The Career Development Journey

Postgraduate Requirements After completing an undergraduate course of five years to gain a Bachelor’s Degree of Dental Surgery, graduates can then register with the General Dental Council (GDC) to practice dentistry in the UK. However, they cannot work in the NHS until they have completed a further 12 months of foundation training, which enables them to obtain a performer number. Foundation training involves working under the close supervision of an experienced dental practitioner (who is approved as a trainer). A newly qualified Dentist in the UK is not regarded as competent to go it alone upon graduation and requires this extra foundation training to enable him or her to confidently treat patients. In other words, the dental undergraduate course is only the start of their career in dentistry and certainly does not equip them to carry out anything other than the most basic dental treatments.

Course Choices & Combination Pathways Tipton Training Postgraduate Certificate and Diploma courses give the opportunity to gain recognised qualifications, once a Dentist has gained experience of treating patients in a general practice. eCPD and U

We live in a world of abbreviations, acronyms and composite words, and in dentistry we are no less guilty of using them than anyone else: BDS, BDA, BDIA to identify just three. The new GDC CPD which came into force on 1 January 2018 is called eCPD, which one might expect stood for ‘electronic’ but actually for ‘enhanced’ and here we look at what it means for all GDC registrants.

Continuing Professional Development (CPD) was first made mandatory in dentistry in January 2002 for dentists and then in August 2008 for all other registrants at the time when the majority of Dental Care Professionals (DCPs) also became registered for the first time (hygienists and therapists had been registered before this). Initially there was a great deal of confusion, some anger and quite a lot of panic as dentists grappled with the idea of having to record their continuing education. In truth, the majority were already undertaking regular updates so that once the details of the scheme, which was also trendily dubbed ‘Lifelong Learning’, were better understood most dentists settled into the pattern with little difficulty. Although issuing guidance, from the beginning the GDC made it clear that they did not have the resources to police the providers of CPD and the onus on standards was placed on the users to give feedback which should then be acted upon as necessary. The GDC also modified the requirements as time went on, starting with what they called four ‘core’ subjects: medical emergencies, infection control, radiology and legal/ethical matters and later added oral cancer, child protection and other topics. The ‘new’ eCPD began on 1 January 2018 for dentists and 1 August 2018 for DCPs. In its document on the changes the Council states “The approach to CPD is not one-size-fitsall for professionals, but needs to be tailored to the individual needs of your role, work setting and your patients. That is why the enhanced CPD scheme is designed to be flexible, so you can plan activity to suit your professional needs, and adapt your activity as required across your cycle.”

An end to non-verified CPD One of the most criticised aspects of the original CPD programme, especially by lay people, was the inclusion of ‘non-verified’ hours which were accrued by the dentist recording his or her own activities, such as reading a journal. The claim was that this provided no guarantee at all other than ‘trust’ in the professional (which one might have contested was sufficient) without the verification of a third party. Non-verified CPD is not included in eCPD.

A change in the required number of hours of verified CPD In line with the above change, eCPD reduces the overall number of hours required but now includes only verified hours. It mandates a minimum of 100 hours per five-year cycle for dentists, with at least 10 hours to be completed every two years to ensure regularity of study and to guard against binge CPD in the closing days of a cycle.

The requirement for every registrant to have a Personal Development Plan (PDP) The idea behind the PDP is that it should be used to help the individual identify her or his CPD needs for their cycle, creating a plan for maintaining and developing skills and knowledge within their relevant field of practice. Additionally however, this has to relate to the GDC’s development outcomes (see below) which adds a further new layer of complexity.

A greater emphasis on personally relevant CPD and on reflection As part of the move towards a more tailored approach to CPD rather than a free for all, the emphasis is to be on an individual making more specific personal and career orientated choices in terms of the activities and subjects chosen. In addition, the registrant is encouraged and expected to ‘reflect’ on the CPD outcomes once completed in order to assess their relevance and value both per se and in relation to the PDP. Borrowed from other educational streams the idea of reflection will be new to most dentists other than perhaps some relatively recent graduates and is one of the aspects that longer-term registrants might find more difficult to embrace.

Development outcomes The GDC’s Standards for the Dental Team, set out the ethical principles of practice that all dental professionals should embody in daily practice. These are now brought to the forefront of eCPD planning and activity through four development outcomes which are designed to encourage registrants to link learning activity more closely to the standards and support and to embed the principles further into working life. So now each planned and completed activity has to be linked to at least one development outcome. CPD providers must also give an indication of the anticipated learning outcome(s) in the evidence they provide to participants.

The GDC encourages choice of CPD to cover all four development outcomes in a cycle, although (further confusingly) this is not a compulsory requirement. Each planned and completed activity has at least one of the outcomes A, B, C, or D:

A.

Effective communication with patients, the dental team and others across dentistry, including when obtaining consent, dealing with complaints, and raising concerns when patients are at risk

B.

