Action for Children Short Breaks: Day care and overnight breaks for children aged 5-18 with complex needs; based at Pastens in Oxted.

This service is accessed through social workers.

Action for Children Cool Club: Based at Clifton Hill School in Caterham. This club is open to children from Clifton Hill School and the surrounding area. Children attending this service will be aged 11– 18 years old. Actionnaires and Young Transitions: A programme of fun and accessible activities for blind and partially sighted children and young people (C&YP) aged between 5 and 19. The combined ‘Actionnaires’ Clubs and Young Transitions Network will provide regular opportunities for engagement with activities ranging from sports and leisure to arts, crafts and essential independent living skills, all conducted by experienced, trained staff, in accessible and safe environments. Domiciliary Care: Specialist nursing support in the home for disabled children and young people aged 0-18.

Young People’s Service: Avenues Young People’s Service: Supporting children aged 12-19 years old in their homes or the local community. *Avenues will soon be able to support children and young people across both the East and West of Surrey. Badshot Lea and Colts SAINTS Football Club: A football club for disabled children and young people aged 5-18 years-old running Saturday practice sessions and competitions during the football season. This club is in Badshot Lea, Farnham, and open to all families to access directly.

Barnardo’s Transition and Independence Service: 12-week life skills programmes focusing on specific topics relevant to teenagers plus 1:1 support available via referral. This will again be a 12-week programme focusing on the individual needs of the young person. Barnardo’s Participation Worker: Barnardo’s run a participation project to support groups of disabled children and young people to get their views and ideas heard. Overnight short breaks, Day care, Tea Visits & Introductory Assessments: Overnight breaks in East Clandon and other support during the day to children/young people aged 0 –19 with severe disabilities.

Holiday Camps: Play schemes for children and young people aged 5–19 years old with profound disabilities and complex needs. The play schemes will run during the Easter, Summer and Christmas holidays and will be based at The Children’s Trust in Tadworth. Personal Assistant/Care-worker Introduction: This service will recruit, train and check Personal Assistants and Care Workers to work with disabled children. This service is aimed at families receiving Direct Payments but is open to any family wanting to employ a worker directly. 2

Freemantles Guildford After School Club: Transport is available to take children aged 4-12 years old from Freemantles School to Challengers in Guildford for an After School Club. The Ridgeway Farnham After School Club: Transport is available to take children aged 4–12 years old from The Ridgeway School to Challengers in Farnham for an after School Club. One to One Sundays: providing support for disabled children aged 4-12 year-olds to attend play schemes in Farnham and Guildford Play and Youth Schemes: More places including 1-1s are being provided at holiday play schemes, youth schemes, Saturday clubs and Youth nights for children and young people aged 4–19 years-old. Youth Activity Weekends: Weekends away for 13–19 year-olds. Including Me: Helping disabled children educated in specialist settings access mainstream after school and holiday clubs. Early Years Service

Early Support Service: offering parents and professionals information about local services for young children aged 0-5 with disabilities and special needs. Early Years can coordinate the different services you may need to make sure everyone involved (health, education, social care professionals, the voluntary sector) is aware of the overall needs of your child and your family. Grafham Grange Residential Activity Breaks: these holidays will be for children and young people aged 10–16 years old who have behavioural difficulties related to their disability and will be run from Grafham Grange School in Bramley, outside Guildford. Halow Project Youth Activities: Youth clubs and activity days based in Guildford and Haslemere for young people aged 16–19 years old with a learning disability.. The weekly youth groups take place on Wednesday and Friday nights.

Transport from Freemantles and The Ridgeway to Challengers After School Clubs will be accessed directly by parents whose children attend those schools. The One to One Sundays, Youth Activity Weekends, Play Schemes, Youth Schemes, Saturday Schemes and Youth Evenings are all accessible directly by families. InterAKtive Holiday Play Schemes: Play schemes specifically for children and young people aged 5-16 years old who have Autistic Spectrum Disorders. To access these clubs at Easter and Summer families must become members of InterAKtive. These play schemes run at Linden Bridge School (Worcester Park) Kids Short Breaks: providing short breaks either in the home or the community which can be booked directly by families.

Escape to Olympia: Saturday activity days in Woking and weekend residential breaks for young people with learning disabilities and additional needs, aged 12-17 years-old Olympic Spirit: Half-term activities in Woking for children and young people with Learning Disabilities & additional needs (5–17 years old) Holiday Playscheme: Activities for children and young people aged 5-17. Based in Woking with trips to zoos, farms and so on. Youth Clubs: A range of clubs in Woking and Camberley for children and young people aged 4+, including swimming, dance and transition. Domiciliary Care: Support at home for children and young people aged 0-18 in Spelthorne.

