We have faced many challenges, but we have celebrated an equal number of victories as we continue to exemplify the characteristics of grit and determination our region is known for

Over 600 credentialed providers and a total staff of more than 3,200 exemplify our mission statement, which is to advance the health and well-being of our region through comprehensive care in a Christian environment. I am very proud of this statement and I stand behind it wholeheartedly because it clearly shows what Pikeville Medical Center stands for

We offer our patients the same advanced level of care as in metropolitan areas with the heart, compassion and feel of a community hospital. Our employees are proud to serve the patients of Eastern Kentucky and go above and beyond to meet their needs.

PMC offers many support groups for patients facing medical challenges such as diabetes, obesity and cancer We understand their healing is dependent on much more than treatment and medications alone, and we want them to have access to an environment where they can learn and share outside of the hospital or clinic setting.

This mindset of caring for our community is also evident in the community events we either host or collaborate with others to offer Teams from different departments regularly participate in community and school events to interact with the community outside the walls of our hospital. From free medical screenings at area community centers to a stroke awareness event in the city park, we are committed to serving our patients. For nearly 10 years, the PMC Colors of Courage 5K has raised money to directly benefit patients of the Lawson Cancer Center at PMC and has grown into a very popular event in Pikeville.

During the summer of 2022, Eastern Kentucky faced the most devastating flash flood in our region's history, claiming 44 lives. I have never been more proud of our staff, who dedicated countless hours outside their responsibilities to help their neighbors in need.

Bottom line, when Pikeville Medical Center sees a need in our community, we are committed to meeting that need. This has been true throughout our past and will continue into our future.

Over the past several years, PMC has been fortunate to have great leadership to guide the organization and help overcome obstacles. This leadership team is able to define problems, develop and execute plans and deliver quality care to our patients.

Members of the PMC board of directors are actively involved, not only on the business side of the hospital, but also in hospital events and activities. The board is comprised of seasoned leaders from both in and out of the hospital who have a true passion for healthcare and are committed to growing Pikeville Medical Center to better serve the region.

ABOUT PIKEVILLE MEDICAL CENTER

Pikeville Medical Center (PMC) is a 348-licensed-bed facility dedicated to providing quality health care. PMC is the single largest transfer center in Eastern Kentucky and the 8th busiest in the state. The medical center has over 3,300 employees which includes 600 credentialed providers, all focused on providing quality patient care, and serves a patient population base of over 450,000.

PMC was named to the Forbes list of Best-in-State Employers for four consecutive years, earning the distinction as a Four-Year Champion. With a progressive and supportive leadership team and board of directors, PMC is able to supply its physicians and staff with the advanced technology and training needed to provide quality health care.

PMC is always striving to expand and further meet the needs of the service area. Being restricted by a rural geographical location, patients in the past were often at least two hours away from specialty healthcare services. Fortunately, PMC has grown into a regional referral center over the past several years and now offers the Commonwealth's only Level II Trauma Center. In 2023, PMC received nearly 350 transfers and admitted over 300 patients into one of the hospital’s Intensive Care Units. Additionally, there were over 40,000 patients referred to PMC specialty providers.

Pikeville Medical Center has made a giant leap in cardiac care with the completion of the $32 million Heart Institute Expansion Project. Three new state-of-the-art interventional cardiac catheterization (cath) labs enable

physicians to provide an advanced level of care with a broader range of minimally invasive diagnostic and interventional cardiovascular procedures, many that have never been offered at PMC before. PMC utilizes advanced technology and has the more cath labs than any other hospital in Kentucky, providing the medical center with the resources and technology to allow patients to remain close to their family and friends.

PMC saw the need to provide better access to pediatric care, which was something that previously required families to travel outside the region to obtain. The last several years have seen an intense push to increase pediatric services, which resulted in a five million dollar endeavor to create the region’s only children’s hospital.

While celebrating the opening of the Mettu Children’s Hospital, officials also broke ground on an expansion project for the Lawson Cancer Center. This expansion provided additional space for full-service cancer treatment and follow-up care, giving patients more options to receive advanced treatment close to home. The cancer treatment area offers 24 new infusion suites. Staff welcomed patients in September 2023.

Another impactful project was a seven million dollar renovation to the PMC laboratory. This created more space which now houses a state-of-the-art automated testing line with 10 new analyzers. The expanded lab increase in turn-around time of lab results by 50%. Due to high volume, another laboratory expansion is underway.

Donovan Blackburn, PMC has entered into a new era of communication directed at engaging the community, staff, physicians, patients and their families. This open line of communication was particularly important and beneficial during the COVID-19 pandemic.

Pikeville Medical Center is recruiting some of the most talented and experienced physicians who are at the top of their class in their specialties. Nearly all PMC physicians are board certified and many are double and even triple board certified. Along with job creation, PMC is also working diligently on job retention through workforce development. The goal is to broaden the knowledge and experience of employees in order to make them stronger, which results in a stronger organization.

The administration and board of directors appreciate the talented, dedicated staff at PMC. There is a priority placed on recognizing those employees who go above and beyond to care for patients and show support for their fellow staff members. Those who rise to this level are recognized through programs such as PMC Shining Star, the Daisy Award, department and hospital-wide Nurse of the Year, the Woonie (Wound Care) Award and more.

QUALITY, PATIENT SAFETY

The PMC mission statement is an integral part of what Pikeville Medical Center stands for. The quality of the technology being used is constantly being evaluated and measured to ensure patients are receiving comprehensive, quality care.

Developing treatment plans and offering medical care to patients begins with proper diagnostics. PMC continues to invest in state-of-the-art radiology equipment to provide accurate and rapid diagnoses. By continually upgrading technology to increase capabilities, PMC can offer a higher level of care to patients. One recent example is the enhancement of stroke protocols with CT Profusion to allow physicians to detect viable brain tissue. Every minute counts during a stroke event and this technology provides lifesaving advantages.

