By JEFF WEBB It is an acute understatement to say these are challenging times in the healthcare industry. So, when you have as many issues to advocate for as does the leadership of the Florida Hospital Association, it stands to reason the responsibility should be entrusted to a person with a skill set that includes the ability to juggle myriad topics that directly affect policies and patients. In October, the FHA Board of Trustees did just that when Allen Weiss, who describes himself as an “inveterate multi-tasker,” became chairman. Trained in internal medicine, geriatrics and rheumatology, Weiss, who also has a Master of Business Administration degree, has been president of NCH Healthcare Systems in Naples since 2001. He assumed the additional title of CEO in 2006. The 67-year-old has served on the FHA board since 2011 and now that he is chairman, he is eager to champion the priorities that will become his legacy at the 87-year-old FHA. “Our top priority is among the most important issues facing our state – securing healthcare coverage for nearly 1 million uninsured, working Floridians whose jobs pay less than $16,000 a year,” said Weiss. “When people have access to
quality care they can afford, it means serious illnesses can be detected and treated sooner. Florida has the opportunity to use over $50 billion to develop a state-specific program that provides coverage for low-income Floridians, which ultimately creates a healthier workforce and state. That’s why we’re working with A Healthy Florida Works, a bipartisan coalition of business leaders, chambers of commerce, statewide business organizations and concerned individuals that supports coverage for 1 million working, uninsured Floridians,” he said. Essential to the goal of expanding coverage, Weiss said, is the future of Florida’s Medicaid Waiver. “Before the Patient Protection and Affordable Care Act, the federal government covered the cost of caring for uninsured patients through supplemental programs such as the Low-Income Pool. Since the law’s implementation, the federal government has changed its approach by providing coverage for people, rather than paying more in Medicaid rates for hospital services,” Weiss explained. “The waiver for Florida’s current Low-Income Pool program is set to expire in June 2015 and we need to be prepared to address the impact.” Attempts to do that have been fraught with
obstacles beyond the control of hospitals and physicians, namely the refusal of Gov. Rick Scott and the Florida Legislature to accept available federal Medicaid funds. However, Weiss said he is encouraged that rigid stance may soften. The FHA “backed off a little (in 2014) to see what would happen with the governor’s race and the Legislature. I think now is our time to ask for this to be closely considered,” he said. Weiss hopes the political will arises from “the people who understand that Florida is losing $18 million a day and we have a million folks who have no insurance. I think they will be sympathetic to the practicalities of it no matter where they are ideologically. … I have every respect for everybody’s opinion, and I understand both sides. But the practical matter is that the states that have taken the money – and I think it’s more than 27 now – their quality of care is improved and their patient outcomes are better,” he said. “Plus, everybody around us who has insurance could pay a little less if we had more people insured.” “The governor is in his second term now and he can focus, as he has done, on what is continued...
GETTING PERSONAL Growing up as the middle of three children in suburban Philadelphia in the 1950s, Allen Weiss said he enjoyed “a happy, productive childhood focused on hard work, education and family values.” But Weiss, who has accomplished a great deal to become a successful physician and now chairman of the Florida Hospital Association, remembers two turning points from his youth. He was not a good reader in elementary school and his parents hired a tutor who not only helped him improve, but who “boosted my confidence, which was important in 3rd grade,” he said. He remained “a rather unremarkable student” until his senior year of high school “when I woke up!” he recalled. “I got all A’s on my report card, which continued as a trend thereafter in college (at LaSalle College), medical school (at Columbia University) and graduate training” (at Presbyterian Hospital in New York City), he said. Weiss was at LaSalle when he picked up a couple of lifelong habits: swimming and running. He did it to take a break from his studies there and in medical school, he said, and “It never dawned on me that it would be healthy later in life.” Decades later, Weiss said he still exercises daily, with a weekly routine of running Tuesday, Thursday, and Saturday for 7, 7, and 10xmiles respectively, and swims a mile and lift weights Monday, Wednesday and Friday.” What about Sundays? “I hit some golf balls or bike about 21 miles,” he said. Weiss began his practice in Naples 37 years ago after completing his fel-
lowship in rheumatic disease in New York. As his career evolved, Weiss said, “I migrated into geriatrics while always practicing internal medicine.” Weiss moved into administration at Naples Community Hospital and North Collier Hospital in 2000, and a year later helped form a Physician Hospital Organization and obtained an MBA, he said. Today he is both president and CEO at NCH Healthcare System. Weiss said he has been married more than 40 years and he and his wife, an educator and writer, have two daughters who are physicians, two successful sons-in-laws and four happy grandchildren. “I couldn’t have asked for more.” In addition to being a voracious reader who does not drink coffee or tea and almost never watches TV, Weiss also writes a monthly newsletter, hosts a weekly radio show and volunteers on a host of health-related boards besides the FMA. His philanthropy includes a gift to Florida SouthWestern State College, the Allen and Marla Weiss Health Sciences Hall. “I find that the charities with which I’m involved give me so much more than I give them. It’s so rewarding,” Weiss said, as is “working elbow-to-elbow with the most competent, compassionate, and fun healthcare colleagues I’ve ever known.” But, without question, Weiss said, “helping people is most fulfilling.” Weiss’ 3rd-grade tutor probably would agree.

