Welcome By Trust Chief Executive Julia Bridgewater WELCOME to this Hospital Guide for NHS Professionals. Here you will find useful information for yourselves, your patients and their visitors and your colleagues working in the NHS. The guide has been designed to be portable so it can be used as an important point of reference. Our new hospital is an important piece in the jigsaw of achieving the highest quality of care for our patients. Our strategic priorities are to: • deliver safe, appropriate and effective patient care • provide efficiency driven by innovation, teaching, research and education • be efficient and financially stable • build a positive reputation and play a key role in the wider community.

Site map Car Parking City General hospital currently has 1,500 car parking spaces for patients, relatives and staff across its three sites. Once the landscaping is complete in 2014, University Hospital of North Staffordshire will have 2,150 car parking spaces.

Entrance 2 From the A34, next to the Undergraduate Medical School and Clinical Education Centre (shown below) This is the main route for ambulances coming to Accident and Emergency from the A34. It also provides access for vehicles visiting Sterile Services and the Catering Department.

It is also the best entrance for: • Visitors to the Undergraduate Medical School and Clinical Education Centre • Patients and visitors attending the Lower Floor of the Lyme Building

Internal signposting Signs have been carefully designed to make it easy for patients, visitors and staff to find their way around. The hospital is built on six floors, encompassing Lower Ground Floor 2, Lower Ground Floor 1, Ground Floor, Floor 1, Floor 2 and Floor 3.

Patients and visitors entering the hospital via the Main Entrance on the Ground Floorwill be directed to the department they require using specific lifts. The wards, which are located to the rear of the building, if entering via the Main Entrance, are roughly 50m-100m walk away. Outpatient clinics, located towards the front of the building, are a similar distance. Patients and visitors who have problems walking distances over 100m are advised to use the wheelchairs provided at the Main Entrance. Toilets, the restaurant, a shop, a cafe and the pharmacy are all accessed via the atrium.

Car parking Patients and visitors can help ease parking congestion by using public transport to get to the site. All public car parks are numbered or lettered, and visitors will be informed of the most appropriate car park to use. Parking will not be permitted in areas of the site where there are restrictions in place, and these restrictions will be enforced by the parking attendants. The Trust has also been working with the local communities to ensure it does not have a negative impact on those who live around us. We would ask all patients and visitors to consider those communities and not park in neighbouring residential areas.

Traffic control The speed limit around the hospital site is 10 miles per hour. Except for vehicles attending an emergency, every vehicle must keep to this limit for safety reasons.

Concessionary parking permits The Trust offers a concessionary parking permit for patients and visitors who visit frequently. The following patients and visitors are eligible: • Patients who visit the Trust for several days throughout the week. • Relatives of terminally ill patients who are visiting on a daily basis. • Relatives of patients likely to be an inpatient for longer than seven days. • Relatives visiting for long periods of time outside normal visiting hours. The cost of the concessionary parking permit is £3.40.

Buying and displaying your Concessionary Parking Permit: 1. Ask for a Concessionary Parking Permit form from the ward and complete section 1. 2. Ask a ward sister/departmental manager to complete section 2. 3. Take the completed and signed form to an APCOA office, either the ground floor, multi-storey car park at the City General or the grey cabin, near the helipad at the Royal Infirmary 4. The form, valid for seven days only, will be authorised by the attendant. 5. You must visibly display your permit inside your vehicle. The Concessionary Parking Permit is valid for one week only. If you need subsequent permits, a new application form must be authorised by ward staff and be purchased at an APCOA office. The parking permit is valid for one vehicle only.

Drop-off and disabled spaces Parking for people with disabilities and drop-off or short stay spaces are provided close to the hospital entrances. Drop-off and short stay spaces will be monitored and anyone parking longer than they should will be given a warning notice and, if appropriate, a Parking Enforcement Notice. All car parking on the site is monitored by the Trust’s car parking provider.

Bicycles and motorcycles Secure, covered bicycle and motorcycle parking facilities are provided close to the hospital entrances to encourage patients and visitors to cycle to the site.

Motorcycle parking is available in the multi-storey car park.

Bicycles should be parked in these facilities and not taken into buildings.

Ambulances Entry points Emergency ambulances for patients on trolleys should use the Ambulance Only entrance at the Emergency Centre (adjacent to the helipad). Patients brought to the Emergency Centre on foot or in a chair should be brought through the walk-in entrance of the Emergency Centre. Patients brought to University Hospital for emergency care should use these two entrances only.

