Dear colleagues, Thank you for allowing me to serve as your President for the forthcoming 2 years! I am very happy that your delegates unanimously approved the by-laws changes and elected Nelson Piccolo from Brazil as my successor for the General Secretary position. I also want to thank the outgoing and the incoming Board of Directors and EXCO members. So many of them are involved in their national society activities and in challenging training programs and yet they serve IPRAS with great dedication! With all their expertise and their transcultural perspectives together we create a stable fundament in IPRAS. I am sure you noticed the member services that were introduced during the past 1 ½ years, since Zacharias Kaplanidis and his team started to work with us: their enthusiasm for YOU and your work is unparalleled! I am delighted that so many of you feel encouraged to share your ideas and visions with us, not only with the Executive Office but also with the Board of Directors and Executive Committee Members and with me personally. We learn from you every day! The congress in Vancouver turned out to be an outstanding event, due to the untiring efforts and dedication of our Canadian colleagues. Our congratulations and thanks to them! In Vancouver we started a direct communication with our trainees and residents throughout the world. I am happy about their activities. We will soon send out the questionnaire about training conditions worldwide. Please encourage all your trainees to participate! They now have the chance to create a better future for themselves and the next generation. The President of the Association of Plastic Surgeons of India, Dr. Raja Sabapathy, has announced July 15th to be THE PLASTIC RECONSTRUCTIVE SURGERY DAY! On this day, every single one of the 1.300 plastic surgeons in India should operate on one patient for free. We congratulate our Indian colleagues for this unique initiative! I dream of having a world-wide Plastic Reconstructive Surgery Day with all of YOU on July 15th, 2012! Let us make the world a warmer place. NOW! Cordially yours,

I am extremely happy to be a member of IPRAS

As the IPRAS’ Board of Directors commences a new term, I must say that I am extremely happy to be a member of it, as General Secretary of IPRAS.

Another is the will to help, to provide humanitarian aid, here, there, everywhere…. The Plastic Surgeon is the Surgeon who will most likely be present in every corner of this world, where humanitarian aid is needed and provided… We are “naturals” where disposition and availability are concerned, and we are certainly technically prepared for it! Being from Brazil and South America, we are also very, very happy that the next World Congress of Plastic Surgery will take place in Chile, in February 2013, when we are certain that a massive number of our members will be present!!!

Under the leadership of our President, Prof. Marita Eisenmman-Klein, we will be participating in the continuous evolution of Plastic Surgery and Plastic Surgeons around the world. IPRAS unites over 100 Plastic Surgery Societies and several other Societies with specific interests in the fields of Plastic Surgery. We understand that there are several differences throughout the world on how patients can reach a Plastic Surgeon and receive adequate professional and specialized services. The necessity for reconstructive procedures in all areas of the world has been the main drive for many decades as the originator (or consolidator) of aesthetic techniques and procedures. We also know, however, that there are many similarities amongst us, plastic surgeons worldwide! One of them is the ever present will to improve techniques and methods, constantly sharing successful recent and timeproven knowledge with other specialists. This has been done since the early years of training in Plastic Surgery and has continued for the entire Surgeon’s professional life, during congresses and several other meetings. History has shown that colleagues from the most varied places in the world have contributed to our ever growing knowledge. 6

WHAT A YEAR!

EDITORIAL

It began with “what a day!”. Now that day has stretched into a year. We’ve had superior performances by all parts of our team. Beginning with the non-stop, world- circling efforts of our Secretary General, now President, Prof. Marita Eisenmann-Klein, the steady hand of our Treasurer, Dr. Bruce Cunningham, the Secretary General Prof. Nelson Piccolo, the Deputy Secretary General for Europe Prof. Andreas Yiacoumettis, the Chairman of the Scientific Advisory Board Prof. Daniel Marchac our Women For Women leader Dr. Constance Neuhann- Lorenz, and the large assortment of dedicated workers from Greece, to India, to Canada, with stops in smaller but no less important ports. There is no question as to the viability and acceleration of the effectiveness of IPRAS. It would be negligent to pass the opportunity to recognize the highly important management skills of Zita-Congress and its leader, Mr. Zacharias Kaplanidis, along with his entire team, especially my right hand in all my efforts, Ms. Maria Petsa. The problem with naming names in a huge effort is that it is impossible to enumerate all the contributors, but they know who they are and this Journal takes this moment to acknowledge their works. All this is about last year. We now have entered into the real life struggle to not only remain active but to move forward and assume the responsibilities incumbent with the position of the largest organization of plastic surgeons in the world. The experience in Canada will be a platform for our next biannual conference to be held in Chile in 2013. It will be a major challenge to exceed, or even match, our success in Vancouver but that effort has begun many months ago. Once again it will require many shoulders pushing and many backs straining to pull the Society’s growing significance to an even higher level. Somehow my intuition, born out of fiftythree years involvement in plastic surgery, indicates to me a grand success in Chile….but intuition is amorphic. Regarding the Journal I must say I am pleased. I am pleased that so many people have pitched in and responded to my requests for contributions. I have been happy with our series, as we have presented each issue of three outstanding members of great quality:

1) Rising Star….a surgeon under the age of forty-five whose contribution has been notable 2) Pioneer….a surgeon whose contributions have significantly changed the way we do things 3) Senior Ambassador….a surgeon in the twilight of his/her career who can recollect his/her own involvements and contributions and extrapolate them into a vision for the future….a path we’d be wise to follow. I am also pleased by the scientific articles that have come forth. My vision at this time is for the articles to be more of a draft form, pointing out the problem, providing a brief analysis of the pursued solution, displaying results, and finally reaching to the conclusion. This leaves the author free to publish the more complete work in one of our sister journals which is indexed. The time may come when we will change this approach, but for now this is the way it is. We have opened our pages to corporations who wish to present the technical aspects of their products in article form in addition to their buying space for advertising. The area where I would like to see more growth is in the “Letters to the Editor”. It would help me greatly to have more feedback from the readers. Finally, “What a Year”!…. it has been a joy for me to be on this ride and I eagerly anticipate a continuing adventure….so I thank all those who have made it so for me. Dr. Thomas M. The 5th edition of the IPRAS Journal coincides with our Confederation making one step forward in both its management methods and its amplitude of aims and goals. For the very first time since its establishment in 1955, the institution of the President is added to its administrative pyramid.

At the same time, important sub-committees are either founded or given a newly expanded agenda, such as the Humanitarian (IPRAS Foundation), the Board of Trustees, the Scientific Advisory Board, etc. The purpose and goals of these sub-committees are analyzed by ex-Parliamentarian and currently Deputy General Secretary, Prof. Andreas Yiacoumettis. We would just

like to underline the magnitude the aims and goals of IPRAS now have in the field of Plastic, Reconstructive and Aesthetic Surgery worldwide. During the IPRAS World Congress in Vancouver (22-27/5/2011), 14 individual meetings of the Board of Directors, the Executive Committee, The National Delegates, the General assembly meeting and the SubCommittees were held, reaching a record number for management and constitutive meetings in the history of IPRAS. A new record was set for the number of IPRAS members, as in Vancouver, with the addition of the newly-established association of Kazakhstan, the number of the National and Regional Associations reached 100. Since we are referring to Vancouver and the World Congress, we would like to congratulate the organizers for their absolute success on both the organizational level and participation. The participants exceeded the number of 1800, as officially stated by the Organizers of the Vancouver Congress, while the presence of Exhibitors and Sponsors was very dynamic. We would like to congratulate the President of the Canadian Society, Dr. Patricia Bortoluzzi and the Chairman of the Canadian Organizing Committee, Prof. Don Lalonde, Prof. Ronald M. Following a flawless election procedure, the countries that were chosen to host the 2 world congresses were China (Shanghai) for 2015, and Belgium (Brussels) for 2017. We congratulate both and we wholeheartedly wish both National Associations every success. We would also like to thank and congratulate the rest of the candidates for their excellent presentations. Namely, the President of the Hellenic Society of Plastic, Reconstructive and Aesthetic Surgery, Dr. Apostolos Mandrekas, the President of the Indonesian Society, Prof. Theddeus Prasetyono, Prof. Reha Yavuzer of the Turkish Society and Prof. Nicolo Scuderi from the Italian Society together will their assisting teams. It is our belief that all the above countries are perfect for the organization of a future IPRAS World Congress. We encourage them to try again reminding them that Chile is not that far away chronologically.

It appears that the financial success of the Congress guarantees IPRAS an amount that exceeds 200.000 CAD (50% of the net income). This amount gives IPRAS the potential to reinforce its management and humanitarian activities for the period 2011-2012. The Confederation’s Management Office (ZITA Congress) manned the mission to Vancouver with a group of 8 people. They staffed the beautiful booths of IPRAS, Chile 2013 and the 1st European-Chinese Congress that will take place in October 2011. The IPRAS Executive Director, Mr. Zacharias Kaplanidis and the Assistant Executive Director, Mrs. Maria Petsa participated and organized 14 meetings of the Board of Directors, the Executive Committee, the National Delegate Council, Sub-Committees etc. It was a difficult and demanding task, met with absolute success. Many thanks are in order for all the Management Office staff as well as the Congress Organizers for their precious help. As you may well know, Vancouver was the place where the selection procedures for the host countries for the 2015 and 2017 IPRAS World Congresses were completed.

Closing the Vancouver chapter, we would like to wish every success in their difficult and strenuous work to all the new “faces” in the IPRAS administration, starting with the new General Secretary, Dr. Nelson Piccolo (Brazil), and the new Deputy General Secretaries, Prof. Yilin Cao (China), Prof. Ahmed Noreldin (Egypt) and Prof. Andreas Yiacoumettis (Greece). Many more positions were filled with esteemed surgeons like our new Parliamentarian Prof. Norbert Pallua (Germany), the head of the IPRAS Foundation Dr. Christian Echinard (France), the President of the newly established (and IPRAS Organized) ISPRES Prof. Gino Rigotti (Italy), the President of the Scientific Advisory Board Prof. Daniel Marchac (France), and of course the 1st President of IPRAS Prof. Marita Eisenmann-Klein (Germany). Furthermore, in this new era of IPRAS, we wish every success to the Deputy General Secretary Dr. Brian Kinney and the precious and very experienced treasurer Dr. Bruce Cunningham.


