Upon successful completion by the student, of the following course assignments and/or tests, the faculty member will arrange the appropriate grade change in Workday and provide a reason for the change. If the student has not completed this contract within one year time limit, the grade automatically becomes an F. (In No Case May the "I" Grade Be Changed to a "W")

I have received a copy and agree to comply with the conditions of this Incomplete Grade Contract.