PMC's mammography services were recently upgraded and improved to complement the care provided at the Eastern Kentucky Advanced Women’s Care Center. In addition to 3-D mammography, PMC now offers cancer detection through the use of the

Automated Breast Ultrasound (ABUS) System. When used in conjunction with mammography, this technology can detect smaller abnormalities of tissue, resulting in a quicker diagnosis.

PMC offers patients a wide range of services to treat various conditions and diseases and is home to many specialty services. The Appalachian Center for Advanced Eye Care at Pikeville Medical Center features high-level eye care provided by board-certified ophthalmologists specializing in cataract surgery, blepharoplasty, glaucoma diagnoses/treatment and other laser eye surgeries.

The Appalachian Center for Advanced Eye Care proudly offers a wide range of Optometry services for adult and pediatric patients, as well. For pediatric patients, it's important to catch any potential problems early and to resolve any issues before they risk impacting the child's development, especially in school.

PMC partners with the University of Pikeville’s Kentucky College of Optometry to offer routine eye exams.

THE NATION’S LARGEST AUTISM CENTER OF ITS KIND

The Appalachian Valley Autism (AVA) Center at Pikeville Medical Center (PMC) is the largest autism center in the country offering Applied Behavior Analysis (ABA) therapy By providing this sought-after and proven therapy for autism, the AVA Center is changing lives and meeting the demanding needs of many families throughout Central Appalachia.

The AVA Center also provides in-house speech, occupational and feeding therapies to children between the ages of two and twelve diagnosed with autism spectrum disorder (ASD) enrolled in its program. These crucial services allow families to remain together and close to home, rather than having to drive hours outside of the region or even relocate to receive this potentially life-changing therapy.

At the AVA Center, each learner (child enrolled in the program) has a dedicated Board-Certified Behavior Analyst (BCBA) who provides ongoing assessments and creates individualized care plans and evaluations of the child's progress. A Registered Behavior Technician (RBT) is responsible for running the child's program on a one-to-one basis while speechlanguage pathologists and occupational therapists address communication, feeding and functional living skills.

The Appalachian Valley Autism Center also offers Autism Diagnostic Observation Schedule or ADOS testing. During this assessment, a specialist interacts directly with the child in social and play activities to accurately assess and diagnose autism spectrum disorder across age, developmental level and language skills.

Soon after opening its doors in 2020 in a much smaller location, the AVA Center was faced with the almost immediate need for expansion. This growth came in the form of what is now a 27,000-square-foot, ocean-

themed oasis featuring autism-friendly spaces with close attention to textures, acoustics and lighting conditions. The state-of-the-art facility consists of multiple learning rooms, a specially designed gym, a BEAM virtual play system, and a special sensory stimulation room. The additional space created the ability for the AVA Center to increase the number of children receiving services to 75, but hundreds more remain on a waiting list.

The Appalachian Valley Autism Center in Pikeville features an Outdoor Sensory Stimulation Area which was funded by a $675,000 grant from the Health Resources & Services Administration (HRSA) through the Community Project Funding/Congressionally Directed Spending Act. This carefully planned area allows learners to receive their ABA therapy in an outdoor, sensory-friendly environment.

Pikeville Medical Center's multi-disciplinary approach to providing care is particularly valuable for families with a child on the autism spectrum because of the unrivaled expertise that exists in meeting the unique needs of toddlers and children with ASD. Particularly beneficial are the behavior therapies that aim to reduce undesired behaviors, improve a child's functioning and address co-occurring mental health conditions such as anxiety and depression. AVA Center clinicians utilize several therapies matched to the child's age, specific symptoms and degree of severity.

At no point since opening has the AVA Center not faced an intense need for expansion. In late October 2022, Pikeville Medical Center celebrated the

opening of a second location in Prestonsburg, Kentucky. This 5,500 square foot center features the same ocean theme and features 12 learning rooms, a feeding room, a BEAM virtual play system and two specially designed sensory stimulation activity rooms. This expansion allowed therapists to provide services for 14 additional learners and offer the same type of specialized, one-on-one services available at the Pikeville AVA Center facility.

Even with the incredible growth and expansion of the AVA Center, there are hundreds of children with an autism diagnosis in Eastern Kentucky who remain on a waiting list to be told the doors of hope for them are opening at the AVA Center. As funds are made available and opportunities present themselves, additional growth will remain on the horizon.

In addition to providing this life-changing opportunity to children on the autism spectrum, the AVA Center at Pikeville Medical Center creates new job opportunities for the region. The AVA Center in Pikeville employs 50 full-time employees and the Prestonsburg location created an additional 16 positions.

PMC and the staff of the AVA Center are deeply committed to providing high-quality care to the learners, which includes providing encouragement and support to help them reach their full potential.

The Pikeville campus has created over 50 jobs and is on track to add 20 more. The new Prestonsburg center created approximately 16 additional positions, all eligible for PMC's robust benefits package. The new positions include Board Certified Behavior Analyst (BCBA), Registered Behavior Technicians, a nurse practitioner, occupational, speech and feeding therapists and manager. EXPAND AVA CENTER SERVICE AREA

When a small percentage of children's health is adversely affected, there are devastating effects on the family and the community. The CDC reports that about 1 in 36 children has been identified with autism spectrum disorder (ASD) according to estimates from CDC’s Autism and Developmental Disabilities Monitoring (ADDM) Network. The rate of autism has steadily grown over the last twenty years and is the fastest growing developmental disorder, but yet the most underfunded.

Comorbid conditions often associated with autism include Fragile X, allergies, asthma, epilepsy, bowel disease, gastrointestinal/digestive disorders, persistent viral infections, PANDAS, feeding disorders, anxiety disorder, bipolar disorder, ADHD, Tourette Syndrome, OCD, sensory integration dysfunction, sleeping disorders, immune disorders, autoimmune disorders, and neuro-inflammation.

With no regional facilities to offer autism therapy, PMC opened the doors of the Appalachian Valley Autism (AVA) Center in May 2020. Soon after opening, the overwhelming response and need required an immediate expansion. So many success stories have been imagined and created with the successful transition of children from a world all to their own, communicating with their family and friends in a comfortable and rewarding fashion.