Since the publication of our last edition, many important events occurred for the evolution and the work of IPRAS worldwide. In the end of April, our President, Prof. Marita Eisenmann-Klein and the Executive Director, Mr. Zacharias Kaplanidis traveled to Kazakhstan after been invited to an ISAPS Symposium. We would like to thank ISAPS President, Dr. Jan Poell and General Secretary, Dr. Miodrag Colic for everything as well as giving us the chance to meet in this beautiful and interesting country of Central Asia. The hard work of the IPRAS President and the Executive Director started much earlier to the journey and arrival at ex-capital Almaty, resulting in the foundation of the National Association of Plastic Surgery of Kazakhstan with 34 members and their admission as the 100th member of IPRAS by the IPRAS National Delegate Council. We wish them the best results in their targets. IPRAS and its management office will always be at their disposal for any kind of support. Another journey included traveling to Armenia at the end of June (28-30) in order to participate in the National Congress in Yerevan. IPRAS and its Management Office supported this organization in every way and we must report that it was successful from many aspects. The congress took place at the exceptional premises of the University of Yerevan (Medical School) with participation from approximately 15 countries, such as Georgia, Russia, South Korea, Egypt, Turkey, Greece, Brazil etc. The Congress impressed all participants as it had a rich and interesting scientific programme (26 International Faculty Members), enjoyed the unprecedented attendance of the National Media (7 TV Channels & 25 Journalists) which covered all activities for 2 days, and ended with a rather successful and interesting Public Day (with the participation of approximately 145 civilians). Thanks to IPRAS’ subsidy, approximately 39 residents from Armenia and Georgia attended the congress free of charge. IPRAS, also, subsidized the congress with

10.000 € in total, an amount that had been accounted for and endorsed by the Executive Committee in Doha in December 2010. This way, the Confederation proved its sensitivity in supporting smaller Associations/ Members once more. ZITA Congress, who handled all the organization of the congress, also had an important financial and organizational contribution of 8000 €. Among others, Prof. Nelson Piccolo (Brazil) in his first official appearance as new General Secretary, the Deputy General Secretary, Prof. Andreas Yiacoumettis (Greece), the Deputy General Secretary, Prof. Ahmed Adel Noreldin (Egypt), the members of the executive committee, Dr. Suk Wha Kim (South Korea), Dr. Christian Echinard (France), Prof. Daniel Marchac (France), Prof. Mohamed Zaki (Egypt), and the President of IPRAS, Prof. Marita Eisenmann-Klein (Germany), honored the beautiful city of Yerevan with their participation. The hospitality shown by the

Armenian Association was exemplary and we would like to thank all of its Members, the President of the Association, Dr. Armen Hovhannisyan and the President of the Congress, Prof. Gevorg Yaghhyan. Furthermore, we would also like to thank the Rector of the Yerevan State Medical University, Prof. Derenik Dumanyan, who perfectly hosted the Congress at its excellent university establishments. Despite its size, the congress left impressions and memories to be remembered for many years. Armenia, IPRAS will always be with you. Thank you for your precious experiences and your hospitality in your beautiful and historic country.

In June and on the beautiful island of Mykonos, EURAPS held their annual congress, which for many years has been a very important event in the field of Aesthetic Plastic Surgery. IPRAS supported this congress immediately and supported the organization of 2011. IPRAS issued relevant reports in all the editions of the IPRAS Journal leading to the EURAPS Congress. Issue 5

The Management Office sent several newsletters to the IPRAS database (37.000 plastic surgeons) and ZITA Congress’s efforts, as partially responsible for the organization, contributed so that the 22nd Annual EURAPS Meeting in Mykonos in 2011 was the largest in participants numbers in the history of the institution. Some of its impressive numbers include: approximately 450 participants, 170 accompanying persons, 17 exhibition companies and 50 exhibitors. Congratulations to all the key persons behind its success, Prof. Moshe Kon (General Secretary), Prof. Othon Papadopoulos (Congress President) and Prof. Julia Terzis (EURAPS President). ZITA Congress would like to thank them for their excellent cooperation and wishes Prof. Milomir Nincovic and his assistant Dr. Sarah Lorenz every success in EURAPS 2012 in Munich. We also wish the best success in his duties to the new EURAPS General Secretary, Prof. Manfred Frey.

IPRAS endorsed and ZITA Congress organized the 14th ESLAS Congress of the European Society for Laser Aesthetic Surgery, which also took place in Mykonos in June. This was a specialty-based congress that satisfied all participants from 19 countries, and we would like to congratulate the President, Dr. Vakis Kontoes for his efforts and to express our sincere support to the efforts and the work of the entire administration. IPRAS will continue to work with ESLAS, since its members are mainly plastic surgeons. Moreover, ESLAS will be participating at the upcoming 1st European-Chinese Congress of Plastic, Reconstructive and Aesthetic Surgery on 27-30/10/2011. To conclude the IPRAS Management Office report for the period April – June 2011, we would like to express our support to all the countries and our members that continue to tackle the consequences that the natural, social and political problems forced upon them in the previous months. Libya, Japan, Syria, Egypt, Tunisia. We haven’t forgotten about you. We are always at your side. We have underlined in these pages in the past, that in hard financial & political times the role of an International Scientific Institution such as IPRAS is very important. IPRAS, with 100 National Members and approximately 40.000 plastic surgeons, constitutes a large scientific, social and economic entity, whose decisive contribution is capable of “healing” wounds, globally. With the new structure of IPRAS with its Sub-Committees and the establishment of the President, this is exactly where the Confederation aims. Therefore, during these difficult times, Leon Tolstoy’s words can inspire us all. “I now understand that my welfare is only possible if I acknowledge my unity with all the people of the world without exception”. The 2011 Vancouver World Congress A “Landmark” in the history of IPRAS

Ever since IPRAS was founded at the beginning of the fifties in the last century, many Plastic Surgeons have participated in the World Congresses over these years. This time in Vancouver, it was the biggest gathering ever! Everything contributed to this success. The excellent organization by the Canadians, with Don Lalonde at the helm, the introduction of new ideas in the scientific program, the modern venue, the social events, the beautiful city of Vancouver and the surrounding area, to mention but a few. We congratulate the Organizing Committee and the Canadian Society for this success. It was also a “Landmark” in the history of IPRAS. Many things happened in Vancouver during the business meetings, which introduced new ideas, new procedures and new officers in the ExCo and Subcommittees, eager to work for the aims of our precious organization. According to the new revised bylaws, starting from the

Prof Marita Eisenmann-Klein was unanimously elected as the first President of IPRAS by the Council of Delegates and the Executive Committee and approved by the General Assembly. Her outstanding service as General Secretary was gratefully appreciated by everybody and the enthusiastic standing ovation following her election was more than an expression of admiration and continued support. In the position of General Secretary Nelson Piccolo was also unanimously elected, the first colleague from South America to serve in this position. For a long time now, it has been felt by all and almost became an obligation, that Senior Members with distinguished long service to the Specialty and IPRAS, disserve appreciation and honor from everybody in the profession and more importantly by our Global organization, which up to now has failed to meet this duty. Hence, the bestowment of this lifetime appointment as IPRAS TRUSTEE which has now come to fulfill this debit. The Board of Trustees (no more than a hundred) will be a “think tank” group of esteemed colleagues who with their experience and wisdom, will deliver opinions and statements on highly important issues concerning our specialty. Their first task will be the development of a “Code of Ethics” . The first to receive this honor are: Garry Brody (US), Güler Gürsu (Turkey), Paulino Morales (Argentina), Daniel Marchac (France), Jean-Philippe Nicolai (Netherlands)and Mohamed Zaki (Egypt). Fernando Ortiz Monasterio (Mexico), Michel Costagliola (France), Jean-Paul Bossé, (Canada) were honored in “in absentia” as they were unable to attend in Vancouver and they will receive their plaquette at the opening ceremony of a future major IPRAS event. The Scientific Advisory Board is comprised of colleagues who are charged with the job of advising the BOD, the ExCo and the membership at large on scientific issues. The Board will Supervise the scientific programs of the biennial Congress and all meetings organized by the 14

BOD. Members of the Scientific Board are expected to assist in the formation of a position statement on scientific issues of concern. The first chairman of this group is Daniel Marchac from France. During the Vancouver Congress we witnessed the birth of a new Society, which will function within the IPRAS structure. This is the “International Society of Plastic Regenerative Surgery” (ISPRES). The inaugural meeting was held on May 26th, 2011 during which the first ISPRES ExCo was elected with Gino Rigotti as President, Roger Khouri as vice President and Sydney Coleman as Secretary. Colleagues from all over the world interested in this modern and inspiring topic are already applying in large numbers to become members of this new Society. The Council of Delegates met twice during the Congress. Among other business the candidate societies for organizing the 2015 and 2017 presented their bids. For the first time the bidding was made well in advance in order to allow the winners ample time for preparation. The presenters of the bids were given time to promote their applications to the Council with audiovisual presentations and booths at the Trade Exhibition site. Between the two meetings of the Council enough time was given to the delegates to consult with their societies regarding their preferences. Countries bidding for the 2015 event were China, Greece and Indonesia. For the 2017 were Belgium, China Italy and Turkey. The winners following a secret vote were China for 2015 (Shanghai) and Belgium for 2017 (Brussels). Warm congratulations are in order to the winners. We are all looking forward to a great success in the organization of these top events of our specialty. The new ExCo and the new members of the subcommittees met in the afternoon of Friday May 27th for a very important deliberation regarding the role and the action plan of each of the Subcommittees. Regarding “Training & Accreditation” and “Board certification” the special subcommittees will focus on developing policies and strategies after Data collection and information from the National Authorities. Help from National Societies belonging to IPRAS on this matter will be very valuable. Issues regarding the practice of the profession will be covered by subcommittees such as the “Guidelines SubCommittee”, the “Specialty Protection Sub Committee”,

the “Quality Management Sub Committee” and the “Cooperation with other societies Sub Committee”. The “Communication subcommittee” made valuable suggestions towards making IPRAS more known to the membership, the various authorities and to the public as well. The newly founded IPRAS Media Office run by our Executive Management will be a great asset in pursuing this task. The Humanitarian Groups of IPRAS also met during the Congress. We have all witnessed the admirable activities of all these special colleagues who spare no effort in pursuing their philanthropic tasks. The protagonist Christian Echinard together with our President and others are continuously engaged in organizing missions and providing help wherever is needed. Special subcommittees on this subject are: the “IPRAS Foundation (IPRAF)”, the “Coordination of Humanitarian Projects Sub Committee”, the “IPRAS Emergency Task Force” and the “Women for Women Sub Committee”. IQUAM (International Committee for Quality Assurance and Medical Devices in Plastic Surgery) also held a meeting chaired by its President Constance Neuhann-Lorenz (Germany). Among other issues, the organization of the next meeting and the consensus conference which will take place in Athens in the autumn of 2012 was discussed. It is evident from the above that IPRAS has entered a new era. Its presence and activities have grown impressively in the last couple of years. New policies, closer communication with individual plastic surgeons on a regular basis, support and responsiveness, the publications of the IPRAS-Journal sent to over one hundred National Societies’ members, the Industry and health authorities, are the tools for achieving even greater results. Last but not least and surely the most important decision we have made, was to entrust to ZCongress the executive management of IPRAS. Their enthusiasm and effectiveness together with the support of the officers of the IPRAS hierarchy and the individual members, have been the driving force in achieving many of our assigned tasks. When and why did you decide to choose plastic surgery as your specialty? In 1999/2000 while still in training for my postgraduate degree in general surgery. I was able to attend an “operation smile� mission in Kenya and was fascinated by the results of the surgery. You are one among 16 plastic surgeons in Kenya. You must be a very busy lady. How many hours do you work per week? Roughly about 40 hours a week; sometimes more sometimes less.

You also are a working Mom. Are you ambivalent sometimes whether staying at home with the baby might have been the better decision? Being in a third world country I did not have much choice as every little extra coin goes a long way to help in meeting our family expenses. However, there are days I wish I did not have to go to work and could stay at home and attend to my son. I try, as much as possible, to spend time with him in the evenings. Who shares your domestic work load with you? I have a domestic worker who lives with me. I also live

with some of my extended family that helps me look after the baby. They have been very helpful and supportive. You always seem to be relaxed, calm and composed. Are you a happy person?

is difficult to get funds to train outside our region. This can be done in collaboration with institutions such as Kenyatta National Hospital and the University of Nairobi, while IPRAS could help us with faculty to teach and examine.

