dear friendS, Our Annual Report theme, Connections & Partnerships: Creating Positive Change, is particularly appropriate to describe this past year. We have made important changes internally to create increased efficiencies and build a greater sense of connection among programs and staff. We have also developed strategic partnerships to better position Family & Children’s Agency for the period ahead. Here are some brief descriptions of these partnerships: We established a new partnership with VRI, Inc. for our personal response service. Now, seniors can not only access an emergency response service, but also receive reassurance calls through VRI’s Care Center. VRI is also assisting us as we promote our own brand—personal alert. We established a partnership with Mid-Fairfield Child Guidance Center and the Norwalk Community Health Center to provide early intervention services for children under age five who are at risk for significant behavioral health issues. Called child first, this innovative service delivery model receives core funding from the Robert Wood Johnson Foundation. We established an information technology (IT) collaborative with Family Centers and FSW to prepare our agencies for the requirements of health care reform. This innovative partnership, which received partial funding from the State of Connecticut, will eventually lead to an integrated IT system and a common structure that will provide for the IT needs of each agency.

We enhanced and expanded our relationship and partnership with Norwalk Community College (NCC). NCC is providing additional classroom space at the college that has enabled our After School Program to expand by 30% this year, while also providing office space for our After School staff. NCC is also, for the first time, offering on-site social work services for its students, provided by a Family & Children’s Agency social worker. Our relationship with the Norwalk Housing Authority (NHA) is also expanding as a result of a choice neighborhoods planning grant secured by NHA with input from us. Professionals from Family & Children’s Agency are conducting comprehensive assessments of the human service needs of each resident of Washington Village as part of this major neighborhood revitalization plan. Over the course of this past year we have engaged all the various stakeholders of Family & Children’s Agency in the process of establishing our Strategic Plan for the next three years. Meetings and focus groups have been held with Board Members, all levels of staff, community providers, funders, and clients. The result of this inclusive approach is that all participants have a stake in the plan moving forward. Through the process, priorities have been revised to more accurately reflect our future emphasis. For example, increased visibility will continue as a priority area, but will now also include increased engagement at both the Board and community level. staff development will now be viewed as the development of Human capital through enhanced supervision and clearly defined training plans. We will also continue to deliver the highest level of outcome driven services while maintaining our strong emphasis on integration and collaboration.

As the Agency continues to navigate during these turbulent times, it is important that we continue to seek partnerships that increase our effectiveness while we continue to identify opportunities to further integrate and connect our programs and staff. You, our volunteers and donors, provide the critical support and connections we need as we look to create positive change for the more than 10,000 clients we serve. children & families Working with children and their families has been at the heart of Family & Children’s Agency since our inception in 1942. Our goal is to help children thrive in safe, nurturing, and strong families. Whatever its composition, the family remains a source of strength, growth, respite, and trust for most people. We help build families through our Adoption and Specialized Foster Care programs. We help strengthen family relationships through our Counseling, Family Support, and Parent Education programs.

Reuniting a Family The first time Wilbur, who is now 10, went into Foster Care, he was three years old. He remembers how happy he was to be reunited with his mom two and a half years later. “I was really, really happy. I was so happy. I was back with my mom,” said Wilbur. Almost two years later, Wilbur was once again in Foster Care as a result of his mother’s drug addiction. “It was worse for me the second time,” said Wilbur. “I saw what happened. I couldn’t get that image out of me.” Over the next year and a half, Wilbur and his two brothers and sister were in and out of four foster homes. Their mom, Delia, age 31, was born and raised in Guatemala. Her childhood was one of abandonment and abuse. At the age of four, her mother moved to the United States and left her to be raised by relatives. She remembers living with her father, who was abusive, and then living off and on with other relatives. “My life was so crazy when I was little,” she said. “There was no stability. I didn’t stay with one family.” At 19, Delia was introduced to cocaine by her former boyfriend and started using it regularly. Two years later, she moved to the United States, hoping to reconnect with her mother who was living in New York. “I just wanted to see her; I used to miss her a lot when I was little,” she said. Although she found her mother, there was little connection between them. Delia continued living with her boyfriend, using drugs, and moving from one part of the country to the next. As the years passed, she and her boyfriend started having children, but also continued using cocaine. After a neighbor reported her case, the Department of Children and Families (DCF) removed her children for the first time, and Delia started down the path to recovery. She recalls how she felt the first time her children were removed, “I felt terrible. I wanted

