Lumpur City Hall (DBKL). The wet market, which mainly serves the residents of the nearby Seri Selangor and Seri Sarawak flats as well as households along Jalan Hang Tuah, Jalan Loke Yew and Jalan Kenanga, is located next to Kenanga Wholesale City, a new fashion mall slated to open later this year. City Hall had proposed a relocation to the Keretapi Tanah

Melayu (KTM)-reserved land at the Pudu LRT station but the hawkers and petty traders signed a petition objecting to it. “The KTM land is about a 30-minute walk from our wet market and moving there will adversely affect business as we believe we will lose many regular customers built up over the years," said Jalan Merlimau wet market action committee president Song Chee Woon, 35, who runs a drinks stall. Lim Saa, 56, who has been selling noodles there for over 30 years, said he a n d other traders were apprehen-

based on some valid points, are severely one-sided and PR-unfriendly, which are detrimental not only to society, but also to their own cause. For example, the word ‘obedient’ is hardly an appropriate tag these days. ‘Obedient’ traditionally carries a submissive or subjugated context, and in the age and society where there are more female graduates than males, you can’t expect it to be accepted, can you? For goodness sake, there are so many other, more friendly terms out there — here’s a simple one: ‘Good’. Or the Malay equivalent: ‘Mithali’, which connotes kind, responsible, understanding, or model. We’ve used ‘mithali’ on mothers, fathers, children … why not wives? And while you’re at it, why limit it to a just a wives club — include the husbands! Marriage is a two-way relationship, so make it a ‘Mithali Spouses Club’. That’s because if you want to push for obedient wives, it has to be coupled with responsible husbands, who are just as obedient to their com-

mitments and promises. Additionally, if you want to provide ‘tips and lessons’ for bedroom activities, it’s not just the wives who need lessons, husbands have a lot to learn too in that department, yes? So I say, let’s just take a chill pill and look at what we all want, happy marriages and families, peace and harmony (or at least, less angst) between the sexes, respect, dignity and fairness. We need each other, so let’s ignore all the idiocy, and take the positive things from this ugly and unnecessary episode and move on. THE 14-year tussle for Subang Ria Park between Subang Jaya residents and property developer Sime Darby Berhad ended yesterday, much to the elation of the residents. Selangor State Appeals Board announced the park will be retained as a green lung area, on the grounds the land was meant to be developed only for recreational purposes. It rejected Sime Darby's plan to build low-density apartments or medical centre suites on the 72-acre site. Subang Jaya Residents Association (RA) president A.S. Gill said: "We are very happy with the decision to bring justice to residents and the public and it's time for Sime Darby to show their corporate social responsibility and hand the land back to the people. "We are also grateful to Selangor Menteri Besar Tan Sri Abdul Khalid Ibrahim, Subang Jaya assemblyman Hannah Yeoh, our lawyer Derek Fernandez and the Subang Jaya Municipal Council (MPSJ) for backing the residents on the matter." Consumers Association of Subang and Shah Alam (CASSA) president Jacob George said the decision was a victory for residents and CASSA itself. "When we launched the 'Save

Subang Ria Park' campaign in 1999, the initiative was to retain the park as it is," he said, adding the decision vindicated CASSA's 12-year struggle on the matter. "Hopefully, this case would also set a benchmark for other decisions involving green lungs in the country." Louisa Lee, a Subang Jaya resident, said she and her neighbours are pleased with the outcome. "We hope the deicision will be accepted by everyone, the park will remain as it is and no longer be an issue," she said. Before the State government's decision, the council had rejected Sime Darby’s proposal, claiming it failed to meet the zoning requirements of Subang Jaya’s Draft Local Plan. Back in 2007, Sime Darby announced its plan to develop the land, much to the disappointment of residents. This led to the forming of a task force in January this year, comprising Gill, Yeoh, Fernandez, MPSJ councillor Dr Loi Kheng Min, Wangsa Baiduri Neighbourhood Association member Haneef Gordon, and USJ Residents Association member Premesh Chandran, to preserve Subang Ria Park to the extent of a buyout.

