Hello Readers! Summertime is here. Happy time is here. The valiant efforts of scientists and medical community have helped us to slowly resume our lives. Basic precautions are still to be followed, but clearly large-scale vaccinations are working. If you know anyone that is vaccinehesitant, give them a gentle nudge, a friendly pat on the back and ask them to join the ranks of the vaccinated. The old cliche applies here: We are all in this together. We have several medical-related articles in this issue. A healthier citizenry can help fight future pandemics before they get out of control. A science panel urges the government to give access to all Americans to a primary care physician. Read the informative article on page 22. Doctors must now provide their patients access to their own data. Go to page 42 for the article. Also, if you have any youngsters thinking about joining a medical career, read the column on page 54 about pre-med journeys. Thank you for continuing to read and support Saathee. Drop us a line with any comments or suggestions. We welcome conversations. See you next month.

The Editor’s Desk by Samir Shukla

The other day I rented a car for a day of business errands. I had booked a small car, but since the rental car company didn’t have any compact cars left, they upgraded me with a Dodge Charger. Black. Sleek. Hood muscles bulging. Motoring down the road, its engine heaving and roaring at the slightest caress of the accelerator, the ride took me back to my first car I bought. A 1972 Chevy Nova. Multi-hued. Muscular. A bit rusty. I paid $200 for it back in high school, in late 1979 or sometime in 1980. Can’t exactly remember the date. It was an American muscle car like the Charger, but ancient in technology compared to the beast I was driving on this day. The Nova’s prime technology was the radio. It also had a tape deck the previous owner had installed. The Charger was loaded with contemporary tech goodies that are now common in most cars. The Nova’s was sparse, classic you might call it. The previous owner had begun to repaint it but never finished. He did a lousy job of covering up the rusty spots. The transmission gave out a month after I bought it, but it ran, groaning and roaring, for couple Saathee.com

years more after I got it fixed. Minor repairs were needed every few weeks. Hey, what do you expect for 200 bucks? In the wide, endless expanse of youthful roads, I sometimes pretended that old Nova was really a starship. Afterall, it had a name that evoked a flashy, starry event. I did a lot of cruising on highways, byways, and alleyways, but the darn thing never did get off the ground. In a moment of reminiscing, the day I drove around the Charger, I closed my eyes for a bit, and pretended it would take off like a spacecraft. Off this virus-ridden planet, somewhere freer. I opened my eyes and accelerated down the road; the stars will have to wait. Both cars were laden with my mix tapes, in their own manners. I had actual mixed tapes, those handy cassettes for those who remember, in high school and later college, that I made from songs off radio and records, and I would stick one into the Nova’s tape deck and roll down the road. There was Van Halen blasting out of the creaky speakers, running with the devil, Kiss rocking and rolling all night long, Pink Floyd taking me on an interstellar overdrive and Black Sabbath oozing out of the open windows and disappearing into the void. Among many other favorites. The playlists on streaming services on my cell phone are contemporary mix tapes and they enhanced the ride in the continued on page 82 14

A Primary Care Physician for Every American Science Panel Urges

By Noam N. Levey, Kaiser Heath News

Strengthening primary care has long been seen as a critical public health need. And research dating back more than half a century shows that robust primary care systems save money, improve people’s health and even save lives. “We know that better access to primary care leads to more timely identification of problems, better management of chronic disease and better coordination of care,” said Melinda Abrams, executive vice president of the Commonwealth Fund, a New York-based foundation that studies health systems around the world. Recognizing the value of this kind of care, many nations — from wealthy democracies like the United Kingdom and the Netherlands to middle-income countries such as Costa Rica and Thailand — have deliberately constructed health systems around primary care. And many have reaped significant rewards. Europeans with chronic illnesses such as diabetes, high blood pressure, cancer and depression reported significantly better health if they lived in a country with a robust primary care system, a group of researchers found. For decades, experts here have called for this country to make a similar commitment. But only about 5 percent of U.S. health care spending goes to primary care, versus an average of 14 percent in other wealthy nations, according to data collected by the Organization for Economic Co-operation and Development. Other research shows that primary spending has declined in many U.S. states in recent years. The situation grew even more dire as the pandemic

The federal government must aggressively bolster primary care and connect more Americans with a dedicated source of care, the National Academies of Sciences, Engineering and Medicine warn in a major report that sounds the alarm about an endangered foundation of the U.S. health system. The urgently worded report, which comes as internists, family doctors and pediatricians nationwide struggle with the economic fallout of the coronavirus pandemic, calls for a broad recognition that primary care is a “common good” akin to public education. The authors recommend that all Americans select a primary care provider or be assigned one, a landmark step that could reorient how care is delivered in the nation’s fragmented medical system. And the report calls on major government health plans such as Medicare and Medicaid to shift money to primary care and away from the medical specialties that have long commanded the biggest fees in the U.S. system. “High-quality primary care is the foundation of a robust health care system, and perhaps more importantly, it is the essential element for improving the health of the U.S. population,” the report concludes. “Yet, in large part because of chronic underinvestment, primary care in the United States is slowly dying.” The report, which is advisory, does not guarantee federal action. But reports from the national academies have helped support major health initiatives over the years, such as curbing tobacco use among children and protecting patients from medical errors. Knowledge Sansad Bhavan (Parliament House) The Parliament House (Sansad Bhavan) in New Delhi is the seat of the Parliament of India. The building houses the Lok Sabha (lower house) and the Rajya Sabha (upper house) in India’s bicameral parliament. It also houses the President’s office. The building was designed by the British architects Sir Edwin Lutyens and Sir Herbert Baker and was constructed between 1921 and 1927. It was opened in January 1927 as the seat of the Imperial Legislative Council. Between August 14-15, 1947, at India’s Independence, power was transferred from the British to India. The format of Indian Constitution was also prepared in this hall. Following the end of British rule in India, it was taken over by the Constituent Assembly, and then by the Indian Parliament once India’s Constitution came into force in 1950. On December 13, 2001, the Parliament House was attacked by five Lashkar-e-Taiba and Jaish-e-Mohammed terrorists. In addition to all the attackers, six military personnel and one civilian were killed. The Indian government in 2019 launched the Central Vista Redevelopment Project, with the construction of a new parliament building as a part, alongside other projects in New Delhi, including revamping Rajpath, making a new office and residence for the Indian prime minister, and combining all ministerial buildings in a single central secretariat. The groundbreaking ceremony for the new building was held in October 2020 and the foundation stone was laid on December 10, 2020. It is expected that the structure would be completed by 2022, coinciding with 75 years of India’s independence. The current building is planned to be converted into a Museum of Democracy after the new Parliament House is operational. The color of the Lok Sabha carpet is green, which indicates that India is an agricultural country and represents common people, the color of the Rajya Sabha carpet is red which reflects royalty and freedom fighters’ sacrifice.

USA  Knowledge The Capitol The United States Capitol, often called The Capitol or the Capitol Building, houses the meeting chambers of the Senate (in the north wing) and the House of Representatives (in the south wing) – the two bodies that compose the legislative branch of the American government. The Capitol was built after Thomas Jefferson, then US Secretary of State, held a design competition in spring 1792 to elicit entries from some of the finest architects in America. A late entry by William Thornton, which both Washington and Jefferson loved, was chosen. George Washington himself laid the cornerstone for the Capitol on September 18, 1793. The original building was completed in 1800. The Congress first met in the Capitol on November 17, 1800. It was partly destroyed in the 1814 burning of Washington, then was fully restored within five years. The building was later enlarged, with the addition of a massive dome in the 1850s, and extended wings with expanded chambers for the bicameral legislature, the House of Representatives in the south wing and the Senate in the north wing. The most recent, and largest ever, addition to the Capitol, the Capitol Visitor Center, was completed in 2008. Atop the U.S. Capitol dome is the Statue of Freedom, an allegorical female figure. There are 100 statues in the National Statuary Hall Collection. Each state may contribute two statues; all have done so. On January 6, 2021, the United States Capitol in Washington, D.C., was stormed during a riot and violent attack against the U.S. Congress.

