You have been referred to the Provide Physiotherapy Service and we are inviting you to attend a Community Appointment Event.
This has been organised with the aim of moving away from traditional healthcare boundaries and combining a comprehensive range of MSK services, including assessments, advice, health promotion, rehabilitation and community and voluntary sector support, all in a non-medicalised environment and all on one day. This is an opportunity for us to work together to understand what is important to you and to get you access to what you need all in one place. This is something new and very different.
Date – Thursday 23rd January 2025
Time – Appointments available between 0900 and 1530.
Location – Chelmsford City Football Club, Salerno Way, Chelmsford, CM1 2EH
Parking – parking onsite.

What to expect?
When you arrive, you will be greeted by a member of our team and checked in.
During this appointment you will have opportunity to discuss your specific musculoskeletal (MSK) problem and ‘what matters to you’. There will be no individual clinic rooms, but there will be the entire experienced physiotherapy team on hand, as well as a range of non-medical services from the community. The aim of the event is to focus on what matters to
you as a whole person and not just individual elements of pain or conditions.
Your appointment may take up to 90 minutes depending on the services you choose to engage with. Please be aware that this will be an open environment and if you wish for a therapist to look at a specific body part, please dress appropriately.
Before the conversation about what matters most to you, it can be helpful to think about:
What do I feel I need from today?
What do I think the problem is? Have I any ideas what it could be?
Have I done any reading or talked to others about what it is?
Have I tried anything that has helped?
You will be given a booklet on the day to keep which is your record. You are encouraged to make notes about what is discussed, who you have spoken to and the outcome of the day. These are then scanned at the end of the day and uploaded to your healthcare record.
