In the Spotlight

The Implants SBU: Passion, partnership, professionalism The Implants SBU has been a premium manufacturer in implant dentistry for over 40 years. Over 2,200 employees develop, produce and support first-class solutions backed by scientific evidence for implant dentistry clinics and professionals worldwide. The Implants SBU is about more than simply selling implants. Rather, it’s about asking how it can help dentists treat their patients in the best way possible and to improve quality of life. Its employees have a passion for implant dentistry with a profound commitment to science, documentation, education and innovation as well as pride in developing comprehensive solutions. Together with dental professionals, they make the difference for patients every day. More than 2,000 scientific studies are the foundation of the products and solution portfolio that includes Ankylos®, Xive® and Astra Tech Implant System® EV, three of the most well documented premium implant systems in the market today. The high quality implant systems are developed and produced in Mölndal (Sweden), and Hanau (Germany). There are also implant-related production sites in Waltham (USA), Tokyo (Japan) and Hasselt (Belgium).

The digital workflow

The digital implant workflows by Dentsply Sirona show how digital technology can bring significant added value to dental professionals and practices. The combination of first class products and their seamless interaction make it an easy decision for clinicians who don’t have to worry about material specifications, compatibilities or interfaces as all components perfectly match one another.

Innovations in the product portfolio

The SBU recently launched its latest innovation, developed in cooperation with one of the world’s most acknowledged implant dentistry professionals, Dr. Marco Degidi. AcurisTM represents the best of two worlds: a conometric retention that is removable for the dentist but fixed for the patient. At the “International Congress on Ankylos®” in Berlin in June 2018, dental professionals got a sneak peek at this new implant solution by trying it out on a model and discussing it with experts. This created a lot of interest and buzz about the new product. The official global launch took place at the annual scientific congress EAO, in Vienna, in October 2018.

To show customers how the Implants SBU’s products and solutions can help them to achieve excellent results for their patients, an extensive clinical education program is offered. Great examples are the company-hosted customer events, such as the World Summit Tour and the Ankylos® Congress, with scientific lectures, live demonstrations and hands-on workshops. Continuous clinical training and education make a difference, nurturing the relationship with customers and differentiating Dentsply Sirona from its competitors. For example, at the dental implant treatment facility in Mölndal, the ability to witness live surgeries is a unique learning

opportunity for visiting guests. In addition, guests are offered the possibility to tour the state-of-the-art Atlantis® production, where patient-specific abutments are designed and manufactured.

Solutions to satisfy rising expectations

The Implants SBU aims to fulfill the different needs and requirements of all parties involved in the various facets of implant dentistry - referred to as “connecting the dots.” Patients are becoming more informed and approaching practices with corresponding expectations. Digital technologies offer an increasing number of possibilities but not all solutions are required for every indication. It is therefore a question of intelligent, logical concepts that simply function and improve the predictability of treatments. This includes not only implants themselves, but also regenerative products, like bone graft materials, resorbable and non-resorbable membranes as well as corresponding instruments. Furthermore, this includes restorative solutions such as patient-specific abutments, crowns and bridges which can be ordered worldwide via Atlantis® from the Implants SBU.

Interview

Driving Innovation Forward Dr. Markus Vollmann,

Director of Research and Development at Dentsply Sirona Lab As the Director of Research and Development at Dentsply Sirona Lab, Markus plays a central role in terms of our innovation pipeline. He is the person who drives new product developments, for example, innovative materials such as Celtra® Press. Thanks to his expertise, great communication skills and inventive thinking, Markus understands our customers’ needs and turns them into new and smart product ideas. His work philosophy reflects Dentsply Sirona’s unique positioning by always laying out the whole picture of the workflow. He is working closely together with other Dentsply Sirona business units to generate valuable links to related workflows. This means that you can benefit from thought out end-to-end solutions, and subsequently benefit from tangible improvements in your daily work. In this interview, Markus explains the various facets of his work as well as the secret behind real innovation. Q. Tell us a little about your role as Director of Research & Development? What are some of your daily endeavors and/or challenges? A. A typical day for me is full of reviewing the statuses of all running product development projects, anticipating and identifying what obstacles or surprises (sometimes positive, sometimes negative) there are or might be, and how we can manage all of these things in order to either meet existing timelines and deadlines or to be able to start new projects which are fitting in the overall portfolio of the company. For the development team, one needs an open ear to absorb every single surrounding detail, which includes being completely receptive and having a comprehensive, up-to-date understanding of our production, quality assurance, product management, and even logistic