The Deception of Authenticity Mirror, Mirror by Jennifer Allen

“It is not he who is revealed by the painter; it is rather the painter, who on the coloured canvas, reveals himself.” ~ Oscar Wilde Recently I watched the 6-episode first season of Marvel’s The Falcon and the Winter Soldier. It’s set during the aftermath of the last two Avengers movies with certain characters who are now gone, other characters who have now returned, and a world that has to cope with the repercussions of both. One of the overlying themes of the show is the search to find the worthiest person to take up the mantle of Captain America after Steve Rogers. The plot then deliberates between four different characters who, in the comic books, had wielded the iconic shield at one point or another. There are two scenes that are particularly important over the course of the show. In both cases, a character donning both the shield and a red, white & blue costume to match with onlookers pulling out their phones to capture that moment. One scene portrays the symbolism of Captain America while at the same time committing murder. In the other, the character uses it in an attempt to build hope. In both scenarios, the characters were not hiding behind the uniform but instead using it as an extension of who they really are. Despite their quite opposite intentions, both men were being authentic. We now live in a world, like it or not, where anything and everything can be captured on camera. With that terrifying realization in mind, there is still that nagging question as to whether anything anymore is absolutely real. Those selfsame scenes captured on our mobile devices can easily be taken out of context given when, where, and how this footage came to pass. The reality shows that have saturated our entertainment outlets are still heavily edited with a general script in mind to highlight the personality traits of certain individuals. While these attributes may indeed be real, the show is often reconstructed to amplify these to the point that this person or people are reduced to a niched archetype instead. continued on page 80 Saathee.com

Doctors Now Must Provide Patients their Health Data Online and On Demand But the rollout of the rule has hit bumps, as doctors learn that patients might see information before they do. Like Ramsey, some patients have felt distressed when seeing test results dropped into their portal without a physician’s explanation. And doctors’ groups say they are confused and concerned about whether the notes of adolescent patients who don’t want their parents to see sensitive information can be exempt — or if they will have to breach their patients’ trust. Patients have long had a legal right to their medical records but often have had to pay fees, wait weeks or sift through reams of paper to see them. The rule aims not only to remove these barriers, but also to enable patients to access their health records through smartphone apps, and prevent health care providers from withholding information from other providers and health IT companies when a patient wants it to be shared. Privacy rules under the Health Insurance Portability and Accountability Act, which limit sharing of personal health information outside a clinic, remain in place, although privacy advocates have warned that patients who choose to share their data with consumer apps will put their data at risk. Studies have shown numerous benefits of note sharing. Patients who read their notes understand more about their health, better remember their treatment plan and are more likely to stick to their medication regimen. Non-white, older or less educated patients report even greater benefits than others.

By Sarah Kwon Last summer, Anna Ramsey suffered a flare-up of juvenile dermatomyositis, a rare autoimmune condition, posing a terrifying prospect for the Los Angeles resident: She might have to undergo chemotherapy, further compromising her immune system during a pandemic. After an agonizing three-day wait, the results of a blood test came back in her online patient portal — but she didn’t understand them. As hours passed, Ramsey bit her nails and paced. The next day, she gave in and emailed her doctor, who responded with an explanation and a plan. For Ramsey, now 24, instant access to her test results had been a mixed blessing. “If there’s something I’m really nervous about,” she said, “then I want interpretations and answers with the result. Even if it takes a few days longer.” On April 5, a federal rule went into effect that requires health care providers to give patients like Ramsey electronic access to their health information without delay upon request, at no cost. Many patients may now find their doctors’ clinical notes, test results and other medical data posted to their electronic portal as soon as they are available. Advocates herald the rule as a long-awaited opportunity for patients to control their data and health. “This levels the playing field,” said Jan Walker, cofounder of OpenNotes, a group that has pushed for providers to share notes with patients. “A decade ago, the medical record belonged to the physician.” Saathee.com

Losing My Religion Who Do You Think You Are? Subcontinental Divide by Ahsen Jillani

I was born and got my early education in an era where science was starting to determine our destinies. Modernism kind of collided head-first with everything from religion to conservatism to colonialism sometime after WW-II because America emerged victorious and its PR prowess was newly battling with the rising threat of socialism and communism. A fresh-faced technocrat PhD fresh out of the University of Chicago, my dad was one of the few early culprits who brought a secular philosophy to a country barely 12 years out of British colonial rule. Truthfully, as he chaired the sociology department at Karachi University straight out of college, I don’t have a single memory of participating in a religious ceremony of any sort (other than my circumcision, at age 4, which I still don’t want to discuss due to trauma). We actually could have been any family in America in the ‘60s. While the Pakistani President Ayub Khan was entertaining Jackie Kennedy and presenting her with race horses, we were traveling around town in evenings going for ice cream and burgers, my dad and uncles were having beers and watching American shows, and weekends, we might go to the beautiful Arabian Sea coast or the aquarium before driving to the drive-in for a western film. Religion technically didn’t exist in our lives. Religion actually was on vacation in the West as well, as America struggled with moral and ethical issues like civil and voting rights, and the raging Vietnam war that practically continued on page 78

Why Do We Hate the Sound of Our Own Voices? Your voice, when played back to you, can sound unrecognizable

By Neel Bhatt As a surgeon who specializes in treating patients with voice problems, I routinely record my patients speaking. For me, these recordings are incredibly valuable. They allow me to track slight changes in their voices from visit to visit, and it helps confirm whether surgery or voice therapy led to improvements. Yet I’m surprised by how difficult these sessions can be for my patients. Many become visibly uncomfortable upon hearing their voice played back to them. “Do I really sound like that?” they wonder, wincing. (Yes, you do.) Some become so unsettled they refuse outright to listen to the recording – much less go over the subtle changes I want to highlight. The discomfort we have overhearing our voices in audio recordings is probably due to a mix of physiology and psychology. For one, the sound from an audio recording is transmitted differently to your brain than the sound generated when you speak. When listening to a recording of your voice, the sound travels through the air and into your ears – what’s referred to as “air conduction.” The sound energy vibrates the ear drum and small ear bones. These bones then transmit the sound vibrations to the cochlea, which stimulates nerve axons that send the auditory signal to the brain. However, when you speak, the sound from your voice reaches the inner ear in a different way. While some of the sound is transmitted through air conduction, much of the sound is internally conducted directly through your Saathee.com

skull bones. When you hear your own voice when you speak, it’s due to a blend of both external and internal conduction, and internal bone conduction appears to boost the lower frequencies. For this reason, people generally perceive their voice as deeper and richer when they speak. The recorded voice, in comparison, can sound thinner and higher pitched, which many find cringeworthy. There’s a second reason hearing a recording of your voice can be so disconcerting. It really is a new voice – one that exposes a difference between your self-perception and reality. Because your voice is unique and an important component of self-identity, this mismatch can be jarring. Suddenly you realize other people have been hearing something else all along. Even though we may actually sound more like our recorded voice to others, I think the reason so many of us squirm upon hearing it is not that the recorded continued on page 94 June 2021

The Adult Indian Dance Student My continuing journey of lifelong learning Dance and Identity My Voice by Preethi Sriram

Nataraja Pathu (Song to Lord Nataraja) Indha ulageerezhu men alithai chollu, ini unnai viduvathillai, Eesane, sivakami nesane, yenai eendra, thillai vaazh natarajane. Translation- Please tell me, why did you create these two times seven worlds, I am not going to leave you, Oh God, Oh darling of Parvathi, Oh Nataraja of Chidambaram who gave birth to me. The lines above are from a Tamil Song to Lord Nataraja (The Lord of the Dance) written by Muniswamy Mudaliar and translated by P.R Ramachandar. Music is one of the most integral parts of dance. At least for Indian classical dance, without poetry and music, it is hard to have dance. I wish to explain the importance of music in dance from my experience this past year. When I was sick for a day this past year, I became delirious. I was mad and upset and cursed the divinities for the pain. It was a completely vulnerable time, and at that moment, hurt and trying to find answers- I.e.- “why this pain? Why this creation? Why this suffering?” I was raging and burning (due to probably a high fever) and hurled my frustration at the creations. Like the poet who wrote the Nataraja Pathu (though not as beautiful, nor with any sort of poetry attached), I became the questioner, asking “why?” and “take pity on us” and “how cruel” and “give us reprieve.” Yet, one of the things that gave me solace was listening to mantras/slokas/bhajans. Strangely, I kept singing to myself all the mantras and slokas I knew. I could not recall much of it. All I wanted to do was listen to the devotional music. In the past, where would one have turned to in this moment? It would be hard to summon a priest to chant. Or it would be challenging to try and find a private musician at that very moment to sing the specific songs that one would desire to take us to a place closer to a greater consciousness. continued on page 82

Your Pre-Med Journey The key to ending that daunting feeling is simple – make smaller goals.