I like to think so and this far I am grateful to God for helping me achieve some of my life’s dreams. Share with us your experience with the “Women for Women” camp at Kenyatta National hospital, which you organized in 2010! It was a wonderful experience. I was able to work and interact with Marita, Debra and Vanessa. We screened about 10 patients. We were able to exchange ideas and share knowledge and generally bond with each other. How did the hospital staff in general react to the “Women for Women” team? They were welcoming and appreciative of this initiative. I have severally been asked by my colleagues when the next one will be organized. We would not mind if this became part of our unit’s yearly calendar at Kenyatta National Hospital.

Do you experience an increased demand for aesthetic surgery in Kenya? Oh yes! As more ladies are becoming economically empowered, they are asking for these surgeries even if they have to save for them. Internet and the media have also allowed them to access a lot of information about the various kinds of procedures and this has increased their interest. Tell us about your vision about plastic surgery in Kenya in 10 years from now!

What happened with the patients who were operated during the camp in October 2010? They all did well postoperatively. Three of them had some minor wound infection that was cleared. For one girl with post burn contracture of the hand we lost some of the correction. What expectations do you have regarding the IPRAS support for the Kenyan Society of Plastic Surgery? There is a big paucity of plastic surgeons in Sub-Saharan Africa and this gap needs to be filled. My hope is that IPRAS would support the establishment of a training centre in this region. It would offer a fellowship for trained general surgeons who are interested in plastic surgery, as it 22

That plastic surgery would be a more easily available service to our communities that need them so that there should be at least a plastic surgeon in all the 8 provincial hospitals in our Country. This will entail a lot of training and also increase the level of interest in this subspecialty. By then, I hope we will be more than 50 plastic surgeons in Kenya and that we will affect our region by helping to train plastic surgeons from the Sub-Saharan region. And what are the visions you have for your children? That they would grow up to be God fearing men and women who would excel in all their endeavours. Is there a motto in your life? To be a woman of excellence and to be a role model for young women in my family, my community and in the world as God gives me opportunity. Thank you! I graduated from the Medical School of the University of Sao Paulo in 1957 and spent another four years training in Plastic Surgery, so it took me 23 years of investment to feel qualified to open my office. With my colleague and friend Dr Merlin Keppke, as pioneers, in 1962 we moved to Campinas City, a city 65 miles from Sao Paulo with 230.0000 inhabitants at that time, and no plastic surgeons. Today there are around 160 Plastic Surgeons working in this area, four Universities and four Plastic Surgery training departments for 1.200.000 inhabitants. Dr Merlin and I created the first Plastic Surgery training program in the Medical School of Campinas University in 1965. After 1,5 years we resigned from this department to dedicate ourselves to private activities. Another point about our partnership: it lasted for 32 years without any misunderstanding. This period finished in 1992, when I closed my office and moved to Bologna to obtain my Board License to practice in Italy. In 1997, my plans changed again, taking me back to Brazil, where I practiced in Sao Paulo for 15 years until 2009, when I decided to retire. For this action there is an interesting message: “When the act of operating gives you not much pleasure and the pleasure of operating gives you too much trouble....it is time to stop”. Months later, the Directorial Board of our Society invited me to be the new Editor - in - Chief of the Brazilian Journal of Plastic Surgery. This trimestrial journal was at the Lilac B international level for 22 years, with no more than 14 articles per issue. After two and a half years we got an upgrade to Scielo Level, with around thirty articles per issue. This year the journal will be published in English, and is scheduled to be bimonthly. Our Brazilian colleague, Dr Antonio Roberto Bozola, wrote an interesting point about our journal: “The Brazilian Society of Plastic Surgery is the second biggest in the world, with five thousand members, but not with the second best quality journal”. A huge job is still waiting for us. It is an “almost impossible mission”.

Changing the subject, regarding our professional speciality, there are three basic groups of Plastic Surgeons. First, those who love to operate, have countless excellent results, but don’t write or publish, but love to present their best results when invited and eventually attend scientific meetings. Second, those who do the opposite; have an extensive curriculum with dozens to hundreds of publications, countless scientific meeting presentations, unlimited theoretic Plastic Surgery culture, most with a University career , but are not so avid with the scalpel and finally have less time for private clinical work, with all the consequences. Specifically “private patients” means those that “Knock on the doctor’s door”. Finally there is a third group, where I have belonged during the past 55 years of my professional life, consisting of Plastic Surgeons who like to both operate and publish. It is the most active and sacrificed group. After ten hours of daily routine, the third period of the day is dedicated for weeks, months and years to preparing scientific contributions. It never ends. There are no explanations. Each one has their own limits and enthusiasm to go on. Before I continue, I am quoting a phrase written by Mark Twain in 1850. It is a lesson in discretion and modesty that I have always followed - “I don’t like to talk about myself but I don’t feel discomfort if the others do”. Breaking this concept and attending Tom Biggs’s request the article above describes our activities in the Brazilian Society of Plastic Surgery, ISAPS, IPRAS and our scientific contributions. SENIOR AMBASSADOR. RICARDO BAROUDI This editor has had the privilege of the friendship of Ricardo Baroudi since his first (of thirty-six) trips to Brazil. I was in Brazil for the annual meeting of their National Society in December of 1971 In San Jose Do Rio Preto. After the meeting I made an auto trip to Iguasu Falls with Melchiades Oliviera and his two sons, passing through Campinas where began one of the most fortunate of many fortunate occurrences of my life….I met Ricado Baroudi. He was living and practicing in this city near Sao Paulo. Since that day forty years ago, we have been in steady communication, and I have benefited from his wisdom, tolerance, and overall innate sense of agape more than I can write with my simple words. I can only say that he is as close as is possible to the template I and all other plastic surgeons should strive.

Humanitarian Plastic Surgery Networks: a world of solidarity

IPRAS now involves 100 National and Regional Plastic Surgery Societies from all over the world, with nearly forty thousand surgeons. Some of these countries are rich, some are poor… Developing countries have nearly no or very little access to Plastic and Reconstructive Surgery. Most of them, however, need this type of specific care very dramatically (acute burns, burn contractures, congenital malformations, nomas, post trauma defects…). One of the goals of our scientific societies should be to provide them with local training, surgical expertise and help, and technical assistance. In 2007, ESPRAS, the European Society, developed a humanitarian network, SHARE (Surgical Humanitarian Aid Resources Europe), to include the most efficient European surgical NGOs.

In 2010, IPRAS, the International Confederation, launched HUGS, the “Humanitarian Union for Global Surgery”, with the support of Marita Eisenmann-Klein, our actual President, and Nelson Piccolo, our Secretary General. HUGS is a global network aiming to provide surgical help to the developing countries. More recently, the idea of an Emergency Force, ran by IPRAS, for disaster relief in the world, was proposed; it will be the IPRAS Emergency Force, with surgical volunteers and mobile operating theaters. In this beautiful but terrible world, everything must be done to help the poorest countries to develop a better sanitary system. There is an enormous need for surgical help. There is a need for teaching surgery, a need for reconstructing surgical wards and operating theaters, a need for rehabilitation. There is also a need for

After the earthquake in Haiti: a need for rehabilitation prevention of major risks, of surgical diseases, of severe burns, flame suicides and acid attacks… There is a need for emergency support and disaster relief, a need for active cooperation with the developing countries... IPRAS is not a Charity organization. IPRAS is a scientific society, the most important scientific Plastic Surgery society in the world, gathering over 38 000 plastic surgeons. IPRAS is not a Charity, but the role of this global organization, besides its scientific activities, and because it is a medical non-profit association, is to help the poor countries. IPRAS Foundation (formerly IPRAF), will participate in the development of the humanitarian activities carried out by the Plastic Surgeons of this planet. Its role is not to sponsor one specific NGO, but to enhance humanitarian activity through worldwide networks, for regular Plastic

Reconstructive Surgery and Emergency Programs. It will organize lectures, courses, and round tables on this topic during the IPRAS endorsed meetings, including, if possible, a plenary session on humanitarian Plastic Surgery. This has already been done in Delhi, Rhodes, and Vancouver… and will be set up in Yerevan, Beijing, Santiago de Chile, Brussels, and many other places in the near future. The Foundation will support and help the three main projects that were elected as priority humanitarian projects in Vancouver, during the last IPRAS World Congress, last May: HUGS, IPRAS EMERGENCY FORCE, and Women for Women. HUGS (Humanitarian Union for Global Surgery) is the name of the project organized by the IPRAS subcommittee for humanitarian help. It will gather big as well as smaller NGOs, such as V.I.P.S., Interplast, Operation Smile, HumaniTerra, Medecins du Monde – Operation Sourire, Sentinelle, Women for Women, the Noma Foundation, Rotaplas and others, in a world network. Its aims are to federate all humanitarian Plastic Surgery activity under the umbrella of the biggest Plastic Surgery society, IPRAS. This will allow exchanges and joint activities, through the IPRAS website and through specific meetings. It will therefore be a great help to all our colleagues in the developing countries, but will also help young Plastic Surgeons to participate in difficult surgical camps under the guidance of senior surgeons. A specific database will be set up. It will include all surgical camps in the world, and Plastic Surgery actions. It will also contain world data per treated pathologies: Nomas, acute burns, burn contractures, congenital hands, cleft lips Issue 5

We are actually working on the mobile operating theater project. One sample of these O.Ts. is right now been built in Haiti, at the University Hospital, and will allow HUGS and SHARE teams to operate very soon. This prototype will be then developed, and carried by boat or

It will be composed of Plastic Surgeons from all over the world, willing to help and provide surgical help immediately after a disaster. It will be a real emergency program, for which we will need mobile structures, such as transportable operating theaters and tents, but also surgical material and consumables. It should be able to go on the site of the disaster (earthquakes, fires, tsunamis, conflicts, famines….) as soon as possible. An appeal to all medical and paramedical companies will be made, to support and help, financially and logistically, this emergency force. The human resources of the IPRAS Emergency Force will be a group of IPRAS volunteer Plastic Surgeons, rapidly available, good willing and ready to go. This is of course not easy to do, because most of the Plastic Surgeons are very busy, and cannot leave their office, on demand, at once, for several days or weeks. However this has already been organized by SHARE, the European 26

plane on the field, as soon as possible after the disasters. A proposal has been made for the IPRAS Foundation to sponsor this common project.

The “Women for Women” project This excellent project has been founded by Marita Eisenmann-Klein and Constance Neuhann-Lorenz, a few years ago, under the umbrella of IPRAS. It is based on the idea, and the real fact, that in some countries (particularly in the developing world) female patients prefer or can only be operated on by women surgeons. A group of women surgeons belonging to IPRAS has decided to join their forces in order to achieve this task. They are currently travelling through numerous countries and are operating on women on a regular basis. Several joint surgical camps have been organized with them, among others a recent co-operation camp in Bangladesh with HumaniTerra.

Fund raising by the IPRAS Foundation We will need a lot of money in order to develop these projects and make them a success. The IPRAS foundation, recently renewed in Vancouver, will collect funds. Some of these funds will come from private donations, through a call to IPRAS members, and through a proposal to drugs companies. This will be done with the help of our partner Zita Congress, which is very much involved in Humanitarian causes. Another part of the funds will come through institutional international sponsors, such as the European Community, the U.N. agencies, international foundations…This goal will be long and not easy to achieve… but we’ll make it.