Effective management of self and effective management of others or effective work with others in the dental team, in the interests of patients; providing constructive leadership where appropriate

C.

Maintenance and development of knowledge and skill within your field of practice

D.

Maintenance of skills, behaviours and attitudes which maintain patient confidence in you and the dental profession and put patients’ interests first.

Transitional arrangements While the above may seem a lot to wrestle with, at least initially, there is one further potential confusion for the hapless registrant to negotiate; the timing of their CPD cycle and therefore how many hours of non-verified and verified can be carried over from their existing cycle (to 31st December 2017) added to the proportion of ‘new’ eCPD verified hours to cover the remainder of the cycle.

Further guidance It is quite possible that many dentists are not yet familiar with eCPD and it may well be that as we come towards the end of another calendar year and minds turn towards managing CPD hours there will be lots of questions asked. Full details of eCPD are given on the GDC’s website (www.gdc-uk.org/professionals/cpd/requirements) and as with most things in life it would be wise to get to grips with it all sooner rather than later. But also, like most things in life, it is easier said than done!

Why Occlusion Matters

If you want to reach the pinnacle of dentistry, it’s important to know what distinguishes ‘great’ dentists from the rest of the field.

We’d like to share one of those traits with you: a deep understanding of occlusion. Most newly qualified dentists are unclear about occlusion because universities don’t tend to cover it. However, occlusion is the major factor which determines whether restorations succeed or fail. It’s why Tipton Training covers the concept in-depth on our Postgraduate Certificate in Restorative Dentistry. Occlusion - when applied in patient assessment and treatment planning - can directly impact the success rate of treatments. With this in mind, it’s vital that more dentists learn the fundamentals of occlusion. Dentists can use their knowledge of occlusion to treatment plan cases and prevent other ailments, such as headaches, TMJ inflammation and muscle pain.

What Is Occlusion? Occlusal assessments evaluate the contact or relationship between the upper and lower teeth as they approach each other - i.e. when a patient bites. By familiarising ourselves with occlusion, we can gain a better understanding of where and how problems are being caused. Occlusion is the cornerstone of successful dentistry. However, it also is perhaps the most misunderstood subject in dentistry. Why do restorations performed with occlusal understanding last the test of time, whilst a lack of occlusal understanding causes iatrogenic damage to patients? Ninety per cent of patients have occlusal disease, so learning the principles of occlusion will enable you to treat occlusal disease confidently. Understanding the language of occlusion will allow you to fully integrate the five principles of occlusion into your daily dentistry. Remember - occlusion isn’t just something that affects tooth contact, but the supportive tissues, neuromuscular system, TMJs and cranial bones. Dentists who wish to develop an understanding of occlusion must be prepared to go above and beyond to spot issues and learn how to solve them. It is this incisiveness that separates great dentists from good dentists.

Choosing A Dental Implantology Course

Dental implantology is an established and a part of everyday treatment for patients. It’s also a significant earner for dentists and their dental practices. That’s why we are seeing an increasing number of young dentists move into this area. However, a yearlong dental implantology is mandatory before you can place and restore dental implants as per GDC guidelines. So how should you choose a dental implantology course? The first thing to consider is whether a course is true live patient treatment programme i.e. does the course fees include live patient treatment? Some education providers price courses in different ways. That’s not the case at Tipton Training, we quote fees on a per day basis - and the fees are all-inclusive.

It’s important to note here that most implant courses charge/collect the patient fees for themselves regardless of where the patient comes from. Another important criterion to consider is professional indemnity. You need to have your own professional indemnity even when learning how to place and restore implants. Speak to your own professional indemnity provider, they will give you a tailored quote. Lastly, it’s of utmost importance to consider the topics covered and the experience of the faculty. There is no easy way of doing this, particularly if you are a young dentist. We suggest considering these key aspects: osseointegration, treatment alternatives, patient selection, surgical and prosthetic treatment planning and live patient treatments. Some dentists choose dental implantology courses that are too advanced. 10 Things To Know When You Step Out Of Dental School

1.

You can be an NHS dentist or a private dentist. Or, like the majority of dentists in the UK, you can work in a mixed practice. As you leave university, you need to choose where you want to work.

2.

NHS dentists earn by delivering UDAs - it’s often referred to as the UDA treadmill. Private dentists can set their own fees - but these are influenced by market demand.

3.

NHS contract reforms have been happening since 2006.

4.

Get professional indemnity. It can be expensive, but it is worth it. There are various options you can choose from.

5.

Litigation levels are increasing within dentistry.

6.

As a dentist, you are expected to submit a certain number of Continuing Professional Development (CPD) hours. In January 2018, the GDC launched Enhanced CPDs.

7.

Enhanced CPDs are quality assured CPDs that have specific learning outcomes.

8.

The GDC wants its members to follow Professional Development Plans (PDP). PDP templates are available on the GDC website.

9.

Continuing education matters. It will lower the chances of facing litigation - and you will stand to earn more income.
