Netcare Union Hospital leads the way in cardiovascular treatment
On Monday, 24 June 2019, a cutting-edge new procedure, which uses ultrasound shock waves to ‘crack’ highly calcified deposits and blockages within coronary arteries, was successfully performed for the first time in Africa at Netcare Union Hospital in Alberton. The Shockwave Intravascular Lithotripsy (IVL) procedure, as it is known, was performed on a 79-year-old Gauteng patient, who had severe coronary artery disease (CAD) and had previously suffered a heart attack. It was undertaken by a team led by interventional cardiologists, Dr Chris Zambakides and Dr JeanPaul Theron, in the cardiovascular centre’s catheterisation laboratory at Netcare Union Hospital. Dr Zambakides and Dr Theron explain that the new Shockwave IVL procedure enabled the team to successfully clear a way through the patient’s severely calcified and blocked coronary arteries, so that these could be successfully supported with stents, which are small mesh tubes that are used to support and treat diseased and narrowed arteries. “The team is most impressed with the results we were able to achieve with the new procedure. The Shockwave IVL system will enable us to tackle even the most challenging of calcified coronary arteries, and reduces the risks associated with traditional coronary angioplasty and stenting in highly compromised cardiovascular disease patients,” says Dr Zambakides. “The patient was also immensely pleased with the results of the Shockwave IVL procedure, which took some 90 minutes to complete, and says that he almost immediately felt relief from the severe chest pains, fatigue and other debilitating symptoms he was experiencing. Indeed, as we were leaving the theatre afterwards, he surprised us all by announcing that he felt ready to go home,” he added. Dr Zambakides explains that severely calcified

They are with Suzanne Wallace, director: medical education and scientific communication at Shockwave Medical. and diseased coronary arteries can make coronary interventions such as traditional angioplasty and stenting considerably more difficult and significantly complicates the procedure. He says not only can this be associated with higher complication rates such as rupture but also with poorer long-term outcomes. “Acutely it may be associated with stent underexpansion and in-stent restenosis [ISR], which is when a blocked artery that was opened up with angioplasty and stent becomes narrowed again. In this case, the patient had occlusions, or blockages, of between 70% and 90% in various coronary arteries.” Dr Zambakides says that due to the patient’s age and the presence of comorbidities, or additional medical conditions, surgery was not
a viable option. Cracking the calcium with a high-pressure traditional angioplasty balloon was also ruled out in favour of the new ultrasound shock wave approach because of concerns of complications due to the large amounts of calcium deposits present. “While rotablation, which involves the use of a tiny drill-like device, is suitable for grinding the calcium out of smaller vessels, it is extremely difficult to achieve the necessary pressure needed to break up severe calcifications in major coronary arteries during an angioplasty procedure,” he adds. “The Shockwave IVL system involves the use of a special integrated balloon with electrodes attached that produce localised sonic pressure waves to ‘hammer’ at the calcium on the arte-
rial wall, and breaks it up. After this, the IVL system’s integrated balloon is expanded, making placement of the stents considerably easier and safer in these patients. As the technology safely selects and fractures the calcium, any trauma to the soft tissue is minimised.” According to Dr Zambakides, the Shockwave IVL procedure is ideal for patients with heavily calcified lesions, particularly where these deposits are thick and subintimal, or deeper down in the vessel. “The severity of calcification is not always obvious but may only be visible once you perform an angioplasty. In such an instance, instead of persevering with the angioplasty stenting procedure, we recommend now rather switching to the Shockwave IVL procedure in these types of patients, which make up about 5% of those seen by cardiologists.” Glenda Pakes, director of ParagMed, the distributors of the Shockwave IVL system in South Africa, says that to date, more than 3 000 of these procedures have been performed globally, with the bulk having been undertaken in Europe and New Zealand. According to Pakes, they have been completed in patients between the ages of 60 and 80 years with a success rate of 99%. Netcare Union Hospital’s general manager, Esmé Abrahams, congratulated Dr Zambakides and Dr Theron for successfully completing South Africa’s first Shockwave IVL procedure and “making this safer new treatment option available to our patients with advanced cardiovascular disease”. “Netcare Union Hospital’s cardiovascular centre is served by some of the leading cardiovascular specialists and has always been among the most innovative and cutting edge units of its kind in the country. We are most gratified to now be able to offer this advanced new treatment option for these complex patients,” she concluded.

The price of a litre of 95-octane gasoline will decrease to R15.81 a litre in the economic hub of Gauteng from midnight on July 3, compared with R16.76 in June, the Mineral Resources and Energy Minister Gwede Mantashe said Friday in an emailed statement. The wholesale price of diesel will fall by 0.75 rand and the cost
of kerosene, used for cooking, and liquid petroleum gas will also be lower. The new gasoline price will be lower than the level of July 2018, helping to stabilise inflation which was at 4.5% last month.

Habana embracing life after rugby

The moment Springbok legend Bryan Habana walked into a recent function at the British E m b a s s y, his erstwhile
teammate Jean de Villiers recognised immediately the one element of life after professional rugby that all players need to come to terms with. Clothing. “The only thing I could fit into was my old number two Springbok suit. As I got to the function Jean saw the suit and said, ‘Whoa’. He opened my suit jacket and said, ‘This is one of your number twos isn’t it?’ He caught me out properly,” says Habana as he reflects on the transition he has made from a career as one of the most decorated rugby players in history to what he terms as “life outside that bubble”.

I’ve also worked with some big brands throughout my career, and was conscious of creating my own brand that corporates would want to continue working with after my rugby career was finished. “So in this 18-month period that I’ve given myself to discover life after rugby, I’ve been fortunate to not have been too stressed about what’s next for me. I still don’t know what I’m going to be doing fulltime, but at the present time I’m really enjoying the opportunities, like being part of this Vodacom Red Final Tour experience.” And he’s certainly embracing this next phase of his life, wherever it may lead.