We have to join our forces… Plastic surgeons are participating in hundreds surgical humanitarian camps and missions every year. They are performing thousands of surgical procedures, free of charge, all around the world. This world of solidarity, born within our own plastic surgery group, is not yet recognized enough. We have to show that Plastic Surgery is not only cosmetic surgery… but that the best aesthetic surgeons in the world can also participate in a global project of free

In Afghanistan, gathering women for a flame suicide prevention campaign: a need for prevention reconstructive surgery for the poorest countries. We have to demonstrate that many of us can be available, in case of disaster, in world consensus of sharing. IPRAS will be the ferment of this chain of surgical solidarity all around the world. Women For Women by IPRAS: 2nd Mission - Chilmari Bangladesh April 23-30, 2011

In Collaboration with “Friendship” and the Emirates Floating Hospital Dr. Constance Neuhann-Lorenz, MD

About the Region of Chilmari Bangladesh, a country of “a thousand rivers” with much of its geography embedded in water, is recognized as the most densely populated country in the world by the World Health Organization. With a surface area of only 144,000 square kilometers and an escalating population of 160 million, diseases of poverty and overcrowding are rampant in the country. Living even below the poverty cutoff line for Bangladesh are the inhabitants of the northern riverbank of the Ganges-Brahmaputra delta. The people of this region are nomadic in nature and dwell on semi-permanent islands known in Bengali as “Chars”. (Figure 1) These islets, formed from the melting snows of the Himalayas, undergo constant formation and erosion from its powerful currents, and typically have a lifespan of only months to a few years. Due to its vulnerable position at the northernmost tip of Bangladesh, Chilmari, in the district of Kurigram, is a frequent victim of natural disasters, such as flooding and cyclones. This was the location of the ninth mission for Women for Women by the International Confederation for Plastic Reconstructive & Aesthetic Surgery (IPRAS).

About the bond between Women for Women and Friendship The inhabitants of the northern riverbank and char areas of Bangladesh are amongst some of the most vulnerable and marginalized people in the world. Recognizing the serious deficiencies in food supply, running water, electricity, basic sanitation and living infrastructure within the chars, Women for Women and Friendship teamed up for a second successful surgical mission between April 23rd to 30th. The local planning and on site organization in anticipation for our mission were coordinated by Friendship, a value-based Bangladeshi organization established in 1998 under the leadership of Mrs. Runa Khan. To fulfill its aim of providing essential basic services to the most inaccessible areas of the country, Friendship delivers both primary and secondary healthcare to the people living on the shifting islands through its floating hospital at the mouth of 28

the Brahmaputra River. Through a generous startup donation and sustained funding from the Emirates Airlines, the Emirates Floating Hospital (EFH) has provided essential medical care to around 4 million people of the chars since November of 2008. (Figure 2) The ability of the EFH to be self-contained and mobile affords the hospital the unique ability to relocate every few months to serve a different char, in addition to surviving natural disasters, common to this region. The EFH is a suitably equipped hospital with one fulltime onsite medical doctor, one optometrist, 4 ward nurses, 2 OR nurses, 2 operating rooms, a pharmacy and a radiology department. (Figure 3) In addition, with the invitation of Friendship, there is a regular roster of foreign missionary teams that provide specialized medical and surgical care a few times a year, ranging from gynecology, orthopedics, cleft lip and palate and, finally, plastic and reconstructive surgery by Women For Women.

About Mission Chilmari Bangladesh In Bangladesh and other developing countries of the world, it is not uncommon for women to suffer from disfiguring physical injuries as a result of ethnic, social, marital, and other domestic disputes. Further compounding the issue of inadequate prevention and

treatment of these avoidable injuries, it is known that female victims generally do not actively travel away from home to seek medical attention. As a result, they further exclude themselves from society and spend the rest of their lives hiding their physical deformities and tolerating the shame inflicted upon them by their perpetrators. The mission of Women For Women, therefore, aims to “help women suffering from physical injuries as a result of domestic abuse, war, or other socio-cultural reasons in third-world countries�. Women For Women (WFW) is a charity program, supported by IPRAS, and its missions are carried out by an international core group of socially conscious female plastic surgeons, who share a common vision to restore the skin and the lives of female victims of war and social crimes. The most common crime of female domestic violence in Bangladesh and other parts of South East Asia are burns, particularly acid burns to the face. This vicious form of attack is an unconscionable act that aims to disfigure the most presentable element of the female body. This is a large-scale problem that has afflicted Bangladeshi women for generations, from young to old, and from north to south. Only recently has it received its much-needed corrective attention, and today there

is a national society based in Dhaka, the capital of Bangladesh, known as the Acid Survivors Foundation. (http://www.acidsurvivors.org) During this mission 6 months ago, the Women for Women team met with ASF Founder Mrs. Monira Raman and her team to evaluate synergies and visited the ASF-center and the surgical facilities.

About the WFW Team to Chilmari Team WFW arrived in Chilmari on April 23rd and began screening potential patients the same day on the EFH. The surgical team consisted of two senior plastic surgeons, Dr. Connie Neuhann-Lorenz (President of WFW, Munich, Germany), Dr. Toni Zhong (Plastic, Reconstructive and Microsurgeon, University of Toronto, Canada), and junior plastic surgeon was Dr. Michelle Locke from Auckland, New Zealand currently completing her fellowship training at the University of Toronto. All three other members of the surgical team came from Munich, Germany, and consisted of Drs. Carola Stenzel and Christina Frey (anesthesiologists) and Mrs. Gabi Kerlstetter, a senior OR nurse. In addition, our team was privileged to have the company of Ms. Ann Christine Weorhl during the entire mission, Issue 5

maximize the safety of the surgery and minimize OR duration, the three surgeons worked in concert and completed the entire surgery within an acceptable time frame with minimal blood loss. The patient was ambulating with assistance by postoperative day 1 and tolerating solid foods by postoperative day 3. Most importantly, for the first time in 3 years, she was able to provide personal hygiene care for herself, a right that should be basic to any adult human being.

Case 2: Dowry Dispute - Burns to the Face and Neck The Operating Theatre on the EFH

About the Patients For five consecutive days, from April 24th to April 28th, our team performed approximately 61 procedures in 32 patients. Many of these patients had multiple sites of burn injuries. The ages of the patients ranged from 2 to 70, and the most commonly performed procedure was burn contracture release and reconstruction using a full thickness skin graft. Since the intent of the WFW is not to be an exclusionary program embedded in feministic ideology, we also performed upper extremity burn contracture releases for two little boys who presented to EFH.

Case 1: Accidental Soot Burns to the Perineum The most complex and demanding procedure of this mission involved the release of a severe full thickness soot burn to the perineum of a 17 year-old girl. She had an extensive burn contracture of the perineum that resulted in webbing of the proximal 1/3 of her inner thigh skin. (Figure 4a) Preoperatively, she had approximately 25 degrees of abduction between her proximal legs. Functionally, due to the distal extent of the webbing in her groin and both inner thighs, she was unable to provide independent perineal care. As a result, she was rendered to an infantile status where she had to have her basic personal hygiene needs provided by her family members. After reviewing her surgical options which were limited due to the presence of full thickness burns to both her thighs and the difficult nature of healing in the perineal region, we decided to reconstruct the anticipated large perineum defect caused by release of the contracture using a pedicled right vertical rectus abdominus myocutaneous flap (VRAM). (Figure 4b) Since the EFH ORs are not equipped with ventilators which excluded the option of providing general anesthesia and paralysis to the patient, we were relegated to performing the entire surgery under regional anesthesia and conscious sedation. (Figure 4c) In addition, to 30

A 22-year-old girl was attacked by her mother-in-law and sister-in-law the day after her wedding, by setting fire to her veiled face and sari, as a result of an unsettled dowry dispute. As a result, she sustained third degree burns to her face, neck, chest and arms. After barely surviving the initial extensive burns, she was finally repatriated by her own family for the remainder of her recovery. Feeling ashamed and worthless with her physical scars, her family had taken her to the EFH to seek our reconstructive expertise. (Figure 5a,b) She underwent release of severe burn contractures to the right side of neck with local flaps and FTSG, ear reconstruction using a superiorly based pedicled tubed flap, and release of flexion contracture of her index finger. (Figure 6a,b) Despite our reconstructive efforts at restoring some semblance of normalcy to her face and hand, she will likely remain a woman who will be permanently physically and emotionally scarred by this tragic and preventable assault.

The Future In a society built on thousands of traditional rituals and customs, intentional burn attacks on women is one ritualistic practice that must be eliminated. Even today, one Bangladeshi woman is attacked with acid every two days. Especially in the “char” regions of Northern Bangladesh, where its residents have been completely neglected by the government and foreign non-government organizations (NGO), WFW intends to strengthen its loco-regional presence in assisting marginalized female patients who have nowhere else to turn. With the support of local organizations such as Friendship, and continued funding from Emirates Airlines and other sponsors, WFW team aims establish sustainability by educating, preventing, treating, and reintegrating female victims of burn injuries. Andrea, together with Women for Women members Alexes Hazen and Haydé Hirmand, with the full support of Andrea`s family, her husband Martin and her sons Michael and Luka, decorated the gallery with posters informing about Women for Women. The unique flower arrangements were done by Alexes Hazen, right after stepping off the plane, after presenting at the EURAPS Congress in Mykonos. We were all enchanted by Andrea`s sons, who assisted to welcome the guests and acted as the perfect hosts

“INTERPLAST – Germany” “INTERPLAST – Germany” performs free plastic reconstructive surgery for social underprivileged and needy patients in developing countries. The volunteering medical personnel of this NGO (plastic surgeons, anaesthetists, operating room and anaesthetic nurses and other specialists) work free of charge, usually during their annual holiday or after retirement. Travel expenses, medical supplies and instruments are funded by donations raised in Germany from industry as well as the general public. In the last years we organized about 60 camps per year all over the world. Specialists of different faculties join the 1600 INTERPLAST members and share their expertise for the benefit of the poor. INTERPLAST teams provide reconstructive operations that transform the lives of children with physical disabilities and thereby improve the future of their families. INTERPLAST has no financial, political, racial or religious interest. The aim of the surgery is to improve function, and not to do cosmetic surgery. Cooperation with local medical staff and working in their hospitals close to the patients´ home is efficient and offers education in a specialist field for all involved. Apart from medical staff, local volunteers with social commitment are essential for the preparation and the smooth running of a successful INTERPLAST camp which should be a humanitarian joint venture. Interplast supports the IPRAS guidelines for humanitarian work which are: I. Registration of humanitarian missions to facilitate and better coordinate humanitarian activities involving plastic surgeons all over the world. II. Improvement of the quality of humanitarian aid given by experienced plastic surgeons so that no harm will be done. In the humanitarian camp there is no place for cosmetic surgery and competition with the local plastic surgeons should be avoided. The humanitarian aid provided should lead to sustainable development of reconstructive plastic surgery in the receiving countries. Therefore Interplast is operating accordingly. Since 2008 all Interplast camps are registered in advance. Quality report with complication review is mandatory after every camp. Finally, integration of the local colleagues is always sought, but, unfortunately, not always managed. André Borsche Interplast Germany

A pilot survey was conducted to evaluate the trainees’ opinions on the adequacy of their own training programs. This survey included 32 residents representing Plastic Surgery training programs from 9 different countries. The results of this evaluation were of great interest and illustrated the potential improvements that may be required to deliver more effective training

There was a general consensus amongst trainees, that progression within the program should be competency-based, however it was acknowledged that limited exposure or lack of structured training may have major implications on this. When trainees were asked about the adequacy of their training in certain areas, aesthetic, craniofacial, and paediatric Plastic Surgery, were areas highlighted as deficient amongst trainees. Although the conclusions that can be drawn from this survey are limited due to the small numbers

of respondents, the results give scope for further investigations on a much larger scale. The survey may be regarded as a platform for trainees to voice their concerns and opinions on current and future practices within training programs. A follow-up survey may need to be conducted, in which trainees from all over the world can be invited to disclose their opinions and suggest improvements for the current training programs, the results of which may produce a highly beneficial impact on the future of Plastic Surgery education and training opportunities on a global scale. The Conference was much more than scientific presentations and educational classes. It was also an excellent opportunity to network with trainees from all over the world. Over the past two years Dr Don LalondeChairman of the Canadian Organizing Committee along with an enthusiastic group of Residents from British Columbia, Nottingham UK, and Texas have worked relentlessly to bring the best out of this meeting, both scientifically and socially. Finally, after another long day of scientific presentations, Residents had a chance to become acquainted with each other and team up yet for another exciting competition “The amazing IPRAS Residents race�! The participants all had great fun and shared unforgettable moments whilst running through the vibrant streets of downtown Vancouver, ending at a local pub watching the Vancouver Canucks and the San Jose Sharks in a memorable game of ice hockey with the Vancouver Residents.

