Chronic sufferers are known to present with a characteristic “waddling” gait as they lack sufficient pelvic stability to transfer loads effectively.

Osteitis Pubis & Physiotherapy treatment Osteitis pubis is a painful condition that presents in both the normal and sporting populations. Osteitis pubis is common in distance running and sports involving kicking and unilateral landing patterns, such as AFL and soccer. This is due to the repetitive shearing stresses placed on the symphysis pubis by the movements of the sport and/or action of surrounding musculature. In many cases it may present following child birth due to excessive movement of the symphysis pubis during the birthing process. Symptoms are characterised by pain in the Pelvis, lumbar spine, Sacro-iliac Joint and legs, with maximal tenderness and discomfort arising from the central pelvis

Due to the complexity of this area, differential diagnoses such as Lumbar disc referral, SIJ dysfunction, Hip pathology, adductor strain and hernia, to name but a few, must be considered. A Musculoskeletal Physiotherapist is able to carry out a succinct subjective assessment and confirm a diagnosis with accurate and appropriate objective examination and testing. Radiograph examination may show periosteal reaction and sclerosis of the pubic rami, flamingo view x-ray to evaluate widening of the symphysis may be used as an adjunct to aid diagnosis. Bone scan examination has not been shown to correlate accurately with symptoms. MRI is the gold standard examination and will detect acute and/or chronic changes. Rest from the aggravating activity is the first line of treatment. Following this the main goal of treatment is to achieve stability in the pelvis, thus reducing shearing stress through the symphysis pubis. In the acute

stage this can be achieved with taping or use of supportive belts. At this stage the Physiotherapist will provide appropriate hands on treatment to reduce symptomatic muscle spasm and aid normal movement patterns in the lumbar spine, pelvis and legs. Patients are educated on effective management in relation to Activities of Daily Living (ADL) and provided with appropriate exercises to complete independently at home to aid recovery. With a reduction in symptoms, treatment will then be targeted towards the underlying cause of injury E.G. weak core stability, muscle imbalance, poor flexibility or inefficient sporting technique. The individual will be guided through a comprehensive rehabilitation programme with a timely return to sport or ADL being the end goal.