Children on the autism spectrum often possess sensory issues making the learning environment critical to their success and is contingent on having varied interventions. It is critical for a child learner to be able to enjoy their day while releasing some of their energy

behavior therapist training program, additional jobs, and expanded much-needed service. The funding will provide 8,000 square ft for additional therapy services for 36 learners in Floyd County at the AVA Satellite campus, including a much-needed outdoor multisensory stimulation area and bring an in-house registered behavior technician (RBT) training program to train 25 students and create 25 additional jobs.

Strategy: Expand the AVA Center to Satellite Locations

The significant need in this area still far outweighs the availability of openings in the AVA program, making it critical to expand the program. Currently, the second satellite location is located in Prestonsburg, Kentucky

The Center for Bariatric & Minimally Invasive Surgery at Pikeville Medical Center (PMC) offers hope to patients who are ready to reclaim their lives from obesity. The Center is Accredited by the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP), a joint program of the American College of Surgeons (ACS) and the American Society for Metabolic and Bariatric Surgery (ASMBS). This accreditation demonstrates that the Center for Bariatric & Minimally Invasive Surgery provides quality bariatric care and produces excellent outcomes for patients.

The highly skilled and experienced bariatric team is led by board-certified PMC Bariatric and General Surgeon Amy Johnson, MD, FACS. Dr. Johnson has vast experience, expertise and compassion for her patients which has raised the bar for weight loss surgery in the region. In addition to demonstrating a high level of skill when performing traditional surgeries, Dr. Johnson has many years of experience performing robot-assisted surgery at Pikeville Medical Center. Her primary goal in surgically treating morbid obesity is to eliminate or significantly improve most medical problems affecting her patients.

TRAUMA

The PMC Trauma Center is a Level II trauma center offering emergency care 24/7. The team consists of experienced professionals in trauma, the emergency medicine, neurosurgery, orthopedic trauma, critical care, interventional radiology and local emergency transportation services. PMC's trauma services include the care of patients who have experienced a motor vehicle or all-terrain vehicle (ATV) accident, traumatic fall, gunshot wound and more.

PMC also offers advanced trauma courses such as: Rural Trauma Team Development Courses, Advanced Trauma Life Support Courses, Stop the Bleed Courses for schools in the area.

In addition, PMC works with the city to train local EMS.

TRAUMA SERVICE AREA

PMC EMERGENCY DEPARTMENT CERTIFIED FOR PEDIATRIC AND AUTISM READINESS

PMC Emergency Department consistently increasing number of pediatric patients. This growing group prompted PMC to begin the certification process in November 2019 to become “Pediatric Ready" by the Kentucky Emergency Medical Services for Children (KYEMSC) Program. Being recognized as "Pediatric Ready" demonstrates PMC's commitment to enhancing emergency care services for children in the region. Only three other hospitals in Kentucky have been certified Pediatric Ready

With the creation of the Appalachian Valley Autism (AVA) Center, the PMC ED recognized a need for further training in treating individuals with autism, as these individuals typically have different needs relating to communication, pain perception, and other treatments. The International Board of Credentialing and Continuing Education Standards (IBCCES) has recognized the PMC ED as a Certified Autism Center. Now, PMC staff is better equipped to help these patients.

ADVANCED EQUIPMENT WARMS INFANTS DURING TREATMENT

The Panda Warmer allows the medical team to focus on the most critical aspects of their job-caring for our youngest patients and their families. With direct heat application, babies, and toddlers stay warm and content. The compact size allows more room for the emergency staff and family to interact with the patient. An integrated x-ray cassette tray eliminates the need to move the baby. Plus, the Panda Warmer provides uninterrupted thermal support during x-rays. Additionally, a built-in scale makes taking the baby’s weight easy There is no need to move the baby to another tool or area. Having this piece of equipment will help treat sick babies more comfortably than before. This equipment has been extremely beneficial in the Emergency Department.

EMERGENCY RESPONSE

Challenging terrain and the location of many remote communities in Eastern Kentucky have long presented unique healthcare delivery challenges. Access to quality medical care can be a hurdle, especially during emergencies when every second counts. EMS personnel are pivotal in bridging this gap, providing expedited medical assistance and transport to those in need. Thanks to two Rural Emergency Medical Services Training Grants by the Substance Abuse and Mental Health Services Administration (SAMHSA), PMC is furthering its commitment to enhancing emergency medical services in rural communities through its Rural EMS Training Course.

In addition, PMC recently purchased two ambulances. These are used strictly for inter-facility transportation, freeing up the limited remaining ambulances in the region to respond to 911 calls.

PMC also procured funding to purchase Narcan. ED Staff has distributed these kits to local fire and police departments and provided these first responders with education and training.

Advanced Women’s Care Center

The Eastern Kentucky Advanced Women's Care Center at Pikeville Medical Center (PMC) is led by seven OB/GYN physicians who make up the largest on-call, board-certified OB/GYN team in the region. This experienced team of physicians is supported by a dedicated and compassionate team focused on all aspects of women's health including well screenings, family planning, menopausal care, pregnancy and postpartum care as well as minimally invasive gynecologic surgeries and more.

For mothers-to-be who are navigating the journey of pregnancy, the Eastern Kentucky Advanced Women's Care Center guides patients through this incredible chapter of life, delivering an average of 1,000 babies each year. Pregnancy and childbirth can be overwhelming, and the team at PMC strives to provide the support patients need every step of the way.

PMC's skilled obstetricians manage the various stages of labor, delivery and postpartum care, working closely with mothers to ensure safe and comfortable birthing experiences and employing medical interventions when necessary.

Where they are going to give birth is vitally important for women to consider as they choose an OB-GYN physician to care for them during their pregnancy. Having access to a Neonatal Intensive

Care Unit (NICU) is critical in the event the baby requires medical attention at birth. For this reason, PMC offers the region's only Level II Advanced Care Neonatal Intensive Care Unit PMC's two experienced neonatologists specialize in caring for infants born prematurely or with medical conditions that require immediate attention. The NICU at PMC can care for babies born as early as 28 weeks and weighing as little as 2.6 pounds, keeping more families together in the event these life-saving services are needed.

