Patch testing with TROLAB allergens Patch testing is the most important diagnostic and investigative method currently available for studying allergic contact dermatitis in clinical practice. The method includes the epicutaneous application of a specific allergen at a defined concentration and in a defined vehicle which will induce a cutaneous inflammatory reaction in a sensitised person, but will cause no reaction in a non-sensitised person. TROLAB patch test allergens have always been developed, revised and produced following the recommendations of the International Contact Dermatitis Research Group (ICDRG) and the European Environmental and Contact Dermatitis Research Group (EECDRG). Their many years experience helped us to select the test substances and determine test concentrations and test vehicles, so that generally the test allergens do not trigger toxic reactions whilst allergic ones can be induced. Today we add new test allergens to the TROLAB programme only, if test concentrations and vehicles have been proven and confirmed in clinical trials. TROLAB patch test allergens are dispersed in petrolatum (if no other vehicle is indicated) and filled in 5 ml syringes. Some test allergens contain additives like e.g. sorbitan sesquioleate or solid paraffin. These additives are indicated on the syringe lable. Almirall’s experience as a pharmaceutical company specialising in the production of topical dermatological products also guarantees that the patch test allergens are produced in a high galenic quality.

TROLAB patch test allergens are produced in accordance with the WHO guidelines of Good Manufacturing Practice. Raw materials are selected carefully. The allergens are homogenously incorporated into the ointment in the form of extremely small particles. The stability of the final product is checked by chemical analysis. AII TROLAB Patch Test Allergens are registered as MEDICINES in Germany with the Paul Ehrlich Institute, Langen. Our overall objective is to offer our customers high quality standardised patch test allergens. However, it should be noted that the interpretation of the test results requires much experience on the part of the testing physician, i.e. careful anamnesis and examination of the clinical findings for the final assessment of the relevance of the test results.
Application of test substances TROLAB patch test allergens can be used with all types of patch test plaster. Very well accepted are Finn Chambers on Scanpor *. It is important to consider that all test concentrations were defined by using this test plaster.The following description is for using Finn Chambers on Scanpor (for other test plasters please follow instructions of manufacturers).

When using Finn Chambers on Scanpor , the serial numbers of the substances to be tested must be clearly marked on the upper side of the adhesive tape (Fig. 1), for example: S1 - S10 (Standard, substances 1-10). The small tape sections carrying the numbers will later remain on the skin and allow to perform the reading with the practical Reading Plate*.

Place the test tape in front of you, as shown in Fig. 3. Now you can fill the chambers downward starting on the top right row. The test ointments are applied directly into the aluminium chambers (Fig. 3). The chambers must never be filled completely but only slightly more than half-full (5 mm length). No substance should be brought on the rim of the chamber!

When using test solutions, a filter paper disc has to be placed into the chamber first (Fig. 4 a). The filter paper has to be saturated with one drop

of the test solution (Fig. 4 b). Please avoid excess. On the other hand, the filter paper disc must not dry up, as in that case false-negative test results would have to be expected. We recommend to prepare the plaster just before applying it to the patient’s skin in order to achieve relevant test results. The test tapes that have been prepared as described are applied to the back in normal upright position, with the lower margin being fastened first. Thereafter they are pressed against the skin with

the palm, gradually from bottom to top, and then carefully stroked down. The air can thereby escape from the chambers and a good fixation is achieved. Subsequently, the test chambers are again pressed against the skin two to three times, exerting slight pressure with the fingers in order to spread the test substances evenly on the skin (Fig. 5 a + 5 b). The adhesive tapes should be applied only to the clean and dry skin (non-greasy, no ointment residues etc.). You might use diluted alcohol to clean the skin.

The Reading Plate is a simple aid to recognize the test reactions without having to mark the skin of the patient with ink. As a preparation for the reading, before the test chambers are filled, the numbers of the substances to be applied have already been noted on the upper ends of the adhesive tapes (see Fig. 1). Fig. 6 demonstrates how to use the Reading Plate when the adhesive tape is removed (usually after 48 hours). The cutting edge of the plate is slightly pressed against the test tape at an angle of about 45 degrees. The broader side of the plate should thereby be held on the left. Now, grasp the test tape and tear it off in a brisk upward movement, thereby leaving the strip (about 1 cm wide) where the numbers are marked (Fig. 7). After the removal of the Finn Chambers on Scanpor , circular imprints on the skin reveal whether the test provided adequate occlusion. If these imprints are not visible, the test has to be repeated (it is known from experience that this rarely happens if the adhesive test tapes were properly applied).

Fig. 7 demonstrates how the Reading Plate is used during the first as well as during the subsequent readings when the circular imprints of the test chambers on the skin have already disappeared. Place the left corner of the Reading Plate on the left corner of the remaining strip of the adhesive tape. The holes in the plate now indicate the locations of the test sites. Please note:
The first reading should be performed about 20 to 30 minutes after the removal of the tape, as some skin sensitive persons will react with redness after tape removal. Excessive test ointments have to be carefully removed with an individual swab for every substance prior to the first reading. Make sure not to wipe the excessive ointments into the adjacent test areas. If no Reading Plate is available, the locations of the individual test chambers have to be marked on the skin with a suitable marking pen before removing the test tape, so that positive test results can be clearly identified during further readings after 72 and 96 hours.

