Food First Approach in Long Term Care Jessica MacDonald NM Season Care Consultant Season Care Dietitian Network What is the Food First Approach? At Seasons Care, we believe in a Food First Approach. Residents who struggle with food intake may not be getting enough nutrients, therefore, nutrition supplements are often used as a means to combat the risk of malnutrition. The Food First philosophy is based on our firm belief that foods are more enjoyable and satisfying than supplementation, and all measures should be taken to adapt the normal diet to fit the Resident’s individual needs, such as providing foods that are high in protein and nutrient dense. The goal is to have Residents consume something before they leave the dining room. What is the Food First Approach? 1. A Resident may require additional nutrition due to a wound, weight loss, malnutrition, poor intake, behaviors, etc. 2. The Registered Dietitian (RD) will provide nutrient dense items using High Energy High Protein (HEHP) Intervention such as High Protein Milk, Super Cereal, High Energy High Protein Pudding, or items and snacks based on the Residents’ preferences. 3. If these are not effective on their own, or if the Resident refuses, the RD will then add in a nutritional supplement to enhance the Food First Approach.

Sysco Nourishing News | Winter Issue 2024
What are the benefits and drawbacks to the Food First Approach? Like all approaches, there will be benefits and drawbacks to everything. Benefits: • Food is one of life’s primal and most innocent delights, both satisfying and eternal • Food uses all 5 senses, supplementation does NOT • It connects with all; everyone has to eat A drawback of this approach would be that food and fluid intake in Long Term Care (LTC) can often be challenging due to common medical conditions, such as dementia, Parkinson’s disease and stroke, that may affect one’s ability or desire to eat. How can we promote the Food First Approach? Promoting the Food First Approach may be difficult in your Home. You need to look at all aspects of your meal and snack service and how everything is presented to your Residents. Everything from optimizing the core menu, individual Resident preferences, snacking between meals, your approach, always having options, and your multidisciplinary approach, are all very important. See below for things that we can do to promote this approach: 2 choice at meal

How can we promote the Food First Approach over the Holidays? Here are just a few examples: HEHP Hot Chocolate with Whipped Cream & Cinnamon To a hot cup of hot chocolate, add in a scoop of your protein powder and top it with whipped cream & cinnamon. HEHP Eggnog Pudding Using a simple instant vanilla pudding recipe, sub eggnog for the regular milk you would use. Incorporate food in activities that your Home hosts over the holidays Examples of this could be Holiday baking and decorating, offering snacks and refreshments at holiday wreath making, stocking stuffing, Christmas tree decorating, holiday dance and carol singing. Jazz up the snack cart for the holidays. If you are using a special cart for snack service, Residents may find it more intriguing and exciting to get a snack. Colour and choices can go a long way in promoting Food First! Food is to be enjoyed by everyone. It is a time for Residents to socialize and share with fellow Residents, staff, and family. If we work together and offer Food First in a more positive way, we can watch our Resident’s flourish!

Making Every Momen Chronicles of a Butter (Part 2) The Butterfly Approach is designed for dementia care settings and focuses on delivering emotion-based, person centered, care that connects people in a dignified, holistic and human way. The Approach is proprietary to Meaningful Care Matters (MCM), an international company supporting culture change within health and care settings. The Approach appealed to the Responsive Group because of its ability to bridge the emotional isolation captured in its 2017 Empathy video Could a greater miracle take place than for us to look through each other's eyes for an instant. Scan the QR code to watch the video:
The video portrays the breadth of emotions and tribulations individuals carry inside - often unbeknownst to others - and offers a glimpse of the value of human connectedness, all the more true and needed as a result of the Pandemic.
In this story we listen to, see and celebrate the voices of family, residents and team members of three Ontario Long Term Care (LTC) Homes, operated by the Responsive Group, all of whom recently achieved Butterfly Accreditation, an important milestone in their continuing quest to improve resident quality of life.

York) and The O’Neill Centre (Toronto) each commenced their implementation of the Butterfly Approach during the Covid Pandemic - choosing to undertake a profound, personcentered, culture transformation journey rooted in deep human connections. “The journey from personcentered theory to practice has been transformative in so many ways. We’ve learned so much about ourselves and each other throughout all of the education, the team building activities, and the engagement of people living in and visiting our home; together we’ve built a stronger foundation of trust and excitement for the Butterfly approach and beyond.

Welcome to O’Neill Centre’s Butterfly “Willowvale” Home Area......
Two-way emotions and interactions between team members and residents are what matter most - sentiments are caring and real……. In the words of frontline team members, a torrent of humanity is unleashed:
Erin’s Bio as shared in the Home Area
Our final accreditation review day was an opportunity to showcase the many changes that have sparked a renewed sense of purpose, passion and people connecting with each other in deeply human ways. The Butterfly light shone bright and true, and we can’t wait to embrace these changes throughout our entire home!” While each Butterfly home is unique in its look and feel, there are some core essentials which they all share:
Something needed to come loose. I didn’t think that I needed it but now I see. I can breathe and I can live. This place is my 2nd home and I love the people living here. Butterfly gave that to me. It gave me hope. “Butterfly helps me express feelings of love, and understand [residents’] emotions and feelings.” “Butterfly is beautiful. We can love now… we love as a profession.” “Finally, I have discovered the best part of me through this project - LOVE. I have so much of it that I want to share, and that makes coming to work enjoyable.”

And in the words of family members...
Dundurn Care Centre
Hawthorne Place Care Centre
“It really does mean a lot to my mom and I think the other residents here that there’s that sense of caring… It’s quite lovely to watch my mother interact with the staff here because I think in her mind they’ve become family for her, and she really likes them.” “Someone on staff gave my mother a therapeutic dog, and she’s so attached to that, and it’s really brought her out of her shell. She used to just want to be in her room all the time, but now that she has this little puppy she loves to sit in the hallway or sit in the lounge, interact with it, show it off, and it gives her a sense of purpose… what it’s done for her, in terms of her personality, it’s fantastic.” ....Time is taken.... to spend and savor together.... .... And work gets done.... tasks get completed.... all in a less scripted pace and manner....

Including transformation of “traditional nursing stations” into collaborative spaces... .... All leading up to Accreditation Day for each home.
“I enjoy hearing [the residents’] stories and sharing my stories.”

Pure joy at Hawthorne Place on Accreditation Day!
Sysco Nourishing News | Winter Issue 2024
Accreditation becomes the starting point for a new normal of emotionbased, person-centered care where team members bring their full selves to work. In the words of Charity Morrison, Personal Support Worker at Hawthorne Place Care Centre, “allow the person they are caring for to be themselves and have autonomy over their care. It’s taking time to get to know the whole person [their] personality, likes/dislikes, preferences, life stories. [It’s] asking questions that begin with who, how, what, where, why, so that they have a chance to speak their truths.”