My Voice by Ananya Koripella It might be my own biased opinion, but it seems like interest in the medical field grows every year. I frequently hear students declare that they are premed but many, including me, never really know how to be pre-med before starting our undergraduate years. We need good grades to get into medical school but what else? What really is the right way to get into medical school? The correct answer is actually the one that no one wants to hear: there is no right way. As nice as it would be for the journey to be simple with a just few basic things that will make us the perfect applicants, the pre-med journey is more complicated than that. It challenges us to dig deeper, find our true inspirations and persevere through the countless hours of studying for the MCAT, running experiments in the lab and applying to loads of internships. The pre-med journey is tough and it’s quite easy to feel like we have bitten off more than we can chew. The key to ending that daunting feeling is simple – make smaller goals. I made a mini checklist of all the things that I knew I wanted to accomplish in my years as a pre-med: I wanted to do research, study abroad, volunteer at the hospital in town and much more. Here’s the catch though. Our plans Saathee.com

never stay concrete. Our interests always change and with that, so should the activities on our list. I replaced volunteering at the hospital with leading a medical brigade abroad. Why? Because I quickly realized that I am passionate about public health. While the essence of my list stayed the same – research, service and leadership – the way I showed my passions began to change. That is the key to your pre-med journey. Your passion. The first question to ask yourself is “why medicine and why a doctor?” The response to this question cannot be something generic like “I like helping people.” Why do you like helping people? There are so many ways that we can help others so why do it through medicine? It’s natural to begin the pre-med journey with just a piqued interest and not a substantial answer. We often start off with that interest and through our various experiences, truly learn why medicine may be the right path for us. However, by the time you do apply to medical school, you truly do need to know the answer to this question because choosing medicine is only the first step to a difficult yet rewarding career. As I tried to deal with my once wavering certainty in pursuing medicine, the next question I had was regarding my major. I had a variety of interests, but I didn’t know which major would help me to get into medical school the most. I thought I needed to major in a subject like Chemistry even though I did not like it that much. I was quick to learn that majors and minors actually do not matter when applying to medical school. I could major in the Arts or Humanities and go to medical school as long as I had completed all my pre-requisite courses. Being pre-med is time consuming and, often, very draining. It’s important to pursue interests that are outside of the medical continued on page 94

Does Taking A Gap Year Affect University Admissions? College Admissions Insider by Lindsey Conger Going to college straight out of high school isn’t for everyone. Some students might feel unprepared and unsure of what they want to do with the rest of their lives. That is why a gap year is encouraged by many schools! For example, Harvard University “encourage(s) admitted students to defer enrollment for one year to travel, pursue a special project or activity, work, or spend time in another meaningful way.” According to a report in the New York Times, students who take a gap year tend to be more successful than the students who did not take a gap year. Gap years have been on the rise due to the uncertainty of the Covid-19 pandemic. This past year, more students were drawn to a gap year, with up to 16 precent of students seriously considering taking a year off before diving back into their studies. A gap year can be a very positive thing, if you approach it correctly. However, if, by the end of it, you learned nothing, done nothing, and didn’t challenge yourself, then it will likely negatively impact your university admission. To make the most of your gap year, you should be taking the time to explore your personal interests and discovering what you want to do for your future career. Volunteering, working, positively impacting the world, starting a business, and traveling can all be useful ways to spend your gap year. By the end of the year, you’ll be able to bring a fresh perspective to the school and be mentally ready to take on the challenging course material. However, every university does approach a gap year differently. Schools like Princeton University, the University of North Carolina at Chapel Hill, Middlebury College, and Texas Christian University don’t just look favorably at gap years, but actually have grants, programs, or scholarships that help you achieve your gap year goals. For example, UNC awards a Global Gap Year Fellowship of $7,500 to help fund a 9-month service-oriented gap year. If you have already been accepted to a university, check to see if they offer something similar. However, other universities might not allow you to take a gap year after accepted. If you have already been accepted to a university and want to defer your enrollment, make sure to call admissions and ask if this is allowed. Read Watch Listen Step It Up and Go The Story of North Carolina Popular Music Author: David Menconi (University of NC Press)

Reviews by Samir Shukla Manika Kaur Ek (Six Degrees Records)

David Menconi is a long-time North Carolina music journalist. He began his music writing career at Raleigh News and Observer and has spent three decades covering the state’s music. He has gathered years of interviews, reporting and historical research and chronicles the state’s musical history in his fine new book Step It Up and Go, the Story of North Carolina Popular Music. The book covers everything from early NC music stalwarts like Blind Boy Fuller to indie rockers Superchunk to North Carolinians achieving fame as winners and runners-up of TV’s American Idol. Breaking it down into concise, cohesive chapters. The south’s intricacies are woven into its music, and North Carolina is among its fertile grounds of American musical history. Many NC-based record labels, Sugar Hill, Merge, Mammoth, and others are covered with their timelines of successes, setbacks, and failures. He interlaces the history with interviews he has conducted over the years with myriad North Carolina musicians to create a narrative of NC’s popular music. Menconi deftly covers the genres, musical eras, and personalities. The varied and diverse musical styles of NC musicians and artists are written about in a clean and concise prose, elucidating its many subtleties as well as traditions. He also writes of music towns and scenes, (Chapel Hill, Winston Salem, Raleigh), including musicians that have achieved widespread fame and indie bands that have influenced the state’s musical directions, though may not be well known. Instigators, influencers, producers, recording studios all are covered while traversing the state’s musical alleyways, including genres like Piedmont blues, jazz, and bluegrass to beach music, rock, hip-hop, and more. Mill and textile towns, mountain music, college-town clubs and radio stations are part of the story Menconi unfolds. Menconi is a journalist, but is also a music lover, and writes of music with a conversational passion, much like a friend turning you on to new music. Sidebars in each chapter, along with photographs, round out the narrative. This book is a solid addition to the documentation of NC music and musicians.

Kirtans are devotional singing or sacred chants performed in South Asian religions, especially Hinduism and Sikhism. I first heard Sikh kirtans at local Gurdwaras (Sikh temples) in North Carolina and during a visit to the Golden Temple in Amritsar, India where they are performed continuously while the main sanctuary is open. They are generally sung and performed in Gurudwaras by men. That is how it has been. Australian-born Singer Manika Kaur has parted this tradition and become one of the foremost proponents of Sikh kirtans. Her latest recording, Ek, melds her soothing voice and meditative musicality, bringing kirtans and their spiritual aura to the greater world. She understands the subtleties of this spiritual art form, where soft repetition creates a trancelike atmosphere. Kirtans can be universal in the way they encounter with listeners, evoking a sense of connection, even when one doesn’t understand the words or lyrics. This recording is among those in invoking universality with its Kaur’s vocals, words and flowing music. You don’t need to follow Sikhism or understand Punjabi, traditional language of Sikhs, to feel the devotion and connection Kaur makes with her voice, which is instilled with a sense of peace and tranquility. The recording is composed and performed with traditional instruments and arrangements. Kaur sings in English and Punjabi. Every song on the album leads with one of several traditional instruments accompanying Kaur’s vocals along with contemporary sounds and production values. The recording works as a cohesive whole, including the album artwork, created and designed by the Singh Twins. Kaur is also very active in charity and social work, including raising funds to educate young girls in rural Punjab. The recording Ek (which means ‘oneness’) is now available on Six Degrees Records.

