Registrations received after March 11, 2016 will require a $15 late fee/person. All cancellations must be made in writing on or before March 18, 2016 to ensure a complete refund. Any cancellation after this date is subject to a $50 penalty. No Refunds will be given for cancellations after March 25, 2016.

• All students must have an adult chaperone that stays at the hotel, and monitors their students. • Checks should be made out to RMD Key Club and sent to: RMD Key Club 1641 S. Parfet Ct. Lakewood, CO 80232


Housing

• List roommates together in groups of three or four on the accompanied Rooming Sheet. • If you have one or two members and desire quad occupancy, we will work our best to room them with other clubs. • Different schools may mix students (same gender) to fill rooms, as long as it is indicated on the rooming list that is enclosed in this packet, or a note is attached to indicate rooming arrangements. • Adults will be housed either single or double. If you would like the District Administrator to find you a roommate, let him know. • Please contact the District Administrator if one of your students has special rooming needs.

Code of Conduct, Waiver, Medical Release

• All Key Club attendees will be required to follow the Rocky Mountain District Key Club Code of Conduct and bring a copy of it signed by both the student and their parent/guardian. • They will also need to bring two copies of a signed Medical Release Form. The Release Forms do not need to be notarized. One copy is for the District, and the other is for the chaperone to keep. • These do not need to be sent in with the Registration Forms, but do need to be brought to the Convention.

Registration and Forms

• Registration fee includes: 2 nights lodging, convention shirt, entertainment (including a talent show that YOU can star in), a dance, keynote speaker, workshops/forums, and two meals (Saturday K-Family Lunch/Farewell Brunch). • Every participant needs to fill out a Registration Form. On the form make sure that they indicate their T-Shirt size. If none is indicated, then an XL will be ordered. Also, they need to indicate their K-Family Luncheon selection. If none is indicated, the Vegetarian Pasta will be ordered. • When sending in the forms, please include a RMD Key Club District Convention Club Summary Form totaling all the registrations from your club.
