Client: Asian Youth Council (AYC)

The Asian Youth Council (AYC) is a non-governmental, youth-serving regional organisation to foster unity among Asian youth and to support national youth organisations in the area. AYC act as the administrator to manage and run the project to youth participation in urban setting.

Ministry of Health, Hospital Rehabilitasi Cheras, Malaysian Mental Health Association and Malaysian Psychiatric Assiociation act as the collaborators which professionals collaborate to provide seamless care across care boundaries. This allows the continuity with a primary care professional with effective management of the transition of care from hospital to community based seting.
