Leave Application Format for Not Attending Online Class Due to Eye Problem From: Samantha Brown 45, Almond Street California Date: January 25, 2022 To, Mr. Henry Smith University of California California Sub: Application For Not Attending Online Classes Respected Sir, I, Samantha Brown, student of final year Business Management, am writing this application for requesting you to allow me to not attend the online class for three days from January 27th, 2022 to January 30th, 2022 as I have got a severe eye infection. Due to the infection, the doctor has prescribed me to stay away from usage of any kind of digital screen so that I can recover faster. Therefore, it will not be possible for me to attend the class.

I sincerely regret that I will not be able to attend the online class but I will certainly listen to the recording and prepare notes.

I request you to kindly consider my situation and approve my leave application.
