Dealing with Frustration, Anger, and Guilt eelings of frustration, anger, and guilt are common in caregiving and can be overwhelming and disturbing. If not dealt with, these feelings can be physically, emotionally, and mentally harmful to you and those in your care. Listed below are some tips to dealing with these strong emotional reactions. Get information. Get an accurate assessment of what you can expect from caring for someone with dementia. Learn about the disease and the care needed. You can do this by reading information from reputable books and websites (contact the Alzheimer’s & Dementia Alliance (ADAW) for information), attending a support group, and talking to others who have cared for someone or to professionals with dementia experience. Denial can interfere with problem solving. Find ways to slowly gain acceptance. Be proactive. Identify your warning signs of frustration, what may be causing these strong feelings, and when you are most likely to feel them so that you can plan ahead for when you might need extra support. If possible, take a step back and get some distance from the stressful situation (e.g., go to the next room or take a walk). Identify what you are feeling. Do you feel angry? Guilty? Sad? Acknowledge what you are feeling even though it may be unpleasant. Feelings are feelings and are not good or bad. It is how you react and respond to your feelings that makes the difference. Analyze what triggers the stressful feeling. What events trigger these feelings, and what is it about a situation that makes you feel a certain way? How do you react? What does the situation mean to you? Talk about your feelings with someone who you trust, or write them down to express them. Talk to a professional if you are getting overwhelmed. Develop a support system (e.g., friends, relatives, professionals). Make a plan. Figure out what you can do differently. Make the plan very

concrete. Decide what specifically you will change and how. Suggestions: • Have a mantra, spiritual verse, or quote that you repeat when stressed • Use mental games such as positive self-talk • Mentally monitor your response (e.g., “How much can I take before responding?” “Can I respond differently from last time?”)

• Keep a private journal of your successes. Set priorities. Determine what is most important and don’t sweat the small stuff. Be realistic about expectations for yourself and for the person in your care.

Accentuate the positive. Focus on the good that remains and not on what’s lost. Take care of yourself and take time just for you. Be specific and realistic. Set limits. You need to have a realistic plan of how you will get time away and set up a specific time when it can be done. Use your support system to help you do this. Practice forgiveness. Nobody’s perfect. You are doing the best you can in a difficult situation. If you are not sure where to begin or are facing a particularly challenging situation, be sure to call the Alzheimer’s & Dementia Alliance of Wisconsin to arrange a care consultation. he brain is a complex organ responsible for controlling many different bodily functions. When working at optimal capacity, the brain is a wonder to behold. When illness or trauma affects the brain, various parts of the body may not work as they should. One of the more devastating things that can affect the brain is stroke. Stroke describes a sudden stoppage of blood from reaching the brain. Harvard Medical School states that if a large number of brain cells are starved of blood supply, they can die. With their demise, a person’s memory and ability to speak and move can be compromised. While many strokes come on suddenly, certain factors may indicate a person is at risk. Such factors may include prior heart attacks, genetics, high blood pressure, smoking, or a prior stroke. However, in a particular type of stroke Ñ a “silent stroke” Ñ symptoms are far more subtle and difficult to spot. Silent cerebral infarction, often referred to as “SCI” or “silent stroke,” is a brain injury likely caused by a blood clot interrupting blood flow to the brain, offers the American Stroke Association. Silent strokes increase risk for other strokes and can be a sign of progressive brain damage. A silent stroke is typically only noticed as a side component of an MRI of the brain. Many times, patients do not recall having a stroke and never felt any symptoms. Silent strokes should not be mistaken for ministrokes. Mini-stroke is a brief but discrete

and memorable event, with symptoms appearing for a few minutes or a few hours.

Silent stroke may not exhibit any symptoms, making it more difficult to detect.

According to a study on silent stroke titled “Functional and Cognitive Consequences of Silent Stroke Discovered Using Brain Magnetic Resonance Imaging in an Elderly Population” and published in the Journal of American Geriatrics Society, silent strokes are quite common and can have serious consequences. Researchers have found that silent stroke is associated with impairments in tests of cognitive function rather than movement-oriented performance tests like rising from a chair. Almost 50 percent of studied silent strokes affected frontal circuit components of the brain, such as the frontal cortex, basal ganglia and thalamus. Lesions in these brain structures compromised executive functions and were related to vascular dementia. Another study showed

associations between silent stroke and visual field deficits, weakness in walking on heels, history of memory loss, migraines, and lower scores in cognitive function tests. The “silent” part of a silent stroke also refers to the areas of the brain that the stroke affects. Experts at Harvard Medical School explain that, during a silent stroke, an interruption in blood flow destroys areas of cells in a part of the brain that is “silent,” meaning that it doesn’t control any vital functions. Researchers say that, over time, the damage from silent strokes can accumulate, leading to more and more problems with memory. Collectively, silent strokes become silent no longer. There are certain ways to reduce the risk of any type of stroke. These include: • managing high blood pressure and high cholesterol levels • quitting smoking • reducing the risk of diabetes and effectively treat the condition if it is present • losing weight to prevent obesity • exercising and avoid a sedentary lifestyle • taking a low-dose aspirin or a drug that prevents blood clots. Silent strokes largely go unrecognized but can lead to significant brain injury. Getting the facts can help men and women reduce their risk for silent stroke.

