In a “Falls Prevention Briefing in Washington, D.C., American Physical Therapy Association member and National Falls Free Coalition representative Bonita Lynn Beattie, PT, MPT, MHA, said that “falls and falls-related injuries are not normal consequences of getting old; there are evidence-based interventions that can help reduce older adults’ risk of falling and can affect the rate of fallsand falls-related injuries and death.” Studies show that people can keep their balance skills sharp as they age – even regain skills – through balance training exercises. The AARP website lists simple exercises one can do at home to help maintain and improve balance – such as practicing walking heel to toe, standing on one leg, or simply standing up from a sitting position and sitting back down while maintaining

proper posture. To up the challenge, you can try doing these while balancing a paper plate on your head. Falls are a leading cause of injury and death among people 65 and older. About one in three seniors above age 65, and nearly one in two seniors over age 80, will fall at least once this year, with serious consequences, according to APTA President R Scott Ward, PT, Ph.D. “Physical therapists are taking an active role in educating consumers about how to safely prevent falls as well as treat those who have fallen and suffered injuries as a result. This education will ultimately improve the health and quality of our nation’s seniors,” he said. Susan G. Komen fought breast cancer with her heart, body and soul. Throughout her diagnosis, treatments, and endless days in the hospital, she spent her time thinking of ways to make life better for other women battling breast cancer instead of worrying about her own situation. Moved by Susan’s compassion for others and commitment to making a difference, Nancy G. Brinker promised her sister that she would do everything in her power to end breast cancer. Though Susan lost her battle with the disease, her legacy lives on through the work of Susan G. Komen for the CureŽ, the organization Nancy started in her honor. Komen for the Cure is the global leader of the breast cancer movement, having invested more than $1 billion since its inception in 1982. Each year, the Siouxland Affiliate has been able to increase the amount of grants and number of programs funded. St. Luke’s Telehealth program targets health issues early With three heart surgeries and a heart attack to his name, William Dufault knows the importance of keeping a close eye on his health every day. That’s why each morning, he has a registered nurse from St. Luke’s Home Care check his vital signs. The 85-year-old Sioux Cityan does so from the comfort of his own apartment. Using a Telehealth monitoring system, he steps on a scale, straps on a blood pressure cuff and slips on a pulse oximeter. Using his phone line, the system sends his weight, blood pressure, pulse and oxygen level to St. Luke’s Home Health, where a registered nurse reads the results. Any readings outside of the patient-specific, physician-set parameters bring a phone call from the registered nurse. With targeted questions, the nurse can further assess Dufault’s health and determine if additional

measures – such as a second blood pressure check, a visit from a St. Luke’s Home Care nurse or a trip to his doctor – are needed, says Candice Curry, R.N., Telehealth case manager at St. Luke’s Home Care. Dufault says he appreciates the reassurance Telehealth and St. Luke’s Home Care’s staff provides him through the monitoring system. Knowing his vital signs are where they should be allows him to focus on more important things, like calling bingo in his apartment complex’s community room or planning a fishing trip with his son. “This service does give a lot of reassurance to our patients as it helps them monitor their symptoms and hopefully prevent any further hospitalizations or emergencies,” Curry says. “As nurses, we’d love to be able to be out there with our patients 24 hours a day, seven days a week, but that’s not possible.

Telehealth is an extra set of eyes and ears that allows us to take better care of our patients.” A major benefit of having vital signs reviewed daily by health professionals is the ability to spot minor issues before they become major ones. “With daily monitoring, we may be able to spot a problem our patients may not even know they are experiencing,” says Curry.” “For example, they may not be having problems at the moment, but we notice that their blood pressure has risen over a two-week time period. While it’s still in normal limits, we may contact the patient and his or her doctor to find out why that’s happening.” Along with the checking vital signs, the Telehealth system can transmit blood glucose information for patients with diabetes. It can also be personalized to provide verbal messages to the patient, such as, “Remember