DESPITE having Malaysian parents, 23-year-old JERRY BOBONG fears he has exhausted all options of securing citizenship. "My parents in Sabah divorced when I was five and since then, I have been living with my uncle and aunt here in Cheras," says JERRY, who cannot read or write and has problems securing employment. "A relative in Sabah had possession of the birth certificates of my four siblings and myself but most of our documents were destroyed in a fire." JERRY says only his el-

der brother and 22-yearold sister have birth certificates. "Me and my four siblings have never attended school because we do not have birth certificates. Even my mother does not have a birth certificate. "Over the years, I tried to apply for an IC at the National Registration Department (NRD) at least six times, and in March, I was told that my identity was being used by someone else. Without a MyKad, companies will not employ me and I cannot even get married."

JERRY says he is cautious whenever he goes out as he is afraid of being mistaken for an illegal immigrant. "I would like to work as a shop assistant or security guard to help my uncle and aunt who have been taking care of me all these years," he says. "My uncle is a contractor and earns about RM1,300 a month and my aunt works in a marketplace twice a week and earns RM30 a day."

SADLY, not all privatisation schemes work out nicely and so there are times when it is better for the government to take over, especially when a seemingly lopsided agreement is reached. This is the case with the privatisation of several bodies in Malaysia over the past few years. These private bodies seem to have more clout than the government itself.

If this is the case, public-listed companies might start considering turning private, as there seem to be more benefits to it. And so, many such “marriages” will dissolve, as many will think there is no point in staying together anymore. I agree with the Selangor government’s stance in terminating Alam Flora's contract, announced on April 18, and take over the task of cleaning drains and cutting grass in

public areas. I feel Alam Flora has not been performing as expected. The political secretary to Selangor Mentri Besar Tan Sri Abdul Khalid Ibrahim, Faekah Husin, says the sub-contractors previously working under Alam Flora can bid for tenders to work directly under the various local councils. No benefit from gas subsidy members in any way profits from any fuel used in the generation of electricity. Under the Malaysian IPP model, most PPAs between our members and Tenaga Nasional Berhad (TNB) operate on a fully despatchable basis (all capacity must be made available to the national grid at any time except during planned scheduled maintenance). Payments to IPPs consist of two core components: Capacity Payments (fixed payments as specified under the respective PPAs to recover the costs of developing power generating facilities and fixed overheads) and Energy Payments for fuels consumed in generation and variable operating rates (to cover maintenance costs incurred during generation). All IPPs only commence generation of electricity, thereby incurring fuel costs or variable operating expenses upon instruction from the off-taker. Once instructed to despatch electricity by the off-taker, fuels consumed in generation by the IPPs are quantified and billed back to the off-taker at the predetermined rates as set by the government – in respect of gas, or at rate dictated by TNB for coal, based upon the quantum of electricity generated and at agreed upon efficiency rates.

As such, the cost of gas as a fuel to generate electricity is a component the IPP industry does not derive any financial benefit from. Instead, the savings in gas costs (difference between international gas prices and fixed price set by the government) are passed on directly to consumers through lower tariffs. Since 1997, the government has elected to shield electricity consumers from the volatility in gas pricing by setting a fixed cost for gas to electricity generators. Prior to this, TNB adjusted their prices to consumers in response to market pricing, which is what is meant by "fuel cost pass-through to consumers". Moreover, it’s important to note IPPs are not involved in decisions relating to electricity tariffs as we are mere contract generators to TNB, whereby