ndividuals are born into the world requiring the care of their parents and other adult guardians to grow and thrive. These adult children, in turn, may end up providing care when their parents reach senior age or face an illness or disability. Becoming a family caregiver frequently is a tough choice to make. It requires patience and time, and can be emotionally and physically taxing. An estimated 43.5 million adults in the United States have provided unpaid care to an adult or a child in the prior 12 months, according to the National Alliance for Caregiving and AARP Public Policy Institute. It’s not uncommon for caregivers of any age to feel stressed and burned out by the

demands of caregiving. The Mayo Clinic says people who experience caregiver stress can be vulnerable to changes in their own health. Some signs of caregiver stress include: • Feeling overwhelmed or constantly worried • Feeling tired most of the time • Gaining or losing a lot of weight • Becoming easily irritated or angry • Losing interest in activities you used to enjoy • Having frequent headaches, bodily pain or other physical problems Some caregivers even resort to drugs and alcohol to self-medicate, which can lead to further issues. To avoid the potential pitfalls of caregiver stress, individuals should always put their needs first and find ways to alleviate the added stress of caring for a loved one. These suggestions are just a start.

Don’t strive for perfection It isn’t possible to maintain a patient attitude and get everything done perfectly each and every day. People are not perfect and mistakes will be made. Do not punish yourself if you lash out or simply need a break

Eat healthy As anyone who has dealt with a hungry toddler can attest, failure to eat well and frequently can result in an emotional meltdown. Be sure to always make time for nutritious meals. This will help keep up energy stores and enable you to better cope with caregiver stress.

Pay attention to mood changes Anxiety or depression can sneak up on you when you least expect it. Ask for help if you feel your tasks are becoming too overwhelming. Seek the help of a doctor if changes in mood, sleeping patterns, appetite, and the like become noticeable.

Take frequent breaks Getting a break from caregiving and setting aside time for yourself can increase patience levels and the ability to bounce back from stress. Whenever possible, have a friend or another relative step in for you so you get a break. Explore resources available for professional aides to come and take some of the responsibilities off of your shoulders. Being a caregiver can be a rewarding, but challenging role to play. Caregivers should keep their health a priority.

Navigating the waters of funeral trusts

iscussing end-of-life issues can be an emotional experience. Making decisions upon the death of a loved one can be challenging, especially if the wishes of the deceased are murky or unknown. Funeral trusts often help families avoid making decisions at times when their emotions may make such decisions more difficult.

What is preplanning a funeral? Preplanning a funeral can save family and friends from having to make difficult decisions. It also enables people to choose their burial space, services and transportation. Preplanning also allows people to pay for or arrange payment for their funeral costs ahead of time. According to Consumer Protection Ontario, a person can pre-plan a funeral without prepaying. However, many people prefer to have all financial components in place as well for maximum convenience.

What is a funeral trust? Funeral trust can be a smart investment for people who have not spelled out their wishes in a will or conveyed them in

When is a trust unnecessary? In certain instances, a funeral trust will not be needed. Those who have the means to fund long-term care services and will not need assistance later in life may not want their money locked up in a trust. Others opt for life insurance policies to pay for funeral needs.

Assistance with funeral trusts Funeral homes may work directly with companies that manage funeral funds. Speak with a funeral director if you want to learn more about trusts. Trusts also can be established by working with an elder care attorney or with a funeral trust agency. Taking care of funeral planning in advance can relieve families of the stress of making funeral decisions in the wake of a loved one’s death.

Did you know? According to the Centers for Disease Control and Prevention, regular physical activity is one of the most important things older adults can do to promote their long-term health. The CDC recommends that men and women age 65 or older who are generally fit and have no limiting health conditions need at least two hours and 30 minutes of moderate-intensity aerobic activity, such as brisk walking, each week. In addition, such people should perform strength-training activities that work all major muscle groups at least two days per week. While many fit older men and women with no preexisting health conditions are capable of these activities, those able to push themselves a little further can opt for 75 minutes per week of vigorous-intensity aerobic activity, such

as jogging or running, combined with the same strength-training regimen. A combination of moderate- and vigorousintensity aerobic activity coupled with strength training may also provide adequate physical activity for aging men and women. But there are also ways aging men and women can preserve brain health in an effort to prevent or delay the cognitive decline that affects millions of seniors across the globe. It’s easy to overlook the importance of keeping the brain healthy. However, a decline in brain function can result in poor concentration, memory loss and a host of other issues. Sometimes, by the time symptoms present themselves, it may be too late to reverse any damage. Research suggests that a combination of nutrition and mental, social and physical activities may have a greater impact with regard to maintaining and improving brain health than any single activity. Harvard Medical School also states that volunteering, caring for others and pursuing hobbies may benefit the brains of older adults. A study published in the Archives of General Psychiatry found participants who reported higher levels of purpose in life exhibited superior cognitive function

Men and women can engage in challenging activities that stray from their routines. Puzzles, strategic games, jigsaw puzzles, or difficult hobbies can benefit the brain. • Keep a close-knit group of friends. Regular conversation and social interaction is a key component of any brain health wellness plan. Slowing cognitive decline and promoting greater brain health should be a priority for adults of all ages.
