API represents Active Pharmaceutical Ingredients and holds significant importance in drug development and manufacturing processes.The active pharmaceutical ingredient (API) is a medication's fundamental element that elicits the desired therapeutic outcome. Simply put, it is the substance that drives the drug's functionality. For instance, in a painrelieving medication such as ibuprofen, the API is ibuprofen itself. APIs can be created synthetically through chemical synthesis or sourced from natural origins, and they possess the capability to address various diseases and medical conditions.
In pharmaceutical manufacturing, active pharmaceutical ingredients (APIs) assume a pivotal role.
Therapeutic Function
APIs are primarily responsible for eliciting the therapeutic effects of drugs by interacting with the body in a specific manner to address diseases or conditions. The efficacy of a drug hinges significantly on the quality and potency of its API.

APIs are amalgamated with inert substances, known as excipients, to formulate the final drug product. Others may use strategies like tax exemptions, tariff discounts, or varying distribution channels for originator brand and generic drugs. Generally, the price components within a pharmaceutical supply chain can include:
Recent media reports indicate that prices of fundamental Active Pharmaceutical Ingredients (APIs) like paracetamol, azithromycin, ibuprofen, and ofloxacin have decreased by approximately 1520% over the past month. This decline follows a period of sharp price increases, exceeding 100% from pre pandemic levels, attributed largely to inconsistent supply from China during numerous Covid related lockdowns.Experts in the industry view this reduction in API prices favorably as it alleviates pressure
on profit margins. However, they caution that the decrease may not be substantial enough. Prices are expected to further stabilize with increased supplies from China and the initiation of production by Indian pharmaceutical companies under the Production Linked Incentive (PLI) scheme.
According to data from the India Brand Equity Foundation (IBEF), India is home to 500 API producers, constituting around 8% of the global API industry. The Indian pharmaceutical sector is projected to reach USD 65 billion by 2024 and USD 120–130 billion by 2030, as per the Indian Economic Survey 2021.To mitigate dependency on Chinese imports, the government has encouraged domestic API production through the PLI scheme, while API companies are diversifying their supply chains for sourcing Key Starting Materials (KSMs) and intermediates.The Indian pharmaceutical industry currently contributes approximately 1.72% to the nation's GDP, with the domestic market valued at USD 42 billion in 2021 according to IBEF.
Fluctuations in the prices of raw materials impact the profit margins of pharmaceutical drugs The pharmaceutical industry operates dynamically and relies significantly on the availability of raw materials. From sourcing raw ingredients to receiving finished products, the pharmaceutical sector demands exceptional precision and attention at every phase. Even a slight shortage of raw materials can pose a challenge, potentially causing delays or halting production altogether.Over the past years, the COVID19 pandemic has triggered a global economic crisis, leading to supply chain disruptions and sharp increases in raw material prices. This health crisis has highlighted underlying structural supply issues arising from fragmented and insecure supply networks. Disruptions and slowdowns in industrial chains, along with rising transportation and energy costs, contribute to fluctuations in pharmaceutical drug prices. Any increase in raw material costs affects all industries, ultimately raising prices of finished products.
According to statistics from the Wholesale Price Index (WPI), drug prices have risen by approximately 10.76 percent, marking the highest increase allowable. Under India's Essential Commodities Act of 1955, the federal government regulates the prices of essential medicines to
ensure affordability and accessibility. While larger companies are maintaining buffer stocks of raw materials despite high prices, smaller companies face greater challenges.
The pharmaceutical industry's reliance on foreign suppliers, mostly located outside the country, exacerbates this situation. Rising raw material prices directly affect suppliers of intermediate products like APIs (Active Pharmaceutical Ingredients), who struggle to pass on these cost increases to their customers, thoursqueezing their profit margins significantly.
Production and demand volumes have surged, with India now importing over 70 percent of its raw materials, especially APIs, from foreign sources. Nearly 70 percent of APIs are imported from China, where costs have reportedly increased substantially, impacting the profitability of pharmaceutical products.
Shortages of drugs can result from insufficient basic materials, including APIs, excipients, or packaging materials, due to political unrest, environmental factors, currency fluctuations during transportation, trade disputes, or low yields from source countries, among other operational challenges.
Drug prices constitute a significant portion of overall healthcare spending for individuals in both developed and developing countries. In many developing nations, public healthcare expenditure is minimal, and health insurance coverage is limited, leading to substantial out ofpocket expenses for households and pushing many families into poverty.Recent reforms in the pharmaceutical sector, such as increased imports, reduced trade margins, and liberalized policies, aim to boost domestic production of critical raw materials for medicines by providing incentives within the country. Government initiatives like the Production Linked Incentive (PLI) schemes introduced in July last year incentivize the production of essential raw materials, including APIs and intermediates, essential for manufacturing medicines in India, gradually impacting pharmaceutical production.

The global situation during the COVID 19 pandemic has highlighted significant shortages of active pharmaceutical ingredients (APIs), essential for manufacturing medicines and medical products. India's pharmaceutical industry heavily relies on China, importing around 70% of its API raw materials from there. However, the COVID19related lockdowns in both countries have severely disrupted international supply chains, exacerbating the scarcity of APIs.The pandemic has particularly impacted the supply of raw materials needed to produce drugs for chronic diseases like HIV, cancer, epilepsy, malaria, and commonly used antibiotics and vitamins. The domestic pharmaceutical industry, which depends on imports for over 60% of its API needs, is vulnerable, especially for critical drugs like cephalosporins, azithromycin, and penicillin, where the reliance can be as high as 80 to 90 percent. Supply chain disruptions and contamination risks further threaten medicine availability.The crisis has led to significant price increases for drugs. For instance, the cost of raw materials for penicillin rose by 40% in a month. Prices of other antibiotics like Azithromycin and essential drugs such as Paracetamol have also surged.The Indian regulatory agency warns of potential shortages for over 50 crucial APIs, including antibiotics, vitamins, and hormones, if lockdowns in China persist. Prices of essential drugs like Montelukast sodium have doubled or tripled due to supply constraints. To address shortages, India's drug pricing regulator temporarily increased the maximum price of critical medicines like heparin by 50%. This measure was taken as these medicines are vital for managing patients, especially those with critical illnesses or COVID 19.In response to these challenges, the Ministry of Health and Family Welfare (MoHFW) has integrated certain drugs into its clinical guidelines for COVID19 treatment, but shortages remain a concern. The National Pharmaceutical Pricing Authority (NPPA) invoked emergency powers to adjust the prices of essential medicines in public health programs, reflecting the gravity of the situation.

To address the rapid increase in Active Pharmaceutical Ingredient (API) prices during the pandemic, the Indian pharmaceutical industry must swiftly develop and execute a robust strategy. Policymakers
should initiate short term objectives, enhancing and incentivizing existing domestic players to prepare for global competition in the long run.