The Importance of Financial Literacy

thinking of retirement the day one earns his first paycheck), how to save for children’s education. This also includes how much can we maintain our lifestyles in retirement, how, when and where to invest. There is no end to these questions. They are as varied as there are people. But they all can be answered and understood well if one shows some inclination/literacy towards financial matters. Out of more than 1200 adults surveyed by the American College New York Life Center for retirement income, just 5 percent scored equivalent of B grade or higher. Worse yet, nearly three-quarters of these respondents failed the quiz. This is troublesome because even today about 10,000 baby boomers are retiring every day and marching into that haze and mental fog of financial illiteracy. Think for a moment of a 65-year-old person who has not spent time educating himself or herself about how best to transition into retirement. At that age they must make decisions about Medicare, social security, expected incomes and expenses, health matters, investment allocations, RMDs, tax implications and much more. These are all big issues and need to be thought of long before the age 65. Financial literacy, once we have the interest in it, can bring forth the most important aspect of retirement, which is planning for it. And as mentioned above, unless you know you are going to inherit wealth, planning starts the day you start working and earn your first dollar. There are twelve aspects to that planning (which we may cover in later article) but the most important one is the acute awareness of it and a strong desire and urge to do some thing about it. Half the solution to any problem is defining the problem properly. If you are working on fixing, say, a broken appliance, half the time goes in deciding what is broken (or wrong); once you determine that fixing it is the other half which is relatively easy. Majority of people are heading into their retirement years without a plan. They are going to make mistakes. Some mistakes may be correctible (e.g., deciding where to live in retirement) while some mistakes will have consequences for the remainder of their retiree life (like starting social security too early or withdrawing too much money during the first few years of their retirement). Maybe they will learn from their mistakes and pass the word on to the next generation and make them wiser (if they care to listen) but the point is that not all mistakes we can recover from unless we start early, way early, and never lose focus of what our goals are. Just like the other hobbies we have and are so serious about, make financial literacy a hobby activity and you will be in for a pleasant surprise when the time comes. Well, your next generation definitely would be.

Personal Finances by Mo Vidwans

We all have our own specific interests, goals and ideas and we follow them with great zest, deep involvement and dedicate much of our private time to it. Generally, most of the hobbies that people follow fall under this category (very rarely they convert that into a business); some collect things, some have interests in stamps, dolls, records, rare books, baseball cards, cars - especially antique cars – and so on. The list is long, but it is clear if we generate specific interest in a subject, we do dedicate ourselves to it. This makes me wonder as to why is it that a subject like finances, something that will affect us all the way to our grave, does not generate enough enthusiasm to dedicate more time to it. Even a little bit of financial literacy will go a long way and will make your family’s financial life much more pleasant. Financial Organizations put out many programs (lectures, talks, shows) to generate interest in financial literacy. This is done to get more people involved, get them interested and at least take a second look at their own situation. While many of you reading this are financially literate, I am sure, a significant number of Americans are not. Financial literacy is not limited to USA either, the lack of it is documented in many other countries as well. I am going to touch on only a few key concepts here. The concept of compounding is one of them. This subject has been honed here before, in this column by me and by other articles in this magazine. This is a simple but very powerful concept. If you invest something today and let it grow at a steady but compounding rate it accumulates so rapidly after a certain length of time. And you don’t have to do anything more than just start the process and don’t disturb it. Compound returns are why investors are encouraged to allocate wealth to stocks. Realizing returns on capital appreciation (which is not taxed until you sell it) and reinvesting dividends is what makes stocks a key creator of wealth especially when you have a distant horizon. On the other end of this scale, debt has the opposite impact on wealth. Fail to pay down the principal and the interest charged will drag you down into a much bigger hole and that makes it that much more difficult to climb out of. Those who have dealt with credit card debt and payday loans will understand this well. Another aspect to financial literacy is “retirement readiness.” Topics in this category include when to claim social security, the relationship between risk and yields, Mo Vidwans question about when one should start thinking and saving is an independent, board certified financial planner. Sunny Jain’s New Project: Phoenix Rise Sunny Jain, Smithsonian Folkways recording artist and founder of seminal South Asian funk-jazzdance band Red Baraat, unites a collective of over fifty artists to contribute to a new album and recipe book. Phoenix Rise is an album of songs and a book of “planet-based” recipes with photos, all encapsulated by an abstract painting of the mythological bird, the phoenix. Phoenix Rise combines art, music, photography, food, and a belief in global citizenship by supporting social justice movements. In the solitude of his studio during the pandemic, Jain built a collective of over fifty artists, all of whom came together virtually to create a project of transcultural scope. There are 10 downloadable songs, and a 72-page book containing 22 vegan, “planet based” recipes paired with instrumental foodie photography. The artists who appear here range from well-known jazz musicians (Vijay Iyer, Snarky Puppy’s Michael League) and session players (Darius Christian, Bubby Lewis, Dave “Smoota” Smith) to jam band darling Joe Russo and vocalists, violinists, dancers, and more. Each is foregoing their royalties to benefit the Center for Constitutional Rights, an organization that Jain partnered with to donate all proceeds from the album. “As a child of immigrants, I appreciate the organization’s work challenging abusive immigration policies and supporting families and communities of color,” says Jain. “Their work also extends beyond the U.S., including a fight for LGBTQIA+ rights and gender justice across the globe. Combatting unjust laws and discriminatory treatment of transgender and gender-nonconforming people is not just an important cause for me personally, but an abhorrent violation of human rights that we should collectively work to end.” It is available via: https://sunnyjain.bandcamp.com. All Proceeds from the project will go to the Center for Constitutional Rights (ccrjustice.org).

Ekal’s Efforts to Beat the Pandemic in Rural India In India, Covid-19 pandemic’s second wave hit hard suddenly and with the force of a tsunami. The country was caught off guard and ill Saathee.com

prepared for the toll it took. All the focus of the grave situation revolved mainly around the urban areas and rural-tribal areas received scant notice. There was a reason too for this apathy towards the rural community. Last year, the villages had fared quite well in arresting the spread of corona, by and large, and most folks there had remained immune to it. As India entered well into 2021, the situation changed. According to Bajrang Bagra, the CEO of Ekal Abhiyan (federation of all Ekal organizations), “the serious aspect of the pandemic, this time around, is that it has struck even in the countryside where, unfortunately, the medical infrastructure and facilities are not as strong as they are in the urban areas”. In rural India, Ekal has the most wide-spread network of volunteers and collaborators in the deepest corners of villages. It has firm presence and unwavering support in over 100,000 villages. That’s why Ekal has undertaken a comprehensive well-coordinated offensive against the pandemic, with support from its allied organizations. Last year, Ekal’s successful initiative against covid was based on self-monitoring, selfreliance, education and cooperative-exchanges. It included a wide-spread awareness campaign about the hygiene, social distancing, food distribution, mask-making and restrictive movements. The new offensive has not only incorporated all these steps in the direct-action roadmap, but also, has started a proactive counter campaign against the misinformation about the vaccine, the danger posed by covid and voodoo-treatments. With extensive reach well beyond its footings, Ekal has mobilized tens of thousands of its schoolteachers for this information campaign. ‘Ekal-Arogya (Health Foundation)’ has established 24-hr Telehealth lifeline (# 011 41236457) for professional medical counselling. According to Ramesh Shah, Ekal Global Coordinator and an advisor to Ekal-USA, Board of Directors, “currently more than 350 National Medico Organization (NMO) Doctors in India and some in U.S. are manning the helpline every day.” In addition to various empowering projects for the economic sustenance of the village folks, Ekal has earmarked one million dollars just for covid relief. It is converting its several centers into corona isolation camps, fully equipped with Oxygen concentrators, PPE kits, June 2021

DHS Announces Continuation of International Entrepreneur Parole Program Program Will Provide Opportunities for Foreign Entrepreneurs

The U.S. Citizenship and Immigration Services has announced that the Department of Homeland Security is withdrawing a 2018 notice of proposed rulemaking that proposed to remove the International Entrepreneur program from DHS regulations. The International Entrepreneur (IE) parole program, first introduced in 2017, will remain a viable program for foreign entrepreneurs to create and develop start-up entities with high growth potential in the United States. The program will help to strengthen and grow our nation’s economy through increased capital spending, innovation, and job creation. The announcement is consistent with President Biden’s Executive Order 14012: “Restoring Faith in Our Legal Immigration Systems and Strengthening Integration and Inclusion Efforts for New Americans.” The executive order requires the secretary of homeland security to “identify any agency actions that fail to promote access to the legal immigration system.” “Immigrants in the United States have a long history of entrepreneurship, hard work, and creativity, and their contributions to this nation are incredibly valuable,” said Acting USCIS Director Tracy Renaud. “The International Entrepreneur parole program goes hand-in-hand with our nation’s spirit of welcoming entrepreneurship and USCIS encourages those who are eligible to take advantage of the program.” The initial IE final rule was published on Jan. 17, 2017 and was scheduled to take effect on July 17, 2017. This final rule guided DHS in the use of its parole authority to grant a period of authorized stay, on a case-by-case basis, to foreign entrepreneurs who demonstrate that their stay in the United States would provide a significant public benefit through the potential for rapid business growth and job creation. Prior to the effective date, DHS published a final rule to delay the implementation date of the IE final rule to March 14, 2018. This allowed DHS additional time to draft and seek public comments on a proposal to rescind the IE final rule. However, in December 2017, a federal court vacated the delay, requiring USCIS to begin accepting international entrepreneur parole applications consistent with the IE final rule. Since then, the program has been up and running, and USCIS continues to accept and adjudicate applications