Patients arriving in non-emergency ambulances for the Main Building should use the Ambulance entrance (shown on the map overleaf). This entrance is controlled by a key code which is available from the ambulance service. For patients arriving by non-emergency ambulance and patients leaving the hospital on a trolley for services in other buildings (see list below), the main entrances are available for use. • Maternity Centre • Cancer Centre • Trent Building • Lyme Building • West Building The Children’s Assessment Unit ambulance entrance is located next to the Ambulance entrance, shown overleaf.

Public transport Buses Bus stops contain up-to-date information about services to and from the hospital. First bus offer a regular service from Hanley (City Centre) (24/24A, 25, 101), Stoke (24, 24A, 25), Newcastle (22, 24/24A, 101) and Longton(22). Other operators’ services include Scraggs’ 31 and Wardle Transport Service 41 from Hanley, Stoke and Newcastle. The following are the recommended services to the various sites and take you straight to the entrance: • City General site – 22, 24/24A, 41 (alternatively 5-10 minutes walk from bus stop on A34 for service 101) Additional services include: 16 (Hanley, Bucknall, Werrington, Cheddleton and Leek) and 58 (Hanley, Stoke, Penkhull, Newcastle via Etruria Vale Road, Hanley, Stoke, Penkhull, Trent Vale, Clayton).

Please note, there is no bus service through University Hospital after 7.30pm Mondays to Saturdays. Service 101 still provides a service via the A34. If you need travel planning advice contact Traveline on 0871 200 22 33 or visit www.traveline.info

Moving around the hospital Wheelchairs Visitors who are moved by bed, trolley, wheelchair or on foot should be accompanied as necessary by appropriate staff. Wheelchairs are available from the Main Entrance. Porters and clinical staff moving a patient will make them fully aware of the purpose of the move and will preserve the patient’s comfort and dignity at all times.

Toilet facilities for disabled people Toilets for independent use by wheelchair users are located across the site. Please ask a member of staff for details about suitable facilities for parents and carers of adults or children with learning difficulties or a physical disability to safely, and with dignity, change the person in their care. Staff are responsible for making sure visitors are aware of these facilities and arrangements.

Emergency Centre The Emergency Centre (or A&E) is open 24 hours a day and is located in the Main Building on Lower Ground Floor 1. Emergency Centre can only be accessed through its dedicated entrance, the public and visitors should not use the Main Entrance to access this area. There is no patient/visitor access to the rest of Main Building using the Emergency Centre entrance. The Emergency Centre itself has two entry points, a walk-in entrance and an emergency ambulance entrance (located opposite the helipad).

Any patient brought to the Emergency Centre in an emergency ambulance who is either able to walk or enter via a chair will be escorted around to the walk-in entrance. Parking and drop-off spaces are available directly opposite the Walk-in entrance.

Main Entrance The Main Building is the name of the new hospital. It can be accessed from either the Hilton Road entrance (Entrance 3) or North entrance of the A34 (Entrance 1) closest to Newcastleunder-Lyme town centre. However, the Trust recommends that you use the Hilton Road entrance where possible as construction work is taking place in the centre of the site. There is blue badge parking available close to the Main Entrance. To access this, please take the second left after entering the site from the Hilton Road entrance.

Maternity Centre The Maternity Centre is located close to the A34/Newcastle Road. Use the North entrance of the A34 (Entrance 1) closest to Newcastle-under-Lyme town centre to access the site. The Trust recommends you use either the pay & display car park next to the West Building or the car park next to the A34 itself (to access this car park you must loop around the West Building).

Cancer Centre The Cancer Centre is located on the A34. Use the North entrance of the A34 (Entrance 1) closest to Newcastle-underLyme town centre to access the site. The car park outside the Cancer Centre is for those using that particular building only. The car park next to the A34 (accessed behind the West Building) should be used as a Cancer Centre overspill car park. There is a short cut through to the Cancer Centre to avoid the need to walk around the perimeter of the West Building.

Children’s Centre The Children’s Centre is located on Ground Floor of Main Building. Patients and visitors can use either the Main Entrance or the dedicated Children’s Centre entrance (located on Lower Ground Floor 1 adjacent to Emergency Centre). It is recommended that parents use the car park directly opposite the dedicated Children’s Centre entrance.