The comprehensive care provided by PMC obstetricians, neonatologists and their dedicated support staff ensures that pregnancies are monitored with precision, deliveries are safe and newborns receive the support they need for a healthy start in life.

NICU GROWTH & SUPPORT

Throughout Pikeville Medical Center's 100-year history, there have been numerous growth and expansion projects that have completely changed the level of care the hospital can provide. One such project took place in PMC's neonatal intensive care unit in 2019. The hospital had applied for and was granted a certificate of need to double the number of NICU beds and treat babies born as early as 28 weeks. This historic expansion also allowed the unit to transition to a Level II Advanced NICU, becoming the only NICU of this distinction in all of Eastern Kentucky.

At Pikeville Medical Center, there is never a moment of rest when it comes to advancing the technology and care offered to patients, especially the most vulnerable. In 2020, the PMC NICU received a generous grant from Kosair Charities to purchase a temperature management “cooling blanket therapy” system, which can cool a newborn's body and brain temperature. In certain situations, this

non-invasive technology has the potential to save a newborn's life by utilizing a technique known as hypothermia treatment. Babies, even those born at full-term, can experience a lack of oxygen and blood flow to the brain and experts say inducing hypothermia in those situations can reduce the risk of brain injury by as much as 50 percent.

In 2023, the Pikeville Medical Center NICU was able to add another piece of advanced equipment, thanks to funding provided by the WHAS Crusade for Children Foundation. While having a Level II Advanced neonatal intensive care unit allows PMC to care for more critical newborns than any other medical center in the region, inevitably, it is sometimes necessary to transport a newborn to a higher-level facility. There are times when Level I care is necessary and time is of the utmost importance. Thanks to the generosity of the Crusade for Children Foundation, PMC was able to purchase a Voyager Transport Incubator to transport the most fragile babies. The Voyager unit essentially becomes a mobile NICU and has a wide range of capabilities from cooling to high-frequency ventilation.

Pikeville Medical Center’s Primary Stroke Center is among the top stroke treatment centers in KY. Its goal is to deliver the most effective care with cutting edge technology. To provide patients with quality care, PMC works with Emergency Medical Service providers from across the region to ensure acute stroke care starts from the moment of patient contact.

PMC’s Emergency Department is highly trained for immediate identification of early stroke symptoms and equipped to provide intervention. The hospital has assembled a Stroke Team which has been trained to respond to stroke emergencies and provide expert neurological care. The Stroke Center has earned The Joint Commission’s Gold S e a l o f A p p r o v a l ® a n d t h e A m e r i c a n H e a r t Association/American Stroke Association’s Heart-Check mark for Advanced Certification for Primary Stroke Centers.

In addition to traditional neurology services, PMC’s Eastern Kentucky Neuroscience Center offers neurosurgery and an inpatient rehabilitation (IPR) program.

IPR provides services for patients suffering from falls, serious illnesses, and most commonly, stroke. The physicians and staff works together get patients back home or back to work as soon as possible. The team working with physicians includes, physical, occupational and speech therapists, rehabilitation nurses, case managers, social workers, dietitians and other professionals.

At the heart of the new cath labs is the Philips Azurion® imaging system. The Lawson Cancer Center (LCC) at Pikeville Medical Center provides patients of the region with expertise and leading-edge technology to fight cancer. LCC provides full-service cancer treatment and follow-up care The LCC is staffed with experienced oncologists, surgical specialists, nurses, pharmacists, patient navigators and many other highly trained healthcare professionals dedicated to providing quality cancer care. The LCC makes it convenient for patients across Eastern Kentucky to receive advanced cancer care close to home.

Due to a wide range of healthcare providers being involved with cancer care, the LCC utilizes a team approach. A dedicated group from different departments provide specific services to each patient with the goal of ensuring quality care and support. Each patient's team is assembled based on their

specific circumstances. Multi-disciplinary teams aim to improve communication, coordination and decision making between healthcare professionals. Cancer care is more than simply treating the disease. The PMC Radiology Oncology Division added two (2) new medical linear accelerator (LINAC). The LINAC customizes high energy x-rays or electrons to conform to a tumor's shape and destroy cancer cells while sparing surrounding normal tissue LINACs are equipped with 3D image guidance to accurately target tumors. Special software then matches radiation beams to unique tumor sizes and shapes. This customized approach makes external beam radiation therapy safer and more precise for cancer treatment.

The PMC Radiology Division has expanded their schedules, offering additonal appointments to accommodate patients The new advanced equipment provides the ability to detect diseases earlier.

The SOMATOM® Definition Flash Computerized Tomography (CT) provides a whole-body scan requiring only five seconds. The Fully Assisting Scanner Technologies (FAST) simplify typically timeconsuming procedures so the scan process is more automated and results become more reproducible. Integrating the capabilities of syngo®.via leads to a fast and reliable diagnosis.

PMC utilizes new ultrasound technology designed specifically for screening dense breast tissue. The Invenia Automated Breast Ultrasound System (ABUS) 2.0, by GE, is the only FDA approved system of its kind. PMC is the only hospital in the area with this technology, and it adds another layer of breast cancer screening capability to PMC Diagnostic Center.

LLCC is among a relatively small percentage of cancer centers approved to offer some of the newest treatments to qualifying cancer patients in the form of clinical trials. Clinical trials are research studies that involve people and are the final step in the long process of developing a new drug. Targeted delivery techniques for radiation therapy are key factors to improving patient outcomes and quality of life. Varian True Beam Linear Accelerator (LINAC) provides the latest, up-to-date treatments and procedures. The LINAC is used to deliver external beam radiation treatments to cancer patients. It can target and destroy cancerous cells in a precise area of a patient's body with minimal exposure to the surrounding healthy tissue. A LINAC can be used to treat cancer in virtually any area of the body. The Xoft® Axxent® Electronic Brachytherapy (eBx®) System® uses an advanced technology to deliver a targeted dose of radiation directly to the surface of nonmelanoma skin cancer (NMSC) lesions. The Xoft System is FDA cleared for cancer treatment anywhere in the body, including early-stage breast cancer, gynecological cancers, and NMSC, including basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). The Xoft System uses a proprietary, miniaturized x-ray source to deliver a precise dose of targeted radiation directly to the surface lesion while reducing the risk that radiation will affect surrounding healthy tissue.

