Did you ever wonder why a baby’s skin is so soft and dewy? It’s because a baby’s skin cells turn over rapidly – every 20 days – as opposed to an adult’s which only turns over every 50 days. “That means that every 20 days, a baby is getting a whole new layer of beautiful, healthy, hydrated skin cells making their skin soft, smooth and radiant,” Dr. Torok says. Also, babies are not as exposed to as many free radicals as adults. This rapid cell turnover lasts until the late 20s and early 30s, then cell turnover slows down. During puberty, hormones come into play in the form of oil and sweat glands going into overdrive producing skin conditions such as acne and oilier hair. Plasma Powerful New Skin Regeneration Technique Now Available Plasma Technology Gives ‘Truer Regeneration’ Than Laser; Can Be Used Around the Eye
Plasma, a powerful new skin regeneration program, is now available at Trillium Creek. The same technology that runs your plasma television or flourescent light has been harnessed to effectively work at the skin’s surface to stimulate remodeling of the skin’s architecture, replacing damaged collagen with significant amounts of new collagen. Unlike lasers, plasma can be used around the upper and lower eyelids – a problem area for many women seeking skin improvement. “We have had wonderful results with this procedure,” says Trillium Creek Medical Director Dr. Leonard Torok. “It provides a more true regeneration of the skin. Once the skin is healed after the procedure, it has a new thickness, it’s healthier with better elasticity.” Plasma can effectively treat fine, moderate and deep wrinkles, superficial skin lesions common on sun-exposed areas, keratosis (the brown patches that result from sun exposure), improve skin tone, texture and elasticity and is pending FDA approval for treatment of acne scars. Dr. Torok says plasma is an excellent choice for the neck and upper chest area as well. He is still using the erbium laser for deeper scarring and wrinkling. In some cases, he uses a combination of the erbium laser and plasma. The plasma technology delivers plasma energy deep into the top layer of the skin while leaving the skin’s surface intact for several days following treatment. As the new skin emerges, the old skin will start to shed. With less trauma than cutting procedures, healing is rapid. Dr. Torok starting using the technology on a test basis in August. Trillium Creek has been offering it to patients since November.
The procedure is simple and relatively quick: A topical anesthetic is applied. The area is then treated with the plasma, which is applied similarly to laser treatments. Treatment of the face takes 30 to 40 minutes; face, neck and upper chest take about an hour. The patient is sent home with homeopathic remedies to reduce discomfort which lasts about a day. Within a week, the old skin starts to slough off and the new, rejuvenated skin appears. “What’s nice about plasma is the skin continues to improve over a six-month period,” Dr. Torok says. “The new skin has a more youthful appearance.” Trillium Creek is now using the plasma technology three times more frequently than lasers for skin rejuvenation. more tissue. Once the margins of the wound are free of cancer, it is sutured, dressed and the patient is sent home and returns in a week to have the sutures removed.
“Even people without malignant melanoma should be outraged by this,” says Dr. Patrick Killian, director of the Mohs Surgery Center at Trillium Creek. “It is inconvenient and very frustrating for the patient to have to come in more than once, especially if they are working and need to take vacation time, or are retired and must find a ride.”
“Because the procedure removes the smallest amount of tissue possible, the size of the scar is minimized. Also, more importantly, we know that all the cancerous cells have been removed,” Dr. Killian says. TC
The Center for Medicare & Medicaid Services (CMS), the government agency that administers Medicare, says that too much money was being allocated to skin cancer treatment, and the rationale was to make treatment harder to get. “The American Academy of Dermatology, the American College of Mohs Surgeons and the American Society of Dermatologic Surgeons have been fighting this for a year, but with such a small subspecialty, they had no lobbying power,” explains Dr. Killian.

