Professional Services Agreement THIS AGREEMENT,madethis Ist day of July 2020, by and between the COUNTY OF ARMSTRONG,a political subdivision of the COMMONWEALTH OF PENNSYLVANIA having its principal offices at Armstrong County Courthouse, Kittanning, Pennsylvania, 16201 (hereafter called the “County”) AND JUSTICEWORKS YOUTHCARE., INC.

Whoseprincipal offices are located at 1500 ARDMOREBLVD;SUITE 410, _ PITTSBURGH, PA 15221 (hereinafter called the “Professional”). WITNESSETH:

WHEREAS, the County requires certain professional services, a specific description of which is contained hereinafter, and desires to retain the Professional to perform such professional

services: and WHEREAS, the Professional is qualified to perform such professional services and

desires to provide such professionalservices for the County: NOW, THEREFORE, in consideration of the foregoing premises and the mutual

obligations and benefits hereinafter contained, the parties hereto covenant and agreeas follows:

1.

The term of this Agreement shall be from July 1, 2020, through and including

June 30, 2021.

2.

The Professional agrees to provide for the County the professional services

described in Appendix B attached hereto at the rate of compensation set forth in Appendix A. The Professional further agrees to provide written reports and court

testimony if requested by the County for any court proceedings relating to any

services performed under this contract. In the event the Pennsylvania Department of Human Services changes its method of reimbursing counties for purchased services during this contract period, the “County” will give thirty (30) days notice to the Professional and the contractwill

be renegotiated, or if unableto arrive at an agreement, terminate the contract. The

ANGER MANAGEMENT$77.25 per hour ANGER MANAGEMENT+ PLUS $84.98 per hour

The anger managementprogram is a cognitive behavioral approach designedto help participants learn effective coping skills and become aware oftheir reactionsin reallife situations. The curriculum consists of participants using a workbook and mini essays which helpthe client better handle anger-provoking situations. ATTENDENCE IMPROVEMENT PROGRAM(AIP) MAGISTRATE REFERRAL $1000 for 4 sessions (3250 per group) for up to 8 families.

AIP is designed as a diversion and educational service for students and families and is a Magistratereferral. The target population is for elementary, middle and high school students that have received three unexcused absences andare in dangerof progressing to habitual truancy. The program is 4 sessions and offered in a group setting, on- site.

CHILD DEATH REVIEW FACILITATOR AND SCRIBE $400 per case Justice Works will provide a facilitator and scribe for child death review meetings. A final team mecting report

will be provided after the conclusion of the meeting. COMMUNITY BASED NURTURING PARENTING $77.25 per hour COMMUNITY BASED NURTURING PARENTING + PLUS $84.98 per hour

Community Based Nurturing Parenting provides a foundation for caretakers. Caretakers are introduced to the parenting constructs and nurturing parenting practices.

Family Finding is a program for children involvedin the child welfare system. Family Finding is a process whereby intensive search efforts are made by JWYCstaffto locate family members of children placed in out-

Theintent of the Independent Living Program is to focus on assessing, supervising, supporting and assisting youth in the developmentof skills that allow for the greater likelihood of future autonomy. NURTURING PARENTING + PLUS $84.98 per hour Nurturing parenting is an evidenced based practice that is family centered initiative designed to build nurturing parenting skills as an alternative to abusive and neglectful parenting and child rearing practices. (NP-SA) Nurturing Parenting for Families in Substance Abuse Treatment and Recovery $77.25 per hour (NP-SA) Nurturing Parenting for Families in Substance Abuse Treatment and Recovery + PLUS $84.98 per hour

Nurturing Parenting for Families in Substance Abuse Treatment and Recovery is to decrease instances ofchild abuse/neglect by recognizing the impact of substance use/abuse on family the family, while strengthening nurturing parenting practices which emphasize safety, permanency, and well-being. RAPID SAFETY TEAM MEETING$1,000 Conference, $1000 for each follow up conference

Rapid safety team meetings are held within 5 daysof the referral. Meetings are specifically for children who are being removed from their home and being placed, a change in placement orto seek a consensus on a placement that protects the child and preserves or reunifies the family. Coordinators will gather family, kin and community supports quickly, directly following an emergent event that will likely result in an out of home placementfor the child.

Seeking Safety is an evidence-based,present-focused counseling modelto help peopleattain safety from trauma and/or substance abuse.It can be conducted in a group format (any size) and/oron anindividualbasis. The modeldirectly addresses both traumaandaddiction without requiring individuals to delve into a full trauma narrative (the detailed account ofdisturbing trauma memories). STAND ALONE DRUGTESTING $89 flat rate, No Shows $30.00 flat rate

JusticeWorks YouthCarewill provide the unable and unwilling client an opportunity to provide urine samples to prove their sobriety. This program provides the ability to go into the community, collect a sample from the client, and test the sample in the home. STOPP® $82.50 per hour

STOPP® + PLUS $90.75 per hour

A 60 day, high impact, 24/7 crisis and stabilization program; proven to keep children safe and at home despite profound dysfunction. Staff is in the homeof a new referral within 3 hours and implements a comprehensive treatment goalplan thatassures child safety, reduces out of home placements, and improves family functioning. SUPERVISED VISITS $61.80 per hour SUPERVISED VISIT + PLUS $67.98 per hour Supervised Visitation allows parents who maybe at risk to their children or to another parent to experience parent-child contact while in the presence of an appropriate third part. The goal of this supervisor is to work

towards building healthy boundaries and strong parenting skills. This service can be in the form of one-on-one

supervision with a family, multi-family units, telephone monitoring, or child exchanges (including transportation). + PLUS SERVICE Drugtesting added to a service is a 10% increase to regular price per hour. + PLUS SERVICESprices are listed under regular service price