9

teams. In R&D you need constant curiosity, great communication skills and inventive thinking. Q. How does science, clinical studies, and research all fit under the Dentsply Sirona Lab name? Can you provide a little background on what goes into testing a product before it actually goes into development, and ultimately goes to market? A. Due to the fact we are working in the medical devices market, we fulfill a great number of demands for testing and design control processes. For example, when developing a new material for dentistry we need to look at the biocompatibility as well as risk management starting from the production process, to the user, all the way to the end result — our patients. Besides proving and surpassing all requirements from applicable standards, we also want to know how our customers accept the workflow of the product. Before we launch, we take a close look at how it fits into the actual workflow of real-life dental laboratories, and we then start additional vigorous clinical testing after the launch as well. We include our customers quite early on in this process to allow us to react to their outcomes and feedback, and then be able to improve the product within the development timeline. What makes Dentsply Sirona unique is that the entire workflow is shown, and it can be linked to other workflows and combined into an integrated solution. When it comes to the prosthetic treatment of an implant using an abutment and full ceramic crown, for example, Dentsply Sirona Lab is the right partner for laboratories. But the treatment workflow as a whole starts from the earlier stage of diagnostics and the implantological treatment, and ends with the restoration finally being cemented or screwed in place. For this purpose, Dentsply Sirona and its Imaging, Implants, and Restorative business create integrated workflows for

both dentists and dental laboratories. Q. One of your roles within Dentsply Sirona is to constantly provide material innovations that expand lab offerings to their dentist clients, while improving their workflow. How do you gather the information needed to improve upon these offerings? A. We use groups of our core customers, labs and dentists. Sometimes the most effective feedback comes from our labs and dentists who are everyday partners because they know their ideal workflow routine, and are able to communicate their emerging concerns and ongoing daily challenges. Q. Can you tell us a little about Celtra® Press, the newest material for laboratories? What was the industry missing (doctor, labs, and patients) that this material now offers? A. Celtra® Press has significantly improved the workflow in the lab by being easier to press (with excellent flowability) and excluding the time-consuming and dangerous use of hydrofluoric acid to get rid of the reaction layer. Despite this, it is stronger than other pressable high-strength glass-ceramic materials on the market. Therefore, Celtra® Press provides a simpler workflow in the lab, the dentist receives a robust material with a very good fit and easy polishability, and the patient absolutely loves the natural looking aesthetics and beauty of his or her new teeth. Q. What do you foresee Dentsply Sirona offering in the near or distant future as far as material advancements? A. We will soon present a new generation of CAD/CAM material, hand in hand with the lab material combining Celtra® Ceram porcelain that is suitable for every allceramic case. We are also planning further material improvements on other material sectors coming very soon!

Practice Management

How to build trust in your dental patients Dental patients repeatedly say: “I trust my dentist. They say this because most dental patients aren’t professionals and they cannot truly assess your work. They must trust their feelings. Patient feelings are particularly important when they consult you for the first time. Potential patients may initially rely on recommendations from friends and family, but it is the impression you make that earns their trust.

So, how do you make dental patients trust your practice?

1

Express yourself

Your page about the team and your dental practice is the information that visitors to your website are most interested in. Dental patients want to see who they will be dealing with. Therefore, invest a lot of energy into presenting yourself, your team and your medical practice in a very personal way. Have you ever considered sharing information on your hobbies or interests? This information helps patients relate to you. Of course, you must feel at ease disclosing such information.

2

Show authenticity

Do not present anything online you do not feel comfortable sharing. If dental patients discover you misrepresented yourself on your website or in your information flyer, you are unlikely to earn their trust and new business. People want and value authenticity. So, represent yourself as you are. Showcase your personal character. Your patients will recognise this honesty and you will be rewarded with their trust.

11

3 Ensure a pleasant and classy first impression

Ultimately, your patients come to you to receive good dental treatment. However, the impression they receive when they enter your practice also plays an important role. This is especially true when they recount their experience in their own head as well as to others. So, do not underestimate the importance of this experience, online and in real life. Most dental patients are impressed by modern furnishings and technical equipment in a practice. It makes them feel they are receiving state-of-the-art care. Pictures of a modern practice also make a good impression on your website.