The ruling is especially dangerous in light of skin cancer being the most common form of human cancer. The American Society of Dermatologic Surgery estimates that every 67 minutes someone dies from melanoma. The term “skin cancer” refers to three different conditions: basal cell carcinoma (BCC), squamous cell carcinoma (SCC) and melanoma. BCC and SCC are referred to as nonmelanoma skin cancers and are the most common forms of skin cancer. They are rarely life-threatening, but can be locally destructive to tissue. Melanoma is generally the most serious form of cancer because it can metastasize or spread through the body quickly. Since the 1980s, the most successful procedure for removing nonmelanoma skin cancers has been Mohs micrographic surgery. “The procedure has a very high success rate in eradicating all cancer cells from the site,” Dr. Killian says. In Mohs surgery, the area in which the skin cancer is found is numbed using a local anesthetic. The visible tumor is debulked or scraped with a curette, an instrument that doesn’t tear normal tissue. A 1 mm to 2 mm area of normal tissue is removed from around the tumor. The wound is temporarily dressed, while the tissue sample is sent to the lab for processing. Dr. Killian examines the tissue for cancer cells, making sure all of them have been removed. If cancerous cells remain, he will go back and remove a little

Dr. Anya Enriquez: ‘My greatest joy is to know I made someone feel better’ There are no “silly” or “unimportant” patient questions for Dr. Anya Enriquez. “I want my patients to know that I am here to answer their questions and find answers for their health problems when they come to see me,” she says. “I understand what it’s like to be a patient and how important it is to understand one’s condition and the plan for treating it. My goal is to make patients’ lives a little better. That is the favorite aspect of my job.” Dr. Enriquez joined Trillium Creek from The Cleveland Clinic four months ago. She says the past four months “have been wonderful.”

We can make a difference in people’s lives quicker without all the red tape and bureaucracy.” Her patients are also some of her greatest teachers. “I learn from my patients every day,” she says. “There’s always more to learn. Skin disease doesn’t always present on a patient the way it looks in a textbook. My greatest professional joy is to know I’ve made someone feel better.”
Born and raised outside of New York City, she started her career as a certified public accountant in Dallas, but found it lacked the personal connection that she finds so compelling in medicine. Taking her pre-med requirements at the Harvard Extension School in Cambridge, Mass., she met her husband, Louie, a former attorney. They both went on to graduate from the prestigious Yale Medical School. It was hard leaving her East Coast roots and large, extended Hungarian family, but she was offered a residency at The Cleveland Clinic in 2003. Louie, a radiologist, still practices at The Cleveland Clinic. As a working mother, wife and physician, she says she can easily relate to her patients and their concerns. “I think my patients will find that I am very easy to talk to. I’m energetic and have a good sense of humor. I love to smile. I like to know my patients on a personal level,” she says. “I also know what it’s like to be a patient and to have all of your questions answered by your provider.” Dr. Enriquez’s professional interest is preventive medicine – especially protecting the skin from the harmful rays of the sun. She says educating people about sun protection and avoidance is one of the most important things she, as a dermatologist, can do. “For women, applying a sunscreen in the morning as a moisturizer, an antioxidant during the day and a retinol at night, is a great way to protect the skin,” she says. In her free time, Dr. Enriquez enjoys reading, going to the movies, spending time with her family (including two dogs—a German Shepherd named Anka and a Beagle, Scout), skiing, traveling, ice skating with her daughter, Acadia, 6, and reading with her son, Emmett, 2. And unlike so many people who make grandiose resolutions at the beginning of the year, Dr. Enriquez says she never makes resolutions. “I try to live my philosophy every day – to be a better mom, wife, doctor and friend on a daily basis.” TC

