Carers Link also offer exclusive dementia support through the popular Memory Support Program which aims to increase and stimulate cognitive functioning and effective working memory.
The Memory Support Program psychologist utilises in-home neurocognitive assessments to understand their clients goals, needs and challenges and then designs an individualised plan to assist them to achieve optimal wellbeing.
Throughout the entire process Carers Link offers personalised contact and case management from a dedicated Aged Care Advisor, who will not only work with the client, but also with their family to support you across the whole journey in aged care.
