YOUR CASE MANAGER Your Home Care Case Manager will be a nurse or physical therapist. They will work with your doctor, other team members and caregivers to develop a plan of care. Your Case Manager will also evaluate your progress towards your goals and communicate these results with your entire care team.

REFERRALS AND PAYMENT Referrals for services can be made by physicians, social workers, family members, friends, healthcare providers, community agencies, and patients themselves. Medicare, Medicaid, most long-term care plans, private pay and private insurance plans cover many home care services that your physician has ordered.