London Youth: Activenture: Adventure holidays for children and young people aged 8– 18 years old.

Children’s Clubs: 5 after school clubs across Surrey for children with ASD and similar communication difficulties aged 7-13 years-old Therapy Clubs: structured social skills programme and/or Lego therapy for 5-15 year olds with ASD or similar communication difficulty Youth Clubs: 6 youth clubs across Surrey for children with ASD and similar communication difficulties aged 13–18 (19 if in full-time education) years old. Each club runs for 2 hours per week during term time and for one full day per week during school holidays.

Extended Day Service: Children at Pond Meadow school (Guildford) will stay for after school activities one day per week during term time. Children who do not attend the school but meet the eligibility criteria may be able to access this service. Play scheme: A two-week play scheme for Portesbery School (Camberley) students to be run in the Summer Holidays. After School Club: An after school club running for 5 nights per week from September for children attending Portesbery School. Children who do not attend the school but meet the eligibility criteria may be able to access this service. Rainbow Trust Community Breaks: Providing a service to children with life limiting conditions. This service takes place in the family home for children and young people aged 0-18 years old.

Yippee & Yip4Youth Holiday Clubs: Two groups, one for children aged 5–12 years old and one for 13-16 years old. This service runs through all school holidays. Yippee & Yip4Youth Saturday Clubs: Saturday Clubs for children and young people aged 5–12 years old and one for 13-16 years old. Yippee & Yip4Youth After School Clubs: After School Clubs that run for two days per week. V Buddies: Providing a youth activity based befriending service for young people between the ages of 11-19 years old with complex needs or disability. YMCA recruit and train buddy volunteers. The Ridgeway After School Club: An After School Club running for two nights per week for children attending The Ridgeway School (Farnham).

Scope Family Support Service: Supporting children aged 0–18 years old in their homes and their local communities.

The Family Support service is accessed through social workers.

Befriending Project. A project to link disabled young people with buddies to support them to access activities in their local community. This service operates in North Surrey for children aged 5-18 years old.

Shooting Star Day Care Service. Providing Day care to children aged 0-5 years old with a life limiting illnesses. Shooting Star Chase Hospice is located in Hampton.

Spelthorne Millennium After School Club

Spelthorne Millennium After School Club: Operating out of Manor Mead School in Shepperton this club supports 5–13 years old children with complex needs and autism for two afternoons per week.

Surrey Special Schools Ski Group: Each January a group of Surrey Special Schools take disabled young people on a skiing trip to Italy. The schools involved include: Freemantles, Limpsfield Grange, Linden Bridge, The National Centre for Young People with Epilepsy, Philip Southcote, Portesbery, Ridgeway, Sunnydown, The Park, Walton Leigh, Woodfield and Woodlands. Crossroads Evening Sitting Service: Parents pay an hourly rate to have a member of Crossroads staff come and ‘babysit’ children in the evenings. This service is for children aged 0-19 years-old Domiciliary Care: Providing 1-1 support for children and young people aged 0-19 years old with complex needs. Elmbridge and Funtastic Saturday Clubs: Providing Saturday Clubs in Walton on Thames and Leatherhead for children and young people aged 5-19 years old. 6

White Lodge Treetops: Treetops is a purpose built short break centre for disabled children aged 0 – 18. White Lodge offers overnight short breaks, tea visits, and day care services. White Lodge Rendezvous Saturday Club: A Saturday Club run on alternate Saturdays for young people aged 13-18 years-old with a physical disability and young people with Autism. White Lodge Play & Leisure: Holiday scheme for children with severe learning and/or physical disabilities and complex health needs aged 4-8 in Runnymede, Walton, Chertsey and Spelthorne (12 in Runnymede)

White Lodge Family Link: A family based befriending and short-term These services are only accessible by care service for children and young people with disabilities aged 3 – referral. 18 resident in North West Surrey. White Lodge Twinks: A specialist holiday playscheme that caters for boys and girls on the autistic spectrum age range 4 – 12. White Lodge Rockets: A monthly Saturday Club for young people age range 9 -15 who have behavioral issues / autistic spectrum. White Lodge Domiciliary Care in the home support for parents of children with severe learning and/or physical disabilities and complex health needs with care tasks, sitting service, taking child to access activities. 7