Maybe you’ve heard it referred to as the “mask of pregnancy,” or seen someone with a uniform brown skin discoloration. Melasma is a light-todark skin discoloration found on the sun-exposed areas of the face. It is sometimes referred to as the mask of pregnancy because some pregnant women get the condition from hormonal changes. It can also occur in women on birth control pills and those taking hormone replacement therapy during menopause. While it can affect anyone, young women with brownish skin tones are at greatest risk. Sun exposure is also a strong risk factor for melasma which makes those living in tropical climates especially at risk. Melasma can fade on its own over time, once a child is delivered, you’re off the pill or through menopause. It is not a dangerous disease, but is definitely a cosmetic concern. Trillium Creek offers several treatments for melasma, including retinoids, cosmeceuticals, peels and laser treatments.
Retinoids TriLuma™ cream has three active ingredients, fluocinolone acetonide, hydroquinone and tretinoin. According to the manufacturer, using these three ingredients in combination gives a more satisfying result than using only one. Environ’s Evenescence Clarifying Lotion is a new product available at The Boutique at Trillium Creek. The pH in this product is controlled at its optimum level which provides a safe and effective product. SepiWhite-MSH™ controls the formation and dispersion of melanin which is responsible for the formation of dark blemish marks. Witch hazel provides astringent and antioxidant properties. Peels The Vitalize Peel® is a peel best-suited for the treatment of melasma. The Vitalize Peel® contains a special formulation of a unique blend of peeling agents: alpha-hydroxy acids, beta-hydroxy acids, resorcinol, and retinoic acid. The Vitalize Peel® causes chemical exfoliation of the epidermis, or outer layer of the skin. Skin regeneration follows this chemical peel and results in a fresh, organized, healthy epidermis. Laser Treatment Fractionated photothermolysis (the Fraxel laser) can dramatically improve melasma with minimal downtime. For best results, usually three to five treatments are needed. Physician Assistants An Important Part of the Medical Team Two of the more familiar faces at Trillium Creek are Francine Johns and Darla Hall. They are the practice’s two certified physician assistants. They are the “second set of hands” for Drs. Helen and Leonard Torok and Anya Enriquez. Physician assistants have undergone two years of formal training and are licensed to practice medicine under the supervision of a licensed physician. During their training, physician assistants attend many of the same classes as medical students. To earn their certification, they must meet a defined course of study and testing by the National Commission on Certification of Physician Assistants. At Trillium Creek, training doesn’t stop, however. Both Ms. Johns and Ms. Hall continue to undergo intensive post-certification medical training under Dr. Helen Torok and homeopathic training under Dr. Leonard Torok. “Drs. Helen and Leonard are always making sure we know the latest techniques, drugs and conditions occurring in dermatology and homeopathy,” Ms. Hall says. “We are in an environment where learning is continuous.” Both physician assistants perform full body exams, examine toenail fungus, treat acne, rosacea, contact allergies, hair loss, warts, eczema, psoriasis and other skin problems. They also perform diagnostic, therapeutic and preventive health care services, take medical histories, order and interpret laboratory tests and X-rays and make diagnoses under physician supervision. They also see patients on follow-up visits from the physicians.
Ms. Hall graduated from the physician assistant program at Marietta College. “I love many things about my job, but my patients are the best!” Ms. Hall says. “I see a range of patients from infants to the elderly and I love them all. It’s rewarding to be able educate and arm them with what they need to feel healthy. I enjoy the people I work with, too. It’s easy to come to work! Dermatology is especially rewarding for me because I’m able to see and feel the skin to make a diagnosis. I don’t always have to rely on special testing.” TC

Intense Pulsed Light “IPL is a light-based system where specific colors of light are put into the skin with a short pulse,” Dr. Torok explains. “Red light is specific to removing abnormal blood vessels, brown light removes brown age spots, while the normal surrounding tissue is unaffected.” IPL has excellent results with spider veins, enlarged facial veins, rosy cheeks, mild to moderate acne, freckles, age marks and facial wrinkles.

“IPL has the secondary benefit of shrinking the sebaceous (oil) glands, which decreases the size of pores, as an added aesthetic benefit,” Dr. Torok says. “For the same reason, it is also a good treatment for controlling mild-tomoderate acne. IPL is combined with extraction of the sebaceous glands for a longer lasting reduction in acne. The IPL treatment can be combined with topical applications that increase the effect of the treatment.” IPL also reverses sun damage to the skin. Preliminary tests have shown it to be able to remove approximately 70 percent of future-developing skin cancers in those areas treated with IPL.
Erbium Laser The erbium laser can be absorbed by water, so it can affect all tissues that contain water. It is mainly used for remodeling tissue that has been scarred or has become wrinkled with age. “The erbium laser allows us to smooth the skin around scars. Since the skin replaces itself every three weeks, the erbium laser creates a new wound with a smooth surface. The skin then replaces itself following the smooth contour of the new wound,” he says. The erbium laser is used to remove scars from acne and other wounds as well as the laxity and pigmentation problems associated with aging skin. Continued to page 11