to kill myself. But then I thought, ‘they were alive and I needed to work really hard to get them back.’ ” Unfortunately, soon after they returned, she relapsed. “Their dad brought drugs back into the home and I relapsed,” she said. During her second rehabilitation program she made an important realization. “The best thing I could do, is not come back to be with their dad,” said Delia. “It was my best decision. Everything changed as a result of that decision.” After completing her rehabilitation program, Delia was referred by DCF to the Family Support & Intervention Services at Family & Children’s Agency. “I was struck by her strong will and motivation to once again become a parent to her children,” said Monica D’Auria, a reunification therapist in the program. “She was able to maintain her sobriety and provide for her children’s needs, thus ultimately being reunified with her children.” As a condition of the reunification, DCF required that Delia secure an apartment. “I worked so hard to get that first apartment,” she said, as she remembered working late nights cleaning an office building. “The reason for my life is to have my kids with me,” said Delia. “It’s the reason why I breathe, why I stay off drugs.”

Unfortunately, the apartment turned out to be infested with vermin. Due to her inadequate housing, and faced with the possibility of having her children removed a third time, DCF again referred Delia to Family & Children’s Agency. In October 2009, Truly Rafalko, a caseworker for Family & Children’s Supportive Housing for Families program in Bridgeport, started working with Delia. “Through a partnership with The Connection,® Inc., we are providing her with a housing subsidy until she receives her Section 8 voucher,” said Truly. As part of the program, Truly makes weekly visits to Delia’s home and works with her to make sure her bills are paid and the house is clean and in good condition. “She does what she needs to be doing,” stated Truly. Delia has since entered a new relationship and gotten married. “He’s the best guy. My kids love him so much.” Wilbur is quick to talk about the changes in his life: how he has a dad that takes him fishing, how his grades have improved, and about one of his favorite family activities. “I love when we are all in the living room and watching America’s Funniest Videos,” said Wilbur. “I feel like I’m where I’m supposed to be.”

Youth At Family & Children’s Agency, our youth programs operate under the philosophy that all young people have strengths that need to be built upon. We believe that our investment of time, energy, and resources into the positive development of young people will have enormous benefits for them now and in the future, as well as for the communities they call home. Through our After School, Mentoring, and Counseling programs, youth are given opportunities to better understand and explore their strengths.

uncovering eXiSting StrengthS “I first met Daje when she was young, about seven or eight years old,” remembers Dhani Spinola, Coordinator of the After School Program at Family & Children’s Agency. “I was working at another agency at the time and helped arrange sibling visits between Daje and her brother.” Little did Dhani or Daje know how their lives would become intertwined over the next four years. The next time they met, Dhani had just begun working as a mental health counselor in the Intensive In-home Child & Adolescent Psychiatric Services (IICAPS) program at Family & Children’s Agency. “Daje, who was either nine or 10, was one of my first clients in the IICAPS program,” explained Dhani. Daje’s life at that time was in turmoil. Over the course of her young life, she had several out of home placements and was struggling with an extreme fear of abandonment. As a result, she would frequently break down and sob uncontrollably or throw tantrums.
“I couldn’t calm her down,” said her mother, Chennel. “I didn’t know how to comfort her.” The situation became so severe that on a couple of occasions, Chennel called 911 for help. Daje would then enter local mental health facilities where she would spend a couple of weeks before returning home. “Her situation at the time was one of crisis,” recalls Dhani. Night after night, Dhani and Stephanie, the other IICAPS clinician assigned to work with Daje, would be paged for help. “We found that we couldn’t help the family build the skills they needed because we were too busy dealing with crisis situations,” said Dhani. After one particularly intense crisis, Dhani and Stephanie helped to facilitate long-term hospitalization for Daje at the Boys & Girls Village. “It is really hard to put your child into a hospital,” said Chennel. “But I didn’t understand the problem or how to fix it.” With Daje at the Boys & Girls Village, both she and her mother started learning the skills they needed in

order to communicate and to help each other. Dhani recalls, “When we learned that Daje was going to be discharged from Boys & Girls Village, we started working with both Daje and Chennel individually. Our job was to help them translate the skills they learned at the Village into real life; and to learn how to communicate more effectively with each other.” Over the next year, as Dhani and Stephanie worked with Daje and Chennel, they started to see positive changes. A major breakthrough came when their conversations changed from “if we are going to make it” to “how we are going to make it.” “We knew something had changed in them,” said Dhani. It was at this point that she knew the family was going to stay together. As the family began to stabilize, and Daje’s risk of being hospitalized again decreased, their case with IICAPS was closed. In anticipation of this transition, IICAPS referred Daje to the After School Program. Daje also started working with a counselor in the Agency’s Behavioral Health program and participating in its Jr. Project Friendship mentoring program. Chennel began attending the Agency's Project REWARD program, an addiction recovery program for women.