our generation facilities are contractually made available to TNB. The association would like to highlight the need for rational and objective discussion on the Malaysian energy generation sector and this must be undertaken from a balanced perspective to allow the public to make informed judgments. Such understanding is important for engagements aimed at the development of a long-term sustainable national energy policy. HIKAYAT Merong Mahawangsa wowed many here but failed to impress international viewers who posted their reviews on Amazon.co.uk's film section (go to http://www.amazon.co.uk). The film, directed by Yusry Abdul Halim (pic) of KRU Studios, features a rather impressive line-up, including Stephen Rahman Hughes, Jing Lu, Gavin Rees StenHouse, Craig Robert Fong and Datin Paduka Umie Aida. However, it did not impress audience in the UK. Yusry, the cast and the entire film have been harshly criticised. Some of the strong words used to describe the film (which has been re-titled Clash of Empires: Battle For Asia by its UK distributor) include "rubbish", "garbage" and "nonsensical trash". When The Malay Mail ap-

proached Yusry for comments, the director said he was not too happy with the title change to begin with, as it was pegged with "different expectations" altogether. "They don't understand Hikayat Merong Mahawangsa. The film was not made for the international audience. In fact, it was meant for domestic and international viewers. It's basically a fusion film that tells an epic journey of a warrior who fulfills his destiny to become one of the great kings in Malay history." As for comments on Amazon.co.uk, Yusry feels it is normal to garner positive or negative reviews and feedback. "Of course, we can't satisfy

everyone's flavour and desire. I'd understand if international viewers were upset watching Hikayat Merong Mahawangsa as I myself was disappointed with the title, too. It was a misleading title, created by the UK distributor and it was beyond my control." He said the new title conjured a different story altogether from the original storyline. She also gave advice on what foods to eat and avoid, and what medications to take to control one’s blood pressure. HIV and AIDS awareness programme — “It Begins with You” commemorated International AIDS Memorial Day 2011 as it marked the conclusion of its 6th year advocacy initiative which includes education to medical students as well as on ground events to reach the public. Spearheaded by pharmaceutical company, MSD, together with the Malaysian Society for HIV Medicine (MaSHM) and the Malaysian AIDS Council (MAC), It Begins With You was first initiated in 2005 to address the worrying doctor to HIV-patients ratio. It also sought to address the stigma surrounding doctors’ interaction and treatment of people living with HIV (PLHIV). Combining the resources and expertise of these partner organisations to address the issues, It Begins With You has touched the lives of PLHIVs in a positive manner. Today, this advocacy initiative has expanded to include medical students from various universities, namely International Medical University (IMU), Universiti Teknologi MARA (UiTM), Universiti Malaya (UM) and Universiti Putra Malaysia (UPM). Background and objectives ● ‘It Begins With You’, initiated in 2005, was conceived in view of the urgent need for more dedicated HIV and AIDS treaters in Malaysia. ● The programme is targeted at students from local medical universities. ● As an HIV and AIDS advocacy platform, the programme has three main objectives: • To increase the number of dedicated HIV and AIDS treaters in the country. • To change participants’ mindsets about people living with HIV (PLHIV). • To inspire participants to make a difference — whether to pursue specialisation in Infectious Diseases or simply in treating PLHIVs with care. ● ‘It Begins With You’ is a collaborative effort between MSD, the Malaysian Society for HIV Medicine (MaSHM) and the Malaysian AIDS Council (MAC). The Programme ‘It Begins With You’ started out with a straightforward agenda to educate students on the current scenario of HIV and AIDS. Today, the programme has expanded to reflect a balanced overview which includes comprehensive perspective on treatment, NGO involvement, patient experience stories and a creative platform to explore students’ perception of the disease. ● The creative platform takes the form of a different competition each year, with entries revolving around the theme ‘Stop AIDS: Keep the promise’. ● There are various components to the program with the first being a medical attachment programme. Selected medical students get the opportunity to gain first-hand experience in treating HIV and AIDS patients at the Infectious Diseases Unit at Hospital Sungai Buloh. ● The second is a pre-awareness session comprising individual sessions conducted at the respective universities. These sessions aim to garner participation and educate medical students on the latest developments of HIV and AIDS both in a clinical and psychosocial sense. These individual events provide the students with a sense of ownership in terms of coordinating the events respectively. ● ‘It Begins With You’ culminates in a one-day event with all universities, where programme partners present information and updates on HIV and AIDS to all the medical students. There are also interactive segments for the medical students to initiate HIV and AIDS education to the public.