really good for the state. I think even he agrees, even though he was not in support before, that it is good for the state. He can help, particularly with the folks in the House of Representatives who are still against this idea. …. I’m hoping everybody can get it together. If everybody stays polarized, nothing gets done. … It doesn’t help employers who want to get their health insurance costs down, or the folks who need health insurance, to keep up this ideologic divide and intransigence.” Hospitals that are affected by this need to be searching for other forms of revenue, Weiss said. “Typically these are safety-net hospitals that are doing a lot of charity and Medicaid care. If they can get more people enrolled in Medicaid and get a better reimbursement rate, it would help their financial solvency.” Weiss also is bullish on the quality of care for Floridians. “Six years ago, Florida hospitals set out to significantly improve care for our patients, increase the value of care in our state and set a national model for ways to improve care. Our efforts are making a difference,” he said. “Through the FHA Hospital Engagement Network (HEN), 77 hospitals are working to eliminate patient harm such as infections, falls and readmissions. To date, hospitals working through the FHA HEN have prevented nearly 6,000 cases of harm and saved more than $20 million,” said Weiss, who has been chair of the FHA’s Quality Committee since 2012. It is widely agreed upon that leaders are only as effective as their ability to build consensus among their stakeholders. To that end, how does Weiss hope to achieve the FHA’s goals? “Through the FHA, hospitals have an important role in shaping policy that helps us provide the highest quality care to our patients and communities. Our membership reflects the diversity of hospitals in Florida – rural, urban, small, large – and there’s opportunity for input at every level through our committees, councils, roundtables and our Board of Trustees,” Weiss said. “There’s also tremendous opportunity to learn from each other. By collaborating and sharing best practices, we are a stronger association and better partners in our communities.” Weiss said he is “proud of our work thus far” and is looking forward to a fertile agenda for multi-tasking in 2015. “Changing the way we deliver healthcare in America is a goal we all should share. No one would design a system like the one we have currently. Collaborating with the four major players — patients, physicians, payers and healthcare systems — to evolve from a repair shop mentality to wellness centers is a goal on which we should all be focused.”

Alan Pillersdorf, MD President, Florida Medical Association
By JEFF WEBB Whether it’s been for his family, his patients or his profession, Alan Pillersdorf has a long history of giving. In 1991, Pillersdorf donated his right kidney to his only sibling, older brother Gary, a prominent personal injury trial attorney in New York. Even though doing so cast him in the reverse role of being a patient, Pillersdorf said, the event “affected my life more than anything else as a doctor.” “I always worshipped my brother,” Pillersdorf said. When they were growing up in Brooklyn, Gary would “tease me that I was adopted and that he was going to send me back to the orphanage.” But after receiving his brother’s life-saving gift, “he admitted I wasn’t adopted. … He’s alive, working and I still worship him.” To this day, Pillersdorf said he uses that personal experience to relate to patients about their pain. “When they tell me ‘Doc, you don’t understand how bad this hurts,’ I can say ‘I actually do.’ It made me more sympathetic to my patients” and helped him understand “that when a family member is sick, the whole family is sick,” he said. Pillersdorf also has given care to countless patients at the practice he founded 24 years ago, Plastic Surgery of Palm Beach. His prac-
tice, with offices in Palm Springs, Jupiter and Wellington, now has five physician colleagues. Pillersdorf also operates an outpatient surgery center, where he continues his habit of inspirational giving by donating a lot of surgery to gastric bypass patients who have had massive weight loss and whose operations often aren’t covered by insurance, he said. But Pillersdorf ’s giving ways also have been to advance his profession through leadership roles. His long list of involvement includes the Palm Beach County Society of Plastic Surgeons, the American College of Surgeons/ Florida Chapter, The Palm Beach County Medical Society, and a half-dozen committee and department chairmanships at JFK Medical Center in Lake Worth. “I was groomed into leadership,” said Pilllersdorf, adding “I’ve always liked being involved in the politics of the hospital and making decisions.” Now, at age 63, Pillersdorf is giving his time and talent on an even larger scale as president of the Florida Medical Association, where he has been deeply involved for more than a dozen years as he prepared to ascend to the top elected position at the 140-year-old organization that represents more than 20,000 physicians. Almost halfway through his one-year term as president, Pillersdorf, in his own words, enu-
merated the FMA’s overarching agenda: • Keeping the FMA the most respected physician organization in the state. Keeping it number one is my highest priority. • Legislative advocacy in supporting issues that help physicians practice medicine in this state. The FMA is the voice of physicians and their patients in Tallahassee. Our principles should always be with strong ethical morals, integrity, quality and professionalism. • Help physicians practice in a rapidly changing healthcare market, where there are new paradigms occurring daily. To that end, Pillersdorf said, he will draw heavily on his experience with the FMA to make sure all members have a stake in accomplishing the specifics of that agenda. “I believe the president of the FMA should be a good listener, and lead by consensus and act as spokesperson. We must speak with a unified voice. Disagreement is okay, but once we decide on an issue, we must all speak as one,” said Pillersdorf. “The Board of Governors of the FMA represents all the stakeholders in the state. Each medical geographic region is represented, all the large specialty groups have a say, and we have representation from the Council of Florida Medical School Deans, a Public Member, continued...