As fate would have it, Dhani would yet again have the opportunity to work with Daje. In January 2010, Dhani left IICAPS to become Coordinator of the After School Program. Over the next year and a half, Dhani once again became a source of support and a role model for Daje. In June, Daje graduated from the After School Program, and she is currently attending Brien McMahon High School. “I can speak about my feelings now,” said Daje, who will turn 14 in November. “I can take a joke easier, and I can take being told ‘no.’ I also have some major life skills. If Dhani and Stephanie didn’t come into the picture, I wouldn’t be where I am now. I’d still be in and out of hospitals. I have a better relationship with my mom. I was attached to her, but I couldn’t talk to her. I am now able to tell her everything.” Looking back Dhani said, “I’m touched by the way they opened themselves up to us for treatment. Their willingness to do the work speaks to their survival spirit. If either of them had given up at any step of the way, they wouldn’t be living with each other now. They overcame every obstacle they could face in this life. There is something deep inside of them that said ‘we’ve got to keep trying.’ ”

Adults Family & Children’s Agency believes that no matter where they are in the life cycle, all adults have the ability to reach their fullest potential and to realize life’s possibilities. For many of our adult clients, this means dealing with complicated issues, such as addiction, mental illness, family dysfunction, and homelessness. To meet their specific needs, we offer an addiction recovery program for women, mental health counseling, and a continuum of services for homeless and formerly homeless adults.

Building a New Life “My moods just change like the weather,” said Deborah, who has struggled with mental illness for most of her life. She describes her adolescent years as difficult ones spent in and out of mental health hospitals. “I was really messed up in my head,” she recalls. “I just snapped. I didn’t know what caused it.” She remembers attending West Rocks Middle School in Norwalk and then spending time on and off at Fairfield Hills State Hospital in Newtown. For the past 11 years, Deborah, who is now 44, has been homeless. At times she has lived on the streets, at times, in a shelter; she describes this as living “here and there.” Last January, after working with a Family & Children’s Agency Community Connections Drop-In Center caseworker, Deborah was able to move into a supportive housing unit in Norwalk, where the Agency provides resident support services. “Look at my place, it’s a beautiful place. I’m happy here,” said Deborah. “It’s been a long time since I’ve had my own place; I’ve slept in shelters and on the streets for over 11 years.” With her housing situation stabilized, Deborah is now starting to build a foundation for her new life. “Deborah is learning how to re-enter the community,” said Jennifer Moriarty, Manager of the Agency’s Supportive Housing Program. “For people who have been homeless for so long, it’s about helping them reconnect to other people and the community, and to start trusting themselves and others again.” Jennifer explained that, when working with formerly homeless persons, the first step is to help them make connections. “We start by helping people make small connections. Then, we help them make bigger ones, and hopefully they will start making more and more connections on their own. Eventually, they will have built so many of their own connections that they no longer need our services and will be able to live independent lives in the community.”

Since moving into her own unit, one of Deborah’s most significant reconnections has been with her mother and sister. “My mom wants to come and have a cup of coffee with me,” Deborah told Jennifer a few months ago. “When Deborah’s mother and sister came, they thought her apartment was beautiful,” said Jennifer. “She really needed her family to see her this way. She was so proud of herself.” Deborah is also slowly becoming a member of the community again. She is starting to attend tenant meetings and barbeques in her apartment building. “These are important steps for her,” said Jennifer. “She contributes at tenant meetings and is learning how certain behaviors have an impact on the community. It’s all things you forget when you’re living on the streets. Additionally, she is starting to explore employment options.” As a result of these connections, Deborah’s selfconfidence and trust are starting to grow. “Deborah has come so far, but it takes time,” said Jennifer. “People had given up on Deborah, society had given up on Deborah. So when people see where she is now, they are truly impressed.” Lisa Roberts, a Senior Residential Case Manger at the Agency, has also seen a change in Deborah since she moved in. “I’ve seen an improvement in her self-worth. She is now putting forward an effort and believes there is

