I. PROJECT DESCRIPTION, RATONALE AND BACKGROUND

The proposed (Project Title) production mainly focuses on the production of _______ through ________. project would alleviate the poverty condition of the beneficiaries as it augments their income while being productive. II. OBJECTIVES

General:  To promote sustainable farming system and provide livelihood projects to farm families in the Barangay.  To create rural community awareness in organic farming practices introducing its benefit and help, rehabilitate, conserve and protect the natural resources for sustainable agriculture.  To achieve (specify what production) production sustainability and provide healthy agricultural food products to the community through proper organic agriculture practices.  To promote economically sound and sustainable (specifify what production) production in the community.  To generate employment and provide additional income to rural farm family through _________ project.  Decrease poverty incidence by ______% (if implemented succesfully and is sustained rightly) by (year). The proposed project will be implemented on a counterpart scheme with the LGU. It would involve the collaboration of DA-RFO10, LGU-(Municipality/City) and the beneficiaries. A steering committee (LPRAT) will be created responsible for the selection and validation of target beneficiaries to oversee all activities during and after the implementation. Orientation, seminar and trainings will be conducted by DA-LGU and other line agencies. Briefing and orientation will be conducted prior to the implementation of the project. The beneficiaries will identify a focal person (Agriculture Technician) to manage the implementation of the project. He will be assited by the Community Mobilizers. A memorandum of Agreement will be made between Association, LGU barangay and LGU(municipality/city), while a separate Memorandum of agreement will be executed between the DA, LGU-(municipality/city), and other line agencies if necessary. SUSTAINABILITY

In partnership with the local Civil Society Organization present in the area, the Municipal/City Agriculture’s Office (MAO/CAO) will have to design schemes and policies with regards to the project being accessed thru the Department of Agriculture, to ensure sustainability of the project. 1. A contract between the LGU and the recipients shall be enacted as guiding

principles. 2. Creation of MANCOM composed of PO members and LPRAT members to ensure

proper management of the project. 3. The PO beneficiaries shall maintain a record on income and expenses of the project. 4. Regular monitoring, assessment and planning with the LGU shall be conducted.