Trent Building Trent Building patients and visitors should use the same car parks as those using the Main Building, there are link corridors connecting the buildings. However, those patients with blue badges can use disabled bays located outside each building entrance.

Lyme Building Patients and visitors attending Lyme Building should use the same car parks as those using the Main Building, there are link corridors connecting the buildings. However, those patients with blue badges can use disabled bays located outside the entrance to the building. It is important to note that the upper floor of Lyme Building, where this entrance is located, is the same level as LG2 in Main Building.

This Lyme Building entrance is set to be demolished in early 2013 to make way for a new entrance that will give direct access to Main Building (where the new Prayer Centre is located).

Lower Floor Entrance Those patients with blue badges will find a small number of disabled bays located outside this entrance to the Lyme Building. It is important to note that the lower floor of Lyme Building, where this entrance is located, is a floor below the lowest floor, LG2, in Main Building.

Undergraduate Medical School The Undergraduate Medical School is located opposite the lower floor of Lyme Building. This building is primarily occupied by Keele University but the Trust does have access to meeting rooms on request. To access the building, contact reception using the intercom on the left hand side door.

A Block A Block is a Grade II listed building which previously housed clinical wards. This building will house Trust Headquarters, Nursing Directorate, Finance Department, Human Resources Department and other office based staff. Visitors to Trust Headquarters should park in the pay & display car parks available.

B Block B Block is a Grade II listed building located opposite the old chapel. The building houses the Clinical Technology Department.

There are a small number of reserved parking spaces for staff only outside this department to allow the transfer of heavy equipment.

C Block C Block is a Grade II listed housing the Trust’s Healthcare Careers and Skills Academy. This former workhouse has been renovated to include IT suites, teaching rooms and office for staff.

Anyone visiting the Healthcare Careers and Skills Academy should use the pay and display car park opposite.

Supplies

The main supplies building is located to the rear of C Block. The Trust has three service yards. Service Yard 1 is located adjacent to the main supplies building, Service Yard 2 is located adjacent to the sterile services building and Service Yard 3 is located next to the Main Building close to the Hilton Road entrance (Entrance 3).

West Building The west Building is located close to the A34/Newcastle Road. Use the North entrance of the A34 (Entrance 1) closest to Newcastle-under-Lyme town centre to access the site. Child Development Centre The Child Development Centre is located opposite the walk-in entrance to the Emergency Centre. There are a small number of disabled bays and drop off spaces outside the centre. All other patients and visitors should use the pay and display car park opposite the Emergency Centre. Access to the Child Development Centre should be via the Hilton road entrance and not the emergency vehicle entrance (which is immediately next to the centre).

Clinical Education Centre The Clinical Education Centre contains the health library and numerous teaching rooms and lecture theatres. Access to this building should be via site entrance 2. There is a large staff car park area adjacent to the Clinical Education Centre and a small number of disabled bays and pay and display parking.

Lifts Lifts designated for a particular use, such as a facilities management lift or bed lift, are clearly marked. These lifts must not be used for anything else unless there is a problem with another lift. In particular, where a facilities management lift is available, it should always be used for facilities management activity, including the movement of goods by Trust staff. This will keep patients’ and visitors’ lifts clear.

If a fire alarm sounds all lifts return to the ground floor unless the alarm has been set off on the ground floor, in which case they automatically move to the first floor. When the fire alarm goes off lifts should not be used. Some lifts have override facilities to be used by the fire and rescue service and may be taken over by the fire service to enable them to fight the fire. If anyone is trapped in a lift the Estates Department will launch an emergency response and manually winch the lift to the nearest floor.

Out-patients check-in How does it work? It is similar to those used in many GP surgeries. It will ask questions to find out who the person is. It will then ask to check some of personal details like address and telephone number. These details are important to us in case the hospital needs to contact the patient. It will check to see that the patient is on the clinic appointment list for today and as long as they are not too early it will allow them to check in. Anyone trying to check-in cannot do so more than one hour before the appointment. It will then look at how many people are in the clinic waiting room and if there is a seat it will send the patient straight there. If the clinic waiting area is full it will ask them to wait in the main waiting hall until a seat becomes available.

The patients are listed for the doctors and nurses in appointment time order. It does not work on first come first serve basis. It will give the patient a printed ticket with their name and a unique identification number on and give them a ticket.

