BAPO 2015
The BAPO Conference at the Lancashire County Cricket Club in Manchester, saw the launch of a new selection of Ottobock Prosthetic products including the 1C10 Terion, the 3R80 waterproof system and the new range of Skeo Liners. Ken Hurst was invited to host a talk on the Saturday of the BAPO Conference, to explain Ottobock’s innovative Targeted Muscle Reinnervation (TMR) technology and spoke with the UK’s first TMR patient Andrew Garthwaite. Andrew, who lost his arm whilst serving in Afghanistan, explained the practical benefits of his ‘mind-controlled’ arm and told the story of his journey from injury, through rehabilitation and onto his future with this cutting-edge technology. Ken, who has worked alongside Andy since his injury, described Andy’s clinical journey, explaining the long but rewarding process of TMR. To finish off the line up of talks for BAPO, Ken Hurst and Mark Rogers teamed up with Perry Diggins to hold an open forum workshop on Hip Disarticulation, giving attendees the chance to ask clinical questions and find out more about the process.

NEW! 3R62 Pheon (EBS) – Polycentric mechanical knee joint with stance flexion feature – An addition to the 3R60 Family. The 3R62 Pheon polycentric prosthetic knee joint with mechanical swing phase control is especially well suited for prosthesis wearers with low mobility, since it provides the corresponding functionality for swing phase control and includes features well known from our 3R60 knee joint. At the same time, the 3R62 supports the therapy goal of restoring the ability to stand and moderate ability to walk indoors and outdoors. Thanks to the integrated optional lock, which can be activated or deactivated by the prosthetist, the 3R62 Pheon provides targeted support for the post-amputation therapy process – from the initial standing and walking exercises with the interim prosthesis through the final fitting. The Pheon has a 5-Axis polycentric unit that permits the separation into two different modes of operation: swing phase mode and stance phase flexion mode. It has a ergonomically balanced stride (EBS) which is a purely mechanical unit with spring damping element, adjustable up to max. 10◦ stance phase flexion. A Mechanical swing phase control is used in the knee, equipped with a novel assist spring and self-readjusting friction element to control the extension movement for a harmonious extension stop – both mechanisms are easy to adjust. The knee has the latest needle roller bearing technology that is durable, robust and smooth-running and comes with an optional manual lock which provides targeted support for the post-amputation therapy process.
User benefits. Stance flexion feature increases stance stability: • Leads to stress reduction for both extremities – reduction of the forces acting upon the residual limb, pelvis and spine; approximation to a sound, natural gait pattern. Comfortable walking with high safety: • Also on uneven ground and small inclines (walking downstairs alternating is not supported) Design provides greater protection in high-risk situations: • The wearer can always flex the joint in controlled manner without delay or prior full extension, so there is less risk of falling than with locking knee joints.
For more information on the 3R62 Pheon contact your Business Manager or email bockuk@ottobock.com
Reduced risk of stumbling: • Via effective leg length reduction in mid swing due to polycentric design • Novel mechanical swing control leads to damped terminal swing and allows for better control of moderate walking speeds (MG 2). • Patients with long residual limbs benefit from a very small system height at 90◦ flexion angle.

New! Additions to the Skeo Range – Available now! Skeo Pure – The difference is clear. Thanks to its transparent silicone material, the Skeo Pure liner allows for a visual check of the fit and the skin’s appearance, which offers a significant advantage e.g. for interim fittings. Benefits of the Skeo Pure Liner: •
Special outside coating makes the liner easy to don and doff, easy to clean and quick to dry
•
Textured inner surface is gentle on the skin when donning and doffing and when bending the knee
•
Integrated stabilizing matrix creates a more comfortable distal connection by minimising pistoning (longitudinal stretching) without restricting radial stretching around the residual limb
•
Because it dries very quickly, the liner is also ideal for use in waterproof prostheses
•
Easy to cut or trim for better fit

New! Additions to the Skeo Range – Available now! Skeo 3D TT – A new dimension for fit and comfort. The anatomy of the lower leg served as the starting point for the development of the Skeo 3D liner. The result is a pre-flexed liner with varying wall thicknesses. Additional material protects sensitive areas of the residual limb against pressure and impacts, while knee flexion is facilitated by a thinner layer of material in the hollow of the knee. This makes the liner comfortable to wear. The Skeo 3D for the lower leg is also made of particularly soft silicone. Benefits of Skeo 3D transtibial liners: •
Material is thicker in sensitive areas to provide protection and thinner in other areas for greater flexibility
•
Preflexed (20 degrees) at the knee
•
Less material in the hollow of the knee reduces the formation of the wrinkles that can cause uncomfortable pressure points
•
The special textile cover facilitates donning and doffing of the liner