to take your medication at 10 a.m.” or “Remember your doctor’s appointment today.” The Telehealth system also provides educational opportunities, Curry says. “If we call someone because she gains three pounds overnight and she tells us, ‘well, my kids brought over fast food yesterday.’ This is a perfect teaching opportunity for us, a chance to tell the patient, ‘remember when we talked about high sodium foods? Now you see what they can actually do to your weight.’ This system provides live feedback on health issues they may not understand.” St. Luke’s Home Care provides Telehealth services free of charge to existing patients. Ophthalmologist Dr. Beth Bruening answers questions Dr. Beth Bruening, an ophthalmologist with Bruening Eye Specialists in Dakota Dunes, answers 10 Questions on Eyes as an Ophthalmologist. 1. What are the most common eye problems after age 40? Bruening: With increasing age, the chances of developing an eye disease also increase. Over age 40, one of the common changes is presbyopia, a condition in which a person is no longer able to read well without glasses. The eyes won’t focus. People get worried, but it’s actually quite normal, and can be treated with reading glasses. In the 50s, common eye changes include cataract formation, glaucoma, and the start of macular degeneration. Every decade that a person lives, the chance of getting these diseases increases. 2: What about cataracts today? Bruening: There are probably 20 million or so people living with cataracts currently. By 2020, about 30 million aging Baby Boomers will have cataracts. Cataract formation is a normal part of aging. Some people show signs of beginning cataracts around age 50. It depends on different factors. 3. How has treatment for cataracts advanced? Bruening: Over the last 20 years or so, we have seen advancements. Cataract surgery still requires manual removal of the cataract

lens with phacoemulsification, but companies are making multifocal lens implants that fold, so we can make smaller incisions when putting the intraocular lens inside the eye. We’re fortunate that current technology is absolutely wonderful. Cataract surgery can be performed in 10 minutes or less. We don’t have to put a patient under anesthesia. We can perform cataract surgery with no stitches and no pain, and patients have good vision the same day without glasses. There are new procedures for astigmatism that use lens implants, incisions or laser treatment. Under development is a procedure using lasers to remove cataracts. The downside is cost. next line of treatment is performing a laser procedure in the office. It’s directed at a portion inside the eye. It’s successful two out of three times for lowering the pressure in the eye. Occasionally, the patient may be able to get off drops. If laser treatments don’t work, surgery may be performed to make an opening from the inside to the outside of the eye. It creates a trap door for fluid to be relieved, drain away and lower the pressure. New medicines are being developed and researched. Surgery is the last resort. 5: Can a person do anything to avoid glaucoma? Does it run in families? Bruening: No, a person can’t do anything to avoid getting glaucoma. If someone in your family has it, your chances of getting it do increase. No special diet or vitamins can be taken to prevent getting glaucoma. 6. What is macular degeneration? Bruening: Macular degeneration is an aging change that affects only the central part of the retina used for driving or reading vision. It does not affect the peripheral vision. A person who has macular degeneration can look at another person’s face but they can’t see the details or features. They can’t see the shape of a nose, but they can see everything around that. At worse, macular degeneration never causes complete vision loss where a person’s vision goes black. It

doesn’t necessarily get to the point where people can’t read or drive. On a continuum of 1-10, only about 10 percent of people get to 10, the end stage where the central vision is severely affected, but it’s not going to get any worse. That’s reassuring. There are two types of macular degeneration: wet and dry. With wet macular degeneration there is bleeding or swelling. With dry macular degeneration there is no bleeding or swelling. New treatments for wet macular degeneration allow us to give injections inside the eye to maintain a person’s central vision. Left untreated, wet macular degeneration can cause permanent scarring. We use ongoing injections with medicine to keep it under control and keep patients from losing their vision. With dry macular degeneration, there are no medicines to stop it. We encourage patients to make lifestyle changes: no smoking, eat a healthy diet with fruits and vegetables and fish, maintain a healthy weight, exercise, and take an AREDS formulated multivitamin. In studies, patients with dry macular degeneration who were given a vitamin preparation were 25 percent less likely to get worse. Adding lutein and xanthine are more beneficial. I recommend vitamins for both

Marilyn Broveak listened to her doctor when she was told it was unwise to recuperate at home following surgery on her shoulder. “In fact,� she said, “they wouldn’t let me go home.� Broveak fell outside her home on May 6 and landed on her right shoulder. While she was waiting for surgery, Broveak was a resident at Regency Square Care Center from May 10 to June 1. “The doctors told me they didn’t have enough parts to fix my shoulder,� she said with a poker face. “I had the surgery in Omaha on June 2 and came back to Regency on June 6.