Privacy and dignity The privacy and dignity of patients and visitors must be maintained at all times. Our staff are familiar with the Trust’s privacy and dignity policy and ensure it is adhered to without exception. Staff who are in direct contact with patients and visitors have a responsibility to ensure that their privacy and dignity is maintained. This is particularly important where patients are being moved across the site, are in a state of undress or partial undress or where they are having a physical examination. Staff entrusted with confidential information have a personal responsibility to ensure it is kept safe and secure. They make sure electronic or printed

information can only ever be seen by those entitled to do so. They do not expose this material to any undue risk of being seen by unauthorised individuals. This includes printed and on-screen information at nurses bases and clinic receptions. Care is also be taken when in discussion with patients and relatives, either face to face or over the telephone. No sensitive or general personal information is asked for or given to patients or visitors which could be overheard by another person, unless the urgency of the situation overrides the need to maintain confidentiality. Any breach of patient confidentiality is be treated as a disciplinary matter.

Blood tests GP patients

Hospital patients

GP patients requiring a blood test should either use one of the three walk-in centres (Bradwell Hospital, Cobridge Community Health Centre and Meir Primary Care Centre) or book a blood test appointment at a number of community locations. Telephone 01782 555506 for an appointment. Information regarding locations is readily available at all GP surgeries.

There is a small phlebotomy unit in the new hospital situated within Medical Outpatients on floor one. This is available for hospital patients requiring blood tests when they attend for clinic appointments. This service is for internal hospital blood test requests only. It is not available for patients given a blood test request card by their GP.

Children’s blood tests Paediatric phlebotomy should be accessed by making an appointment at the new Children’s Centre at the City General site. Please contact 01782 675122 for an appointment. This service is mainly for children below the age of 14.

Patient Advice and Liaison Service The Patient Advice and Liaison Service, or 'PALS' for short, is here to help both your patients. Our staff aim to provide your patients with the best possible care and environment during their hospital stay. However, occasionally patients ask for assistance and support if they have a concern. PALS’ role is to be an impartial, welcoming, open and confidential service for people who would like information or advice, or would like to comment about any aspect of the services provided by University Hospital. The PALS team liaise with all staff members on all levels to aim for an acceptable outcome for patients and the public. In return our staff ensure that if a patients or the public raises a concern with the ward or department the initial action is to try and resolve the concern locally.

If the patient or public is unhappy at this point and all attempts to resolve have been made, PALS information can be provided, and or the ward staff can request a visit from PALS on behalf of the patient if they have consented. Hospital Chaplaincy Support for staff Chaplains are NHS staff employed by the Trust and covered by Trust policies and guidance and work as allied health care professionals. The staff provide support by offering a confidential listening ear, reflective practice or debriefs. Funeral prayers are also offered following the death of a colleague where staff are unable to attend the funeral. Advice on matters of faith and religion is also provided. You can contact the Chaplaincy Team on 01782 676400. The Chaplaincy Team provides a 24-hour service including response to emergencies out of hours. Normal working hours are 8.30am to 4.30pm, including weekends. A 24-hour on-call service is available for urgent referrals.

Trust Structure Our services are organised into four divisions, each of which is made up of a number of individual directorates or departments. The clinical divisions of surgery, medicine and clinical support services are led by an associate director, the clinical directors of each directorate and, in surgery and medicine, a divisional nurse. They are supported by a human resources manager and a finance manager. Each of the 35 service line teams are led by a clinician who manages the safety, clinical quality, patient experience and financial aspects of their service. As performance improves, they are given increasing levels of freedom to control and manage how they deliver the best possible quality and service to patients.

Our non-clinical staff support the work of the Trust’s clinical teams. Some work within clinical departments and others in central function departments. These include our executive directors, human resources, operations and performance. The corporate services division includes estates, facilities, catering, cleaning, portering and security, although many of these functions will transfer to our PFI partners through the course of next year.

Infection Control The prevention and control of infection together with patient safety remains a top priority at University Hospital of North Staffordshire. The Trust is pleased that the number of Trust apportioned Meticillin resistant staphylococcus aureus (MRSA) and Clostridium difficile cases is significantly lower than last year. University Hospital achieved an 80 per cent reduction in MRSA bacteraemia in 2012 compared to 2011. This reduction to just six cases of MRSA bacteraemia is even more significant when considering the figure of 100 MRSA bacteraemia reported for the same period in 2006-07. The Trust achieved an 82 per cent reduction in Clostridium difficile cases in 2012 compared to the previous year with year on year reductions in this area since 2007. Cases of Trust apportioned Clostridium difficile infections are subject to a root cause investigation and analysis. Wards

that have two or more cases within 28 days present their findings to the Chief Executive. Learning outcomes are shared throughout the organisation. To achieve and sustain year on year reductions, the Trust has developed policies and procedures as well as an audit programme to ensure that they are being adhered to. A new Lead Nurse for Infection Prevention and Control was appointed in April 2011.

