Registration Instructions Rocky Mountain District Key Club 2013 District Convention 1) Three registration forms are enclosed in the packet. Feel free to duplicate if more are needed. 2) Each registrant needs to fill out a form. 3) Three Medical Release Forms and Codes of Conduct are enclosed. Feel free to duplicate if more are needed. 4) Each student needs to have TWO Medical Release Forms filled out. One for the chaperone and one for the RMD Key Club District Administrator. These can be brought to the convention. 5) Medical Release Forms DO NOT need to be notarized. Housing Policy 2013 RMD Key Club District Convention Advisors!! When sending in registration forms, please indicate whether the housing is single, double, triple, or quad occupancy. Make sure that the roommates indicated are definite and set. If you want your students to stay with the desired students from your school, make sure you have the correct number of students that corresponds with the housing option checked. For example, if you have seven female students, do not mark all of them as quad occupancy and give me a rooming list of four students in one room and three in another. Rarely do we fill empty single slots in a room. In years past, we would let it slide. We cannot afford to do that anymore. If quad occupancy is checked and fewer than four names are submitted, there is an excellent chance the students will be split up. Triple occupancy is not that much more expensive than quad occupancy.
The RMD Key Club will be giving away a trip to the 2013 Key Club International Convention in Washington, DC. This is a $1299 value. All paying Key Club members who register for the 2013 RMD Key Club Convention will have an opportunity to win this trip.