New! Additions to the Skeo Range – Available now! Skeo 3D TF – A new dimension for fit and comfort. With its innovative shape, the Skeo 3D for transfemoral amputees is the brand-new addition to the Skeo range of liners. A transfemoral residual limb typically exhibits an especially large amount of soft tissue. Optimally suited to this anatomical characteristic, the Skeo 3D tapers at the distal end. The tapered shape softly cushions the tissues, rather than constricting and pushing them upwards. Meanwhile, the upper section of the liner is cylindrical in shape, thereby supporting the residual limb from all sides. A drop-shaped textile inlay on the outside of the liner serves as a visual and tactile positioning aid and can be used to attach the KISS Lanyard System. Benefits of Skeo 3D TF: •
Conical shape in the distal area encloses and protects sensitive soft tissues
•
Cylindrical shape provides a better and tighter connection in the proximal area
•
The special textile cover facilitates donning and doffing of the liner
•
Special textile area for simple attachment of the KISS Lanyard Velcro

Genium Prosthesis fits ‘The Bill’. John was an active father of two who enjoyed running, cycling and keeping up with the everyday demands of a young family. Working as a Police Constable, much of his official duties were performed on his police motorbike and in May 2013, he was involved in a road traffic accident. As a result of his injuries, John tragically lost his right leg above the knee. Initially John was supplied with a prosthetic leg from his local NHS centre. He underwent a course of physiotherapy and gait re-education and left the centre using a single walking stick. He found walking tiring especially concentrating on positioning his foot during walking and he tripped and fell on many occasions. John wanted to put the trauma of his accident behind him as quickly as possible and try to get back to his previous activity level but found that he now needed help for even the simplest of tasks, such as getting in and out of the bath at home. On a family holiday, he had managed to swim without a prosthesis but had to be assisted getting to and from the water. As John had been injured whilst performing his duties as a police officer, he was able to make a personal injury claim for compensation, which included receiving treatment from a private prosthetic supplier. Private prosthetic companies are able to provide a broad range of world-leading, bespoke products and services as they are not restricted by the same budget and resource constraints as the NHS. As a leading private supplier in the UK and already well known to John’s Lawyer and Barrister, he was referred to Dorset Orthopaedic initially for an assessment and preparation of an expert witness report, and secondly to start him on a course of prosthetic rehabilitation to bring back his independence. During the private clinic assessment he was questioned about his expectations and long term goals for the future. “I wanted to get back to normal life and longed to return to cycling and possibly running again,” said John. “I was also really keen to get back to work – even if it was just desk duty.”
Following his assessment, Dorset Orthopaedic provided recommendations detailing an immediate treatment plan, to ensure he had the best possible chance of returning to his pre-accident lifestyle and activities. “As part of my claim, I was recommended four prosthetic legs - two for everyday activities with the second of these also being used for water related activities and the other two being dedicated sports prostheses, i.e. for cycling and for running. I was also recommended an extensive rehabilitation programme of amputee-specific physiotherapy, which was designed specifically for me – to help me regain my independence.” John underwent a comprehensive Prosthetic rehabilitation programme at Dorset Orthopaedic in May 2014 and was awarded the four prostheses recommended – including two of the world-most advanced prosthetic legs from Ottobock, known as Genium. He also attended a residential rehabilitation programme while some of the fittings were undertaken and finished by attending an extreme outdoor rehab clinic where he was able to undertake trekking and kayaking with his new prostheses. “The private care I have received has changed my life,” said John. “I have returned to work on a full time basis assisting with office duties and I am back running and cycling socially. I’ve even learnt to ride a Jet Ski using my new Genium X3 prosthesis, something I had always wanted to do but never thought possible!”

Derma ProFlex – Now Available to order in black! Derma ProFlex is an anatomically shaped sealing sleeve made from a hard-wearing copolymer (TPE) with outer textile. The anatomical shape results from a combination of 3 different features: • Pre-flexion of 15° for easier knee flexion and reduced formation of wrinkles in the hollow of the knee • Conical shape for comfortable pressure distribution in the area of the thigh and optimal adhesion to the prosthetic socket • Pre-shaped patella section for reduced pressure on the patella during the entire range of movement The interplay of these 3 factors provides for a previously unmatched level of functionality and wearer comfort for the user. The Derma ProFlex knee sleeve can be used as the primary suspension system or with a valve or the Harmony System. It is suitable for users with a low to high activity level.
