4. Referral Process In order to access any of our training, we require a completed referral form from your workplace. The referral form can be obtained from our administrator and should contain: Contact information of the child and the referrer. Details of the child’s healthcare needs. Details of who needs to be trained (usually this is five staff maximum per child). Signed consent from the child’s parents or person with legal parental responsibility. Referrals are processed upon receipt and we will notify you of the proposed action plan for completing training. They are then presented to a multi-agency panel, which meets on the first Friday of every month. Urgent referrals, such as training to facilitate an emergency placement, can be fast-tracked and started before presentation to the panel. If your child accesses a service where you feel the staff will need training, please advise the manager or your child’s social worker to contact us to make a referral.