Striving for high levels of evidence-based medicine clinical trials is key to guide and support clinical decision making1. As such, monthly journal clubs and self-directed assignements have been shown, in a recent study from London, Ontario, Canada2, to improve evidence-based skills in Plastic Surgery residency training. Randomizedcontrolled trials (RCTs) are believed to reflect the gold-standard in plastic surgery3 an beyond. However, the conception, planning and execution, as well as the reporting of RCT in plastic surgery is challenging4. As such, Dr. Rohrich stated: “So you want to be an evidencebased plastic surgeon? A lifelong journey�5.

Evidence-based medicine in Plastic Surgery journal publications Given the broad field of Plastic Surgery, with its subdisciplines of reconstructive surgery, aesthetic surgery, hand as well as burn surgery, a lack of randomizedcontrolled trials (RCTs) is evident. As far as journal publications in the four major plastic surgery journals are concerned (Plastic Reconstructive Surgery, Annals of Plastic Surgery, Journal of Plastic, Reconstructive and Aesthetic Surgery, American Journal of Aesthetic Surgery) a survery among all published clinical trials in 2007 revealed that only 2.2% of those were Level I evidence studies6 (i.e. meta-analyses and randomized controlled trials with narrow confidence intervals). The average level of evidence was Level III, which are cohort studies. Comparing the Level I evidence from 1978 to 2009 among various Plastic Surgery Journals, there was a steady increase of such studies7, a fact supported by two other studies8,9.

As such, at the ASPS 2008 meeting, a total number of 9% of all conference 159 abstracts were randomized-controlled trials, with 5% being presented as oral abstracts and 4% as poster presentations10. As far as burn surgery is concerned, the American Burn Association (ABA) meeting in 2008 included 252 abstracts with a total number of 7% being RCTs (5% oral presentations, 2% poster presentations)11. In line, the 2008 annual meeting of t he American Society for Surgery of the Hand (ASSH) included 10% RCTs.

sixty-seven clinical trial abstracts were analysed according to the underlying study design (figure 2). Among all clinical trials, three meta-analyses of randomizedcontrolled trials were presented, accounting for 0.5%. Fourteen randomized-controlled trials accounted for 2.5% of all clinical trials. Observational studies were the predominant clinical trial design in 52%. Case series and case studies accounted for 29% and expert opinions for 16% of all clinical trial abstracts. Overall, the mean level of evidence was 3.54±0.9, with no significant difference between the 480 oral abstracts (3.5±0.9) vs. 255 poster abstracts (3.7±0.7).

IPRAS: A Personal Recollection

I was a young apprentice, together with Tom Rees and Ralph Millard, when I first heard that a small group of young plastic surgeons decided to create the first society to expand the knowledge of this new specialty. This happened at a meeting in London, in 1951. The organizer of the group was someone with whom I would create a solid friendship: Tord Skoog. With his deep sense of dedication to teaching, allied with his personal leadership, Skoog founded the Confederation in 1955, and became its first Secretary General. Some of his cofounders I came to know quite well: Gillies, McIndoe, Barrett-Brown, Webster, Dufourmentel, Burian, among others. The 1st Congress in Uppsala marked the beginning of a long tradition of meetings that gathered both the experienced and the novice plastic surgeon, every four years, in cities around the world. In the following decades, as each country aggregated their members in national associations, IPRAS spawned several major societies that today represent the whole range of our specialty. Yet we should not forget the mission-statement of our very first Confederation: to promote plastic surgery clinically and scientifically, educating our younger colleagues, and tightening the bond between specialists in all countries. When I returned to Brazil, in the late 1950’s, after a very enriching educational experience in the USA and Europe, I was determined to teach what I had learned. By uniting my private clinic with the 38th Ward of the Santa Casa General Hospital (a charity institution), in 1962 I began a residency program that – over these many years – has trained over 500 plastic surgeons, in 42 different countries. With a group of dedicated assistants, our school has been actively involved in all aspects of plastic surgery, both reconstructive and aesthetic: education and clinical practice; research and investigation. I firmly believe that this has been done in the true spirit of IPRAS. As a privileged actor, and also spectator, it has been 38

exciting to follow the development of our specialty, from a young seedling to a mature, multi-branched tree over more than 50 years. Participation in all of the IPRAS quadrennial meetings was a personal commitment that has given me great pleasure. Revisiting old friends, meeting new ones and exchanging experiences are some of my fondest memories of our Confederation. Looking to the future, I am sure that plastic surgery will carry on thriving, absorbing new technologies and techniques. Cell reproduction and derivation will set the stage for whole tissue regeneration, and it will continue to be the work of the plastic surgeon – together with a multidisciplinary team – to restore function and aesthetics following trauma or the ravages of senescence. There will be no shortage of skin envelope to cover large wounds, once dermal cell replication becomes a staple of the reconstructive laboratory. Heterologous grafting, today in an unsure footing, will become safe and predictable. Many of the operations currently performed – with large scars and risk to flap survival – will be noticeably improved. The healing process itself will certainly be controlled in the age of biotechnology, allowing less noticeable scars. It is my hope that IPRAS and its national societies will continue to be the forums where innovation will be presented, where the inquisitive mind will find others equally curious, so that plastic surgery may evolve within the framework of two principal objectives: to generously pass on knowledge to the next generation, and to assure safety to our patients. Formation of the “IPRAS Trustees”

I suppose I should start this article by congratulating the IPRAS Executive Board for taking a brave and much applauded decision to create a new Board entitled “Lifetime Trustees”, and then continue by thanking them for honouring me to be one of the members of this new force. When I was asked to write my thoughts and feelings about this new body and IPRAS in general, I must admit I did not realize that I would be both bewildered and uncomfortable. Bewildered, because, my relation with IPRAS goes way back to 1955 and I am not sure how to talk about a relation without making it personal. Uncomfortable, because I am sure that, there are so many other members of IPRAS, who are more knowledgeable then I. However, I shall try to give you my own experiences with IPRAS, and what it meant to me for the past 45 years. Above all, I shall try to share my thoughts with you, about, what this new responsibility means to me, as well as what it should really mean to our younger generation of plastic surgeons. Here we are; yes, the summer of 1955... I was an exchange medical student in Göttingen University Medical School. I had chosen this university because of the information I had received concerning a professor doing Cleft Lip & Palate operations, which attracted me like a magnet but had, thus far, observed no such operation. However, to my great disappointment the professor was not in the hospital, at that time, because he was attending a meeting in Sweden. Therefore, I had ended up in Thoracic Surgery instead. Yes, this was the year of the first International Congress of IPRAS (then IPRS) in Stockholm, Sweden. Like most relations, it had started on a wrong foot. Later on, I learned that the second Congress took place in London, Great Britain, in 1959. Even though I was not there personally, this particular date also holds an important role in my life, because two unforgettable pioneers, the fathers of Plastic Surgery in Turkey, Drs Halit Ziya Konuralp and Cihat Borçbakan were there

(as in 1955) and demanded an explanation “why the Turkish Flag was not displayed along with the others? ”The answer was simple .They were told that, after those two meetings, it had been decided to turn this group into an international organization This was going to be a group, formed by the National Plastic Surgery Societies. Therefore, unless they had a Society, they could not be officially included. Receiving this response and information, upon their return to Turkey, they immediately brought a group of surgeons together and formed the Turkish Society of Plastic Surgery in 1961. What a wonderful coincidence that, this year marks the 50th anniversary of both IPRAS and TSPRAS. However, this platonic relation from a distance, without knowing each other personally, finally became a reality in 1963, when, as a young trainee from New York, I registered in the 4th IPRS Congress in Washington, D.C. and attended for the first time. So, from 1965 on, we have never parted. IPRAS became my guiding light in my adventure of creating a teaching program in Turkey. This relation was blessed in 1967 with my first paper presentation of 8 Pseudo Median Clefts, 2 posters; “Statistical study of 172 CL & CP in a period of 18 months” and a case presentation of a” Unusual Facial Cleft with 6 nostrils, and Related Orbital Deformities”. This was the beginning of an international involvement, therefore since the Rome Congress, Article 5a of the By-Laws (then) became our rule in creating teaching centres and in preparation of the curriculum. We adopted the Anglo-Saxon model for training programs and had our first National Congress in 1968, with Prof. Tord Skoog as a guest. This has continued, uninterrupted, until today, every other year. The evaluation and progress of Turkish Plastic Surgery, as well my own history since then, in relation to IPRAS, is another, personal as well as professional, story.

However, in respect to our professional relations in summary, I was involved as the National Representative of Turkish Society of Plastic and Reconstructive Surgery to IPRAS, for a very long time. Following the establishment of chapters, Turkey had the honour of hosting both the 6th Congress of the European and 5th Congress of the Asian-Pacific Section in 1989 in İstanbul. I believe this occasion is still remembered by many.