Eastern Kentucky is nationally ranked in the top five (5) percent among all congressional districts for cancer diagnosis, particularly in the early-stages. The reasons are yet to be discovered and potentially could be a combination of environmental factories and disparities of health care access. Patients undergoing oncology treatments are some of the most vulnerable patients in the medical environment.

With a group of oncologists, surgeons, radiologists, nurses and other support staff, this team collaborates to design tailored treatment plans that consider each patient’s unique medical history, preferences and emotional needs.

GOAL: EXPAND ONCOLOGY SERVICES

Strategy: Created an Oncology Treatment Area

With the assistance of grant funding, PMC opened a 7,000 square foot oncology treatment area located on the 11th floor of the PMC Clinic building. This area provided an additional 18 exam rooms to compliment those on the 10th floor. We built an additional 24 chemotherapy treatments spaces. This new treatment area is located adjacent to our state-of-the-art cancer specialty pharmacy making for easy and immediate delivery of treatment medications.

Not only was this area constructed to efficiently deliver the treatment, it was designed to maximize the comfort of the patients with a home-like feel. Patients can receive treatment in a comfortable environment, complete with recliners, internet access, massage equipment and other new features that will provide a calming experience during treatment.

Strategy: Upgraded Existing Oncology Treatment Technology

To continue offering the highest level of oncology treatment, PMC will continue to replace equipment by upgrading our second linear accelerator to the new Varian True Beam unit. The expansion has allowed us to offer additional treatment plans to our patients, including Stereotactic Body Radiation Therapy (SBRT), which is a treatment procedure specially designed to coordinate-systems for finding the exact location of tumors in the body by delivering radiation in limited, but highly precise, treatment fields. SBRT has significantly improved the cure rates for the tumor in a way which PMC could not previously deliver.

PMC treats cases for prostate, liver, and breast tumors without damage to the surrounding tissue. This technology will also allow PMC to treat multiple brain lesions simultaneously.

In 2021, PMC celebrated the opening of the Mettu Children's Hospital (MCH), the only dedicated children's hospital in Eastern Kentucky, offering over 13,000 square feet of space for both inpatient and outpatient services. Everything about the MCH was designed to care for children, from the junglethemed decorations and child-friendly exam rooms to comforting patient rooms and a distraction therapy area for times when the environment is just too overwhelming.

The inpatient section of the Mettu Children's Hospital encompasses over 8,100 square feet and consists of a family-friendly waiting room, 10 private patient rooms (two of which are isolation rooms), a consultation room, a treatment room, a medication room, a nourishment room and more.

TELEHEALTH SERVICES FINDING SUCCESS IN SCHOOL PROGRAM

Living in the rural area of the Appalachian Mountains can be a barrier to many things, but healthcare should never be one of them. A generous USDA Rural Health Grant enabled PMC to offer state-of-the-art telehealth services to all 19 Pike County Schools. The Healthy @ School Telehealth program launched in 2020 and has been very successful. In 2023, there were nearly 350 telehealth visits from elementary, middle and high schools. The PMC Healthy @ School Telehealth program is available for students, faculty and staff.

The administration of Pikeville Medical Center recognized the need to offer pediatric specialty services not currently offered in-house, which prompted the establishment of a relationship with the University of Kentucky (UK Healthcare) Children's Hospital. Through this partnership, PMC is bringing the known expertise of Kentucky's top pediatric heart program as well as other pediatric specialty programs to Eastern Kentucky. The success of this partnership is being seen in the improved health of pediatric patients from the region who require specialty care.

PEDIATRIC SERVICES

PMC, like most acute care hospitals has historically focused on both ends of life's spectrum, from the beginning of life with our obstetrics and neonatal services to the end of life with our adult specialists. However, there is a void of service offerings to the pediatric population. This can cause several problems, including financial and emotional stress to families when traveling with children and children often being scared of the unknown and new surroundings. This can affect the healing process. If we could render services nearer to home, it could make a significant difference on the family burden, the stress of the younger patient and potentially increase the likelihood of a child receiving treatment.

The growth of the AVA Center and our pediatric service line is PMC's foundation to becoming this region's premier pediatric healthcare provider. In December 2021, the opening of the Mettu Children’s Hospital launched PMC to establishing itself in this role.

The over 13,000 square foot wing of the hospital is fully dedicated to the treatment of children, complete with child friendly exam rooms, comfortable and comforting patient rooms and a distraction therapy area. Patients, parents and visitors will have separate waiting areas for sick and well children. The Mettu Children's Hospital at PMC is on the main campus for both inpatient and outpatient specialty treatment, and is located close to the testing and diagnostics centers for the convenience of both the patients and the providers.

EXPAND PEDIATRIC SPECIALTY CARE

Strategy: Expand Relationship with Uk’s Children’s Hospital

The existing relationship with University of Kentucky (UK) is the foundation on which we grow additional specialty services. By expanding our relationship with UK, we will develop a team of medical providers including specialists, with expertise in specialized medical needs and anatomy of children. Additionally, PMC has developed the strategies to offer virtual services, for both consultation and outpatient services, when it is appropriate for the delivery of the plan of care. These specialties include cardiology, endocrinology, BMI, pulmonology, and plastic surgery.

Strategy: Expand Pediatric Care Offerings

We have evaluated offering additional services such as pediatric endoscopy, EEG's and general surgery, with our partnering physicians. To support those services we will review the need for additional medical pediatric hospitalists to manage the services rendered by the specialists and or the local pediatrician. With additional patients, we expect to have the need for a Pediatric Intensive Care Unit (PICU). As we evaluate the need going forward, we will be able to utilize tele-ICU services with our partners and then expand to having a fully functioning PICU as demand warrants. PMC is fully vested in caring for the children of the region in all aspects of their medical needs.

