Long-Term Disabilityinsuranceprovidesyou with amonthlycash benefit tohelp payfor everyday expenses (suchas mortgage/rent,utilities,childcare, or groceries) if a covered disabilitylike a backinjuryor chronic illness takesyou away from work for an extendedtime.

Monthly benefit (after your claim is approved) Get a monthly check of$500to $2,500 in any $100 incrementyouchoosethatreplacesup to 60% of your Total MonthlyEarnings.

Guaranteed IssueAmount $2,500

When benefitsbegin Benefits begin as soon as 90 days

Benefits may be paid for Up to 5 years graded,or until youreach retirementage.See Q&A section for scheduleof benefits.

NOTE:This is anincrementplan.Thereare no offsetsfor NC Teachers RetirementDisability IncomePlan, the Public Employees’Retirement System (PERS),the StateTeachers’RetirementSystem(STRS),or forSocial Security DisabilityBenefits,butincomefrom other sourcescould reduce your benefitamount.

What happens if I become disabled?

Contact youremployer toreportyour disabilityandto checkwhether youare insuredunder the policyand eligibleto file a claim for long-termdisabilityinsurancebenefits.You will have to wait a certain number of days(see“when benefits begin”),for your benefits tokickinafter youareno longer ableto workdueto acovered disability.

How do I qualify for benefits?

You’llstartreceiving disability payments if yousatisfytheElimination Period (see“Whenbenefits begin” in the table) and meet the definitionofdisability.

What if I have a pre-existing condition?

If you submit a claimwithin 12 monthsof your insurance taking effect, or 12 monthsfollowing any increase in your amount of insurance, we will not payany benefitfor any pre-existing condition. A pre-existing condition includes anything you havesought treatment for in the 3 months prior to your insurance becoming effective. Treatment can include consultation,advice,care, servicesor a prescription for drugsor medicine.

How do I file a claim?

To file yourclaim,weneed toreceiveinformationfrom you about your doctor, your incomeand yourcriticalcondition. We’llask for youto authorize the release and disclosure ofinformation,likemedicalrecords,to help us evaluateyour claim. Your doctor will also need to fill outa form thatprovidesuswith specificmedical information aboutyour conditionand expected recovery.Formsmaybe downloadedfrom www.sunlife.com/us. Makesureto completeand sign all forms, asmissing information or signatures can delayyourclaim.