Therefore, as a proud member of the organization, it is my firm belief that the creation of the “Lifetime Trustee Board” sprung from this necessity. This title was neither given to us, nor accepted by us just to be recognised or honoured. Just the contrary, I strongly believe that we were chosen to continue to work for the excellence of our Profession by sharing our knowledge and experiences and,

All educational and ethic-oriented Organisations in the field have rightfully and wisely followed the path of IPRAS in these past years, consequently being members of IPRAS, which now is the one and only real, unquestionable identity in our specialty. Other identities of course were and will continue to be added as time goes on. As I would like to emphasize here, the fact remains that the validity of our training and recognition as Plastic Surgeons is established by IPRAS. In an age of global changes in every aspect of our lives, and in any specialty, plastic surgery is going through a painful period as well. After years of studies, both clinical and experimental, when we had achieved to be the top of all possible excellences in all domains of plastic, reconstructive and aesthetic surgery,

whenever possible, to assist in the formation of the new developments of either the training or the practice of our profession, within the established guidelines of IPRAS. Young men and women need to know that they are not alone at the wolves table; we have all gone through difficult times due to the circumstances of every period we were in. As long as you have a sound training, respect for your patients, your colleagues and, above all, yourself, you will be successful and, most importantly, content and at peace with yourself. This path is going through IPRAS, sincerely believing in it, just as we must believe in the Hippocratic Oath as a word of honour to the profession and to our patients, to whom we owe our existence…. (like microsurgery, transplantations of tissues, organs etc just to mention few) we are facing unequal, unethical and, above all, even non-medical interventions to our specialty. Not only is the value of knowledge by training and the evaluation of skills by proper, legal channels being interfered with, but also, these necessary requirements are under attack by so many. These developments are targeted to destroy and demolish our specialty. There are so many on the waiting list to take away one part of this deliciously prepared meal, the right recipe of which took almost a century to prepare. 40

‘‘Lunch Time’’ Suture Suspension Lifting

Surgical lifting has been the standard of care for providing a radical solution and achieving dramatic improvements in the face and neck. There is, however, a growing trend nowadays for patients to seek the least invasive treatments possible with less risk of complications and downtime. Minimally invasive suture suspension of the face has gained prominence with the advent of the socalled ‘‘lunch-time’’ face-lift. This procedure entails the passage of sutures under the skin of the face and neck. It avoids large incisions and significant undermining. Although there has been substantial mention in the popular press, little information exists in the plastic surgical or dermatologic literature regarding the safety, efficacy, longevity, or complications of these suspension procedures. Multiple techniques now in vogue with simple transcutaneous elevation of soft tissues with various smooth looping or barbed sutures placed at various points in the face have been described; they may provide simple solutions to some conditions, but all lack evidence of follow-up beyond the immediate or very short-term postoperative period. It is really surprising that serious long-term studies and peer-reviewed data about longevity and patient satisfaction following such suture lifting procedures are notoriously lacking. The few clinical studies that exist do not offer any level of evidence more than level III. Most fall short of confirming the claims of fewer complications and improved patient satisfaction compared with standard techniques. Yet that did not stop numerous television shows and even physicians

from heavily advertising and promoting with unbridled enthusiasm transcutaneous suture lifting. Although there may be many theoretical and real advantages to using suture suspension and despite some reported satisfactory results of minimally invasive facelifts in cases involving the jawline and upper neck, tissue relaxation invariably occurs. Results are disappointing and have failed in terms of long-term maintenance of repositioned soft tissue and often result in visible threads, extruded threads, broken threads, or traction lines at rest or with animation. Moreover, evidence-based information on the fate of the implant materials is lacking. Unfortunately, Aesthetic Surgery relies heavily on patients’ feedback to determine success. However, the type of feedback received depends on how carefully questions are selected. The most well-intentioned physician can easily end up promoting a procedure of limited benefit. Plastic Surgeons are quite justified in being skeptical about new procedures that sound too good to be true. Currently, no consensus on an optimal subdermal suspension thread or an optimal placement pattern has been reached, and unfortunately, a safe and effective ‘‘lunch hour’’ procedure for elevating and redraping the soft tissues in an upward direction, with minimal or no patient downtime, is still eluding us. CO2 Fractional laser skin resurfacing A new era in facial rejuvenation & combined plastic surgical techniques

Introduction Lasers have a long history in aesthetic plastic surgery. First used as cutting devices, in the early 90’s, they were subsequently applied for skin resurfacing procedures, many times in combination with a surgical procedure (1). Lately, fractional resurfacing has emerged as a “limited adverse effects” procedure (2) for skin rejuvenation either stand-alone or in conjunction with cosmetic surgery.

Material & Method From June 1997 until June 2009, 1157 patients underwent laser assisted blepharoplasty (3) and 317 patients underwent face lift combined with ablative CO2 laser skin resurfacing by the senior author (PK). Laser skin incision and laser skin resurfacing of the lower eyelid was performed in 397 and 872 blepharoplasty cases respectively. Full face or partial skin resurfacing was performed in 207 of the patients who underwent face lift combined with CO2 Laser operations. Since September 2007, 495 sessions of CO2 facial fractional laser resurfacing were performed in 187 patients. Patients were given questionnaires to evaluate the quality of their scars following laser or cold blade incisions in laser assisted blepharoplasty. Patient and physicians evaluation of the procedures’ outcome was recorded.

Results The quality of scars in laser skin incisions fared significantly lower compared to steel ones. Laser skin resurfacing of the eyelid or the whole face skin yielded satisfying end results, but with a significant number of patients experiencing side-effects and long lasting postprocedure sequelae (erythema, hyper-pigmentation etc) which sometimes required extra treatments to resolve (4). Return to everyday activities was achieved a lot

earlier by patients who did not have their skin laser resurfaced. Fractional laser skin resurfacing achieved inferior results regarding skin texture compared to full ablative resurfacing and required multiple sessions. However, fractional resurfacing was rated by most patients as an “over the weekend” procedure and patient downtime was significantly less than full ablative resurfacing. In both groups the final results were rated higher than if face lift or blepharoplasty were conducted alone.

Conclusions CO2 laser is an excellent adjuvant to traditional facial aesthetic procedures as a cutting device (excluding skin incisions) because it minimizes blood loss and surgical time. Laser skin resurfacing has shifted over the years from full ablative procedures to the more sophisticated fractional ones which can complement the results of surgery in terms of skin texture.

Obituary of D. Ralph Millard, Jr., M.D. June 4, 1919 - June 19, 2011 The world of Plastic Surgery has lost one of its most iconic figures with the passing of D. Ralph Millard, Jr. A Life Member of ISAPS, Dr. Millard died quietly at his North Miami Beach home on June 19th, at the age of ninety-two. An indefatigable scholar, lecturer, author, educator, innovator, and master clinician, he will be most remembered for the power of his thought. At once creative, original, and brilliant…he was also disciplined (principled, he would have said), bringing an aspect that is not always, or even often, associated with the concept of genius. From an early age, he shunned the prosaic and routine: in his approach to his own education and training in Plastic Surgery (with a minimum of time devoted to prerequisite general surgery, his was a patchwork exposure to many of the greats of the day, with his transformational year under Gillies falling early rather than later within the cycle), in his extensive authorship (wherein he broke all the rules then in place for medical writing, beginning with his classic collaboration with the now Sir Harold, The Principles and Art of Plastic Surgery, as unconventional in its day as it remains entertaining, even to this day), in his scientific presentations (when his name was announced from the podium of the First International Congress of Plastic Surgery in Stockholm to present his revolutionary paper on unilateral cleft lip repair, the young unknown began his formal presentation in a stentorian voice from the rear of the hall and continued all the way to the dais, adding the additional “in transit” minutes to his total

presentation time), and, of course, in the daily planning and execution of the repairs he brought to his most difficult reconstructive challenges. That some prickly, more senior members of the plastic surgical establishment would find him brash was no surprise. But this penchant was then compounded by his compulsive commitment to straight talk and frank opinion, a bluntness that, while admirable and refreshing to many, was impolitic to some and incendiary to a few-who would take unconcealed umbrage at his remarks and assessments. Thus, early on, his membership in national societies would be blocked for extended periods. In spite of this, he was elected President of both the Plastic Surgery Educational Foundation and the American Association of Plastic Surgeons, as well as Vice-Chair of the American Board of Plastic Surgery…all positions that brought him pride. The arrogance that some attributed to him was never in evidence to this trainee, who spent two years with him following a residency taken elsewhere; instead, I saw only profound humility for his best efforts (and they were always his best efforts) on behalf of his patients, especially those with more serious deformity. In common with other great individuals, he remained his harshest, most unforgiving critic. It was also refreshing to be in the company of one who, in the mid 1970s, approached aesthetic surgery with the same dedication and respect that he did all of Plastic Surgery. He was a pioneer in rejuvenation of the neck and the first to advocate submental lipectomy. He


also confirmed for me that great modern Plastic Surgeons were most often masters in both arenas: the aesthetic and the reconstructive. His interests in Plastic Surgery were wide-ranging, and he found himself especially drawn to some of its most intractable challenges, only two of which I will note here. Cleft lip and palate, of course, presented a lifelong devotion, and his rotation-advancement operation for repair of the unilateral cleft lip remains a universal standard, still embraced in the over-whelming majority of repairs world-wide. So is his extensive, three-volume text, Cleft Craft, which garnered multiple awards for medical writing, the definitive work on the subject. Nasal reconstruction, and especially total nasal reconstruction, was another such area where Millard created outcomes never before realized. He established sound planning and operative principles for this most humbling of plastic surgical undertakings and brought them together (along with those for primary and secondary aesthetic rhinoplasty and congenital--especially cleft--rhinoplasty) in A Rhinoplasty Tetralogy. It is no coincidence that current leaders in the field, Gary Burget and Fred Menick, are both Millard trainees. Among his major works I must also cite Principlization of Plastic Surgery, which focuses not on any specialty area within the field or any atlas of its operations, but rather on the thought processes that should underpin proper diagnosis and treatment throughout Plastic Surgery. This unique work brings to a culmination a career of reflection and analysis in the complex field of Plastic Surgery, a thought process that had been awakened in him those many years ago by Gillies. Millard was an internationalist in every sense of the word and appreciated early on the world-wide distribution of talent in plastic surgery. He was understandably fond of things British, dating from his time with Gillies, and was proud of his Honorary Fellowships in the Royal College of Surgery of both England and Edinburgh, as he was the Order of Distinction from Jamaica, bestowed by the former British colony in appreciation for his many missions to that island to perform volunteer reconstructive surgery. In his many years as Professor and Chief of Plastic Surgery at the University of Miami, he trained more than 180 residents and fellows, many of whom he followed throughout their careers. Less than 60% of these, however, were American; the remaining 40% were international, many of them now in positions of leadership within some 35 countries. And as he had also drawn so much from his early visits to overseas masters of Plastic Surgery, so did he open wide the doors of his Miami program to visitors from around the world. And visit they did; a typical morning in Operating Room Number One at Jackson Memorial Hospital presented

the sleeping patient, with Millard seated comfortably on an oversized purple cushion with embroidered crown (a gift from his nurses), assisted by a much-less-comfortable resident or fellow craning from a distance to keep his attached headlight beamed exactly on the operative site (Millard demanded precise, focused lighting, but would not be bothered with the wearing of a headlight himself), a highly efficient and practiced scrub nurse, and a gaggle of some half to a full dozen visitors, all jockeying for a better look…while trying to avoid the attention that might well send an unsettling question their way from the intimidating Millard. His three greatest loves were, foremost, his family, followed in turn by plastic surgery itself, and then athletics. While an undergraduate at Yale, he lettered in boxing, wrestling, and football. He often credited the self-reliance he found as Captain of the Yale Boxing Team with the success in many of his life struggles that followed. This same love of sports is among the many gifts he has passed on to his three children: to elder son Duke, who played football for Harvard (where Millard had received his M.D.); to son Bond, who enthusiastically coaches scholastic football to this day; and to daughter Meleney, some of whose track records remain unbroken at her Miami high school (more than three decades later). Those who had the privilege of getting to know him found a surprising shyness about this man, who had so little penchant (or patience) for small talk. But if the topic turned around a legitimate controversy in Plastic Surgery (or boxing), he opened up. He knew and counted his enemies, perhaps more than he needed to, but he was blessed with a wealth of uncommonly true friends. But among the many, his most ardent supporter, his most trusted advisor, and his most loyal friend remained his wife, who sadly preceded him in death by more than a decade. When his remarkable and beautiful Barbara fell fatally ill, he abruptly retired from the practice of Plastic Surgery that he loved so well to devote himself entirely to the final days of one he loved more. And with her passing, he was left—in the brief span of a few months--bereft of both. He moved from his house in Miami (he couldn’t bear to be there without her) to an apartment near Bond, to which he gradually and increasingly confined himself. Tony Wolfe, his loyal resident and then partner in practice, visited him regularly throughout his final decade, bringing enthusiastic young residents and fellows to meet the Great Man…attention he clearly enjoyed. Our specialty is fortunate that this special man chose to come our way. He left it a richer and deeper discipline than it had been before. The Congress of 2011 took place in Yerevan, and was co-organized by the Yerevan State Medical University and hosted at the Conference Hall of Yerevan State Medical University. The Congress was organized under the auspices of IPRAS (the International Confederation for Plastic,