EXPAND THE SURGICAL AREA & OPERATING ROOM PROGRAM

With the opening of the PMC Clinic Building, PMC expanded the surgical suite capacity to total of 20 suites to provided a comprehensive array of surgical services of the highest caliber Many would no longer have to travel out of the area for care, saving the cost of ambulance or helicopter travel, and ensuring that patients receive the care they need as soon as possible.Rising costs and diminishing reimbursements require hospitals to continually find ways to improve efficiency and productivity. The operating room (OR) is a cost-intensive environment requiring efficient and effective management. The power of performance of the OR is crucially dependent on the cooperation of the surgical, anaesthesia, nursing and allied health professionals involved.

Strategy: Add four (4) additional operating rooms

Each operating room will be a well- designed system that meets the various needs of operative patient care. The addition of these four OR suites will dramatically expand our ability to perform the most complex tertiary surgical procedures while also caring for our trauma patients. The new, high-tech operating rooms is part of a significant expansion to expand the surgical program.

The expansion would allow procedures to be scheduled in a more timely fashion, cutting down on patients’ wait time and ensuring that care will remain local. The expansion would allow more physicians to utilize

PHARMACY SERVICES

The goal of Pikeville Medical Center Pharmacies is to provide the community with fast, safe, effective and reliable service. PMC pharmacists work hard to achieve that goal. There are several retail and internal pharmacies at PMC, each offering prescriptions to serve our patients’ needs, whether they be in the hospital, visiting our retail location or needing the services of a specialty pharmacy.

Outpatient/Retail Pharmacy

PMC’s Outpatient Pharmacy has taken the necessary steps to ensure convenience for all patients. Recently, drive-thru lanes two and three have been upgraded with a new tube system to accommodate larger medication packages. The new credit card readers have eliminated the need to send a payment through the tube system. With drive-thru services available 24 hours a day, extended walk-in hours, online prescription refills, mail delivery, home delivery for 41501 service area and consultation services, these features simplify access to medications and healthcare products.

Inpatient Pharmacy

PMC’s Inpatient Pharmacy dispenses and prepares medications for patients that are hospitalized or in the emergency department. Pharmacists work alongside physicians to ensure proper medication treatment, dosage and safety for each patient. A satellite pharmacy operates in the ICU to provide more dedicated care to those in critical condition.

Specialty Pharmacy

Unlike traditional retail pharmacies, specialty pharmacies cater to patients with complex and often rare medical conditions. The PMC Specialty Pharmacy is equipped to provide specialized medications tailored to the unique needs of these patients. Specialty pharmacies primarily handle high-cost, specialty medications often used to treat complex and chronic conditions .

The PMC Specialty Pharmacy offers a wide spectrum of patient support, including medication counseling and management, ongoing monitoring and financial services. These services ensure patients achieve the best possible outcomes while managing their side effects and potential complications.

Oncology Pharmacy

PMC’s oncology pharmacy prepares chemotherapy for patients at the Lawson Cancer Center. Pharmacists work in close conjunction with oncologist to formulate chemotherapy regimens appropriate for the needs of each individual patient. These pharmacists are regularly consulted to aid physicians in searching for drug information that will better serve our patients.

GOAL: ENHANCE SPECIALTY PHARMACY SERVICES

Pharmacists are now a large part of the healthcare team. Pharmacists conduct consultations with patients and providers to help recommend the most appropriate medications available for treatment. Most patients requiring medications for short-term illnesses often visit traditional retail pharmacies. Pikeville Medical Center has been one of the most relied upon retail pharmacies in the region for years by offering 24-7 service to our community. With the continued growth of specialized medical care, PMC patients are being treated for more complex diseases here at home. Many of those treatment plans utilize pharmaceuticals that are highly specialized that they are not prescribed in large volumes. Furthermore, certain medications need to be handled or distributed in nonconventional ways, such as refrigerated or frozen until administration. These medications are not available at smaller pharmacies. PMC has become the regional provider of specialty medications providing significant advantages to residents of this region.

Strategy: Continue to Partner with Providers and Pharmacies in the Area

Partnering with the providers and other pharmacies in our area have continued to allow us to harness purchasing power through the 340B Program. This initiative will significantly help both PMC's purchasing power and the purchasing power of other pharmacies in the area. As we continue to develop the strength and network of these partnerships it will allow us to help other organizations, and ultimately more patients, regardless of their retail pharmacy location.

SLEEP MEDICINE

The Sleep Center at Pikeville Medical Center (PMC) is fully accredited by the American Academy of Sleep Medicine. It is staffed by a highly skilled and dedicated team of providers and sleep medicine professionals led by boardcertified PMC Sleep Medicine Specialist David Carter. Dr. Carter’s experienced team includes a nurse practitioner, certified polysomnographic technologists, and respiratory therapists, who treat a wide range of sleep disorders together

“So many people don’t realize the health benefits of getting enough sleep,” explained Dr. Carter. “Sleep health contributes to all aspects of a person’s health, and getting proper, restful sleep can help improve overall health as well as many other related medical conditions.”

Dr. Carter added that patients often don’t know they need a sleep study, and most are unaware of the benefits of having one when they arrive for their initial visit to the PMC Sleep Center. One of the most common symptoms patients seek help for is snoring. Others include morning headaches, being sluggish or tired, pauses in breathing during the night and difficulty focusing or concentrating during the day

The PMC Sleep Center provides a wide range of services, including overnight sleep studies and at-home sleep studies. In addition to Dr David Carter, the sleep center staff includes certified polysomnographic technologists and respiratory therapists.

For many, not getting enough sleep can be more than just an inconvenience. Too many sleepless nights can aggravate both physical and mental health problems and can be responsible for the inability to focus as well as general irritability The inability to sleep has been linked to increased stress and the weakening of the body’s immune system. Chronic insomnia is also linked to a spectrum of serious health problems, including diabetes, cardiovascular disease, depression, obesity and hypertension.

