When Every Second Counts
Jim McLandsborough was an active 72-year-old. He enjoyed long bike rides, playing tennis and even competing against men 20 years his junior in spirited games of racquetball.
So the last thing he expected while working outside at his lake home on the Cass Lake chain in May of 2013 was a heart attack. But when the sudden sharp pain in the middle of his chest hit, Jim immediately knew what it was. “I stopped what I was doing, walked inside and took some aspirin,” Jim says. “Then I asked my wife to call 911 because I was having a heart attack.” For Jim, everything else after that was a blur. Every second became crucial to his survival. Emergency services transported Jim by ambulance to Sanford Bemidji Medical Center and the cardiology team from Sanford Bemidji Heart & Vascular Center quickly got to work. “When Jim arrived at our hospital, he was already quite sick,” says Dr. Mohammad 'Nur' Jameel, interventional cardiologist at the Sanford Bemidji Heart and Vascular Center. “Not only was he having a heart attack, he was in cardiogenic shock, which is a very dangerous situation.” continued to next page

Jim was immediately transported to the cardiac catheterization lab where Sanford’s experts went to work. Before they could even perform the procedure, Jim went into ventricular tachycardia and then cardiac arrest. Dr. Jameel and his team responded quickly and put their training and experience into action. “We began CPR at that time, which When Jim McLandsborough needed immediate treatment for a heart attack, Dr. Nur Jameel and Sanford Bemidji's cardiology team were there. Jim attributes his survival to having the proved successful for Jim,” Dr. Jameel right team in the right location. says. “An intra-aortic balloon pump was placed to support his blood pressure. Once we were able to stabilize Jim, we performed the catheterization.” It was then they discovered his distal left main coronary artery was 99 percent blocked; this artery is more commonly known as the “widow maker.” With balloons and stents, the artery was opened, but Jim was still quite sick. The skilled and compassionate team of experts at Sanford were determined to help Jim through it all. And they did. “With Jim’s condition, the risk for complications was high,” says Dr. Jameel. “But getting blood flow restored as quickly as possible was the key to his survival.” Jim spent six weeks in the hospital and then returned to his lake cabin to be near Sanford’s facility for follow-up visits. He required extensive therapy while in the hospital and after. “I was extremely weak and I knew I had a significant recovery ahead of me,” Jim says. “My lake house was the perfect place to recuperate and I was still close to Sanford if I needed help.” Jim firmly believes it was the proximity of Sanford Bemidji’s Heart and Vascular Center that saved his life. “I do not think I would be alive to tell this story if that facility weren’t there,” Jim says. “If I had needed to be transported very far, I don’t think I would have been able to make the trip.” According to Dr. Jameel, Jim was probably right. “If our facility weren’t here, Jim probably would have been transported to Fargo, and time was of the essence,” says Dr. Jameel. “The sooner we restore normal blood flow the better, and this would have delayed the process for Jim. That could have led to less than ideal results.” Now Jim has returned to his home in the Minneapolis suburb of Eagan. He continues pushing himself and walks nearly every day, with the hopes of returning to a more active lifestyle. “I know I was very sick, and I’m trying my very best,” he says. “But through it all – I am so grateful just to be alive.”

My Sanford Chart: Take Charge of Your Health
Becky Lueben is proactive when it
comes to her personal health care. So when Sanford Bemidji rolled out My Sanford Chart last summer, Lueben quickly embraced this free online system that gives her an efficient way to manage her health care needs. Lueben appreciates having immediate access to medical records such as her health summaries, prescriptions and test results. And there's no need to call her physician's office with questions any more; she simply zips her questions directly to her family medicine physician, Dr. David Wilcox, via Sanford My Chart's secure e-mail system. She knows she will get a timely reply. "I think what's been most important for me is that I can go online and look at my health data any time and that helps me be a more proactive consumer," Lueben said. "At my last appointment, I had my lab done the day before my appointment. Then I checked the results on My Chart before my appointment. That way I know if there are changes and I'm prepared to ask questions and talk about adjustments we might need to make. I feel more in charge of my own health care."
Becky works at Target in a job that requires physical stamina. She likes that with My Chart she can graph her test results to look for trends or patterns that may need medical attention.
Lueben, 61, is on medication for diabetes, high blood pressure and high cholesterol. She laughingly calls her weak knees the "arthritic dime store junk version that I've had since I was a kid." While chronic conditions like these sideline some people, Lueben works at Target in a job that requires physical stamina and she also regularly participates in Zumba dance classes. Using My Sanford Chart helps her monitor and maintain her health. She appreciates the appointment reminders. She likes being able to easily access relevant data, such as when she needs to schedule annual procedures

