Even with half of the year in the books, many small business owners wait until December or January to assess their business and identify ways to improve its financial performance. Yet making time in June or July for a mid-year check-in – when you have a good idea of your business’ needs – may be one of the best times to help your business save time and money, and operate more efficiently in the long run.
As a banker, I see the benefits of doing a mid-year review firsthand. A mid-year review can help a business owner evaluate what lies ahead on the fiscal horizon and plan accordingly. The key to making the review effective is to keep it focused on the outcomes you want to attain. Here are key areas every business owner should consider at mid-year to help make the rest of the year a success: EVALUATE BUSINESS EXPENSES | Understanding how profitable your business has been year-to-date can help you evaluate your current position and modify your financial goals as needed. If you don’t already do it, keep your business and
personal accounts separate. Business checking and credit accounts that are kept separate from personal accounts can help you maintain accurate and complete records of all businessrelated income and expenses. Keeping separate records will also help you as you look for ways to minimize expenses and improve cash flow. ASSESS YOUR BUSINESS PLAN | Every small business should have a formal, written business plan to help with business decisions and strategic planning. According to a recent Wells Fargo survey, business owners who said they had a formal plan reported much greater optimism for 2015. If you don’t have one, or if your plan hasn’t been updated in a long time, now is a great time to consider writing or updating your business plan. THINK TAXES | It can be easy to lose sight of the need for year-round tax planning for your business. The more proactive you can be with managing and filing your tax returns, the better. Spend time reviewing your tax entries for the first half of the year to ensure you’ve captured all expenses, especially for things like cars that are used for business and personal use. If you haven’t met with your tax professional recently, now is a good time to go over key filing dates and deadlines like quarterly tax payments, and staying organized and prepared on your business taxes.

A Look Back with LACMA President

Last June we welcomed Dr. Vito Imbasciani as the 145th LACMA president. For this final column, we’ve pulled together some of his previous words of wisdom as a marker of all that’s happened over the past year. A recent meeting of the entire LACMA Board of Directors discussed this issue at length. The pain in the room was palpable and visceral, and it creates unfortunate divisions within the House of Medicine.”

FEBRUARY: Leadership | “I presume, somewhere out there in the world of literature, there is a vision of Utopia where the author has taken pains to create a society without leaders, where citizens lead their lives in the absence of rule-setters. ... “But, let’s face it, we need leaders. We need them at the local, regional, national and international levels, because humanity has not yet evolved to a point where we all agree on what our needs are, let alone agree on the solutions. We have myriad needs, for the maintenance of biological life, for reproduction, for emotional and spiritual health, for peace in the world. “Physicians have the education, the experience, the interest and the passion to make a positive impact in the lives of the people in the communities they serve. Be a leader — find your voice and speak out. ... A physician speaking at city council meetings will be listened to. There is no better time to lead than now.” APRIL: Legislative Advocacy Day | “Hundreds, if not thousands, of groups flock to Sacramento every year to lobby their cause, and many of them try to distinguish themselves and magnify their visual impact by wearing a uniform ... . But those groups cannot hold a candle to the simple white jackets that are synonymous with the nobility of the healing arts. And half of those jackets were worn by medical students, giving our elected officials a snapshot ... a glimpse into the future. That future will be populated by hosts of physicians who are actively involved in advocating for the profession at the local, state, and national level — and they learned how to do it while a student, under the aegis of LACMA and the other county medical associations throughout California.” Thank you Dr. Imbasciani for all of your service to LACMA.

