Community Based Assessment Case Manager
The NFI St. Albans WRAP Program is seeking a specialized case manager to join our clinical team. This position will provide clients and families with 60-90 day community-based assessments. We are looking for someone to provide children and families with an accepting, caring and honest relationship to support preservation, guide healing and nurture dreams. Job duties include: Provide a comprehensive community based assessment of clients, and families mental health needs, develop and support treatment teams, work with community partners to create appropriate plans of care, and completion of clinical documents. NFI offers opportunities for training in models of trauma treatment for children and families. This is a FT 40 hour/week position which includes a comprehensive benefits package. This is a community-based position which requires travel in Franklin and Grand Isle counties, reliable transportation and a valid drivers’ license required.
