Inspection Text This inspection report is in regard to the rooftop of the college. Areas of the roof which may be accessed by workers should be delineated to direct travel away from proximity to the roof edge. Discussions with Mr. S. Traviss (Safety, Security and Parking Coordinator)on this issue produced the following possible solutions: A raised walkway with nonslip material that could have handrails affixed should be considered for rooftop pedestrian control. This would serve to direct foot traffic away from possible danger areas. Signage should be posted at the rooftop access in regard to the hazards in the areas. Other specific rooftop projects may require risk assessments before work commences. Handrails should be installed on the side of the incline ramp joining one roof area to the other. They could be made removable for access by large HVAC equipment etc.

Occupational Safety Officer (OSO) Ron Morehouse and Occupational Hygiene Officer (OHO) David Ogilvie, on the morning of April 9, 2002, in the company of Steve Traviss, H&S Coordinator, and Karen Hourston, Safety Specialist Facilitator. This is the older campus of Vancouver Community College (VCC), while a larger campus is located at 1155 W. Broadway. That facility falls within a different WCB inspectional region, and so is periodically inspected by a different set of WCB Prevention Officers. Employees may be members of CUPE, or the Vancouver Community College Faculty Association. VCC has addressed office ergonomics over the last 3 years, conducting risk assessments and changing problematic work-stations. A full inventory of evident Asbestos Containing Materials (ACM) was conducted roughly five years ago, and a large amount of ACM was subsequently removed. The First Aid Room is in the Health Services Area. The door to the First Aid Room needs to be clearly identified as such. Areas inspected included the Bakery Department, Culinary Arts, and the Jewellery Department. , Instructional Assistant, guided us through the 5 kitchens of the Bakery & Pastry Arts Department. It was noted in these kitchens that there had been a lapse in cleaning service to the exhaust hoods, these had been marked by the service provider Commercial Vent Cleaning as due for follow-up cleaning as of November 2001. A major safety issue noted in these kitchens was the lack of safety-guarding on most of the dough mixers. The majority of these were older Hobart mixers. This issue has been dealt with in Inspection Report 2002117080182, prepared by OSO Morehouse. Fire suppression equipment in the Bakery area is inspected by Pacific Coast Fire Equipment Ltd., and these inspections are up-to-date. About half of the 8 kitchens of this department were inspected. As is the case in the Bakery Department, the exhaust hoods were due to be cleaned as of November 2001. The ventilation hoods were apparently cleaned on the previous weekend by the maintenance department, although the hoods continue to provide less than adequate capture of smokes. It was noted that hoods serving grills and fryers exhibit small stalagmites of grease. Bench top steam ovens are in use in some of the kitchens. A safe work procedure in effect requires waiting until the oven cools to 150 F before cracking open. Large Bain-marie food warming baths were present in at least 2 of the kitchens inspected. These had an open hot water bath with approximate dimensions of 8 feet long by 26 inches in width and rose off of the floor to a height of about 28 inches. The water in these baths is maintained at temperatures up to 200 F. Skin contact with water at 155 F (68 C) for 1 second will result in a 3rd degree burn. At 200 F a 3rd degree burn would occur practically instantaneously. The open layout of a large bain-marie presents an unacceptable risk of partial to whole body scalds, if an individual slips, trips or loses balance and falls into such a bath. A method of guarding access to these or providing covers must be implemented. A worker in the dishwashing area has developed upper back problems over the last 5 years, and has identified a source of strain. Dirty dishes arrive from the cafeteria via a gravity fed roller conveyance. These dishes must be lifted and placed on an automated conveyor to the rear of the first set of rollers. This involves a long reach on the part of the worker to place the dishes, and may involve twists to pick them up. The risk presented by the reach is further compounded by the substantial weight that a stack of dishes may have. Prior to the lunch hour, there is a two hour rush period during which the speed at which this task must be performed involves much repetition. A risk assessment of this task must be completed and the layout of this work area will most likely need to be modified. As a temporary measure to reduce the exposure of a single worker to this risk factor for musculoskeletal injury, a rotation scheme should be considered to limit the amount of time that individual workers are performing this task. The Asian Food Kitchen was briefly inspected. It was noted that the exhaust hoods have been cleaned more recently by Commercial Vent Cleaning, stickers indicate that these hoods are due for follow-up cleaning in July 2002. 2002. The Jewellery Department was inspected with the assistance of Department Head , and Program Assistant . In the burn-out area wax casts are burnt out of jewellery investment molds in ovens at 1250 F for 5 hours. The ovens are placed under a large overhead canopy hood of unknown exhaust flow. The exhaust of this hood is also is meant to exhaust an adjacent canopy hood over the jewellery casting rotor, to which it is connected via a square opening between the hoods. An appreciable flow of fresh air is supplied to the room area via a ceiling duct immediately adjacent to the secondary hood. The proximity and orientation of this layout may result in a turbulent air flow within the hood, interfering with its capture efficiency. Air flow rates and direction of air movement needs to be investigated by VCC in this area. Vertically extended exhaust pipes attached to the burn-out ovens may assist in directing oven emissions to the overhead duct. Pickling solutions (approx. up to 2 litres) are heated in a crock pot on a bench along the wall beside the burn-out area. One of these contains a solution of sodium bisulfate, which although only mildly corrosive, was reaching high enough temperatures to produce vapour (steam) emissions that may transport irritating acid aerosol into the air. The other solution was 