Advance Diploma in Midwifery overview of the course: The programme is delivered over a one year period by the MAHSA University College and it is under the Department of Midwifery Programme. The Nurse-student midwife who successfully completes the programme will be awarded a certificate in Advance Diploma in Midwifery. The Nurse student midwife must pass the examination certified by the Midwifery Board of Malaysia. When you are too breathless for your baby She only began to notice she was getting out of breath towards the end of her pregnancy. Walking on the same level wasn’t so bad but when it came to climbing stairs it was a gallant effort each time. It was Aishah’s (not her real name) first pregnancy and naturally she attributed her symptoms solely to the normal physiological process of carrying a baby almost to term. Not long after, came her bundle of joy in the shape of a baby boy. She had been preparing herself for months for his arrival, expecting that motherhood was both exciting and demanding, but nothing could have prepared her for what happened in the weeks that followed. Her symptoms persisted. Two months after the birth of her son, her feet became swollen. By this time, she could hardly walk from her bed to the bathroom. Sleeping was a real struggle, and only happened in a few short periods when sat upright in a sofa. A consultation with her doctors led to the diagnosis of a rare condition affecting the heart called peripartum cardiomyopathy (PPCM). Peripartum is a condition that occurs in the last month of pregnancy and a few months after delivery. Cardiomyopathy is a medical term for a disease or condition affecting the heart muscle. PPCM is, therefore, a condition presenting with heart failure secondary to weakening of the heart muscle occurring sometime at the end of pregnancy or a few months after, as mentioned earlier. The heart is dilated (enlarged) but not always. PPCM is not so well understood. Various theories as to its cause have been postulated, such as viral infection, inflamma-

tion of the heart muscle (myocarditis) and autoimmune process (own immune system acting against the heart). The incidence varies between countries. It can be as high as one in every 350 to 400 live births in Haiti. In Japan, it is about one in 6,000. The only study on the incidence of PPCM in Malaysia is that by Professor Dr. Wan Azman of University Malaya Medical Centre (UMMC) which revealed one case in every 3,000 live births during the study period. PPCM can occur in any woman of any race or childbearing age although it is more common in older women, twin pregnancies and those with preeclampsia (a condition characterised by hypertension, protein in the urine and leg swelling during pregnancy). Diagnosis of heart failure is usually by clinical assessment and supported by echocardiogram (ultrasound of the heart). In the case discussed, Aishah had classical symptoms of heart failure. She was short of breath on exertion, unable to lie down because of breathlessness (orthopnoea) and frequently waking up at night also because of breathlessness. On top of that, she also had leg swelling, another cardinal feature of heart failure. An echocardiogram performed on her revealed a dilated heart which was only pumping less than a quarter of what a normal heart would be able to do. Treatment depends on whether the person is pregnant or not. If PPCM is diagnosed after delivery, then treatment follows the usual management algorithm for heart failure with diuretics to relieve the body and lungs of

excess fluid, and ACE inhibitor and beta-blocking drugs to help improve the heart’s function in the intermediate and long term. Management of PPCM if discovered towards the end of pregnancy is understandably more complicated as the wellbeing of the baby in gestation needs to be considered. The overall increase in blood volume in pregnancy also makes heart failure difficult to treat. So what is the prognosis for someone with PPCM? Is heart failure for life or can a person recover fully from it? As in any heart failure, the risk of death from PPCM is real and usually occurs in the first three months after delivery. Recent studies have shown figures as high as 27 percent at six months. A significant number, however, can achieve full recovery of the heart. In the small UMMC study, three out of eight PPCM patients identified during the study period had restoration of normal heart function. In the United States, over 50 percent of PPCM experience complete recovery and almost all are eventually able to discontinue medications with no resulting relapse and have a normal life expectancy. For the rest, heart failure persists and medications will need to be continued indefinitely. Long term prognosis is also not good. Is it the end of the road as far as future pregnancies are concerned? Very likely so for those who remain symptomatic and with hardly any recovery of heart function on echocardiogram. However, for those who do very well, opinions vary as to whether they are allowed on medical grounds to be pregnant again.

