5 YEAR OLD SISTER GIVES GIFT OF LIFE TO HER AILING 10 YEAR OLD BROTHER FIRST HAPLO IDENTICAL BONE MARROW TRANSPLANT SUCCESSFULLY DONE AT FARIDABAD
A
ten year old boy Ferdawos Sediqi from Afghanistan came to Sarvodaya Hospital with a diagnosis of Aplastic Anemia. A rare and serious condition, aplastic anemia can develop at any age. It is a condition that occurs when the body stops producing enough new blood cells. Aplastic anemia left the boy feeling fatigued and with a higher risk of infections and uncontrolled bleeding. The boy’s condition required a Bone Marrow Transplant. The cancer care specialists at Sarvodaya took him for Haploidental Transplant, as his five year old sister had a 5/6 HLA match (incomplete match), while no other sibling had HLA match. Haploidental transplant provides an opportunity for nearly anyone to receive a bone marrow transplant even without an optically HLA matched donor (6/6). The boy was given a reduced intensity conditioning as we didn't have a full match. The transplant was successful without any acute complications of graft rejection (In haploidental transplant, chances of graft rejection and complications are upto 50%). ‘Cancer` has always been one of the most dreaded diseases known to mankind and usually has been equated with poor outcomes. But in the last five decades with tremendous scientific advances, the cure rates for cancer treatment have gone up markedly from dismal 10% to around 65-70% but still some cancers have poorer rates of cure. Pediatric tumors (Childhood Cancers) and Blood cancers have shown most promising results with more than 80% of them being cured, the treatment has changed from conventional toxic chemotherapy to more targeted drugs (which act only on cancer cells and do not harm the normal cells) and immunotherapies. Bone Marrow Transplants have become more common due to affordable costs and very few side effects due to better drugs and techniques.
HEMATOPOIETIC STEM CELL TRANSPLANTATION Bone marrow transplantation is a procedure wherein stem cells from bone marrow that produce various blood cells like red blood cells, white blood cells and platelets are injected into a recipient. These stem cells are also referred to as hematopoietic stem cells. Nowadays, hematopoietic stem cells may also be

obtained from peripheral blood after treatment with growth factors or from umbilical cord. Thus, the term “Hematopoietic stem cell transplantation” is now preferred to “Bone marrow transplantation” to include these other sources of hematopoietic stem cells. The bone marrow is a soft, spongy portion within a bone. It contains immature cells called stem cells that have a continuous ability to produce different types of blood cell i.e. red blood cells, white blood cells and platelets. These are released into the blood stream. Bone marrow is of two types, red marrow and yellow marrow. In children, most of the marrow is red marrow and is rich in stem cells. In adults, however, a large portion of the bone marrow is converted into yellow marrow due to infiltration of fat cells. Red marrow in adults is usually restricted to a few bones like the hip bones, breast bone, ribs, shoulder blades, skull, backbone, and the ends of the arm and thigh bones. BMT means infusion of stem cells (collected either from bone marrow or peripheral blood or cord blood) into the recipient.
The type of bone marrow that is for the transplant distinguishes different types of BMT. 1. Autologous BMT is done by using the patient's own stem cells. 2. Allogeneic BMT involves using the stem cells of a donor. The donor may be a

relative, usually a brother or sister, or an unrelated (anonymous) donor. The donor needs to be Histocompatible Leukocyte tigen (HLA) match. BMT PHASES There are three main phases of the BMT process, Although the experience during these phases may be quite different for each patient, a general overview is provided. to the patient. Bone Marrow Transplant is a major and complex procedure & requires thorough and meticulous planning & forethought. The donor is most likely to be a brother or sister of the patient. However, it is only in 35% of the cases that sibling’s marrow is a perfect match. Sarvodaya Cancer Care Centre Faridabad launched its Bone Marrow Transplant Unit in November 2015. It is a dedicated facility, equipped with ultra-modern infrastructure trained and experienced faculty. The Centre is supported by state-of-theart laboratory services and transfusion services. The Centre offers both allogenic and autologous transplant using bone marrow, peripheral blood stem cells and cord blood stem cells for various benign and malignant disorders like Thalassemia, Aplastic Anemia, Leukemia, Lymphomas, Multiple Myelomas and Advanced Pediatric Solid Tumors. In its short span it has already started giving successful transplant results under dedicated care of Dr. Sumant Gupta (DM Medical Oncology) and his team.