'Liver of Sulpher', which is a sulfurated potash that produces the gases hydrogen sulfide and sulfur dioxide as decomposition products. Both of the solutions must be placed under an exhaust hood so that airborne emissions may be removed. As well, each of these containers need to have a separate tray or pan placed under it to contain any spillage (each tray should be able to contain the entire volume of the solution). The jewellery work studio area has a series of work benches at which jewellery is built or assembled. At most benches a flux solution (BATTERNS Self-Pickling Solution (Boric acid) is present in an open jar. These jars need to be promptly closed after use to prevent spillage. Work around such volumes of corrosive materials requires the use of safety eyewear. A drop-wise dispensing jar for these flux solutions is recommended. Each bench is supplied with a gas/oxygen fed burner for soldering jewellery. Local exhaust ventilation is not supplied at the work benches, rather the room is reported to have had its ventilation specially balanced for the purpose at hand. Production of combustion products requires that air from these processes be exhausted separately so that such contaminants do not re-enter into the general building air. carbon dioxide and carbon monoxide levels needs to be performed to evaluate the adequacy of the ventilation for this room. Provisions for the ventilation in this area shall reviewed with the engineering and maintenance departments of VCC. Dust masks are worn while buffing jewellery with rouge. A diamond lapidary saw mounted in a box contains very dirty, old cutting oil. Cutting oil routinely becomes colonized by bacteria, usually resulting in production of biofilm. Stagnant cutting oil is prone to anaerobic growth resulting in noxious by-products and potentially endotoxins. A schedule for routine changeout of the cutting oil needs to be maintained. Review the use and/or adequacy of biocides used in the cutting oil. Operators exposured to cutting oil need to wear protective glasses, gloves, and may require an apron. This saw was not in operation at the time of this inspection. It is likely that an aerosol of the cutting oil may be produced during use of this tool. These materials present a hazard to the respiratory system. The conventional control measure is to use a dedicated exhaust near the point of operation to remove airborne contaminants. In the meantime, appropriate respiratory protection is recommended. Wax models of jewellery are used to produce a negative mold called an investment. The investment is made using a cementious material that contains free silica. Approximately 3 x 100 lb. drums of Ransom & Randolph ULTRA-VEST Jewellery Investment are used by the Jewellery Department each year. This material is mixed at a sink at the end of a bench that is adjacent to the grinding, buffing and blasting room and is open to the adjacent area where students eat their lunches. A thin layer of fine white dust is evident on cabinet and bench surfaces in proximity to the investment preparation area. The facility for mixing of this material should be reviewed taking into account the silica content of the investment material (reportedly very low), and the frequency of performance of investment preparation (reportedly infrequent). The presence of an exit to the hallway directly adjacent to the investment mixing area is problematic. The opening and closing of this would be expected to cause turbulent air flows that contribute to the mixing of investment dust into the air and further distribution of this contaminated air. Although 3M 8210 N95 disposable dust masks are worn while handling unmolded investment material, mixing should be done in an area that is isolated from other work areas so that contamination does not get into those other areas. These have an open hot water bath with approximate dimensions of 8 feet long by 26 inches in width and rose off of the floor to a height of about 28 inches. The water in these baths is maintained at temperatures up to 200 F, which could result in 3rd degree burns. The open layout of these large baths exposes workers to an unacceptable risk of partial to whole body scalds. Critical Incident Stress counseling has been offered to the workers at this workplace. The cause of death has yet to be determined. A copy of this report must be submitted to this officer upon completion. This worker , performing the activity he performed in the absence of appropriate protection, has been potentially exposed to biohazardous material. reasonable care: Occupational Health and Safety Regulation Section 3.21 - First aid attendant responsibilities (3) The first aid attendant is responsible, and has full authority, for all first aid treatment of an injured worker until responsibility for treatment is accepted (a) at a place of medical treatment, (b) by an ambulance service acceptable to the Board, or (c) by a person with higher or equivalent first aid certification. Workers Compensation Act Part 3 Division 3 Section 116 - General duties of workers (1) Every worker must (a) take reasonable care to protect the worker's health and safety and the health and safety of other persons who may be affected by the worker's acts or omissions at work, Given the above, the first aid attendant did not maintain responsibility over the injured worker and the attendants authority was circumvented by a worker who did not possess the necessary level of qualification as defined in 3.21(3). Additionally, this worker did not take reasonable care to protect himself by performing this act (i.e. performing mouth to mouth without a pocket mask). It was reiterated to this worker that he should immediately seek medical aid as he was potentially exposed to biohazardous material (as a result of performing artificial respiration without a pocket mask). Post exposure protocols would ensue upon his report to medical aid. It is incumbent upon the occupational first aid attendent and worker involved in this incident, to ensure that their respective duties and responsibilities are understood and followed in this workplace. Violations of these duties and responsibilities may expose the incumbents to further action by order. Recommendations for proceeding may ensue upon review of the circumstances and facts associated with such violations. The aforementioned investigation as noted in ii) must, in addition to the main incident (the fatality), pursue this additional incident, of a worker exposure to potential biohazardous material.