Most ofour offered services can be combined with our + Plus Service. This is a 12 panel drugtest, testing up to two family members twice a month randomly. A urine specimenis collected andif the specimenresults in a presumptive positive the sample will be sent for lab testing. Lab results will be provided to the County showing results and levels from a presumptive positive. VIP (VIOLATION INITIATIVE PROGRAM)® $82.50 per hour VIP (VIOLATION INITIATIVE PROGRAM)® + PLUS $90.75 per hour

VIP® utilizes STOPP® stabilization methodology for chronic non-compliant juvenile delinquents whoare at high risk of being detained and placed for violating probation, family or community standards. VIP® staff meets with the youth in the community, coordinate with school officials, and can-do curfew checks. VIP® follows the BARJ competencies andoffers a flexible range for high to moderate risk cases. (T4C) THINKING FOR A CHANGE(T4C) $360.50 group session (Max of 10 youth per group) $74.16 per hour for individual missed appointments —*Travel not included in rate

T4C encouragesparticipants to recognize the connections between thinking errors and anti-social behavior traits. The curriculum introduces cognitive self-change, pro-social skill development, and a formal problemsolving framework using both cognitive and behavioral elements of skill exploration. This strong commitment to the CBT framework improves outcomesfor those involved.

(TSP) TRANSPORTATION SERVICE PROGRAM (TRP) 832.00 per hour plus IRS mileage reimbursement rate per mile

The Transportation Services Program (TSP) will provide linkage to community resources for children and/or parents that are involved with the Armstrong County. Our goals are to provide cost-effective, safe transport services, TRUANCY REMEDIATION $77.25 per hour TRUANCY REMEDIATION + PLUS $84.98 per pour

WhyTryis an internationally recognized, research-based intervention model targeting risk factors and issues related to drug and alcohol use, school violence and aggression, truancy, anti-bullying and dropout prevention. The emphasis of this curriculum is to build resiliency in youth.

TRUANCY DIVERSION $77.25 per hour individual TRUANCY DIVERSION + PLUS $84.98 per hour individual TRUANCY DIVERSION GROUP$231.75 per group session WhyTry is an internationally recognized, research-based intervention model targeting risk factors and issues related

VISIT COACHING $66.95 per hour VISIT COACHING + PLUS $73.65 per hour A visitation service using a “coaching” system based on the principles of empowerment, empathy, responsiveness, and active listening. Coached visits allow parents who maybe at risk to their children orto

anotherparent, to experience parent-child contact while in the presenceof an appropriate 3“ party. VISITATION $135 per visit for cases within 65 mile radius of county seat. Cases 66 miles and beyond $100

per visit, $30 per hour travel time + travel allowance to equal rate established by the IRS, not to be below 51 cents.

Conduct monthly visits to assess placementsafety for settings such as foster care, group home,residential placement, kinship, etc. All safety assessment forms will be completed as indicated by CYS. Billable Performance Guide

Direct Contact:

1.

Billing includes face to face, actual client contacts by phone, accompanyingclients to appointments, attending court, specific case functions as indicated by CYS/JPO, or other collateral contacts.

2.

Maybill for up to 2 failed consecutive contacts without approval by CYS/JPO to

continue. No more than 30 min at a time.

3.

Direct contact time includes meetings with the client, family member, worker, another community provider, supervised visits, observations, participation in a Family Group

Decision Making or similar service (All staffin attendanceis billable separately). 4.

Aspart of the engagementprocess, families will have an opportunity to take part in the

developmentof progress notes. Family’s that help in note documentation will be clearly identified in the note that they participated in writing the specific entry.

Indirect Contact — Indirect contact is defined as time used to write case notes, preparing monthly reports, attending court AT the request of the family, supervision, driving to and from the client’s residence orlocation,

and attending CYS/JPO reviews. Drug and Alcohol Screening - Can be addedto any active service for 10%.

Twotests per month will be provided for the identified client and/or one household member. i.

Initial screening will be performed with the client. If results present

with a presumptive positive, the specimen will be sent for further lab analysis. ii.

b.

Reports reflecting lab results will be sent to the county.

Each additional individual tested will be charged at cost. If testing is requested for more than two individuals within the household the county will be billed at cost for testing supplies (e.g. panel test, collection device) and lab requisition testing.

c.

Testing can be performed for clients more than twice per month. The county will be billed for the testing supplies (e.g. panel test. collection device) and lab requisition testing ifinitial results result in a presumptive positive.

Postage: Postage will be reimbursed by the county for FGDM, FF, FCAP, IL and for all SWAN related services.

Discharge Planning: Notification of case closure must provide a minimum of 7 days for JWYC to appropriately close a case and set upall aftercare supports.

Collateral/Client Service Report: Collateral contactis billable in the form of building resource contacts for a family or youth.

“Step Down”in Care: JY WC requires 7 days notice to prepare for case step downor upto adjustto the case needs and ensure safety and well-being ofall parties involved. This time allows JW to notify other caregivers of the modification and to make their own personal adjustments to support the family and youth. Phone Calls:

1. Phonecalls to clients are billable. 2. Phonecalls are billable as related to a crisis situation.

3. Phonecalls are billable when contacting a case worker, or other community provider. Court: All court ordered time is billable.

Travel: Travel is reimbursable to and from locations, as defined by the countyor state, and at the IRS rate and guidelines identified by this contract service guide.

Authorization of Services: If it is agreed that services are approved to be completed- via email, letters, fax — county must honorthis service request by this agent of the county.

Expiration of Services: JWYC will provide at least 10 days notice prior to expiration of authorization. Invoicing: JWYC requires an itemizedlisting ofall invoices to accompany payment. Please notify the program director of any adjustments madeto the invoice.