Guy Hilton Research Centre The Guy Hilton Research Centre, opposite the old Pathology Lab, offers patients an opportunity to take part in high quality research projects, which continue to be a priority at UHNS. In 2011 we recruited 3,167 patients into 103 National Institute for Health Research (NIHR) projects across 20 medical/surgical specialties. The Trust is committed to increasing the number of people involved in research. Our target for 2012/13 is 4,800 patients. We are on target to double the number of participants recruited by 2014. Our NIHR portfolio work was supported by £1.4m of NIHR research network investment to provide the infrastructure that supports patient recruitment into studies, including research nurses, midwives and key support services such as pharmacy, pathology and imaging. During 2011/12 the Trust was ranked 34 out of 347 research active Trusts based on number of patients recruited. In partnership with Keele University, we secured £1.7m of research grant income during 2011/2012. This partnership delivers a ‘bench to bedside’ approach to

research, enabling the results of research to feed through to daily clinical practice in clinics and on the wards. Notable research grant successes include an i4i award evaluating point of care testing in the management of COPD and two Research for Patient Benefit awards. One looked at the management of peritoneal dialysis and the other focused on memory loss in patients with Parkinson’s Disease. Research was well supported by UHNS Charity during the year with projects funded in areas such as paediatrics, obstetrics and gynaecology, cardiology, orthopaedics and diabetes. Staff at the Centre continue to build and expand the research activity of the NIHR Clinical Research Networks based at University Hospital (Medicines for Children, Stroke and Cancer). They continue to support the on-going development of the Local Clinical Speciality Groups, six of which are led by our consultants (Diabetes, Neurology, Nephrology, Paediatrics, Critical Care and the Care for Women Network).

Major Trauma Centre University Hospital was granted Major Trauma Centre (MTC) status in 2011-12 after the West Midlands Strategic Commissioning Group recommended the Trust become an MTC serving the population of Staffordshire, South Cheshire, Shropshire and beyond. The Major Trauma service is underpinned by expertise in resuscitation and specialist surgery and focuses on coordination, standardised communication and real-time governance. It is coordinated by a team of dedicated trauma leaders. The Trust provides comprehensive, specialist care in an integrated and timely way from when patients receive injuries to the completion of rehabilitation. Our specialist surgical teams deliver definitive care for patients with complex, life-threatening injuries 24-hours a day. The Centre has excellent links with our partner trauma units, and the West Midlands Ambulance Service. The North Staffordshire Rehabilitation Unit supports us in Neuro and General Rehabilitation.

The vision for the centre, within a cohesive Major Trauma Network, is to become a world-class provider of acute and specialised services. We have all of the necessary ingredients to provide a full range of immediate and emergency interventions, delivered within a purposebuilt, single-site facility. The major trauma service builds on existing links with Keele University and increased excellent opportunities for research and academic development. Major Trauma Centre status also provides opportunities to develop our regionally recognised medical and nursing education.

Keele University/CEC/UGMS Keele University Medical School trains around 130 students in each of its five years of the Keele MB ChB course. 2012 is an important year as it is the first year that this degree will have been awarded. Before this, doctors qualifying from Keele had been trained in a Manchester based system and awarded a Manchester degree. The Keele curriculum is an innovative, modern medical curriculum that includes problem-based learning whilst still using traditional methods of teaching. The distinctiveness of the course is that it has been designed to allow diversity and integration. It allows students with different personalities, aspirations, preferences, learning styles and strengths and weaknesses to be successful, to enjoy their undergraduate time and to be able to build on these experiences during postgraduate training.

Career options for doctors have never been greater and, although our primary aim is to deliver competent Foundation Year trainees, the course helps students to experience more specialised activity by recognising and developing natural aptitudes. This is achieved through flexibility in the student-selected components, consolidation periods and final year electives that allow for maximum variety of choice in terms of activity and learning environment. Students are based in the Clinical Education Centre and the Under Graduate Medical School.