Interpretation of test results When recording the results in the patient’s test sheet, it is important to score according to the reaction actually seen and not according to the subjective interpretation placed on the reaction by the reader. Irritant reactions should be recorded as positive irritant (IR) and not as negative. To recognize irritant reactions it may be helpful to know of the irritability of the skin at the test side and the time of testing. Almirall is offering Sodiumlaurylsulfate (SLS) 0.25 % in water as a control irritant for patch testing. Whether or not the positive test reaction demonstrated by the patch testing is relevant to the patient’s dermatitis, either as the primary cause or as an aggravating factor, must be determined by the critical judgement of the physician. If the clinical and historical facts are consistent with the patch test result, one can be reasonably certain of its significance. In doubtful cases, a repeated open application test (ROAT) in the antecubital space or on the outer aspect of the upper arm may be helpful.

Information on the irritancy control patch test with Sodiumlaurylsulfate (SLS) 0.25% in water Why to do an irritancy control when patch testing with contact allergens. Every contact allergen produces an irritancy effect on the skin to some degree. It is thought that this irritancy is important for a substance to be able to act as a contact allergen. Due to this particular property, irritant or false positive test reactions may occur in diagnostic patch testing with contact allergens. The frequency and extent of such unwanted reactions differ not only between allergens but also between individuals, depending on their particular sensitivity to irritants. This irritability may vary between different skin regions and over the course of time. Therefore, the German Contact Dermatitis Research Group (Deutsche Kontaktallergie-Gruppe ; DKG) recommends to perform an irritancy control patch test when patch testing with contact allergens, in order to assess the individual sensitivity to irritants at the time of patch testing in the test area. Which irritant is suitable? There are different kinds of irritants, e.g. weak bases or acids, or detergents, which disturb the epidermal barrier. For the purpose of an irritancy control patch test, Sodiumlaurylsulfate (SLS) has proved useful. SLS is not an allergen and, tested at a low concentration it causes visible reactions only in very sensitive individuals. DKG studies have shown that patients, in whom SLS 0.25 % in water elicited an irritant reaction, had questionable and weak positive patch test reactions to contact allergens more frequently than others. In most cases, these patch test reactions, too, are irritant, and not allergic, reactions. Thus, patch testing with the irritant SLS 0.25 % in water as a marker for sensitive

skin facilitates the assessment of erythematous and weak positive patch test reactions to contact allergens. How to perform and read the SLS patch test and how to interpret the results. The irritancy control patch test with SLS 0.25 % in water is performed in exactly the same manner as the classical contact allergen patch test. For reading and assessment of the test reactions, the DKG recommends the following categories (modified from Tupker et al. 1997): Score sls0 sls1 sls2 sls3 sls4

When patch testing with SLS 0.25 % in water, only very rarely reactions of grade sls3 or sls4 occur. In the DKG studies, less than 30 % of the patients tested had a reaction; most of the reactions were classified as sls1 or sls2. In patients having a visible reaction of this kind, an increased irritability of the skin is probable.

Significance and limitations of the irritancy control patch test. An irritant reaction (sls1 or stronger) to SLS 0.25% in water does not automatically mean that every weak patch test reaction to an allergen is also an irritant; however, the probability that an irritant erythema or a weak false positive reaction occurs in this patient is markedly higher. This is particularly true for test allergens known to have an increased irritant potential. The irritancy control patch test facilitates the assessment of this kind of test reactions; however, it does not replace the need for a thorough interpretation of the test reactions (allergic versus irritant) by the physician and for the assessment of clinical relevance. 15
As mentioned above, the irritability of the skin may vary between body regions and over the course of time. Thus, the irritancy control patch test with SLS only gives information about the irritability of the test region (upper back) at the time of patch testing. It can be considered a regionally limited snapshot, which does not allow any generalisation concerning the patients’ skin irritability. The Baseline series consists of the minimum recommended test set to be used during routine diagnostic patch test investigations and which will be ordinarily supplemented by additional series and other allergens as appropriate to adequately investigate the clinical problem. Assuming that this recommandation is followed we have not placed any substance from the Baseline series in one of the other series to avoid double patch testing. Statements given by the ICDRG: a) Thiuram Mix lacks high specificity and sensitivity b) Some cases of hair dye dermatitis could be missed with the use of PPD alone. c) Mercapto Mix lacks high specificity and sensitivity. Comments by Almirall to this proposal: 1. Nickel Sulphate is in the North American Standard Series 2,5 % as the waterfree salt; in Europe the NiS04 路 6H2O salt 5 % is approved by the authorities. The molecular content of nickel ions of both test allergens is almost equal. Testing NiS04 路 6H2O at 2,5 % as proposed by the ICDRG means testing only 50 % of the Nickel content that is being tested in Europe and North America.