Be Hair Aware Say Goodbye to Plucking, Tweezing and Waxing Laser Hair Reduction Can Diminish Hair Growth by 70 Percent
It’s the time for winter getaways and thinking about spring. Retail stores are already showing swimwear and warmer weather clothes. As the seasons for exposing more skin rapidly approach, is your skin as smooth and hairfree as possible?
Hair shouldn’t be too long and it’s OK to shave a day before the treatment. However, once the laser treatment is done, there should be no waxing, plucking or use of depilatories. Tanning should be avoided. It can cause hyperpigmentation or skin discoloring.
If not, laser hair reduction can give you fabulous, long-lasting results, according to Jessica Canfield, LPN, a laser hair reduction specialist at Trillium Creek.
Trillium Creek has two state-of-the-art lasers – The Aesthera PPX and the Sciton.
Laser hair removal can be performed on many parts of the body including the upper lip, chin, underarms, abdomen, chest, back, face, legs, top of the feet or toes, backs, ears, sideburns and can help with folliculitis (inflammation of the hair follicle.) The eyebrow area, however, is too close to the eye to be able to use laser technology safely.
“The Sciton is used for darker-skinned patients,” Ms. Canfield says. “It has one head attachment, but allows me to choose different parameters based on the skin type. The Aesthera has different power and vacuum settings and uses different heads or tips.”
“Laser hair reduction usually allows for 70 percent permanent reduction of hair,” explains Ms. Canfield. “If it does reappear, it’s never as thick. There are millions of dormant hair follicles under the skin and the laser only affects those in the first stage of growth.”
Certain health conditions preclude laser hair reduction. They include a history or current case of cancer, an active infection, a history of herpes, lupus or porphyria, immunosuppressive diseases or taking medication for immunosuppressive diseases, hormonal disorders, taking anticoagulants, broken or very dry skin, a recent suntan, women who are pregnant or nursing or anyone taking Accutane.
Laser hair reduction is quick – usually from five to 10 minutes for the chin or upper lip to an hour for a full back or both legs. Dark hair reacts better to the laser. The light from the laser attaches to the pigment in the hair, allowing the follicle to be destroyed.

Bouncing Baby Basics Continued from page 1 Dr. Torok advises against using premoistened, heavily scented baby wipes. They can irritate the skin rather than soothe it. She also advises against using baby powder with cornstarch. “Cornstarch contains sugar and it can cause yeast to grow irritating the skin,” she says. Choose a powder with talc instead. Moisture that collects in the folds of baby’s neck can also be problematic. Make sure the area is washed and kept dry. A thin application of powder without cornstarch will help keep the area dry. Milliaria or prickly heat is a common rash seen on some babies. It is caused by plugged sweat pores. A baby doesn’t perspire on the body and loses heat from the head and scalp only. “A baby’s body temperature doesn’t regulate easily,” Dr. Torok says. “It takes a few months for them to thermoregulate. They get overheated and chilled easily and a lot of their heat escapes through the top of their head.” In their first six months, a baby’s immune system is also developing. During this time they are more prone to infections. Keep babies indoors and away from crowds during flu and cold season. Wash your hands before handling a baby. If you have a cold or flu, your hands can spread the infection to the baby. Cradle cap is a condition seen in the first six months of life. The thick, greasy patch on baby’s scalp is actually a form of dandruff. Baby will usually outgrow it, but it may reappear at puberty. Eczema, an inherited, itchy allergy of the skin, usually appears after six months. It is best treated by using mild, bland cleansers and lotions such as Cetaphil or CeraVe. Heavily scented products, soaps, detergents and lanolin must be avoided with this condition. Diaper rash occurs if the baby’s diaper area is not kept clean and dry. It is best to use a gentle soap or cleanser, such as Dove for sensitive skin or Cetaphil, to clean the area. Avoid scented products, especially lavender oils and lotions on boys and products with lanolin. Products containing glycerin and shea butter are best. Cold weather is a dehydrator and robs moisture from baby’s skin. Dr. Torok suggests keeping all of baby’s skin covered, even his or her face, with a light blanket. A light application of Vaseline® or CeraVe moisturizer can help keep the skin as soft and dewy as it was at birth. TC

Lipodissolve Lipodissolve is a nonsurgical treatment that uses a combination of medications to dissolve unwanted fat in any part of the body. Results can be seen in as little as one or two sessions. A combination of phosphatidylcholine and other medications administered through a series of micro-injections permanently dissolve fat. The fat cells break down within a few weeks and are removed from the area by a natural healing process.