I hope I have not scared any expecting mothers with this article. PPCM is not common. It occurs sporadically, and I have seen two in the last two months, but none in the previous three years. The key is early recognition. If symptoms of heart failure as we discussed earlier occur – anything which is out of proportion from what is expected during a normal pregnancy – and especially if they persist after delivery, then the alarm bells should be ringing. A visit to your doctor is what you need. During the last visit to the clinic, Aishah walked at a normal pace into the consultation room without a hint of breathing difficulty. She was almost unrecognisable from the person who could hardly put two sentences together only a few weeks ago. Whether she recovers fully from this ordeal, only time will tell. What is optimal blood pressure

Dear Dr. Haizal, I am in my mid-forties and I have been monitoring my blood pressure at my company’s panel clinic for the past few months. The readings have always been around 130/85mmHg. From my understanding, this is normal but in one your articles earlier, you mentioned that the optimal blood pressure is below 120/80mmHg. What did you mean by “optimal” and do I really have to get to that level? Bryan Dear Bryan, Hypertension is the single biggest risk factor for illness and death globally. It is im-

portant that this silent killer is detected early and managed appropriately. Yes, a blood pressure below 140/90mmHg is considered normal but the optimal or the most desirable level is below 120/80mmHg. This is where the risk of getting heart attack and stroke is at the lowest. A level in between, such as your usual blood pressure reading of 130/85mmHg, is called prehypertension. This means that you do not have hypertension now but are likely to get it in the future if steps are not taken to address the situation early. The latest guidelines for the management of hypertension have incorporated this relatively new category indicating

its importance in the overall treatment strategy of combating this serious condition. It is, therefore, important that you try to get your blood pressure below 120/80mmHg. However, medicines are not generally indicated for levels such as yours. Like any good hypertension management, always start off by adjusting your lifestyle. Avoid exposure to tobacco smoke, exercise regularly, consume a healthy diet especially one which is low in salt, achieve and maintain a healthy weight and lead a stress-free life. And remember to see your doctor for regular medical checkup. As important as hypertension is, it is only one of many

modifiable risk factors for cardiovascular disease. Others include smoking, abnormal cholesterol levels, obesity, lack of exercise and diabetes. Take charge of your health by monitoring your blood pressure in out-of-clinic settings, such as at home. You might want to invest in an automatic digital blood pressure machine. It gives a more accurate assessment of your true blood pressure. For all you know, your blood pressure is normal at home but higher than usual during your clinic visits – a phenomenon known as white-coat syndrome. This can, of course, be discussed in another Heart2Heart issue.