Indiaspora Opens Giving Platform for COVID-19 Relief Efforts to India Indiaspora, a nonprofit community of global Indian diaspora leaders, has launched the giving platform ChaloGive.org for the public to donate to COVID-19 relief efforts in India after raising an initial USD $1 million last week through their network of Indiaspora members. “Our hope is that this urgent ChaloGive campaign for India will inspire the broader community to give generously and support India’s fight against COVID-19. The scale and magnitude of the situation in India is currently beyond any one person or any one group’s ability to tackle,” said Ashish Shah, Senior Director, Philanthropy and Community Engagement at Indiaspora. “We need the force of our entire diaspora behind COVID-19 relief efforts in India so that we can make the biggest impact possible. The need is of enormous proportions.” The grassroots initiative ChaloGive for India will facilitate donations to trusted and vetted high-impact nonprofits that are working on the ground during the second wave of COVID-19 relief. Visitors to ChaloGive.org will have an option to give toward three major areas of COVID-19 relief: the creation of urgently needed COVID care centers and makeshift hospitals through nonprofit WISH Foundation, direct cash transfer to families who have lost a primary earning member through nonprofit GiveIndia, and food relief and livelihood assistance for migrant workers and other underserved populations through EdelGive Foundation to nonprofits Goonj and Jan Sahas. U.S. donors also will receive a tax exemption. “India needs all the help it can get and many I know are impacted. This is the time for all of us who can to give generously,” said Indiaspora member, Jay Vijayan, founder and CEO of California-based Tekion Corporation, who donated toward the campaign. consistent with existing DHS regulations. Under the IE program, parole may be granted to up to three entrepreneurs per start-up entity, as well as their spouses and children. Entrepreneurs granted parole are eligible to work only for their start-up business. Their spouses may apply for employment authorization in the United States, but their children are not eligible for such authorization based on this parole. Additional information on eligibility and how to apply is available on the International Entrepreneur Parole page. USCIS will plan information sessions and other outreach activities to ensure foreign entrepreneurs are aware of this opportunity and how to pursue it. For more details, visit Uscis.gov.

storage, and other in-orbit resources for daily use. NASA will purchase from Axiom the capability to return scientific samples that must be kept cold in transit back to Earth. “The first private crew to visit the International Space Station is a watershed moment in humanity’s expansion off the planet and we are glad to partner with NASA in making it happen,” said Axiom President and CEO Michael Suffredini. “A thriving commercial marketplace in lowEarth orbit begins with expanding access to serious, nontraditional users and that is exactly the aim of our private astronaut missions.” NASA has opened the space station for commercial activities, including private astronaut missions, as part of its plan to develop a robust and competitive economy in lowEarth orbit. The agency’s needs to achieve that goal – such as research on the effects of the space environment on humans, technology development, and in-flight crew testing – will remain in place after the retirement of the International Space Station. Commercial entities can meet those needs, providing destinations and transportation capabilities. Enabling Ax-1 is an important step to stimulate demand for commercial human spaceflight services so NASA can be one of many customers in low-Earth orbit. For the Ax-1 mission, Axiom has proposed Michael López-Alegría, Larry Connor, Mark Pathy, and Eytan Stibbe as prime crew members. These private astronauts will be reviewed by NASA and its international partners, as is standard for any space station crew, and undergo NASA medical qualification testing to be approved for flight. López-Alegría will serve as the mission commander, with Peggy Whitson and John Shoffner as backups. Once the proposed crew passes review and qualification, the four members will train for their flight with NASA, international partners, and SpaceX, which Axiom has contracted as launch provider for transportation to the space station. Trainers will familiarize the private astronauts with systems, procedures, and emergency preparedness for the space station and the Crew Dragon spacecraft. Based on current mission planning, training is scheduled to begin this summer. The development and growth of the low-Earth orbit economy continues. In January 2020, NASA selected Axiom to provide at least one habitable commercial module to be attached to the forward port of the International Space Station’s Harmony node in late 2024. Most recently, NASA announced the agency is seeking input from industry on future commercial low-Earth orbit destinations that will provide services, such as crew training, scientific research, and advanced systems development for both government and private-sector astronauts and customers. For more than 20 years, NASA has supported a continuous U.S. human presence in low-Earth orbit. The agency’s goal is a low-Earth orbit marketplace where NASA is one of many customers, and the private sector leads the way. This strategy will provide services the government needs at a lower cost, enabling the agency to focus on its Artemis missions to the Moon and on to Mars while continuing to use low-Earth orbit as a training and proving ground for those deep space missions. For information about the Ax-1 mission, visit: Axiomspace.com/ax1

NASA, Axiom Agree to First Private Astronaut Mission on Space Station

Credits: NASA/Roscosmos NASA and Axiom Space have signed an order for the first private astronaut mission to the International Space Station to take place no earlier than January 2022. “We are excited to see more people have access to spaceflight through this first private astronaut mission to the space station,” said Kathy Lueders, associate administrator for human exploration and operations at NASA Headquarters. “One of our original goals with the Commercial Crew Program, and again with our Commercial Low-Earth Orbit Development Program, is that our providers have customers other than NASA to grow a commercial economy in low-Earth orbit.” The spaceflight, designated as Axiom Mission 1 (Ax-1), will launch from NASA’s Kennedy Space Center in Florida and travel to the International Space Station. Once docked, the Axiom astronauts are scheduled to spend eight days aboard the orbiting laboratory. NASA and Axiom mission planners will coordinate in-orbit activities for the private astronauts to conduct in coordination with space station crew members and flight controllers on the ground. Axiom will purchase services for the mission from NASA, such as crew supplies, cargo delivery to space, Saathee.com

Amma’s Upma

Saathee RECIPES

Hi Saathee family! I’m back with another super easy recipe for June. Wait, how are we already halfway through the year? 2021 is absolutely flying by! I’m not complaining, I’m definitely looking forward to warm weather and a fully vaxxed summer filled with friends and family I haven’t seen in a while. And honestly, what better way to bring our favorite people together than with food-filled events! Personally, I plan on having loads of picnics in the park and even hosting a few potlucks so that’s why I had to give this month’s dish a try. Amma’s upma has always been a staple snack, side-dish and even breakfast dish throughout the years. This recipe is so quick and easy to whip up and will be a perfect lightbite throughout the summer! Upma is essentially a savory porridge filled with vegetables and cashews, first made popular in South India as a breakfast dish. It’s now widely enjoyed throughout India in a variety of different ways. In this version we’ve chosen to add in a few vegetables and yogurt (Amma’s secret ingredient). Be sure to check out the “How-to” video at saathee.com/taige and enjoy!

Ingredients 1 Cup rava (or cream of wheat) 1/2 onion (finely chopped) 1 tbsp Urad dal (split and husked black gram) 1 small green chili (finely chopped) 1 inch piece of ginger (minced) 10-12 curry leaves 1 tsp mustard seeds ½ tsp cumin seeds ½ tsp sugar 2 tbsp ghee or oil 1 tbsp lemon ½- 1 cup of frozen mixed vegetables (thawed) 10-12 cashews (halved) Cilantro (1/2- 1 cup finely chopped) 1 ½ tbsp yogurt 2 1/2 cups water Salt to taste

Saathee.com

Serves: 3-4 people Prep Time: 30 mins By: Taige Shukla

Directions

1 Prep your ingredients. For the rava (cream of wheat) lightly roast it until it becomes fragrant & crisp, do not let it brown. Set aside. Now chop and mince the onion, garlic, green chili, & cilantro and set aside until ready to use.