such as mammograms. Being able to check past procedures has also been valuable. "I work on a loading dock around heavy machinery so I'm a walking bruise and it's important I'm current on my tetanus vaccinations," she explained. "I can easily go into my chart and find out if I'm up to date. I hate to admit it, but My Sanford Chart is much more reliable than my memory." Lueben is enthused about My Chart's flow chart capabilities that provide a historic graph relative to specific medical information. For instance, she can graph her blood pressure readings and A1C results over specified time periods to look for trends or patterns. (The A1C laboratory test measures average blood sugar control over a three-month period.) "Dr. Wilcox encouraged me to use My Chart and I think I was one of the first to start using these flow chart capabilities," she said. "What we learn from them can help me be better regulated and give him a better idea of what works for me." Getting started on My Sanford Chart is easy and free. The program's prompts lead users through the steps necessary to register and obtain a secure login and password that ensures no one else can access their personal records. The East Grand Forks native and University of North Dakota School of Medicine graduate is married to Bemidji native Julie Papenfuss (Breyen). Following his residency, they settled in Bemidji where he was a general surgeon at the hospital from 2008 to 2012. With completion of the surgical oncology fellowship, Dr. Papenfuss is board eligible in Complex General Surgical Oncology. He has begun the process of becoming board certified.

"A few years ago we weren't seeing a lot of complicated cancer surgery cases in Bemidji because people were often referred elsewhere," he commented. "With Sanford's commitment to becoming a regional referral center and building up the cancer center, it's an exciting time to be returning to Bemidji. With the surgical, medical and radiation oncology expertise we have in Bemidji, patients won't need to travel far from home for specialized cancer care."

Explaining his specialty area more precisely, Dr. Papenfuss said surgeons generally physically remove cancers from the body. The fellowship prepared him to better evaluate the best practice for each patient. Surgery may not always be the first or best option. Using a multidisciplinary approach, the surgical oncologist works with medical and radiation oncologists to develop an individualized treatment plan best suited to an individual patient's needs.

Wesley Papenfuss left Sanford Bemidji two years ago knowing he wanted to return. A general surgeon, Dr. Papenfuss moved to Buffalo, NY, to specialize in cancer care by completing a fellowship in surgical oncology at Roswell Park Cancer Institute. He recently returned to Sanford Bemidji. "I have always had interest in this specialty area and there is a need for surgical oncology in our community," Dr. Papenfuss said. "I look forward to being able to offer specialized surgical procedures for cancers of the breast, colon, lung, liver, pancreas, esophagus and thyroid in addition to other surgical areas. The team approaches to the complex surgical cases I participated in during fellowship have been beneficial because I understand how to help different patients and situations to achieve the best outcomes."