JOHOR coach Yong Kien Ann wore a wide smile just after the second round draw was conducted at Maba House yesterday. Kien Ann had hoped his lads would not be drawn against Malacca and his wishes came true as the defending champions were roped into Group Y with Kedah and Pahang. Soon after the draw, Kien Ann confidently said Johor would secure a place in the final. "It was a relief as I didn't want us to play against Malacca. Lady Luck must have been on my side," said Kien Ann. "The draw looks good. We don't want to sound cocky but I believe in the team and they are capable of going all the way to the final." Hurting Tiger pulls out of US Open FORMER world No. 1 Tiger Woods (pic), a 14-time Major champion chasing the all-time record of 18 won by Jack Nicklaus, said this morning he will not play in next week's US Open golf tournament. Woods suffered a sprained left knee and strained left Achilles tendon in the third round of the Masters in April and withdrew in pain from last month's US PGA Players Championship after completing only nine holes. "I am extremely disappointed that I won't be playing in the US Open, but it's time for me to listen to my doctors and focus on the future," Woods said in a posting on his website. "I was hopeful that I could play, but if I did, I risk further damage to my left leg. My knee and Achilles tendon are not fully healed." Woods made the announcement with a posting on his Twitter microblogging website, saying: "Not playing in US Open. Very disappointed. Short-term frustration for long-term gain." The injuries have prevented Woods from putting in the practice time he needs to properly prepare for the 111th US Open, which begins a week from Friday at Congressional Country Club in suburban Washington. Winless in 20 months since the revelation of his secret sex life with multiple mistresses that led to his divorce from Elin Nordegren, Woods has fallen to 15th in the world rankings, seventh among Americans. Lille's Hazard, Gervinho attract endless interest from big clubs LILLE manager Rudi Garcia said he does not want his star duo Eden Hazard and Gervinho to leave in the summer. The players formed a vital cog in Lille’s march to the Ligue 1 title in the just concluded campaign and have been linked with a host of clubs, especially in the Premier League. Both Hazard and Gervinho have given hints they would welcome a move to major European clubs, with the former being linked with Liverpool and the latter supposedly on Arsenal’s radar. But Garcia wants to keep them in his squad for next season. N EW Fu l h am m a n a g e r Martin Jol cannot wait for the chance to have another crack at the Premier League and will waste no time in adding to his squad. The 55-year-old former Tottenham boss agreed an initial two-year contract with the option of a further 12 months and replaces Mark Hughes, who resigned last Thursday after just one season at the helm. Jol saw his hopes of taking charge of Fulham following Roy Hodgson's departure a year ago blocked by Ajax, but having left the Dutch club in December, he is relishing a second opportunity in the

English top flight. "I am very happy to be back and especially at a club like Fulham," Jol told the club's website. "I remember coming here with Spurs and I even think I played here when I was a player. Craven Cottage is a real football stadium, I loved going to Fulham. I could have gone to other countries, but I was waiting for the opportunity to come back to England." Young set for Old Trafford MANCHESTER UNITED are on the brink of landing England ace Ashley Young from Aston Villa in a near £20m deal. Aston Villa have reluctantly allowed Young to move on in his quest to play regular Champions League football. Chilean star Sanchez agrees personal terms to join Catalan giants HIGHLY-RATED winger Alexis Sanchez has agreed personal terms with Spanish and European champions Barcelona, claims Udinese president Franco Soldati, just 24 hours after he said the Catalans were in pole position for the Chilean's signature. The deal will be based on a €3 million (RM13.2m) per annum contract, but Soldati insists it does not mean a move is finalised as the two clubs have yet to agree a fee. However, Udinese will not stand in Sanchez's way as he seeks a move to Nou Camp. "From what we were told by the player's agent Fernando Felicevich, Sanchez has an agreement with Barca for a contract worth €3m a season. This does not necessarily mean he will go, but it is what the player wants," Soldati told La Tercera. "We hope to fulfil the player's wish, but I don't think negotiations will be finalised until at least the end of this month." GERMANY all but assured their place at the Euro 2012 finals in Ukraine and Poland as they maintained their 100 per cent qualification record in Group A with a 3-1 win over Azerbaijan in Baku this morning. Azerbaijan, who celebrated 100 years of football with visits from Fifa and Uefa supremos Sepp Blatter and Michel Platini, started well but goals by Mesut Oezil and Mario Gomez in the 30th and 41st minutes gave Joachim Loew’s side a clear lead. Murad Huseynov pulled one back with a minute to go for the hosts but Andre Schurrle restored the twogoal advantage in stoppage time and guaranteed Germany’s seventh victory out of seven games. The result means Germany moved 11 points clear

of third-placed Turkey, who have four games to play. For the record, the most the Germans need is three points from their final three games but they will also qualify if Turkey, the only team who can catch them, fail to win all their matches.
