The benefits described in this guide are effective January 1—December 31, 2022. If you and/or your dependents have Medicare or will become eligible for Medicare in the next 12 months, a federal law gives you more choices about your prescription drug coverage. What’s New This Year? Medical Benefits and Premiums 

To meet your needs, we will continue to offer three options through Blue Cross Blue Shield of Texas (BCBSTX) and one Kaiser California HMO plan, giving you the power to make decisions on how you spend your healthcare dollars.

There will be some benefit changes to the Premier PPO Plan, as well as enhancements to the Core CDHP. The 2022 plan designs for each Medical/Rx plan are outlined in more detail on page 5 and 10.

Premium rates for the Premier PPO Plan and Kaiser California HMO Plan will increase effective January 1, 2022, while premium rates for the Core CDHP and Base CDHP will remain the same.

New Pharmacy Carrier—CVS Caremark 

Effective January 1, 2022, CVS Caremark will be the new administrator of the pharmacy benefits on the Premier PPO Plan, Core CDHP, and Base CDHP.

You will receive a welcome kit in the mail from CVS Caremark containing important information about the transition, including pharmacy network information and instructions on how to transfer your home delivery prescriptions.

When filling prescriptions at the pharmacy on or after January 1, 2022, you will need to present your new BCBSTX ID card (see section below for more details).

New Medical/Rx ID Cards 

If you enroll in the Premier PPO Plan, Core CDHP, or Base CDHP you will receive a new ID card in the mail from Blue Cross Blue Shield of Texas (BCBSTX).

This new ID card will contain updated Medical coverage information as well as your new CVS Caremark Pharmacy information.

You will need to present this new ID card when receiving care or filling prescriptions at the pharmacy on or after January 1, 2022.

Eligibility & Making Changes As a regular, full-time employee who is scheduled to work at least 30 hours per week, you are eligible for benefits on the first of the month following your date of hire (or date of eligibility if assignment status change). If you are a part-time, temporary employee, you may not be eligible for all of the benefits described in this guide. If you have questions about your eligibility, please contact totalrewards@totalsafety.com. Total Safety extends benefit coverage to your eligible dependents. Eligible family members include:  Your legal spouse;  Your children up to age 26, regardless of marital or student status;  Unmarried children of any age who are incapable of self-support due to a mental or physical disability and who are totally dependent upon you; Children include your natural children, stepchildren, legally adopted children (or placed for adoption), or other child for whom you are the legal guardian. Age limits and other restrictions may vary by plan.

When Can I Enroll? As a New Hire

Qualified Life Event

You can enroll in benefits online within 31 calendar days of your eligibility date, which is typically your date of hire.

Annual Enrollment is your once-ayear opportunity to review and make changes to your benefit elections for the upcoming year.

You have 31 calendar days from the date of a qualified life event to submit any election changes to Human Resources (see below).

The election choices you make when you first become eligible or during annual enrollment are in effect for the remainder of the plan year (January 1—December 31). It’s important to review your benefit options and choose the best coverage for you and your family.

Changing Your Benefits During the Year The benefit choices you make during enrollment will remain in effect for the entire year unless you experience a “qualified status change.” Qualified status changes can include, but are not limited to, the following:  Marriage or divorce  Birth or adoption of a child  Change in employment for you or your spouse that affects your benefit eligibility  Loss of other health coverage You have 31 calendar days following the date of a qualified status change to complete and submit your enrollment change, including any dependent verification documentation, to the Total Safety Benefits Center. Any changes you make must be related to the status change, as required by IRS regulations. You cannot switch medical plans mid-year due to a qualified status change.

Dependent Verification In order to add family members (eligible dependents) to your benefit plans, you must submit verification documentation including Social Security or qualified alternate ID’s. The required documentation will vary based on the relationship to the dependent. When you enroll a dependent, a member of the Total Safety Benefits Center will contact you requesting the appropriate documentation. Please note: if documents are not received within the time period allowed, your dependent(s) coverage is subject to cancelation. 3

The most you have to pay for covered services in a plan year. After you spend this amount on deductibles, copays, and/or coinsurance, your plan will pay 100% of the cost of covered services for the rest of the year.

Comparing Your Medical Plan Options The main difference between a traditional PPO plan—like the Premier PPO Plan—and a Consumer Driven Health Plan (CDHP)—like the Core CDHP, the Base CDHP, and the Kaiser HMO—is how and when you pay for your health care. You can save on your out-of-pocket costs by choosing generic drugs. Brand and generic drugs are regulated by the FDA and are chemically and medically the same. However, generic drugs typically cost significantly less. Talk to your doctor to find out if generic drugs are right for you.

Preferred Brand Drug

A commonly prescribed name-brand drug that is selected based on its clinical effectiveness, safety, and cost. Preferred brand drugs will generally be more expensive than generic drugs, but less expensive than non-preferred brand drugs.