Medical Home Gives Patients New Lease on Life
Ben Hein was discouraged and frustrated
when he learned his blood sugar levels were still way too high after he had done everything he thought he should to manage his diabetes. So he wasn't very receptive when Medical Home Health Coach, Marty Veo-Wilson, RN, suggested he consider the Medical Home program. Now he's one of its best advocates. Coaches, who are all registered nurses, work closely with patients and their primary care provider to assure care consistency. Some patients are in the Medical Home to prevent chronic disease development; for instance, some current patients were referred for weight loss counseling, pre-diabetes or borderline hypertension. Any patient can ask his or her Bemidji doctor for referral to the program. "The Medical Home program is remarkable. Ben’s primary care physician Dr. Rod Will discusses Ben’s care plan with his specialist, Dr. Samar Malaeb, an endocrinologist. It's taken me to a whole new level," Hein crowed. "Marty has been the biggest pain in my butt and I love her for it! I spent years on my own poking myself with needles and trying to eat no sugar, less carbs and all those things. Now I have a much higher level of success controlling my diabetes." Sanford Medical Home is a concept, not a physical place. Begun in 2011, Sanford's model provides a team-based environment where the patient is the focus, and an actively participating member, of the team. The patient's primary care provider and a dedicated health coach, as well as other health care providers involved in the patient's care, are team members. Working with the patient, the health coach develops an individualized health care plan specific to that patient's medical needs and cultural and religious beliefs. Patients in the Medical Home benefit from improved care coordination and fewer hospitalizations and emergency room visits. The Medical Home is designed to meet needs of pediatric and adult patients with chronic illnesses, multiple illnesses, newly diagnosed illnesses, those on multiple medications and/or with special health needs. The coach helps the patient set goals to encourage self-management of conditions, refers the patient to resources that can help, and confers regularly with the primary care provider and team members about the patient's progress. Patients are encouraged to contact their coach whenever they have a question. "I've talked to lots of people with diabetes who aren't in control of their disease," Hein said. "At first, I thought diabetes just meant I shouldn't eat sugar and I should quit eating doughnuts. Then I learned carbs matter too. There are so many different things you have to know about trying to manage diabetes." Diabetes is a progressive, chronic disease that affects individuals differently, Veo-Wilson said. "I see a lot of people who don't understand how diabetes works even when they have had it for years. They get so frustrated they just give

up. Being in the Medical Home helps them understand the disease better and what we can all do to help them feel better." When his primary care provider Dr. Rod Will diagnosed him five years ago, Hein, 38, said he reeled in disbelief and denial. He was young, had no family history and thought he was the picture of health. His only hospitalization had been a gift of his kidney to his father when he was 20. He learned how to check blood sugars, inject himself with insulin and tried to eat differently, but still, his blood sugars easily spiked or plunged. Nor did his efforts to manage his disease bring his A1C down to an appropriate level. (The A1C is a test that measures how well the body has controlled blood sugars over a period of two to three months. Normal A1C levels range from 4.5-6. People with an A1C over 6.5 are diabetic; Hein's A1C was 12.1.) Observing Hein's diabetic instability, Veo-Wilson suggested a referral to Dr. Samar Malaeb, a board certified endocrinologist. Dr. Malaeb ordered specialized tests, which revealed Hein had Type 1 adult onset diabetes instead of Type 2, which is most common for adults.

Dr. Malaeb adjusted Hein's treatment plan and determined that he would be a good candidate for an insulin pump. These pumps have a catheter that is inserted under the skin. The computerized mechanism measures blood sugars and continuously releases insulin into the body. "Pumps are not an easy solution," Dr. Malaeb cautioned. "For people who are working hard and doing their best to control their diabetes, the pump can give them more security and flexibility." Since he began using the pump, Hein's A1C has dropped to 7.1. What impresses Hein the most is how good he feels. "The pump was expensive, but worth every penny now that I've seen what a difference it makes," Hein said. "With this machine and the good people I have at Sanford, I have a much better quality of life. And Dr. Malaeb is fantastic. She saw the hope when I didn't. Now I see it too." The man who didn't think the Medical Home was right for him now says it has given him a new lease on life and the chance at a better life ahead. Sanford Medical Home is a concept, not a physical place. The patient is at the center of the home, surrounded by the patient's personal physician and a dedicated health coach, as well as other health care providers involved in the patient's care.

