COMING SOON Easier Access to the Meds Your Patients Need Medication non-adherence – ie, not taking essential medications or not taking them as prescribed – can lead to serious outcomes and, as a result, readmissions to hospitals. Indeed, in this country, nearly one in five hospital readmissions is related to medication issues. p patient’s lack of understandingg about the medication or an inabilityy to obtain the drug. This is especially true in patients with diseases that require complicated medication regimens. Now, a new program at Penn Presbyterian Medical Center has the potential to significantly reduce these occurrences. MyPennPhamacy delivers prescribed medications to inpatients at the time of discharge, eliminating the need to stop at the local pharmacy on the way home and ensuring that the patient not only has the correct medication but also understands how to use it. The MyPennPharmacy staff comprises of outpatient pharmacy pharmacists and a decentralized pharmacy technician to ensure that the correct prescriptions are delivered to the correct patient, prior to discharge. Workingg with the unit-based pharmacist – as well as with nurses, social workers and other staff on the unit – the technician identifies patients who may be eligible to participate, based on specific criteria. The tech explains the program to these patients and works to obtain timely prescriptions. Bedside delivery and payment are also available. The unit-based pharmacist then works to complete Medication Reconciliation and, with wi th med edss in han and, d, edu duca cate tess th thee pa pati tien entt ab abou outt th thei eirr dischargge medications. Over the next few months, we will be ramping our pharmacy staff to include more units throughout thee hosp th hospit itall wit ith h hous housee wi wid ide de imp mplleme lementtat ati tion ion occurring by early 2014.
