ADDENDUM #1 June 23, 2015 Barron County Housing Remodeling Project Rice Lake, Barron and Chetek, Wisconsin Bids Due: 10:00 am, Monday, June 29, 2015 This addendum is issued to modify, explain or correct the original drawings, specs and bidding documents and is hereby made a part of the Contract Documents. Please attach this addendum to the plans in your possession. Make a note of receipt of this Addendum on your submitted Proposal.
PLANS Sheet G2: Construction Note #2: Include stall striping, including a HC stall with symbol and diagonal stripes at adjacent “loading stall”. (2 @ RL, 1 each @ Barron & Chetek) Contractor to install handicapped parking signs for all handicapped stalls. Sign to be 12”x18” and to conform to all WI DOT standards for type II sign. Mount signs to 2” galvanized steel post. Barron & Chetek-one sign each, Rice Lake-one sign between the two HC stalls, with right/left arrow and small sign below indicating “van accessible”. Construction Note #3: 3” tapered fill is average around Rice Lake building. (Some areas will have no fill, some will have up to 8-9”) Construction Note #6: accessible units to be RL#4 in addition to RL#2. Construction Note #7: Remove existing note. New #7 to read: Dark, dashed line on plan indicate location of backing installation. Shower stall to be ordered with reinforcing to accommodate future grab bar installation. Reinforcing beyond shower stall to be 2x8 or 2x10 blocking installed horizontally between studs and centered at 33” AFF. Drywall removed for installation to be patched with new ½” drywall containg 90% recycledcontent gypsum. Drywall to be taped, textured & painted to match adjacent wall. Sheet G3: Construction Note #15: General Contractor to provide, to Owner written documentation that the removed carpet has been recycled. Construction Note #16: Kitchen sinks/faucets to be disposed of, not turned over to Owner. Sheet G4: Accessories Schedule: Toilet paper holder to be Bobrick B-7685, surface mount on concealed brackets, stainless steel. Towel bars to be Kohler “Alteo”, chrome, K-37050CP (18”) and K-37051-CP (24”). Robe hook to be Kohler “Alteo”, chrome, K-37055-CP. Mirror and medicine cabinet to remain as shown. Appliances, ADA refrigerator: may be over/under style, standard 2’-6” width, 50% of freezer space, 100% of refrigerator space and all controls must be located at less than 54” AFF. Must be self – defrosting. Appliances, range: Alt Bid to read: Replace ranges at 22 apartments, 18 to remain.
Sheet G5: Plumbing Fixtures, HC dishwasher to be: Equal to GE - GLDA690FWW, 24” wide, to fit under 34” countertop, ADA compliant, Energy Star. Sheet G6: Electrical Equipment Schedule: Add: Furnish to Owner (1) “Alertmaster 100” signal system which will be provided for one “hearing impaired” apartment. Door Schedule, Doors A,A-1,B: Include Reese weatherstripping and sweep to all exterior doors. Door Schedule, Door H: Include Hardware Group 5. Sheet G7: Window Schedule: Windows A and B to be slide-by units (Door A with stationary center sash, slide-by on both ends) Both windows to meet egress requirements and with ‘U’ value = .28 Sheet G8: Cabinet/Countertop Description, Greaseshield: Include the same Broan wall shield on sidewall of the fully accessible apartments. To be 24”x24”. Sheet G9: Note #7: See changes to note on Sheet G2 Add Note #26 – All apartments and Laundry Sheet G10: Notes for “partial accessible” units: Refrigerator to be over/under style, ADA refrigerator as described on this Addendum under Sheet G4. DW to be HC as described in plumbing fixture schedule. Only Unit #1- Barron and Unit #1- Chetek are “partial accessible”, not Rice Lake. Provide matching, structural, finished end panel adjacent to DW to provide countertop support. Sheet G12: Fully accessible apartments are to be Rice Lake #2 and #4. Electrical panels to be moved to new wall and lowered to side reach of 54” AFF to top breaker. (Two apartments only) Electrical notes: @ Kitchen move HD range outlet to new location, add new outlet for new refrigerator location, add new outlet for DW, rewire for new hood location and remove existing refrigerator outlet. @ Bath move GFI outlet to new wall location. @ Hall move outlets & switches to new wall location. Lower shelves & poles at entry closet and two HC bedroom closets to 54” max. side reach (top of shelf). Patch and paint. Base cabinets (L-R) to be: range/24” std. door-drawer/3 x 3 lazy sue/3’ open with finished apron/24” DW/33” open with matching & finished end support and apron/refrigerator Wall cabinets (L-R) to be: range hood and 18” std. wall cabinet/3630 std. wall/2430 corner wall/1230 std. wall/3618 std. wall/2430 std. wall/3330 std. wall/3618 std. wall Sheet G13: 10” diameter, round sono-tube acceptable for pedestal foundation. Provide two, surface mounted weatherproof, duplex receptacles with spring covers.
Sheet RL1: West parking lot: Add 10’ x 35’ cut-out & replace area with Note #1, (10) 5’x7’ heat patches along east facing stalls and (2) 5’x7’ heat patches over oil spills near west facing stalls. East parking lot: Add (10) 5’x7’ heat patches along east facing stalls. Sheets B2, C2 and RL2: Apartment # designations are incorrect. Change Barron B3 to B1, change Chetek C3 to C1, change Rice Lake RL8 to RL2 and RL2 to RL4. (These are the “partial” and “fully accessible” units) Exterior laundry doors to be Door B, with notes #10 and #11 Include note #26 at Laundry Change exterior hall Door B to Door A1. Add storage room doors under stairs as Door C. Add Note # 27 On Note #13, add “and laundry” Sheets B3, C3 an RL2: Windows shown on 2nd floor hall ends are “rescue deck doors” and are Door C. Include Note #27 at each “rescue deck”. Sheets B4, C4 and RL3: Add Note #8 to each Living Room AC location on 2nd floor. Change 2nd floor bathroom Door E to Door F. (Already designated F on RL plan) Note #6 to read: except at enlarged D doors Add notes #24 & #26 to all apartments. SPECS AIA A102, Standard Form of Agreement between Owner and Contractor: Delete liquidated damages paragraph under 4.3 Remove sections 15.6.1 and 15.6.2 from contract form. All references to WHEDA to be disregarded. Bidders to include overhead, general conditions and profit that they deem fair and competitive. Owner is willing to negotiate a reasonable schedule with the contractor.