Reconstructive and Aesthetic Surgery), ESPRAS (the European Society for Plastic, Reconstructive and Aesthetic Surgery) and ISAPS (the International Society of Aesthetic Plastic Surgery). Aside from their endorsement, they also sponsored 36 young Armenian residents and 4 residents’ participation in this Congress. This significant scientific event attracted many highly esteemed Plastic Surgeons from all over the world, including members of the Armenian Diaspora of Plastic Surgeons. The renowned attendees of the Congress engaged in a constructive dialogue on Plastic Surgery complications and made groundbreaking scientific contributions on the latest Plastic Surgery developments. Another goal of the Congress was to establish and maintain cooperation with Plastic Surgeons from different countries throughout the world, ensuring the improvement of Plastic Surgery quality and treatment outcome in Armenia, through the exchange of information and professional experience. The scientific importance of the Congress is demonstrated by the presence of many members of the IPRAS Board of Directors, such as IPRAS President Prof. Marita Eisenmann - Klein, the Secretary General Dr. Nelson Piccolo, the Deputy Secretary General for Europe Dr. Andreas Yiacoumettis, and the Chairman of the Scientific Advisory Board Dr. Daniel Marchac. Opening Ceremony The 3rd International AAPRAS Congress officially began on June 26th with an Opening Ceremony attended by several important Armenian personalities and internationally renowned Plastic Surgery experts, including a few of the newly elected Board of Directors of IPRAS. Welcome speeches were given by the Minister of Health of Armenia Mr. Harutyun Kushkyan, the Rector of Yerevan State Medical University, Mr. Derenik Dumanian, IPRAS President Prof. Marita EisenmannKlein (Germany) and ESPRAS President, Dr. Andreas Yiakoumettis (Greece).

Press Conference The opening ceremony followed the launch of a successful Press Conference, organized by IPRAS Media Office, with the help of the Armenian Local Organizing Committee. The Press Conference was covered by some of the most popular Armenian Media, including 6 TV stations, 3 News Agencies, 1 Radio Station and various online Media. The total number of participating media is estimated on 30. Key Speakers of the Press Conference were the IPRAS President, Prof. Marita Eisenmann Klein, IPRAS Executive Director, Mr. Zacharias Kaplanidis, ESPRAS President, Prof. Andreas Yiacoumettis, AAPRAS President, Prof. Armen Hovhannisyan and the President of the Local Organizing Committee, Prof. Gevorg Yiaghjyan. In an attempt to reach the public via the media, the speakers presented new and interesting data on Plastic Surgery and confirmed how much progress has been made lately in Plastic Surgery in Armenia. IPRAS President, Prof. Marita Eisenmann-Klein, welcomed the journalists, congratulating all the Armenians for the efforts made to re-establish the institutions of their country. During her presentation on Breast Reconstruction after Oncological Resections, Prof. Eisenmann-Klein explained that reconstructive and aesthetic surgery have to go together and underlined that breast deformities are a very common phenomenon in Armenia Additionally, introducing IPRAS to the Armenian Media, Mr. Zacharias Kaplanidis, Executive Director of the Confederation, stated that every qualified Plastic Surgeon in the world has to be an IPRAS member and briefly presented the IPRAS aims and scopes, which are the promotion of educational and research activities worldwide, the creation of a code of ethics in Plastic Surgery, the provision of advice on international law, on medical practices and on security guidelines and highlighted that the most important dimension of IPRAS is its humanitarian missions.

Plastic Surgery thematic: face, rhinoplasty, breast, body contouring, oncology and skin tumours, reconstructive microsurgery and humanitarian surgery. Over 120 Plastic Surgeons from 23 countries worldwide took part in the congress workings; about 60 reports were delivered within 7 sections, as well as 6 state-of-the-art lectures by renowned experts. The Congress Book is specifically designed to cover the Congress material, including over 90 scientific papers. The Congress also included 10 round tables, the agenda of which encompassed specific topics, concerning analyses of complications, by the leading Plastic Surgeons. In the Special Session, 3 residents presented their case reports and presentations. Issues related to the European and American Plastic Surgery resident education were also incorporated in this important section. The Humanitarian Plastic Surgery Round Table was chaired by Prof. Jean – Philippe Nicolai and attracted many Plastic Surgeons from Armenia to participate in the Humanitarian Mission in the world. A short video film about the Women for Women Program and a short explanation about this program was presented by the President of IPRAS, Prof. Marita Eisenmann-Klein.

Public Day The Yerevan Public Day was successfully launched during the last day of the Congress, inaugurating a dialogue forum with the citizens and demonstrating that Plastic Surgery is not a medical discipline used only for aesthetic reasons, but one that can also alleviate both physical and emotional pain. Aiming to increase awareness on health issues and prevent medical malpractice, some of the most renowned Plastic Surgery experts, Prof. Marita Eisenmann-Klein, IPRAS President, Prof. Gevorg Yaghjyan President of the 3rd AAPRAS Congress, Prof. Armen Hovannisyan, AAPRAS President, Prof. Andreas Yiacoumettis, ESPRAS President, Prof. Nelson Piccolo, IPRAS General Secretary, By Prof. Daniel Marchac, Chairman of the IPRAS Scientific Advisory Board presented topics, such as fat grafting and breast reconstruction and augmentation, the safety of liposuction, the challenge of face transplantation, and skin tumors. Almost 200 Armenian citizens participated in the Public Day, actively engaging in a dialogue process, posing their own interesting questions.

Besides the “rich” scientific program, a nice social program was available to Congress participants. The Opening Ceremony took place on June 28th, 2011, at the Cafesjian Center for Art. The Reception started with a tour at the Armenian Center for Contemporary Experimental Art and was followed by a Cocktail Reception and Music from a classical quartet. The faculty dinner took place at NOY BRANDY FACTORY on June 29th, 2011. The dinner started with a brandy tasting and a tour in the Museum. The objective of NOY FACTORY tour was to reveal the secrets of true Armenian brandy and make it attainable to those who appreciate the majestic flavour and aroma of the legendary drink of the Biblical land. The Conference Dinner took place at the Bellagio restaurant, on June 30th, 2011. In his letter, Prof. Yiacoumettis states: “IPRAS and ESPRAS are very proud of our Armenian colleagues. Your team worked hard for the success of this event and you must be very pleased with the outcome. I am looking forward to future cooperation in both the European and the Global activities of our precious organizations. Once again, many thanks to you personally and to your colleagues for making our meeting in Yerevan unforgettable”. We think that the congress success was insured by the close cooperation with IPRAS, ESPRAS and “Zita Congress”, the official Congress Organizer of this event. EURAPS 2011 Letter by the Outgoing Secretary General Dear colleagues and friends, The 22nd Annual EURAPS Meeting on the island of Mykonos belongs in the past. With nearly 450 participants and 150 accompanying persons, it was the largest EURAPS Meeting ever. The event evaluation questionnaires showed enthusiastic remarks about the scientific and social programs. I have attended EURAPS Meetings, from the first one in Strasbourg in 1990 and only missed one meeting. For many years I served the Association; the past four years as Secretary General and I enjoyed doing so. During these years I was able, with the help of others, to achieve several things.

A new modern website was built, 61 new active members and five new corresponding members joined EURAPS, the EURAPS-JPRAS affiliation with two EURAPS members on the Editorial Board became reality, the EURAPS By-laws were adjusted, the transition of ECSAPS into the EURAPS Research Council was initiated, a EURAPS representative on the AAPS Board (reciprocal voting Board member position in each Association) was appointed, the EURAPS’ financial seat in Switzerland was established and CME credit points (12) for the Barcelona, Manchester and Mykonos meetings were obtained. It has been a great pleasure, for the past two years, to work with Dorothee and Georg Noever, at the

Treasurer’s Office. They are extremely professional and we worked well together as a team. Thanks to their efforts, EURAPS is now, from a financial point of view, a more transparent and official organization. During several years I worked together with Eric Arnaud, Dominique Erni, Moustapha Hamdi and Rafic Kuzbari, Benoît Lengelé Hans-Günther Machens and the different local hosts as the Scientific Program Committee. They are all dedicated, hard working colleagues and we did our utmost to compile balanced programs based on the scores of the reviewers but also taking every submitted abstract into account. We know that we have had to disappoint many, who sent in abstracts which were not accepted, but that is a result of a very stringent selection and because of meetings without simultaneous sessions. I served with four Presidents: Françoise Firmin, David Soutar, Erkki Tukiainen and Julia Terzis. Everyone had his or her way to contribute to EURAPS and to fill in the presidency. It is now up to Manfred Frey, the new Secretary General, to move upwards and onwards with the Association. I wish him success in doing so, together with Andrej Banic, EURAPS President 2011-2012. I want to thank all the members for their support and input and I hope to serve our Association for many more years, in another capacity. Mykonos was a great place to step down and have 600 people, including my family, attend my farewell party! I look forward to seeing you during the 23rd EURAPS annual meeting in Munich 2012. Finnish Association of Plastic, Reconstructive and Aesthetic Surgery (CPF) Finland is a small Nordic country situated in the eastern corner of Scandinavia.

Throughout history, Finland was often ruled by either one of its powerful neighbours, until 1917, when the country became an independent republic. Finland was the first country in the world to allow females into the parliament and the second country in the world to give women the right to vote (1906).

The national Society: The Finnish Association of Plastic, Reconstructive and Aesthetic Surgery (Chirurgi Plastici Fenniae) was founded in 1957 and nowadays holds 128 members, out of which about 70 are actively practicing, fully licensed members. Two thirds of the active members work in public hospitals, some with a little private practice on the side. One third of the Finnish plastic surgeons work full time to cover a small, yet steadily increasing market of cosmetic surgery. With 4 annual meetings this small but, active society keeps up good standards and close collaboration among its members. Knowing each other well is mostly a strength. Plastic Surgery related research gives the foundation to good clinical practice and supporting research among our members is one of the goals of the Society. The crop of papers from our last National meeting in November 2010 was huge and it was a hard job to put them in order. Association of Plastic Surgeons of India (APSI) The Teaching Department at Victoria Hospital, Bangalore, India celebrated their silver Jubilee on 8th January, 2011, by holding CME courses on various topics in Plastic Surgery by National & International Speakers, including Dr. Niri Niranjan from the UK. Inauguration of the Department Library by the Donor, Dr K S Shekar, also took place the same day. Later in the day, the alumni meeting with all past HOD’s, was attended by all past Academic Staff, PG’s and paramedical staff from the past 25 years. Several senior staff members were honoured.

