Cambridge Pulmonary Rehabilitation For Healthcare Professionals
MRC breathlessness scale Grade Degree of breathlessness related to activities 1 Not troubled by breathlessness, except on strenuous exercise. 2 Short of breath when hurrying on the level or walking up a slight hill. 3 Walks slower than most people on the level, stops after a mile or so, or stops after 15 minutes walking at own pace. 4
Stops for breath after walking about 100 yds or after a few minutes on level ground.
5 Too breathless to leave the house, or breathless when undressing.
Pulmonary rehabilitation Patients attending our exercise and education programme will benefit from improvements in exercise capacity, reduction in breathlessness, and improvement in health-related quality of life. They are also less likely to need to call a doctor out of hours or be admitted to hospital. The majority of patients considered for pulmonary rehabilitation programmes will have chronic obstructive pulmonary disease (COPD). NICE suggests that pulmonary rehabilitation should be offered to all patients who consider themselves functionally disabled by COPD. Pulmonary rehab is not suitable for people unable to walk, who have unstable angina or other unstable cardiac conditions or a recent, myocardial infarction (heart attack). Rehabilitation should be considered at all stages of the disease, usually at an MRC dyspnoea scale grade of two or higher. Patients with serious comorbidity such as cardiac or locomotor disability will not derive as much benefit.
* Programme content Patients should attend twice a week for eight weeks and will exercise for an hour with rests every two minutes, depending on the patient. Exercise programmes are tailored to the individual. The 14 education sessions include breathing exercises, nutrition, pacing, medication and inhaler technique, what is COPD? etc.