GETTING PERSONAL As it turned out, Alan Pillersdorf’s donation of a kidney to his big brother (see main story) would not be the last time he would find himself on the receiving end of care for an uncommon medical circumstance. The weekend of May 30 was the beginning of what Pillersdorf described as one of the biggest challenges in his life. On that Friday, he had a busy day in the operating room performing eight surgeries. That night he went to a Miami Heat basketball game. The next day he attended a surprise birthday party for a physician friend in Boca Raton. On Sunday morning “I got up early and by 7 a.m. I knew something was wrong. I couldn’t physically get myself out of bed,” he recalled. Pillersdorf went to JFK Memorial Hospital, where he is the former chief of surgery, and was placed in the intensive care unit with symptoms of severe fatigue and a high white blood cell count. It took about four days for doctors to diagnose that he had contracted viral encephalitis from a mosquito bite. “I was in the hospital three weeks, and in rehab for three weeks” before being discharged to go home and continue outpatient rehabilitation, he said. And all of this transpired right before he was installed as president of the Florida Medical Association. Pillersdorf said he “has not resumed my life as a very busy plastic surgeon,” but he is “functioning at about 86 percent now.” As he has looked for positive side ef-
fects of his illness, it is that he has and has been able to devote even more time to his responsibilities with the FMA, and to spend more time with his wife of 40 years, Rhona, whom he married when he was in medical school at Georgetown University. “My recovery has been amazing … My wife has been even better,” he said. The Pillersdorfs have four children, three of whom live within a mile of where he grew up in Manhattan as the youngest son of an attorney and a school teacher. Pillersdorf has made good use of his convalescence at home. He has resumed working on a children’s book that he hopes to one day share with his four grandchildren. He also is indulging his love of acrylic painting, his subjects being mostly family members. And this year he started piano lessons, which he had stopped when he was about 10 years old. “I have a teacher who comes here twice a week and I’m re-learning how to read music,” he said. If you are attending the FMA meeting in Palm Beach in February, you may be lucky enough to hear the doctor play several pieces at a meet-and-greet he’s hosting. But Pillersdorf said he is most looking forward to getting back into rhythm at his practice, Plastic Surgery of Palm Beach. “The best part of being a plastic surgeon is when you see a patient from a few years ago and they tell you how you have changed their lives. It is the best.”

the Florida Medical Association Alliance, the Secretary of Health, the Chair of the Board of Medicine, the Medical Society Executives, Young Physicians, Resident Physicians and Medical Students,” said Pillersdorf. “I use my board to guide me in all decision-making. Every voice must be heard.” Pillersdorf ’ said his term as president of the FMA has been a personal growth experience with special challenges and opportunities (see sidebar) but his history with the organization has guided him. “Having been on the Board of Governors for the past 10 years, and observing the style of the presidents before me, I have grown a sense of what my leadership should represent. I think as president, I am but one voice, an important voice, but I must lead with the consensus of the organization. There are issues every day that I deal with, but I represent all the doctors in the state, and I base my decisions with that very thought. This is a very large state, with many different communities. To be a leader, one has to be extremely fair,” he said. The FMA board is “35 people who are very, very diverse. It is broken up into eight districts. You must ask them at our meetings what they think. You ask everyone’s opinion and you come up with a fair idea based on that. I think that is how my success will be measured in years to come,” said Pillersdorf. Why is unity of physicians so important? In an interview with Florida Medical Magazine earlier this year, Pillersdorf elaborated: “We’re the only organization that truly works only for physicians. We are stronger if we are united. Our first and foremost priority is to work for physicians – not for insurance companies and not for government,” he said. Achieving consensus among a group of professionals who are known for having above-average egos is no small task, said Pillersdorf. “But there are professional people who teach you how to run effective meetings. Your whole job is fairness. … There can be conflict in opinion (and debate), but not in action,” he said. This summer Pillersdorf will pass the reins of leadership to incoming president Ralph Nobu, Jr., MD. Segueing again to Pillersdorf ’s penchant for giving, Medical News, Inc. asked him what advice he would give to his successor: “Listen carefully to your board and get input from previous leaders. They have a tremendous amount of wisdom to give. Never make a decision when you are upset. … And remember, we are involved in an awesome association, so enjoy it. Listen carefully and always do the right thing,” said Pillersdorf.