The “new look” for the magazine is one piece of an ever-changing mosaic that is a smarter, more relevant organization. In fact, this issue of Los Angeles Medicine is an example of the evolution of LACMA. Our focus will continue to be laser-like when it comes to supporting physicians across the region. What will change is the tempo, the reach, influence, engagement and results. The practice of “medicine” will continue to fuel us as we move forward, while the execution of strategic goals will drive growth and brand awareness because, after all, LACMA is a brand that has historically not resonated with 30,000 physicians — until now. The first step in this evolution is to improve what we deliver for physicians: better resources, smarter solutions, pricing, incentives, service and support. Gone are the days of promising help; today, with a renewed focus, we are about one thing: giving you help when you need it the most. There is a sense of urgency to deliver options and solutions for independent doctors, especially in light of last month’s American Medical Association study that featured a sobering trend: For the first time ever, less than half of physicians (47.1%) had ownership in a medical practice, which is down from 53.2% in 2012. Curing Cancer?
In 2015, in his State of the Union address, President Obama announced, “Last year, Vice President Biden said that with a new moonshot, America can cure cancer. Tonight, I’m announcing a new national effort to get it done.” The White House Cancer Moonshot continues to engage scientists, clinicians, healthcare organizations, advocacy groups and others. A recent Popular Mechanics article called “It’ll Take an Army to Kill the Emperor” focused on some of the top doctors, scientists, policy makers and others who are starting to join forces to win the war against cancer. Doctors from LACMA groups such as City of Hope and Southern California Permanente Medical Group (SCPMG) are part of the effort. Here we take a closer look at what’s going on locally in the fight against cancer.
If you want to see the impact EHR and data collection can have in the fight against cancer, look no further than the Permanente Medical Group. They point to the use of electronic health record (EHR) as “a tool for multiple physicians from different specialties to coordinate care, communicate critical clinical information, and deliver medical treatment quickly and safely.” Michael Kanter, MD, regional medical director of quality and clinical analysis, Southern California Permanente Medical Group, is part of a large multi-disciplinary team working on the SCPMG Colon Cancer Moonshot. The goal, which Dr. Kanter called “large and audacious,” is to cut mortality rates from colon cancer by 50% in 10 years. Three years in, they’re making good progress toward that goal with a 17% reduction in mortality rates. Another impressive feat is that they’ve gotten their colon cancer screening rates up to almost 82% in commercial patients. That statistic jumps to 90% with Medicare patients. The national average hangs at about 45%. EHR is an essential element in the Moonshot project. Reminders about screenings and care gaps go out to both patient and doctor. If a patient is due for a colonoscopy and sees a dermatologist at SCPMG, a reminder will come up. Patients can also check their records for care gaps, and in the case of screenings for colon cancer, get an in-home test sent right to their door. Advances in using EHR for this project are helping the SCPMG team “review deaths with a program called E-Autopsy ... and we’re also able to monitor care and care paths,” said Dr. Kanter. He acknowledges that there’s an advantage at SCPMG in that they have a defined denomination of patients with their healthcare system. He compared it to an organization such as UCLA where they take patients in who aren’t part of the network. The result is a project that spans a decade and allows the team to work toward the goal with a populationbased approach. It also allows for “a little more patience” to see how interventions perform over time.
City of Hope is a logical place to look to for these advances. It is a world leader in the research and treatment of cancer and other serious diseases. It is one of only 48 comprehensive cancer centers in the U.S., as designated by the National Cancer Institute. Recent advances in immunotherapy and gene-based targeted therapy as part of a precision medicine approach have placed City of Hope in the news lately. The National Institutes of Health defines precision medicine as “an emerging approach for disease treatment and prevention that takes into account individual variability in genes, environment, and lifestyle for each person.” Advances in genetic science and research have made precision medicine possible. William D. Boswell Jr., MD, FACR, interim chief medical officer and professor and chair, Department of Diagnostic Radiology, has attributed their “bench-to-bedside approach” as one key to their overall success. He describes it as an ability to move things into action, from research to clinical, within their specialized focus. Many of their physicians are involved in both the clinical and research work at the facility. Their research findings are then applied directly at the community centers. Steven T. Rosen, MD, is the provost, chief scientific officer, director of the Comprehensive Cancer Center and the Beckman Research Institute for the City of Hope. He recently published an article in Oncology Times detailing the partnership between City of Hope and Translational Genomics Research Institute (TGen) out of Arizona. He points to how their areas of specialty dovetail “to fast-track the future of precision medicine and speed translational research for patients.” Dr. Rosen wrote that “precision medicine is the future of cancer care.”
SCPMG is well-respected in the healthcare industry for their effective use of EHR. Dr. Kanter pointed to a couple of factors in that success. He said that “someone has to think about how to use it. It takes systematically thinking about who needs to be screened.” Doctors are often focusing just on what has brought a patient into their office. Stepping back and looking at the need for screenings and other preventative measures has been made easier at SCPMG with the coordinated EHR system. In continuing to develop best practices with the technology, educating patients about care gaps is another factor and plays into the strategy for the Colon Cancer Moonshot.
