San Joaquin County ElderCare Guide • August 2014 • Advertising Supplement

What is hospice, and when is the right time to call? Many people turn to hospice when their loved one has very little time left in their life journey. Hospice is a service that is unfortunately underutilized because it is often misunderstood. Hospice neither hastens nor postpones death, it does however provide the needed support that patient and families are frequently lacking when it comes to end of life care. Hospice provides comfort to persons nearing the end of life’s journey; it is about providing hope and quality of life. Services are designed to educate patients and families, and empower them to make decisions about how and where they want to spend the rest of their life. One of the biggest misconceptions of hospice is that it is only for cancer patients. Cancer is just one of many diagnoses that meet the criteria for hospice services. Additional diagnosis may include: Alzheimer’s Disease; Amyotrophic Lateral Sclerosis (ALS); Cerebral Vascular Accident/Stroke; Heart Disease/CHF; HIV disease; Huntington’s Disease; Liver Disease; Lung Disease; Multiple Sclerosis; Muscular Dystrophy; Myasthenia Gravis; Parkinson’s Disease; and Chronic Renal Failure among others. Hospice may be appropriate when a person has been diagnosed with an advanced illness that could end their life in six months or less, if the illness runs its normal course. Hospice is designed for patients who desire to have palliative treatment rather than

curative treatment, and who wish to stay in their home as long as possible. Hospice focuses on caring for the patient and their family. The focus is on the care needed, not a cure. Hospice considers the entire family’s needs and provides services to support the entire family, not just the patient. Hospice addresses patient and family’s medical, emotional, social, and spiritual needs. Through advancing medical technology, symptom and pain management allows patients to live as fully and comfortably as possible; emphasizing quality of life. When is the right time to inquire about hospice? • I have or know someone that has a diagnosis of an advanced illness. • I spend most of the time in a chair or bed. • I require continuous medication to help my physical pain. • In the last six months, I have made multiple trips to the hospital or emergency room. • I am unable to perform daily activities like showering, getting dressed, eating, cooking, walking, or getting out of bed without help from other people. • I feel tired all the time and need frequent naps. • I have lost a lot of weight in the last six months without trying.

If you have answered “Yes” to four or more questions, you may be eligible for hospice care. Hospice is an option that one may choose to start and stop. Hospice does not decide what is right for the patient; it is the patient that decides what is right. Hospice is simply there to support, educate and make sure all patient and family needs are being met to optimize quality of life. Choosing hospice can be one of the most difficult decisions a person makes, whether it is for themselves or a loved one. Many

feel like they are giving up when the word “hospice” is first mentioned. The truth is that once people understand what hospice is really about, it is an easy choice because they are choosing to live; to live the best they possibly can. It is never too early to know your options. For more information regarding hospice or for a free confidential consultation call Community Hospice at 209.477.6300.

Caring for Your Aging Parents When We Become Our Parent’s Parents By Kimberly Ripley

It’s become a national phenomenon. A much higher number of people these days are caring for their aging parents than this country has seen in several decades. Many of these families are still caring for dependent children at the same time. That particular concept is referred to as “sandwiching.” Welcome to the sandwich generation! A very delicate matter comes into play when we find ourselves in this precarious situation. It changes our entire family dynamic when we become our parent’s parents. Certainly we still respect the boundaries of parenthood when we care for our aging parents. We don’t insinuate that we are now the “Mom” or “Dad,” and we continue to call them by their respectful names. However, when all is said and done, and responsibility and safety issues become a priority we in fact wind up being the parental forces. Help! Help is imperative and help is available. Those parenting their parents should repeat these two facts like mantras. There is a lot of emotional baggage that comes with this job even for those who had seemingly idyllic childhoods. Check with your local center for aging or senior citizens center. Most communities have programs where your parent or parents can enjoy a day out. This occurs in a supervised setting with trained personnel. There are dinners, luncheons, programs and day trips. Many of these are free of charge. Those that require payment are typically nominal. Most folks attending these programs must qualify. The terms are usually simple. They must be of a certain age and/or disabled. There’s nothing wrong with asking for help. Caring for your parents can become a trying situation. You

need and deserve a break every now and then. Unfortunately, aging parents don’t always agree with this philosophy. “After all, we took care of you,” they sometimes say. And yes, this is very true. But didn’t Mom and Dad have the occasional vacation, Friday night date or dinner out? You need and deserve the same for your own well being as well as that of the relationship with your parents. If local community services aren’t available, considering hiring someone to come in and “baby sit” every now and then. Depending on the overall health condition and demeanor of your parent or parents, older teens, housewives or young seniors might be well suited to this job.   Boundaries Boundaries need to be carefully and tactfully laid in these types of situations. Parents (yours) don’t always relinquish the role of being parents, and sometimes might try to step on your very own toes. Yes, some grandparents like to point out their children’s parenting flaws and this can be especially trying in sandwich generation families. No one needs to have their parenting skills critiqued by an aging parent. It’s time for them to gracefully pass the torch and allow you to parent your own kids especially under your own roof. Lay down firm but kind rules regarding who has the right to discipline your children. Ask your mother or father to tactfully point out things that bother them, rather than complaining or whining and promise that you’ll do the same for them. Respect that this life change is enormous for them. Ask them to respect that it is quite a large change for you and your family as well. If your housing situation allows it, it is best to provide a private sitting