2 Start by heating ghee or vegetable oil in a pan on medium heat, add in the mustard seeds and cook until crackling. Add cumin seeds and urad dal, sauté until dal is golden, then stir in the cashews and let fry for another 2 mins.

3 Now add in onions and sauté. Once translucent stir in green chili, ginger, thawed mixed vegetables and curry leaves. Sauté for a few more seconds then stir in 2 ½ cups water, sugar, and salt.

4 Once the water comes to a boil, place heat on low and star to add in rava

(cream of wheat) a few tbsp at a time. Be sure to continue to stir the mixture as you add in each scoop until all the rava is added and well combined. Now cover and let steam for about 3 mins or until the water has completely soaked into the rava.

5 Finally, turn off the heat and stir in lemon juice, yogurt, and cilantro. Feel

free to add extra salt or lemon to taste as well. Now your upma is ready to serve!

India – UK Trade Deal

Why the timing is crucial for both nations By Sangeeta Khorana

The deal is particularly interesting due to its size and intended scope. The UK and India are the 5th and 6th biggest economies in the world. India is the largest single market, of about 1.4 billion people, that the UK has committed to negotiating a free-trade deal with to date. India is Britain’s sixth-largest non-EU trading partner, whereas Britain is barely inside India’s top 20. This points to significant scope for growth on both sides. Once agreed, the free-trade deal is likely to be extremely significant, fostering innovation and technology cooperation as well as skills transfer and knowledge-sharing between the two nations. At present, however, India is facing a particularly devastating health threat following the latest outbreak of COVID-19. The UK government and the diaspora have been supporting India with things like ventilators, oxygen generation units and a clinical advisory group, but the subcontinent is so large and populous that there is only so much that can be achieved. The new agreement can potentially help by easing the pain of economic contraction for India in 2021, while supporting both partners as they commence rebuilding efforts to recover from the pandemic.

The UK and India have announced a new enhanced deal on trade at a virtual summit held on May 4, 2021. The deal aims to double trade between the two countries by 2030 and declares their joint commitment to start working towards a comprehensive free-trade agreement, for which discussions are due to commence in the autumn. Britain and India announced £1 billion of new trade and investment as part of this new Enhanced Trade Partnership. Indian investments worth £533 million will be made in Britain, including £240 million by the Serum Institute for production of Oxford-AstraZeneca vaccines and sales business. At the same time, £446 million worth of export deals were announced by British businesses in India. This builds on a trade relationship that was already worth £25.5 billion in 2019. At the summit the UK prime minister, Boris Johnson, and his Indian counterpart, Prime Minister Narendra Modi, also launched a Roadmap 2030 to expand and deepen bilateral cooperation in five areas: people-to-people relationships, trade and prosperity, defense and security, climate action and healthcare cooperation.

Britain and the Indo-Pacific The deal will likely strengthen the geopolitical positions of both nations in a part of the world that is dominated by China. The UK’s recent Integrated Review of Security, Defense, Development and Foreign Policy articulated the importance of an Indo-Pacific region with “open societies”. The UK signaled a willingness in the review to play a larger role in the region, committing to a larger naval presence to ensure freedom of navigation. It has applied to join the Comprehensive and Progressive Agreement for Trans-Pacific Partnership (CPTPP), a free-trade bloc of 11 Pacific nations including Japan, Australia and Canada. The UK has also become an official dialogue partner of the ten-member Association of Southeast Asian Nations

Deeper ties The new deal is expected to generate additional employment in both countries, grow bilateral trade and unlock new opportunities in sectors such as food and drink, business services such as law and accounting, advanced engineering, defense, education, energy, life sciences and healthcare. It will also reduce barriers, both tariff and nontariff, for businesses at a time when the prospects for global growth after the pandemic remain uncertain. Saathee.com

but they’re not rare, said Dr. Jack Resneck Jr., an American Medical Association board trustee. The AMA is advocating for “tweaks” to the rule, he said, like allowing brief delays in releasing results for a few of the highest-stakes tests, like those diagnosing cancer, and more clarity on whether the harm exception applies to adolescent patients who might face emotional distress if their doctor breached their trust by sharing sensitive information with their parents. The Office of the National Coordinator for Health Information Technology, the federal agency overseeing the rule, responded in an email that it has heard these concerns, but has also heard from clinicians that patients value receiving this information in a timely fashion, and that patients can decide whether they want to look at results once they receive them or wait until they can review them with their doctor. It added that the rule does not require giving parents access to protected health information if they did not already have that right under HIPAA. Patient advocate Cynthia Fisher believes there should be no exceptions to immediately releasing results, noting that many patients want and need test results as soon as possible, and that delays can lead to worse health outcomes. Instead of facing long wait times to discuss diagnoses with their doctors, she said, patients can now take their results elsewhere. “We can’t assume the consumer is ignorant and unresourceful,” she said. In the meantime, hospitals and doctors are finding ways to adapt, and their tactics could have lasting implications for patient knowledge and physician workload. At Massachusetts General Hospital, a guide for patients on how to interpret medical terminology in radiology reports is being developed, said Dr. William Mehan, a neuroradiologist. An internal survey run after radiology results became immediately available to patients found that some doctors were monitoring their inbox after hours in case results arrived. “Burnout has come up in this conversation,” Mehan said. Some electronic health records enable doctors to withhold test results at the time they are ordered, said Jodi Daniel, a partner at the law firm Crowell & Moring. Doctors who can do this could ask patients whether they want their results released immediately or if they want their doctor to communicate the result, assuming they meet certain criteria for exceptions under the rule, she said. Chantal Worzala, a health technology policy consultant, said more is to come. “There will be a lot more conversation about the tools that individuals want and need in order to access and understand their health information,” she said.

Patient Data

continued from page 42

For Sarah Ford, 34, of Pittsburgh, who has multiple sclerosis, reading her doctor’s notes helps her make the most of each visit and feel informed. “I don’t like going into the office and feeling like I don’t know what’s going to happen,” she said. If she wants to try a new medication or treatment, reading previous notes helps her prepare to discuss it with her doctor, she said. The new rule will have less impact on Ford and the more than 50 million patients in the U.S. whose doctors had already made their notes available to patients before the rule kicked in. However, only about a third of patients with access to secure online health portals were using them. While most doctors who have shared notes with patients think it’s a good idea, the policy has drawbacks. One recent study found that half of doctors reported writing their notes less candidly after they were opened to patients. Another study, published in February, found that 1 in 10 patients had ever felt offended or judged after reading a note. The study’s lead author, Dr. Leonor Fernandez, of Beth Israel Deaconess Medical Center, said there is a “legacy of certain ways of expressing things in medicine that didn’t really take into account how it reads when you’re a patient.” “Maybe we can rethink some of these,” she said, citing the phrase “patient admits to drinking two glasses of wine a day” as an example. “Why not just write ‘two glasses of wine a day’?” UC San Diego Health started phasing in open notes to patients in 2018 and removed a delay in the release of lab results last year. Overall, said Dr. Brian Clay, chief medical information officer, both have been uneventful. “Most patients are agnostic, some are super-jazzed, and a few are distressed or have lots of questions and are communicating with us a lot,” he said. There are exceptions to the requirement to release patient data, such as psychotherapy notes and notes that could harm a patient or someone else if released. Dr. David Bell, president of the Society for Adolescent Health and Medicine, believes it’s unclear exactly what qualifies as “substantial harm” to a patient — the standard that must be met for doctors to withhold an adolescent patient’s notes from a parent. Clarity, he said, is especially important to protect teenagers living in states with less restrictive laws on parental access to medical records. Most electronic medical records are not equipped to segregate sensitive pieces from other information that might be useful for a parent in managing their child’s health, he added. Some doctors say receiving devastating test results without counseling can traumatize patients. Dr. James Kenealy, an ear, nose and throat doctor in central Massachusetts, said a positive cancer biopsy result for one of his patients was automatically pushed to his portal over the weekend, blindsiding both. “You can give bad news, but if you have a plan and explain, they’re much better off,” he said. Such incidents aren’t affecting the majority of patients, Saathee.com

when I was growing up and isn’t one now. Science changed its mind. But wait, another planet may fly by every 700 years, so that could be the 9th planet we haven’t yet seen. That’s where we are – infants. Maybe all the newly declassified UFO info will show that we are total morons about a billion years behind other star systems. And maybe those star systems don’t believe in god, or have a deeply ingrained belief in god. So, we geniuses now claim that the universe is 90 billion light years across. That’s subject to change before this article is even printed, because we don’t really know crock. We don’t know much about god. We haven’t seen the entity. It could be within us. It could be nature. It could be the invisible breeze. It could be an animal. It could be everything. It could be nothing. Approaching the bitter end, I’m just breathing. I can pray, but can’t understand what the various gods want – they want me to beg? Cry? Crawl? And then eternal life, or a new life happens? No thanks. You know, you god dudes are supposed to see everything. Just see me birth to death – then rebirth me as something, send me to your own version of hell or heaven, turn me to ash. Do something awesome. I’m bored. Hand me a beer – then do whatever god-like.