On 25th and 26th February, The Baroda Association of Plastic Surgeons organized a unique program on Unfavourable Results in Plastic Surgery and How to Avoid and Treat. The Association of Plastic Surgeons of India supported this program through the Scientific Activities of the Gujarat Fund. Lectures were oriented towards discussing plans to salvage. Dr. C R Sunderrajan National CME course was held at Ahmedabad on 26th and 27th February, its theme being Cosmetic Surgery. The full-day operative session on 26th February included a demonstration on fat harvesting and fat grafting, as well as split ear repair using conchal cartilage. The following day’s program included a demonstration on Obagi Blue Peel by Dr. Arumugam, and a demonstration of fractional CO2 Laser

PLASTIC RECONSTRUCTIVE SURGERY DAY – 15th July. President and Members of the Association of Plastic Surgeons of India call on Union Minister of Health, Hon. Shri. Ghulam Nabi Azad. The Association of the Plastic Surgeons of India has decided to observe July the 15th of every year as Plastic Reconstructive Surgery Day. Dr. Raja Sabapathy, Director in Ganga Hospital, Coimbatore, who is the President of the National Association, explained that this move was taken to increase the awareness of the spectrum of work done by Plastic and Reconstructive Surgeons and also to enable the members to reach out to the masses.

The President of APSI, Dr S Raja Sabapathy has appealed to all the 1300 members of the Association to do at least one free surgery on 15th July. Surgeries could vary from correcting birth defects, post burn problems, hand injuries, deformities related to trauma, cancer reconstruction to aesthetic surgery. This way, they should be confident that they will make a difference in the lives of thousands of people and in turn their families and the community. In addition, the members will also organize awareness programs as per the need of each place. Shri Ghulam Nabi Azad, Union Health Minister, appreciated the initiative of the Association and wished them well. He also suggested to them to explore the possibility of a project, where the Association will provide the technical manpower and the Government could provide the infrastructure and the resources required to perform reconstructive surgery in places where it is not available. On 20th June, the team comprising of Dr. S. Raja Sabapathy, President of APSI, (Coimbatore), Dr. Atul Kumar Shah, Secretary of APSI, (Baroda) and Dr. Rakesh Khazanchi (Medanta Hospital, New Delhi) met the Hon’ble Minister for Health Shri. Ghulam Nabi Azad at New Delhi and made a presentation of the plans. Dr. S. Raja Sabapathy explained that most of the people remained in the dark about the possibilities of Plastic and Reconstructive Surgery. This is because, unlike other specialties, like cardiac surgery, neurosurgery or orthopedic surgery, this specialty does not relate to any organ system. Plastic Surgeons operate from head to foot; they operate on both sexes and on all age groups.

Korean Society of Plastic and Reconstructive Surgeons (KPRS) Founded in 1966, the Korean Society of Plastic and Reconstructive Surgeons celebrates its 44th anniversary as the forefront academic congregation of Korean Plastic Surgery. Plastic Surgery in Korea has achieved enormous growth over the past years. Recently, there have been a growing number of patients visiting Korea from neighboring Asian countries for plastic operations. Given the circumstances, the Society has devoted itself in planning and carrying out various charity works related to Plastic Surgery, in an effort to facilitate the diffusion of this discipline with humanitarian spirit.

It is mainly targeted at underprivileged patients living in governmentsubsidized residential areas, who are suffering from scars and functional disability sustained from burns, facial deformity, congenital malformations of the face and hands/extremities, hemangiomas and vascular malformations. This undertaking begun in 2005, and is benefiting from participations of surgeons from various institutional hospitals and private clinics, who have provided plastic surgical care to 221 patients from 2005 to 2010, all free of charge. The patients either apply directly to the Society for the service, or are referred

In Hussein Bajkara’s times, in the 15th century, a surgeon by the name of Hussein-sheikh used catgut suture. After the conquest of Turkestan by Russia, medical and, in particular, surgical service of the population changed. In April 1903, at the initiative of Peter F. Borovsky, the Red Cross Hospital opened a surgical branch with 10 patient beds. In 1920, the Turkestan State University in Tashkent founded its Medical faculty. This faculty became independent in 1931 as the “Central Asian Medical Institute”, the first University in Central Asia to offer higher medical education. The opening of high schools and scientific-research institutes, the arrival of scientists from abroad and the work of local doctors were factors that contributed to the development of medicine and surgery, as well as the public health system in the republic. Surgeon Pavel P. Sitkovsky and his students dealt with the problems of appendicitis, hernia, biliary dyskinesia and plastic operations. He is also the founder maxillofacial surgery in Uzbekistan. In the last 60 years of the 20th century, Academician Vohid V. Vahidov and his pupils developed and widely applied plastic operations of the cicatricial stricture of the esophagus. Academician Uktam A. Aripov and his students contributed greatly in the areas of abdominal, vascular and transplant surgery. He conducted experimental studies on transplantations, and in September 1972 performed the first renal transplantation on a patient with chronic renal failure. During the 1970s, in the Surgery Department of the

The great scientist Abu Ali ibn Sina (Avicenna), one of founders of surgery in the East, who was born in Bukhara, in the volumes III and IV of the book “Canon of Medicine”, which are devoted to surgery, stated theoretical and practical recommendations. Ibn Sina assorts methods of operations developed by Hippocrates and Galen in detail, and describes improvements and changes to these operations. Concerning treatment of stomach wounds, Ibn Sina describes the technique for wound closure after repositioning of the dropped-out and restrained guts. The recommended resection of the dropped-out omentum in the subsequent suture of the wound is still of value until today. In the 14th – 15th centuries, during Amir Timur’s Empire and his generation, great attention was given to medicine. Samarkand Medical Institute Hospital, serious and deep burns were treated with autologous and homologous skin grafts. The first Department of Plastic and Reconstructive microsurgery was established in 1984, based in the Surgery Science Centre and performed 104 autologous skin graft procedures from 1984 until 1995. Based in the Pediatric Clinic of the Tashkent Medical Institute, the Children’s Plastic Surgery Department is very well developed and maintains excellent co-operation with other countries like Korea, Singapore, Turkey, Germany and the USA, as well as with charity organizations such as “Smile Train” and “Operation Smile”. Aesthetic medicine is a young, developing specialty of medicine in our country. It is of great social value, as it can affect every aspect of life, from the family institution to business development. Experts recognize that Plastic Surgery in Tashkent was somewhat late in starting to develop, but, nevertheless, interest in it increases every day. It is only logical to assume that interest increases thanks to the successes and achievements of Plastic Surgery. On April 5th, 2007 the first Constituent Assembly was held, with the participation of 30 doctors working in the field of Plastic Surgery. Plastic surgeons from private clinics and official government hospitals, ENTs, traumatologists, orthopedists and ophthalmologists participated. The meeting ended with the founding of the Society of Plastic, Reconstructive and Aesthetic Surgery of Uzbekistan (SPRASU). The number of private clinics in the republic has increased. Such well proved clinics include the Center for Aesthetic Surgery “CHIROY-TIB”, “KASMED” Medical center, “ESTIMED” Private clinic, “MEDAS” Clinic of Aesthetic Surgery, “HEIBERG CLINIC” ТМ and “KALISTO”. In these clinics, the full spectrum of aesthetic interventions on the face, trunk and extremities is carried out, such as various kinds of face-lifting; upper and lower blepharoplasty; otoplasty and rhinoplasty; mammoplasty; fat grafting and reconstructive operations on genitals. Another step in the development of Plastic Surgery in Uzbekistan was the organization of the First Conference on Plastic Surgery in Central Asia in 2008, where we welcomed 21 participants from 6 countries. That same year, Uzbekistan became a member of the International Confederation of Plastic, Reconstructive and Aesthetic Surgeons. In the second Conference, which was held in 2010, 45 visitors from 16 countries of the world participated. At that conference, we were joined by the General Secretary and the Executive Director IPRAS, a fact indicating increase of interest in Plastic Surgery in Uzbekistan. The visit of Executive Director Zacharias Kaplanidis in January 2011 marked the continuation of our cooperation with IPRAS. During his visit, Mr. Kaplanidis met with First58

Deputy Minister of Health Republic of Uzbekistan, Prof. Anvar Alimov and the President of SPRASU, Prof. Murod Jafarov. On those meetings, the prospects of the development of Plastic Surgery in Uzbekistan and the questions connected with the organization of the 3rd Conference of Plastic Surgery of Central Asia were discussed. The First-Deputy minister was glad to join IPRAS in carrying out the conference, and promised support from the Ministry of Health of the Republic of Uzbekistan. During his visit, Executive Director Zacharias Kaplanidis held meetings with members of SPRASU and presented them with the report «Prospect of development IPRAS», and also met Heads of private clinics.

International Society of Plastic Regenerative Surgery (ISPRES) A new Society is born and will function within IPRAS. It is named International Society of Plastic Regenerative Surgery (ISPRES). All those interested in this particular field of science, plastic surgeons and collaborating scientists, will find a forum and a group of colleagues with whom they will share knowledge and interests in this type of Surgery. Plastic Surgeons have been practicing fat injections for over a century but it was only recently that we realized that this procedure involved more than augmenting soft tissue in the various parts of the body. “Stem Cells”, “Pre-adipocyte cells” and other related terms are entering not only our vocabulary but also are influencing our everyday practice. A golden future is ahead of us and this topic needs to be studied in depth. In our perpetual efforts for improving our results, new applications will be evolved and it is expected that our methods and techniques will be significantly enriched. Dear friends, It gives me great pleasure to invite you as members of our newly created International Society for Plastic Regenerative Surgary (ISPRES). As some of you are already aware, this society was founded at the last IPRAS meeting in Vancouver where I received the honor of becoming its first president. A basic reason for having a separate body is to emphasize the need to focus and specialize in the regenerative modalities of this niche area. The last half century was characterized by a leap in almost every field of medicine and in plastic surgery, where necessity, as always, has been the mother of invention. The 70s and 80s has been an era of local and regional flaps. With the 90s microsurgery came to help us find near precise solutions to impossible situations. However, only in the last decade have we started looking beyond duplication and substitution to actually remaking and recreating what age or disease has taken away. The genius in a plastic surgeon has finally found his comfort zone in regenerative surgery. With stem cell research and its possible applications gaining momentum in every facet of medicine, lipoaspirate has become the most dependable elixir to restore or recreate. We have always learnt to use our skill in aesthetic surgery as a means to fulfill the aspirations of the patient but we must also recognize the importance of reconstructive surgery which is indeed the “soul� of our specialty. Today this newly found society is an endeavor to amalgamate both for achieving synchronicity in function and form. The near future will see tremendous progress in the versatility of lipoaspirate to solve many problems, from the simple to the most complex, and we will be fortunate to be a part of this exciting journey. I therefore appeal to all of you to join the society and empower it with all of your experience, knowledge and skill. Legends in this field such as Tom Biggs, Sydney Coleman, Roger Khouri.... have already laid the path for all who wish to participate in, contribute to and enrich these pages of history.