ACA Preventive Drug List

When you are prescribed medications on the Affordable Care Act (ACA) Preventive Drug List for preventive purposes, you pay $0, even if you or your family have not met your annual deductible. As a reminder, these drugs could also be prescribed for treatment or other purposes, making them subject Non-Preferred Brand Drug to the deductible or a copay. Mandatory Generics You can save money by using safe, effective generic medications when possible. According to your prescription benefit plan, you may need to try a generic medication first before certain brand-name medications will be covered. When a generic is available but you choose to fill the brand-name medication instead, you will pay the difference between the brand-name medication and the generic plus the generic copay or cost share. Please note this penalty amount will not apply toward your annual deductible or out-ofpocket maximum.

CVS Caremark Maintenance Choice Medications taken regularly to manage an ongoing condition (such as diabetes, asthma, or high blood pressure) are called maintenance medications. The coverage of maintenance medications is limited to two fills at a retail network pharmacy other than a CVS pharmacy or in a 30-day supply. Before your third fill, you will need to fill your maintenance medications through the CVS Caremark Mail Service or a retail CVS Pharmacy in a 90-day supply to avoid paying the entire cost of medication. Health Savings Account (HSA) A Health Savings Account (HSA), gives you the opportunity to set aside money via pre-tax payroll deductions, which you can then use to pay for qualified medical expenses for yourself and your eligible dependents using your HSA debit card. This includes doctor visits, prescriptions, dental and vision expenses, and more. Because HSA contributions are made through pre-tax payroll deductions, you can decrease your taxable income and increase your take home pay by taking advantage of this account. Please note that the HSA is ONLY available to employees enrolled in the Core CDHP, Base CDHP, and Kaiser HMO Plan. For employees enrolled in the Core CDHP and Kaiser HMO Plan, Total Safety will contribute annually to your HSA. Total Safety’s contribution to your HSA will be funded in equal amounts per pay period over the course of the plan year (January 1—December 31, 2022). For 2022, Total Safety will contribute $500 for individual coverage or $1,000 for family coverage. Employees must contribute at least $1.00 per paycheck to their HSA in order to receive Total Safety’s contribution. Combined employer and employee contributions cannot exceed $3,650 for individual coverage or $7,300 for family coverage in 2022. Individuals age 55 and older may contribute an additional $1,000 in 2022.

The Dental PPO Plan helps you with the cost of preventive, basic, and major dental services for yourself and your eligible dependents. This plan gives you the freedom to select any dentist, but you will pay less out-of-pocket when you choose a provider in the Blue Care Dental Network. Critical Illness Insurance It takes a lot to beat a serious illness. Unfortunately, it can also cost a lot. When you or a family member suffers a serious illness like a stroke, heart attack or cancer, Critical Illness insurance can help with expenses that Medical insurance doesn’t cover, such as deductibles, out-of-pocket costs or services like experimental treatment. The lump sum benefit is paid directly to you when you need it most, upon diagnosis, so you can focus on your health.

Hospital Indemnity Insurance Being hospitalized can be financially stressful. While your Medical insurance is designed to cover the cost of treatment, Allstate’s Hospital Indemnity policy pays a cash benefit directly to you as the policyholder, which can be used however you choose. Hospital Indemnity insurance helps provide financial peace of mind, allowing you to focus on recovery during your hospital stay—not your expenses. Women’s Health and Cancer Rights Act Federal law requires a group health plan to provide coverage for the following services to an individual receiving plan benefits in connections with a mastectomy:  Reconstruction for the breast on which the mastectomy has been performed.  Surgery and reconstruction of the other breast to produce a symmetrical appearance.  Prostheses and physical complications for all stages of a mastectomy, including lymphedema (swelling associated with the removal of lymph nodes). Pre-Tax Payroll Deductions: Medical, Dental, and Vision In most cases, Total Safety U.S., Inc. employees’ contributions for medical, dental and vision coverage are deducted from their paychecks on a pre-tax basis meaning before federal income taxes, state income taxes (in most cases), and FICA taxes are calculated. Internal Revenue Code (I.R.C.) Section 125 defines what dependent contributions are eligible for pre-tax deductions. The IRS does not allow employees’ contributions for dependent health coverage to be deducted on a pre-tax basis unless the dependent(s) meet the definition of a tax dependent under I.R.C. Section 125. If they do not meet the definition of a tax dependent, they may be either ineligible for the Plan, or in some cases, the IRS taxes the additional fair market value of these benefits and treats it as Imputed Income. Contributions for medical, dental and vision coverage for eligible dependents that do not meet the definition of a tax dependent will be made on a post-tax basis and the Imputed Income will be included on your paycheck and IRS Form W-2. 21
