A new chapter in rural health care begins The opening of OSF Center for Health – Streator is a major milestone for the community, and we at OSF HealthCare are thrilled to share it with all of you. A historic building with deep roots in Streator has been redesigned and repurposed to serve your needs. We believe the future of rural health care will be defined by collaboration between community leaders, community organizations and the health care system. It will focus on preventing illness and sickness as much as possible. Reduced costs, easier access and improved health will result from a health care model that promotes healthy lifestyles and provides you with the guidance, tools and opportunities to take control of your health. The future of rural health care is being built right here in Streator, and you are our partners in this bold endeavor. And as we navigate this new path together, a lot of hopeful eyes are watching our progress. Rural communities and health systems all across the U.S. are curious to know if we can work out the right formula for rural health care that thrives. Embracing a new vision for the future does not mean that we abandon our traditional values. We will rely on faith, family, charity, love, compassion and effort to guide us along the path toward a healthier community. In that same way, we have not abandoned the old hospital where we sought care when we were sick. Instead, we have turned it into the headquarters – the epicenter – for our new approach. The existing foundation is strong enough to build our future upon it. The opening of OSF Center for Health – Streator is a great achievement. It has required a lot of work – a lot of planning, re-evaluating our methods, collaborating and elbow grease. But this isn’t the end of our work. A History of Health Care in Streator July 26, 1887 After an appeal to Springfield, Sisters Crispina, Bonaventura and Methodia of the American Province of Hospital Sisters of St. Francis responded to the Streator community’s request for health care. They began providing nursing care in residents’ homes. The following year, St. Mary’s Hospital was built across the street from the Van Skiver home.
1963 St. Mary’s Hospital expands by opening a new $7 million on-site facility. The addition was designed to handle the modern needs of its patients.
1913 Portions of the building are converted into a residence for the Hospital Sisters.
2000 The hospital celebrates the 125th anniversary of its founding Sisters.

January 4, 2016 After Hospital Sisters Health System opts to close St. Mary’s Hospital, OSF HealthCare acquires the building and turns it into an outpatient center that includes the state’s first rural freestanding emergency center. After collecting community feedback, OSF HealthCare leadership vows to renovate the space into an innovative health and wellness center.
Streator community members rely on OSF Center for Health – Streator for a variety of health services – behavioral health, social services, primary care. The building is a beloved part of the community and provides community organizations a home where they can easily collaborate with other organizations and help make Streator healthier than it has ever been.

New IMPACT Streator website offers resources and strategies to improve community health Residents of Streator, and community partners working to improve health and well-being, have a new, simplified community website to support their efforts. Innovating and Mobilizing Partners and Communities Together — or IMPACT — is a digital platform that provides a centralized place where comprehensive, up-to-date health data can be accessed by anyone. It includes more than 100 health and quality-of-life indicators, mapping and data visualization tools and measures of socio economic needs. The IMPACT Streator website features a summary of the efforts spearheaded by Live Well Streator community members who are focused on three priority areas:

A reduction in opioid usage and overdose
“Our new website equips community partners with the tools to better identify and prioritize the needs of our population by fostering collaboration and communication of priorities. Together, we can implement strategies to maximize and measure the impact we’re having on community health outcomes,” according to Ellen Vogel, Community Health Engagement Program Manager for OSF HealthCare. The new community website includes a dashboard that makes it easy to: • Promote community engagement through tools such as an event calendar, latest news, community polls, and healthy recipes. • Locate resources including grant opportunities to support community initiatives. • Share motivational stories and useful case studies. • Track and evaluate the impact of community efforts on health outcomes.