ate the soul of the nation. As America slid into an abyss, those who followed it globally went along with the early post-modernist tinges of what became symbolic of what we now admire about western thought. Us, we left for Thailand in the early-70s on my dad’s newest gig with the UN. Bangkok had no religion. It was a confusing mix of superstition and nightclubs. Even at a young age I started noting the confusion between people just being alive and religion. The UN consisted of the usual diversity of people from all cultures and religions. It was liberal before liberal was cool. We attended Hindu festivals, Buddhist events, Christian celebrations, and Jewish bat mitzvahs. You drank and danced with all of humanity. You didn’t judge anybody; you were never better than anybody. And then American college happened. While my many religion and philosophy professors never expressly said anything about their views on god(s), I kind of got the impression over seven years of college that they were hinting something. Arguments about the physical nature of god, and the metaphysical nature of god, or just the importance of god to balance the brains of us wobbling idiots – none of that solidified into an argument on either side. Last many, many months I’ve been spending my quality time reaching into the depths of space. We are such geniuses that we can’t even decide if Pluto is a planet. It was a planet Saathee.com

Primary Care Physicians

forced thousands of primary care physicians — who didn’t receive the government largesse showered on major medical systems — to lay off staff members or even close their doors. Reversing this slide will require new investment, the authors of the new report conclude. But, they argue, that should yield big dividends. “If we increase the supply of primary care, more people and more communities will be healthier, and no other part of health care can make this claim,” said Dr. Robert Phillips, a family physician who co-chaired the committee that produced the report. Phillips also directs the Center for Professionalism and Value in Health Care at the American Board of Family Medicine. The report urges new initiatives to build more health centers, especially in underserved areas that are frequently home to minority communities, and to expand primary care teams, including nurse practitioners, pharmacists and mental health specialists. And it advocates new efforts to shift away from paying physicians for every patient visit, a system that critics have long argued doesn’t incentivize doctors to keep patients healthy. Potentially most controversial, however, is the report’s recommendation that Medicare and Medicaid, as well as commercial insurers and employers that provide their workers with health benefits, ask their members to declare a primary care provider. Anyone who does not, the report notes, should be assigned a provider. “Successfully implementing high-quality primary care means everyone should have access to the ‘sustained relationships’ primary care offers,” the report notes. This idea of formally linking patients with a primary care office — often called empanelment — isn’t new. Kaiser Permanente, consistently among the nation’s bestperforming health systems, has long made primary care central. (KHN is not affiliated with Kaiser Permanente.) But the model, which was at the heart of managed-care health plans, suffered in the backlash against HMOs in the 1990s, when some health plans forced primary care providers to act as “gatekeepers” to keep patients away from costlier specialty care. More recently, however, a growing number of experts and primary care advocates have shown that linking patients with a primary care provider need not limit access to care. Indeed, a new generation of medical systems that rely on primary care to look after elderly Americans on Medicare with chronic medical conditions has demonstrated great success in keeping patients healthier and costs down. These “advanced primary care” systems include ChenMed, Iora Health and Oak Street Health. “If you don’t have empanelment, you don’t really have continuity of care,” said Dr. Tom Bodenheimer, an internist who founded the Center for Excellence in Primary Care at the University of California-San Francisco and has called for stronger primary care systems for decades. Bodenheimer added: “We know that continuity of care is linked to everything good: better preventive care, higher patient satisfaction, better chronic care and lower costs. It is really fundamental.” Article Courtesy of KHN.org Saathee.com

Ironically, modern culture has become obsessed with the concept of authenticity. Just about every major story franchise centers around a character or a series of characters who must partake on some sort of journey to ‘discover who they really are’. Just about all of us at one point or another has spent part of our lives trying to figure out who we are while also trying to stay true to ourselves. The thing is, that while we can attempt to be as ‘legitimate’ as we want… there is still that stigma towards a certain image we must adhere to depending on the expectations of our culture and societal norms. The paradox is that all that effort to be ourselves, we instead become just like everyone else. Our uniqueness becomes just another drop within the sea of other individuals who ultimately share similar traits. One of the largest contributing factors for this has been our growing attachment to social media. In this medium, we can construct fantasy versions of ourselves to which we then perform as if always on a stage. To this end, some often develop an acute sense of narcissism to stand out. While this stance may seem to create a posture of authenticity, it instead becomes the exact opposite. Often the narcissist is hiding confusion and insecurity behind this façade, which in turn makes this person about as non-authentic as anyone can imagine. Unfortunately, we now live in a world where the concept of privacy has become a rare commodity; the very idea that there are aspects of a person’s life that one might not want to share with the public at large. There is also a distinguishable generation gap between those who have always known a world with social media in it, and those who have survived without the desire to chronicle every little detail for the whole world to see. For some it is just another part of life, while others see it as a vice they can do without. So, the question ultimately becomes… how does one be authentic in a world defined by diverse perceptions? Well for starters, it requires finding a way to resist the temptation towards self-absorption and fantasy. Secondly, we need to acknowledge that despite us all being individuals, almost everything in our culture encourages us to rely on others in order to escape our past and eliminate any chance of further misfortune in our lives. Referring back to The Falcon and the Winter Soldier, the character who used the image of Captain America for wrongdoing… did so in part because of his decision to strike off on his own despite others offering to help him out. The other character, in contrast, utilized his allies and henceforth made a much more positive impact on not only himself but those around him. I guess the true answer to being authentic is to humbly acknowledge our individual limitations, while at the same time use those same limitations to enrich the lives of others. As Socrates once put it, true wisdom is found in acknowledging what you don’t know and being completely comfortable with it. When put into that perspective, we all have the potential to discover who and what we are really meant to be.

But the next best thing to that is the current accessibility some of us are fortunate to have through the internet. I asked our home device to play songs to my desires. After, I slept a little better. I felt blessed just that it continued playing and did not stop. I listened in my feverish haze. I thought to myself, if I ever am in such a moment of sickness in the future, or worse, to have bhajans/shlokas playing to help me. Technology seems to be a great connector. For instance, during the pandemic, many dances and performances that would have been live, were offered online. Instead of having to travel somewhere, the options available to be able to view shows increased. Also, during the pandemic, I have been taking the dance classes online, an option that may not have been available only a few years ago. There are shows in the past I could only have wished of seeing, or having to see by travelling, that now I could participate through watching on the internet. Of course, seeing live in-person performances is always preferential for many, but sometimes it is not always possible. Many of us have family in various countries or locations, and while seeing them in person is always the optimal scenario, there may be circumstances that they cannot travel to certain events. We live in such amazing times that people can potentially livestream their events. Now, if our relatives in another country do not have the means to attend the salangai Pooja or arangetram, they can watch through their phones, computers, laptops or other devices, if the event is organized to have that set up. Afterwards, you can live chat with your family as well to get their thoughts and blessings. Those who may be homebound or even to go further, hospitalized, can still participate. Technology can lessen that distance between people and the arts and provide greater accessibility as well to those who may need it the most. Hopefully, even after the pandemic, many performers think about these aspects of a show and implement the livestream aspects to those who cannot physically attend. Not only will it broaden their reach, but it can also, on the individual level, improve the quality of a person’s life who may not have the means or abilities to travel, whether it is near to their house or another country. Dance and arts are a connector and a form of communication and let us utilize technology to reach those who may most need it. For me, the music and dance has eased some of the anxieties of the pandemic and having technology to help us access these forms of communication has been the most essential in these trying times.