Those who suffer from lack of proper sleep may find answers for a better night’s sleep at Pikeville Medical Center’s (PMC) Sleep Center. The Sleep Center’s highly trained staff works together to diagnose and treat adult patients with sleep disorders.

The PMC Sleep Center is fully accredited through the American Academy of Sleep Medicine.

“Sleep disorders can be much more complicated than just not being able to sleep,” said PMC Sleep Center Supervisor Todd Werner. “Extended sleep loss and sleep

disorders can have long-term effects on patients. “

For many patients, the first step is to have a sleep study performed. For those qualifying patients, thanks to new technology, the study may be conducted in the comfort of the patient’s home.

There are several options available to treat sleep disorders, including a bi-level or CPAP sleep device. Recent advancements in technology have allowed manufacturers to produce much smaller devices than in the past, allowing patients to sleep more comfortably.

WOUND CARE CENTER

USING HYPERBARIC OXYGEN THERAPY TO IMPROVE WOUND HEALING

Patients battling a chronic or non-healing wound often see positive results following hyperbaric oxygen therapy. Wounds resistant to other types of treatments are not a disease in and of themselves, but rather the result of one or more underlying conditions which often require specialized care to proceed through an orderly and timely set of healing stages.

The Wound Center at Pikeville Medical Center (PMC) offers two hyperbaric oxygen chambers which deliver 100% pure oxygen to patients in a pressurized chamber. Non-healing wounds are particularly prevalent among the estimated 25.8 million Americans affected by diabetes, a disease affecting many who live in Eastern Kentucky. Of those diagnosed with type 2 diabetes, 15 to 25 percent are at risk for developing foot ulcers.

Hyperbaric Oxygen Therapy (HBOT) is an advanced way of healing these types of wounds, and can also be beneficial in treating carbon monoxide poisoning, decompression illness, radiation necrosis and open bone fractures. During HBOT, patients experience pressure equivalent to diving 40 to 65 feet underwater. HBOT is usually scheduled in consecutive sessions over many weeks and offers several patient benefits. It enhances the delivery of oxygen to tissues, which promotes the healing of wounds and helps fight infection.

Increased oxygen levels also stimulate the production of collagen, a key component in tissue repair and regeneration. This type of therapy can also improve the effectiveness of certain antibiotics and enhance the body's natural defense mechanisms by increasing the amount of oxygen blood can carry.

COMMUNITY NEEDS & ACCESS TO CARE

Living in the rural area of the Appalachian Mountains can be a barrier to many things. As a tertiary referral center, Pikeville Medical Center has become the axis of care delivered to a significant part of the tri-state areas of Kentucky, Virginia, West Virginia, and even parts of Tennessee and Ohio.

Our facilities are located within five (5) major market areas including: South Williamson, Whitesburg, Pikeville, Martin and Prestonsburg. These communities have an approximate population of 450,000 which derive about 75% of PMC's patient volume In addition to the overall economic situation, many in the region have fall victim to the opioid epidemic and have found themselves in addiction treatment facilities with little access to preventative care.

EXPAND ACCESS TO CARE

Strategy: Expand Specialty Services in Clinic Locations & Telehealth Offerings

Currently, PMC has specialty clinics in Whitesburg, Prestonsburg and South Williamson, KY and Grundy, VA.. Those clinics have been designed and built with additional exam room capacity for expansion of specialty services. The family practice locations in Prestonsburg and the new Urgent Treatment and Wellness Center in Pikeville, also have the space to add specialty care.

More specialists from various services lines, will now be able to see patients in their homes, at least for their initial visits and evaluations. Depending on the level of treatment and follow up required, patients could come to the main campus or continue to be seen at the satellite locations.

Additionally, for some patients and specialists, we will expand the availability of telehealth services for an added level of convenience

Strategy: Healthy at School

Expanding the focus on pediatric services, PMC and local school districts have launched a collaborative initiative called “Healthy at School.” We intend to expand this service in other school districts in the region. The intent of this program is to provide access to care to the students of this region who may have otherwise gone untreated.

Strategy: Providing Care to the Addiction Recovery Industry

Some segments of our population cannot travel to seek care for reasons other than economic ones. One example is patients who are actively engaged in inpatient addiction recovery. Many treatment programs cannot offer cost prohibitive wellness programs nor transport residents efficiently. Therefore, the residents of many addiction rehabilitation institutions have limited access to wellness programs further complicating their recovery efforts. IMPROVING SUPPORT SERVICES

A hospital has many different departments, each responsible for providing differing levels of service that make a hospital run efficiently. These departments comprise a wide variety of support services including biomedical engineering, facilities engineering, environmental services, laboratory, dietary, radiology, information technologies and many others. All of these support services are necessary to keep the hospital going, so hospital staff can provide care and patients can receive treatment. These support services also have an opportunity to drive operational improvements. Typically, these departments operate independently of each other. Many times, each department relies on its own software systems to manage workflow From an end-user/customer support perspective, however, these silos become problematic, making it difficult for hospitals to meet the needs of the staff and patients alike.

One of the biggest initiatives a healthcare organization can take on is the replacement of the Electronic Health Record system (EHR), followed closely by the replacement of a revenue cycle system. Most major healthcare organizations have recognized the value in having all of that on a single platform. Pikeville Medical Center is no different. As the healthcare industry becomes more competitive, and as reimbursement is increasingly based on the performance of quality metrics, PMC must have the tools with which to work.

The PMC wireless network, as it is today, provides wireless coverage for mobile devices in most areas of the hospital. We offer wireless connectivity to both our corporate network as well as a public network for guests. The existing systems face issues ranging from several dead spots throughout the facility, outdated hardware that can't be updated, non-compatibility of our old hardware with newer technologies, and the inability to upgrade our wireless phone system to use Wi-Fi technology. These issues will be intensified with the implementation of the EPIC system.

