SERVICES The monthly rate for tenants in the Hodroff Pavilion for Memory Care includes: • Weekly housekeeping and laundry s ervices, daily bedmaking and trash removal • Individualized personal cares • Escorts, cueing and redirection • Home care services 24/7 • Licensed nursing personnel on-site daily and on-call 24-hours per day • Response to pendant calls • Structured and purposeful memory care activities that fulfill tenants’ physical, mental, spiritual and social needs • Transportation for outings • All utilities

Each potential resident will be assessed by a Registered Nurse prior to admission and assigned a Care Level. There is a one-time initial assessment to determine services needed costing $245.00. Potential residents will be placed according to the highest need assessed. For example, one need in Care Level 3 places the potential resident in that level. Care Levels may be reevaluated and reassigned as care needs change.