Charger on this warm spring day. I rolled the windows down and let the guitars wail through the air, the sound waves bouncing off the road, into the ether and then out to outer space. Adding to the sound on this day, along with the guitars of old favorites, Coldplay laying down a hymn for the weekend, Bob Marley singing about so much trouble in the world, Tom Waits growling about how the earth died screaming and Led Zeppelin once again transporting me to a mythical Kashmir. It’s been forty years since the end of high school, graduating and diving into the summer of 1981. It was a summer of open-ended dreaming, working crappy jobs, prepping for college, driving around in the Nova. It was a summer where the Nova saw a lot of the road. I didn’t travel much outside the state, but had a good time driving in and around Charlotte. The radio was king of the car in high school and college days, driving around listening to the DJ spinning song after song. Radio knobs were as important parts of the car as the gears and steering wheel. The tape deck was always ready for a mix tape when the radio signal was fuzzy, or the Disc Jockey wasn’t spinning what your mood of the hour wanted to hear. I imagined one of my old mix tapes breaking when Eddie Van Halen passed away last year. I downed a couple of cheap beers in his honor that night, thinking of the first time I heard his guitar magic. Summer 1981 opened a new era of faster adventures, beginning of self-reliance, making new connections, seeing high school friends begin to scatter to their destinies toward the end of the summer. Summer 2021, a bookend marking 40 years, is strangely similar in its seeking of and finding connections, but of course it is more about reconnections this time, a renewal for the post isolation era, and resuming adventures, yes, but slower, taking time to feel the breeze. The open roads don’t seem endless anymore. There are limitations, destinations to be reached. Loose ends to be tied up, lest time runs out. Streaming music on the phone, roaming the roads, one can call up a song in an instant, but I miss sticking a mix tape into the tape deck, charging into that sunset ahead, emanating from a very distant, bright and starry Nova.

Dadaji’s moral stories for kids THE SWAN AND THE CROW

A swan and a crow lived in a large tree. Once, a weary traveler approached the tree and decided to enjoy the shade. He placed his bow against the trunk and soon fell asleep underneath the tree. After a while the shade disappeared, exposing him to the harsh sun. The swan felt pity for this traveler and spread his wings to block the sun. The crow did not approve of such kindness for a human. He dropped his nest on the traveler’s head before fleeing. The man woke up and saw the swan in the tree. He thought the swan had dropped the nest and so grabbed his bow and pierced the swan with an arrow to vent his anger. The benevolent swan lost its life despite good intentions. Moral: Be wary of those who are wicked.

June 2021

Aries (March 21 – April 19)

Libra (September 23 – October 22)

Now is the time to enjoy the fruits of success you’ve achieved over the past few months. Don’t seek any new changes this month. Family happiness is clearly indicated. You may purchase luxury items to please your partner or loved ones but try not to overdo it. Watch your tongue during the last week of the month. A minor health issue could be there.

This will be a month of traveling, a period of enjoying life, and celebrating your achievements and success. Your long-cherished wish will be fulfilled this month. Just watch your health. Don’t dispute with your partner. Try something different to achieve success. Don’t contemplate profit or loss; just continue to walk a few steps along this unknown path.

Taurus (April 20 – May 20)

Scorpio (October 23 – November 21)

Things will gradually improve for you but won’t allow for any grand achievements this month. A boost in positive energy will help with financial gains. Victory over your opponents should help in controlling them. Your will enjoy good health if you control your dietary habits. Avoid any disputes with family members. A new job opportunity may come your way but be sure to weigh all your options before accepting it.

The hard work you pursue during the first half of the month will produce dividends in the second. These results will not be promising, however, but satisfactory. You will experience delays and disturbances in your day-to-day pursuits so try to avoid getting involved in too many projects. This is a good time for traveling for a vacation. A religious or social ceremony is also possible for your family.

Gemini (May 21 – June 20)

Sagittarius (November 22 – December 21)

You may be forced to compromise with your morals this month. Your skillset should help boost your income. Destiny is on your side. You may become terribly busy at your place of work. This is a good time to make new friends and consider marriage related decisions. You may earn fame due to your creative activities. Take extra precautions for your health and avoid your enemies whenever possible.

Even though there will be no financial problems, your hyper personality and paranoia will rule you this month. Try to develop mental strength. If you feel it necessary, spend some time to do meditation. This period is not negative, but your aggressive actions won’t let you enjoy it. At the same time watch your health. Any risk-taking tendencies must be avoided.

Capricorn (December 22 – January 19)

Cancer (June 21 – July 22)

A very busy schedule and unexpected travel may disrupt your everyday plans. This is a good time to invest in real estate. Go against your nature and try to maintain a practical work ethic. A misunderstanding at work may create tension which cannot be avoided, so try to remain calm. An improvement in your health is clearly indicated. Bad news from your partner is possible during the second week of the month.

Life should fly by this month as you could experience many major ups and downs. You should achieve great success in the end, but only if you maintain strict morals. Any potential wrongdoing must be avoided. You can count on your business partners, but do not trust them blindly. Minor family problems may also disturb your peace of mind. A heavy workload could keep you on your toes. Don’t take any unnecessary risks.

Leo (July 23 – August 22)

Aquarius (January 20 – February 18)

A very hectic month is shaping up for you. You may become stuck between a busy schedule and enjoying time with family. Added responsibilities along with this overly aggressive schedule could cause you to make mistakes. Any rash decisions and aggressive actions must be curbed this month. Listen to your partner before making any commitments. Your health may also decline. Just be sure to slow down and control your anger.

Uncertainty may delay critical decisions and lose an important opportunity for you this month. Your mistakes could put you in hot water. Financial loss is also unavoidable; however, your regular income should remain undisturbed. You may need spiritual guidance throughout the month. Try to protect your morals. Don’t take any large risks.

Virgo (August 23 – September 22)

You have now entered a difficult phase in your life. You could face tough competition at your place of work or business. Luck may not favor you at this critical time. You may gain some support from friends and see relief during the second half of the month. Try to control your aggression and act wisely to avoid loss. Avoid taking any substantial risks during this time. A minor health ailment is also possible.

Pisces (February 19 – March 20)

Take things slow during first half of the month as major familyrelated problems may disturb your usual routine. However, you should be back in control by the second half. Don’t rely too much on luck to see you through. Your partner may prove to be of great help for you this month. Try not to cheat to make easy money. Remain calm, avoid aggression and act wisely instead.

India - UK Trade Deal

Sound of Your Voice

voice is necessarily worse than our perceived voice. Instead, we’re simply more used to hearing ourselves sound a certain way. A study published in 2005 had patients with voice problems rate their own voices when presented with recordings of them. They also had clinicians rate the voices. The researchers found that patients, across the board, tended to more negatively rate the quality of their recorded voice compared with the objective assessments of clinicians. So, if the voice in your head castigates the voice coming out of a recording device, it’s probably your inner critic overreacting – and you’re judging yourself a bit too harshly.

All this, including the trade deal with India, seeks to help the UK to “unlock opportunities in the region” and establish an outward-looking global Britain. Finally, announcing an UK-India agreement before the EU-India summit on May 8 gives the UK a first-mover advantage over the trading bloc that it has just left behind. In keeping with the UK-India deal, facilitating investment, regulatory cooperation and trade barriers are at the top of the agenda at the summit. The EU is also seeking to make progress on a free-trade agreement, as well as several other treaties on specific aspects of trade. Other activities like research, service, leadership and, especially, clinical experience are extracurriculars that a premed student definitely needs. However, like I mentioned, make sure to modify these to be interests that you actually enjoy and not activities that you grudgingly do to cross them off your list. For example, you may not like doing research in a lab setting like I did. Some find it boring and that’s okay. There are other types of research to pursue in clinical or public health settings. If you like the hospital more than the clinic, instead of working as a medical assistant in a primary care office, shadow more surgeons, serve as an interpreter or even get trained to be an EMT. All it comes down to is finding your passions and staying true to them. Next to your passions, the second most important part of the pre-med journey is to give yourself grace periods. As pre-med college students, there is so much to do and so much to figure out. The pre-med journey does not have to be limited to exactly four years while we finish our undergraduate degrees. I attend medical school with peers of a variety of ages, backgrounds and most importantly, premed journeys. We each arrived at medical school with varying timelines filled with unique experiences and interests.
