What to Ask Your Clinician
Patient Information Questions
We want to make sure we are taking care of all your health care needs. Some people are more comfortable answering questions on paper. Others prefer to talk with their doctor about personal concerns in person. Both are okay! The more we know about you, the better your care will be. Thank you for your willingness to be open with us.
We want to make sure there are no gaps in the way we treat our patients by race or ethnicity. The more we know about you, the better we can serve you. It is your choice to share as much or as little as you want. All information you share with us is kept confidential.
There are some needs that are unique to people who identify as LGBTQ+. The better we get to know you, the more we can work together to make sure you are as healthy and supported as possible. All information you share with us is kept confidential.