Persistence Pays Off: Healing Wounds
Paul Braaten is thankful for his feet and for the
Sanford Bemidji Wound Care & Hyperbaric Center team that never gave up on him. "They are top notch," he says. "I'd go back to them in a minute but I hope I don't have to!" Braaten is confined to a wheelchair due to cerebral palsy and has peripheral vascular disease, which also compromises circulation. Over a 14-month period for treatment of a foot wound that refused to heal, Braaten was unable to wear shoes and at one point considered having his foot amputated. "This was a painful process for Paul, and the wound's location between his toes made it challenging to treat," said Angela McCarthy, CNP, a certified wound care nurse practitioner.
Paul Braaten is thankful he still has two feet and that the Bemidji Wound Care team never gave up on him.
Braaten, who continued to work throughout his ordeal, had to make a lot of lifestyle adjustments too. His wife became a home caregiver, regularly changing his dressings. His feet were swollen and pressure aggravated the wound, so Paul went shoeless for months. Various treatments appeared to be working and then the skin would regress again. "We tried every dressing known to man," wound care nurse Lindsey Stull, RN, said. "We also tried cellular matrix products, which mimic skin. Fortunately, we had success with the last one." Physician specialists conferred with team members about possible options. "Our whole team agreed we should stay the course and try everything possible to save his foot," McCarthy said. Braaten has a lifetime of practicing persistence. He was told cerebral palsy would put him in a wheelchair by the age of 8 but he didn't succumb to using a wheelchair until he was 48. Despite his health issues, he proudly notes that he has always worked and never been unemployed. Currently on medical disability, he still works as an unpaid volunteer at Braaten Repairs, a business owned by family members. "I look at both feet when I get out of bed in the morning to go to work and I just think how right on top of it the people in that clinic are," Braaten said. "One of the ladies there told me she even prayed for me when things just didn't work for me. I know the Good Lord had a part in this. I was a really tough case. I'm glad nobody gave up."

Pediatric Surgery Leaves Good Memories
Surgery isn't exactly the type of thing friends plan to do together, but it worked well for Erika Ahlbrecht and Maria Eastman. The women have been good friends for several years and their children are buddies too. So when Jasper Ahlbrecht, 6, and Claire Eastman, 4, needed to have their tonsils and adenoids removed, their moms scheduled the surgeries on the same day with Dr. Mitchell Marion, an ear, nose and throat physician.
Claire was the "fearless leader," Erika said, which gave the more hesitant Jasper some extra confidence. Maria waited for Claire's tears when the time came to go to the operating rooms, but off Claire went with a smile and a wave of her hand. Once back from their surgeries, the children were placed in private rooms. It wasn't long before they were cuddled in the same bed enjoying popsicles and television. And then they decided to explore their new play area.
Good buddies Claire Eastman and Jasper Ahlbrecht were able to have their surgeries on the same day, making their time in the hospital a lot more fun.
"Maria and I expected the kids would be tired and subdued after surgery and we could read magazines and sit and chat," Erika said. "Instead we ended up chasing them around the hospital. They were having a good time!" When Jasper's dad, John, and Claire's dad, Dan, arrived, they realized their wives and children didn't really need extra support. "It was a pretty neat situation," Maria laughed.
Sanford Bagley Awarded Grant to Purchase New Ambulance
Sanford Bagley Ambulance is upgrading its services again with the addition of a new ambulance. According to EMS/Support Services Manager Seth Tramm, a grant award of $125,000 has been approved for purchase of the new ambulance.
"The Minnesota Department of Health Rural Hospital Capital Improvement Grant award is a 75-25 matching grant for capital improvements," Tramm said. "We completed the pre-application process and were invited to participate in the second round.

With our final application, we were given the full award of $125,000, which will be coupled with a 25 percent local match.” Since 2012, Sanford Bagley Ambulance call volumes have increased by approximately 40 percent. The increase places more demand on the ambulances, which cover over 1,000 square miles in Clearwater County and it's important to have vehicles that reach their destinations quickly and reliably. "This grant will not only let us purchase a new ambulance now, but will also assist us in setting up a scheduled replacement program for future ambulance needs," Tramm commented. “The new ambulance is currently in design phase, and will transition to build phase in the near future, with an expected arrival date near the first of January 2015.”

Regional Highlights This spring Sanford Bemidji received 3-year accreditation from the American College of Surgeons’ Commission on Cancer (CoC) with commendation and the National Accreditation Program for Breast Centers (NAPBC). A celebration was held in recognition of these honors and the hard work staff put in to receive these accreditations.
Go 100: Over 160 community members registered for Go 100, Sanford’s weekly summer walking program. Participants meet at the Sanford Center every Tuesday, June 10 - through August 19.
The Sanford POWER Program is a strength and conditioning program offered by Sanford Bemidji through Bemidji High School (BHS) for area BHS and Bemidji Middle School students.
Pow Wow: Over 750 attended the annual Healing Powwow co-sponsored by Sanford Bemidji, Red Lake & Cass Lake IHS Hospitals and White Earth Health Center. The Powwow honors the physicians, health care workers and holistic healers who are all dedicated to promoting physical, mental and spiritual health in our communities.