4

Have the right online presence

Today, many dental patients search for dentists online. Therefore, you should ensure you are present online where they are, for example, on social media. It is not enough that your picture and your offer can be found on your website. Our tip: Plan your social media postings and offer your followers a good mixture of inside-the-practice information— e.g., summer parties, new staff, birthdays, interesting situations from everyday practice life— as well as medical results, information on treatment methods and offers, and appealing things you found elsewhere (ideally relating to dentistry). It is important to choose someone from your practice to be responsible for these social media postings. And do not forget to fill portals, search engines and other lists relating to dentistry with your offer and, preferably, with pictures of yourself, your team, and your practice.

5

Seek recommendations

Most dental patients rely on recommendations from family and friends when it comes to choosing a dentist, but they also trust a good recommendation. Satisfied patient referrals represent the biggest potential for building your practice. Talk to your patients and encourage them to recommend your practice actively. Consider a “bring-a-friend” promotion, which provides additional motivation. But please check if such incentives are legally acceptable in your country first.

your online 6 Manage reputation Whether we like it or not, online reviews are steadily becoming more important. In nearly every country, there are medical professional review sites. It is in your best interest that you keep track of your reviews on these pages. Likewise, actively promote your position with your patients. Talk to your satisfied customers and ask them to leave a positive review of your services on Google and other services.

7

Be yourself Our best advice: when representing yourself, always be yourself. No one does it better than you.

Moreover, don’t leave your external presentation to chance. Patients want to build a relationship and get an idea of who you are. It is your task to actively steer their perception and positively influence it. You should seize this opportunity.

New study lets children chat and play away their dental anxiety Researchers at Karolinska Institutet in Stockholm, Sweden have developed an accessible therapy for children and adolescents suffering from dental anxiety. The study, published in the Journal of Medical Internet Research, shows that a playful exercise package in combination with a guided Internet-based CBT was highly effective in reducing anxiety and increasing the ability to deal with dental treatment. One year later, half the children were completely free of their phobia. Dental anxiety, also called odontophobia, dentophobia or dental phobia, is a common fear in most Western cultures. It is estimated that as many as 75% of US adults experience some degree of dental fear, from mild to severe. Dental anxiety often begins in childhood or adolescence, and can develop into a phobia with avoidance, strong negative feelings and thoughts aimed at dental care. Avoidance of dental care often leads to poor oral health, untreated caries or other dental problems. That’s why it is vital to start treating this anxiety or fear as early as possible. Cognitive behavioral therapy (CBT) is an effective treatment for most forms of specific phobia. However, with regard to children and adolescents with dental phobia, there are organisational, financial and geographic obstacles to providing such therapy. Researchers in this study have therefore developed an Internet-based CBT treatment that they have tested in an open, uncontrolled study of 18 patients between the ages of 8 and 15.

Alleviate dental anxiety – play dentist at home During the study, participants used an Internet platform to obtain weekly online guidance from a psychologist via a chat system. Treatment continued for 12 weeks and included texts, animations and dental-related video clips. The treatment also included an exercise package with a dental mirror, probe, local anesthetic and cannula delivered to the home of the child/parent(s) with detailed instructions for the exercises. Through therapy and guidance from the psychologist, the home-based exercises were linked to real exposure and training visits to dental clinics around Sweden.

53% were free from their dental phobia The results show a statistically and clinically significant increase in the children’s ability to handle dental treatment. The Internet-based CBT also increased children’s and parents’ self-efficacy, led to fewer negative thoughts, and reduced anxiety about dental treatment. 15 children attended the 1-year follow up visit, where 53% were found to be completely free from their dental phobia.

Q. Please briefly explain how long you have been working at your clinic and your background in dentistry. A. I’ve worked as a dentist for 15 years now. I joined Dr. Rizk Girgis’ clinic as a general dentist in 2005, before travelling to Saudi Arabia in 2011 to join Andalusia Dental Group in Jeddah where I started working as a prosthodontist. Q. Please tell us about your specialisms within your practice? A. During my early years I worked as general dentist. Now I focus solely on prosthodontic procedures. Q. What triggered your initial interest in digital dentistry? A. The digital impression and CAD/CAM systems.