GOAL: IMPROVE ACCESSIBILITY AND SECURITY

Strategy: Implementation of EPIC

PMC's Board of Directors and Executive Leadership Team have committed to one of the single largest projects in this hospital’s history by moving to a single integrated EHR platform. Implementation of EPIC, the top EHR system in the world, has helped decrease our IT application footprint and moved the majority of our departments and staff to a single unified system. Preparing for the EPIC system involved intricate details of planning and software customization to meet the needs of all departments Existing hardware was evaluated and replaced as necessary. A comprehensive process of data migration ensured that all patient data from the legacy systems would be available on this new system. Cordinate between departments followed by intense training for all staff on all levels was conducted before we can begin to utilize the system.

Strategy: Implement Security Enhancement

In order to keep PMC safe from outside cyber threats which is vital to the organization, additional security

solutions will be added for more layers of protection. set up identity access management to manage user identities, govern access to resources, enforce security, and ensure compliance. Also implement a solution for Data Loss Prevention (DLP) ) – Varonis For Cloud Storage and network shares

Strategy: Improve Wireless Communication and Access

In order to improve our wireless communication ability, we need to replace all current wireless access points in the facility as well as add new access points to remove any dead spots in coverage. The newer system is compatible with our new wireless network. In order to save cost, we will be repurposing the current access points from the hospital and reinstalling those in outside offices. Those access points will be more than sufficient to handle the speed and number of users. This will also upgrade their access points from older technology, currently in use at these locations, to the current technology that is in place within the hospital.

Strategy: Providing Care to the Addiction Recovery Industry (continued) programs cannot offer cost prohibitive wellness programs nor transport residents efficiently Therefore, the residents of many addiction rehabilitation institutions have limited access to wellness programs further complicating their recovery efforts. If the healthcare needs of these residents heighten to a level of urgent care, the logistical and financial burden on the treatment facility can be extensive, resulting in the decline of their program capacity.

PMC is pursuing a new project working with Addiction Recovery Care's (ARC's) 13 residential sites in Kentucky to provide telehealth services to their residents. This will allow us to offer a level of care many of these individuals have never previously had which will assist in their recovery. This is a new concept of offering health care for the addiction recovery, and will serve as a model for many other organizations in the state, region and nation.

Implement Virtual Nursing

Virtual nursing is the delivery of nursing care and services from a remote location. Virtual nurses are responsible for monitoring multiple patients while collaborating with the nurses, physicians, therapists, and other staff who provide care at the patients' bedside. There are several benefits and models of care with Virtual nursing.

Virtual nurses augment the care delivered by bedside nurses by alleviating their workload and providing greater patient satisfaction. Nurses at the bedside can focus on clinical tasks and patient care, while a highly trained, flexible, virtual workforce is connected to inroom technology to manage the more administrative tasks.

Virtual nursing workflows for patient observation, companionship, care concierge, and administrative tasks can ease workloads and improve care delivery.

Hybrid patient care, combined with bedside and virtual care, allows hospitals to keep nurses working at the top of their licenses and create scalable workforce options. Meanwhile, patients enjoy greater convenience, access to caregivers, faster quality care, and an all-around improved patient experience.

GOAL: EXPAND PROJECT H.E.A.R.T. & WORKFORCE CENTER PROGRAM

Strategy: Expand Workforce Education

PMC's collaboration with organizations like East Kentucky Concentrated Employment Program (EKCEP) and others will provide training and education to expand labor participation, and enhance communities through skill building and provide a better standard of living.

To further grow local nursing programs and increase nurse graduates, PMC will enter into a partnership with the University of Pikeville to creatively enhance their program. We developed incentives to ease the cost of school and make it easier for these students to complete their education. This program has been replicated in other fields that apply to the healthcare industry, such as environmental service and hospitality management, both of which are integral parts of hospital operations.

Strategy: Provide Second Chance Employment

PMC's leadership has made a bold step to become a second chance employer. Second chance hiring is a great opportunity for both organizational and individual growth. By hiring previously incarcerated individuals, people in recovery or other applicants disadvantaged by life's choice, we can help them develop the skill sets they need in order to obtain and maintain stable employment.

We work with ARC to provide skill development and job opportunities for qualified people that have been a part of addiction recovery. Individual recovery plans will include training of skills for positions within the hospital and for which those individuals are suited. Additionally, our partnership with the local jails will offer telehealth services, allowing us to assist those incarcerated find their way to a better life.

Strategy: Build a Medical Training Facility

We designed and built a Medical Training Facility where all area individuals involved in the healthcare industry can work together and develop an active learning community. This has provided a place where medical staff, students of health care and members of the community can work in collaboration to create robust teaching and learning experiences. This has built vital community partnerships to create an intellectually diverse and active learning center as it pertains to healthcare.

• Pikeville Medical Center is proud to collaborate with the University of Pikeville, home to one of only three medical schools in the state (UK, U of L & UPIKE).

• PMC provides a medical facility for clinical rotations. Additionally, PMC plans to work with UPIKE to offer dental services after the planned opening of the university's new College of Dentistry.

• UPIKE is also home to the only optometry school in the state – while PMC clinic hosts students for their clinical training (UK, U of L and Western KY each only have a preoptometry program).

• Soon UPIKE will be home of the third dental school in the state alongside UK and U of L.

• The Nursing Fellowship Partnership Program is a collaboration with UPIKE and PMC to provide financial assistance to nursing students.

• Big Sandy Community and Technical College is officially located the Pikeville campus and opened for class 2021.

• Galen College of Nursing is located on our PMC campus and opened for class 2022 .

• As a third nursing school, ANU relocated to PMC campus this year, 2023.

• In addition to offering nursing degrees, ANU will also be offering associate degrees for positions in the laboratory, respiratory, radiology or certified pharmacy techs.

Pikeville Medical Center collaborates with the Kentucky College of Osteopathic Medicine, Kentucky College of Optometry, all area nursing schools and area colleges that offer 2-year associate degree programs in the medical field (respiratory therapy, medical laboratory technology, radiologic technology) to provide students with clinical rotations and training. PMC also offers an extern program for nursing students to gain valuable training experience.