area and bedroom for your parent or parents. This allows him or her to entertain guests, while enjoying a respectable amount of privacy. It also allows you and your family some much needed time to regroup and breathe deeply! Some families declare a certain hour as “private time,” when Grandma or Grandpa go off to work on a crossword puzzle, while the rest of the family has a group chat or schedule planning session.   Laugh! A sense of humor can make the difference between night and day when taking care of aging parents. There are going to be days when you have all you can do to regain small bits of your composure. There will be extra laundry. There may be dietary restrictions and perhaps even finicky eaters. The best rule of thumb is to think back to when your children were small. For those without children, try recalling a neighborhood toddler you once cared for. Put yourself in the place of your aging parent, and you may very well find yourself in the place of a very small child. Be kind. Be patient. And by all means laugh about these situations! A laugh in the midst of a stressful moment or careless mishap might be just the medicine to remedy the situation. Laughter relieves stress.

It is said to help us live longer. It will be an additional lifeline while caring for your aging parents.   Cry It’s perfectly okay to cry at those those moments that become overwhelming. It is sad to watch your once agile and athletic father confined to a walker or wheelchair. It’s heart wrenching when your own mother no longer knows your name. Share your thoughts with your family, especially your spouse. Check with local senior citizen agencies about support groups for people just like you. This is becoming more and more common and so many people share these same feelings. Finding out you’re not the only one in this situation is healing. The very best advice I ever heard or accepted with regard to caring for aging parents (or not-so-aging parents for that matter!) is this: Honor thy father and mother. Regardless of the hand life has dealt you; if you find yourself in this place in life, make the best of it. Get help. Talk to others who understand. Regardless of the present burden this lifestyle bears, there is one burden you will never experience after caring for your own parents and that is the burden of regret. Copyright © Publishers-Edge

How to Select an Assisted Care Facility By Joe Talent

There are many things to consider when preparing to move into an assisted care facility. One important facet to consider is the living space and accommodations. These will vary greatly but are most important when deciding where to live. Some things to consider are listed below. Are there adequate community areas for resident use? One of the key components of moving into any multi-family community is the relationships you can build with your neighbors. The community areas will facilitate friendships. Is equipment (such as hobby supplies, easels, yarn, etc.) available and are the residents using the equipment? Whether you are continuing long time hobbies or want to develop new ones, you may want to know if the facility can accommodate your hobby. Are the resident rooms furnished or unfurnished? Regardless of how much furniture you now own, some of it is probably going to have to go into storage or be sold. Your new residence is likely to have a smaller living space than where you currently live. What is the policy about personal belongings? You may have to give up even your most prized possessions. What is the policy for overnight guests? If out of town family come to visit where are they going to stay? If your new residence has restrictions on overnight guests you may want to find a nearby hotel that will accommodate visiting friends and family. Are there guestrooms available? What are the guest fees? Is outdoor furniture available for residents and visitors? A smaller residence can restrict your young visitors. An outdoor living space can help relieve some of the small space stress of the grand kids

coming to visit. Is additional storage space available? Is there an extra fee? You are going to have personal belongings that you will just not have room for. Check on available storage areas for residents or find a low cost storage facility. Does the residence meet the rules for people with disabilities? Most assisted care facilities are going to have many accommodations for disabled residents. However, an older facility may not include some of the conveniences most new facilities will include. Can residents have automobiles? Many facilities provide transportation for residents and discourage self transportation or charge an additional fee for parking. Are there patios and courtyards available for resident use? The outdoor community living space is an important feature for residents who expect many visitors. Is there an area for resident gardening? If you enjoy growing flowers or a few of your own vegetables you will want to ask about a community garden area. Is exercise equipment available? You might not want a large selection of free weights, but to keep in shape and feeling good an exercise facility with a nice selection of easy to use equipment can be a feature that will keep you active and healthy. Does the residence provide security? An on-site security agent can provide a sense of comfort and feeling of a safer community. Are pets allowed to reside in the residence? If so, are there additional fees and or deposits? If you have a pet you will want to be sure and find a facility that allows pets. You are going to have plenty of changes just by moving into a new home. Giving up your friend for life can add more stress.

Asking some of the questions above can help you find an assisted care facility that is a great fit for your lifestyle and help make your

experience smooth and a move to look forward to. Copyright Š 2006 Publishers-Edge
