In the assessment we ask questions about various areas of your life, such as things you do to take care of your body and other things that affect you. We will then give you advice and information based on your answers, including details of other services that can help you. You can also contact a health advisor to talk in more detail about what is important to you. You don't have to answer any questions you don't want to answer, but the more you do answer, the more advice we can give you.