Best Practice: Evidence-Based Standards of Care
Therapy for Venous Insufficiency The mainstay in treating venous insufficiency be it varicose veins, spider veins or advanced skin changes-ulcers, is elastic compression. The elastic compression should be graduated – greater pressure at the ankle, than the upper leg. Adequate compression does not include support hose or TED stockings. TED’s give 15mm Hg compression which is adequate while a patient is lying (thus the name antiembolic stockings) but is inadequate with the leg in the dependent position. They really have no place other that the hospital setting. Medical grade stockings include compression over 20mm Hg. The most commonly prescribed compression is 20-30mm Hg. For preventative purposes in those who have jobs that entail a lot of standing-beauticians, nurses, cooks, surgeons and others 15-20 mmHg is adequate. The stockings only need to extend over the calf. Compression in the thigh is only useful for those with large varicosities which ache. They are also used with treatment and for considerations of comfort or where a dress would expose the top of the knee high. Wearing compression hose in Florida is difficult due to the weather and our summertime clothing, but daily usage will usually result in improved hemodynamics and thus symptomatic improvement. In fact, improvement with compression is a good prediction that surgical treatment of the diseased veins will result in long term symptom improvement. Many changes have occurred in the last 10 years in our approach to treatment of chronic venous insufficiency. Previously surgery was the mainstay for therapy. Sclerotherapy with liquid, although frequently successful in the short term had a high rate of recurrence in 20 to greater than 50% of patients. Surgery, in most cases, entailed general anesthesia and multiple incisions with the attendant problems of wound healing in the obese and when incisions were placed in an area of advanced stasis changes. Dealing with perforator veins-often the underlying culprit in patients with ulcers was often challenging, especially when located under an ulcer or in an area of significant lipodermatosclerosis. Even in the best of hands there was a significant recurrence rate, especially if there was a subsequent pregnancy or the patient had a job that entailed significant standing. With the new millennium there has been a significant paradigm shift with the advent of endovenous therapy. The two modalities available are radiofrequency closure and laser ablation. There are also devices on the horizon which glue the vein, or abraid the inner vein wall and inject a sclerosant at the same time. In essence, laser and radiofrequency use heat to denature the proteins in the vein wall causing the vein to fibrose. Because the target vein isn’t actually removed, there is much less discomfort and recovery is shorter. More importantly, the long-
term results are actually better than surgery. Another modality which shows great promise is transcatherer chemical ablation. A solution of sclerosant is converted to a foam and injected into the vein while pulling back on the catheter. The results have been nearly as good as radiofrequency and laser and the procedure is very well tolerated. After treatment of the larger and deeper veins with the laser, the more superficial and visible veins can be treated using ultrasound guided sclerotherapy. This involves injecting a solution into the vein to close these veins and allow the normal veins that remain to more efficiently return the blood to the heart. The keystone to all of the procedures is a complete detailed ultrasound using compression maneuvers with careful attention to the perforators. Nothing can replace experience with venous and arterial disease and an understanding of venous physiology. Patients with significant arterial disease or who lack mobility should generally not be treated. Treatment of patients with ulcers can be gratifying, and should be coordinated with wound care. Not only can wound healing be accelerated but recurrence rates are lowered. Treatment of spider veins is generally accomplished using sclerotherapy. This is performed using asmall 31g needle. Some spider veins, especially those which are very small or those on the face are best treated with the appropriate surface laser, Again, experience and appreciation for the underlying venous pathology is invaluable. Spider patterns in certain locations on the legs often indicate underlying disease which will compromise results if not addressed. New developments in therapy have made treatment available in many who, in the past, would not be considered because of the risks of general anesthesia. There is still a need for experience that comes from years of training and practice in matching the right modality to the patient.