a chance that she can live a life like anyone else,” said Lisa. “She doesn’t give up as easily now. She has faith and trust that the system can work for her.” Recently, when Lisa asked Deborah, “How can I help you,” Deborah’s reply was simply, “Don’t give up on me.” Lisa quickly reassured her that she “would never give up on her.” Lisa has also seen a difference in the way Deborah presents herself. “Deborah’s perception of how she presents herself has become more important to her,” said Lisa. “She now takes pride in herself, in her appearance, and how she maintains her apartment.” Jennifer explained that while Deborah was homeless, she became well known to local police and emergency service workers. “Now when they see her, they tell her how well she is looking. When she hears this, her face lights up and she has greater self-confidence,” said Jennifer. As another measure of her growth, Deborah is starting to think about dreams that have been on hold for 11 years. “I’d like to go bowling, out to dinner or maybe to a movie one day,” she said. “It’s not too easy; society scares me a lot. But I can’t give up, I’ve worked hard to get here,” said Deborah. “I just need to keep positive.”

SENIORS Through our Home Care Services, Family & Children’s Agency offers seniors the ability to remain safe and independent in their homes. Both the physical limitations of aging and the increasing impact of medical conditions play a decisive role in whether a senior can age successfully at home. Our services, which range from having the security of a Personal Alert button to having a full-time, live-in home health aide, allow seniors to retain their safety and independence throughout the aging process.

increaSing Safety and indePendence For John, like many seniors who live alone, there will come a time when a decision has to be made about whether it’s safe to remain in their own home amidst ever changing health conditions. Today, for many seniors, the conversation is moving away from whether to age in one’s own home to what resources will it take. After being discharged from the Westport Health Center for Rehabilitation, John’s physical condition mandated that he either bring in outside help, which would allow him to stay at home, or find another housing alternative—such as living with a relative or moving into assisted living. John chose to remain at home. “Currently, there are several options available to people who want to age successfully in their own homes,” said Randi Ederle, Nurse Supervisor, Home Care Services. “The options range from having a homemaker on a regular basis to having a full-time, live-in home health aide. It all depends on each senior’s environment as well as their medical needs,

and the risks that accompany each of these areas.” Randi explained that one of the risks, and maybe the most common one facing adults aged 65 and older, is the risk of falling and the secondary issues that can occur if the person isn’t helped immediately. In fact, research shows that more than one-third of adults aged 65 and older fall each year, and 50% of those who fall require assistance from someone else. A year ago, after having his legs amputated as a result of heart and kidney problems, John knew he needed extra assistance around the house, but he didn’t want to completely give up his independence. With the help of home health aides that came during the day, and regular visits from a visiting nurse, he was able to remain semi-independent in his own home. However, not long after returning home, John started having trouble at night. “Most of the time, I’d get stuck between my bed and my chair,” said John. He still remembers the feeling of panic and helplessness when it first happened. “I’d have to scream for help until someone would

hear me,” he said. Because John lives alone in his apartment, that usually meant yelling loudly and long enough for a neighbor or someone who was passing by to hear him. “While we can’t prevent people from falling,” said Randi, “there is an effective solution for quickly getting help to those who do fall. With our Personal Alert service, seniors are able to push a button that establishes two-way voice communication with our Care Center. The Care Center then contacts a loved one or emergency services depending on the situation.” Basically, she said, “if seniors can get help within the first hour after a fall, they are less likely to be hospitalized and more likely to remain in their own homes.” It wasn’t long after his first fall that John’s visiting nurse recommended that he try Family & Children’s Personal Alert button. “If I didn’t have Personal Alert, I’d have to have someone with me 24/7,” stated John. “John was one of my first clients,” remembers Shari Troost, a Case Manager for Family & Children’s Agency. “I went into his home about a year ago and we talked

about his needs. I showed him how the service works, and we did a trial call. I also did a fall safety assessment that was later reviewed by our Nurse Supervisor.” As part of the fall assessment, Shari conducted a walkthrough of John’s apartment. “We looked to see if his doorways are wide enough for his wheelchair, whether he has properly installed grab bars, as well as a whole list of other things that may cause John problems,” said Shari. Since John received his Personal Alert button, it has become a valuable tool for him. “I think it’s a fantastic thing,” said John. “I’d feel kind of lost without it.” His visiting nurse said, “If anybody has benefited from these services, it’s John.” With the extra security and support of his button, John is now able to rest easier at night knowing that help is just a “push of a button” away. “I’m very comfortable with this service, it’s invaluable,” he said. He is also able to go outside and visit with his neighbors once again. “I can pretty much go anywhere I want in the (apartment) complex,” said John. “I have the security of the system.”