Q. How long have you been working with Dentsply Sirona products? A. More than 10 years now. Q. When did you invest in your first piece of Dentsply Sirona equipment, and what was it? A. The first pieces of equipment that we bought was a Cone Beam SL, Omnicam and MCXL milling machine. Q. Your practice uses Dentsply Sirona products and equipment predominantly. What would you say is the advantage to your workflow and your practice by making this decision? A. Ultimately it means that the end product that I am delivering to my patients becomes more accurate, faster and safer. I trust the technology and the quality of the equipment. Q. How do you find the CEREC workflow? From scanning, designing, to grinding/ milling and sintering/finalsation. A. Put simply, it’s more than perfect. Q. Why did you choose CEREC over other CAD/CAM systems? A. CEREC is the most accurate chairside CAD/CAM system I’ve encountered. The software is easy to use and versatile, and the speed of digital impressions with the Omnicam is fantastic. Q. Can you explain your experience with the Dentsply Sirona sales process, from initial interest through to purchase and aftersales support? A. The service from all at Dentsply Sirona has

been excellent throughout each stage. Q. What are the types of cases you do with CEREC? E.g. restoration, implantology, orthodontics. A. Primarily we are focusing on crowns, endo crowns, veneers and implants – for now! Q. How has CEREC, and single visit dentistry, affected the satisfaction of your patients? A. Single visit dentistry has had a significant effect on my patients satisfaction. It gives them their time back in the week, and it also helps me to have more time for additional patients. Q. How has CEREC and digital dentistry impacted your business? A. Most importantly I save my time. I stop using impression material and I save on lab costs. Again this gives me more time to see more patients and have quality time with them and their procedures.

accuracy, and moving chairside to avoid lab costs and errors. Q. What do you think your biggest achievement was in your career to this point and how did Dentsply Sirona help to make that a reality? A. When I was child I loved drawing, and then in my faculty I loved the carving. My biggest achievement now is that I can draw and carve up to the finest of details for my cases very easily. In the past, I had to go to the lab and carve the wax or build the porcelain which for me took too much time and effort, but now my dreams have come true and I can draw and carve my crowns very easily without losing my time. I would like to say thanks to Dentsply Sirona for letting me achieve this.

CEREC is the most accurate chairside CAD/CAM system I’ve encountered.

Q. In your opinion, can you achieve a high return on investment with CEREC and digital dentistry? A. It can certainly be achieved but you must have a turnover of patients using the equipment, which is more often than not. Q. What would you say was your goal with CEREC, and would you say you have achieved this yet? A. I can definitely say that I’ve achieved my initial goals of saving time, increasing

Dr. Ayad Mouayad Al-Obaidi BDS, MSc Dr. Ayad Mouayad AlObaidi is an Assistant

Case Report

Lecturer in Conservative Dentistry, at Baghdad University, Iraq. He graduated in 2004 from the University of Baghdad from where he then received his MSc in 2011. Between 2004 and 2015 he attended a number of refresher courses and developed skills in aesthetic adhesive

A 28-year-old female patient had the primary complaint of spacing in the upper front tooth region. The patient’s medical history did not reveal any systemic diseases and an intraoral examination revealed presence of midline spacing between maxillary central incisors (~2mm) (Figure 1 and 2). For a more conservative, economical, aesthetic, and quicker option, a direct diastema closure was considered.

15 seconds on the mesial surface to be restored, then rinsed for 20 seconds (Figure 5), and dried with air gently (Figure 6). Then a universal adhesive system was used (Prime&Bond universal™), using a micro brush with a light scrubbing motion for 15 seconds (Figure 7). This was then gently air dried for approximately 5 seconds. Blow to margin, or to thin if necessary, using a light application of air and then light cure for 20 seconds. The key for papilla regeneration is to provide aggressive cervical curvature that starts subgingivally and this can be done with a mylar strip placed subgingivally with a high viscosity flowable composite or bulk flow composite injected to the contact point area as reference (emergence profile) (Figure 8). The key to success in diastema cases is to finish the first tooth completely before starting the second tooth (Figure

One of the main challenges clinicians face during anterior diastema closure is preventing or eliminating a black triangle between the teeth.

9). Then some corrections can be made to the size of the first tooth so the final size of the centrals will be the same at the end of treatment (Figure 10). In this case the proximal wall was completed with a single shade universal composite (ceram.x® SpereTEC™ one universal) and then began the emergence profile to complete the proximal wall of the second tooth (Figure 11). The teeth were then finished with polishing discs and rubber points. The rubber dam was then removed and as you can see there is a small black triangle in the cervical area (Figure 12). The recall visits in diastema cases are very important to see patient satisfaction, to check the periodontal health and to do some polishing and texture (Figure 13 and 14). With time, the interdental papilla filled the space of the black triangle completely (Figure 15 and 16).
